1.The detection of hemoglobin A1c in clinical application

The hemoglobin A1c(HbA1c) results can reflect the average level of blood glucose over the past 2-3 months,its clinical application value is transforming from evaluation indicator to diagnosis indicator.The application of HbA1c as a diagnostic indicator of diabetes is limited by many factors,such as multi-factor influence on measurement and its late start on standardization process in our country,optimal cut-point and the defect of HbA1c existing in the molecular level,etc.Therefore,clarifying the limitations of HbA1c in the practical application is of great significance for its clinical application.
2.Effects of alterations in extracellular pH on the respiratory burst in neutrophils

The respiratory burst of neutrophils is an important response in inflammation. The alterations in extracellular pH per se is not a sufficient trigger to the respiratory burst in neutrophils. While neutrophils in respose to conventional agonists, the respiratory burst in neutrophils is affected by the alterations in extracellular pH, and there is a direct relationship between intracellular and extracellular pH. O-? 2 production in neutrophils is decreased while H 2O 2 production is increased at acidic pH. O-? 2 release in neutrophils is enhanced at weak alkaline pH. Na+/H+ exchanger plays an important role in the regulation of the respiratory burst of neutrophils.
3.Research of correlation between polyunsaturated fatty acids and postpartum depression

Objective To research whether the polyunsaturated fatty acids (PUFAs) level in prenatal and postpartum women af‐fect the incidence of postpartum depression .Methods 102 eligible and voluntary parturients of full‐term normal delivery were se‐lected and the serum specimens before labor and on postpartum 3 ,42 d were respectively collected for detecting serum PUFAs lev‐els .The perinatal depression related factors questionnaire and the Edinburgh Postnatal Depression Scale (EPDS) were filled in and the scores were evaluated for analyzing the incidence of postpartum depression .Results Among 102 pregnant women and parturi‐ents ,the postputum depression appeared in 28 cases with the positive rate of 27 .5% .The incidence rate of postpartum depression in the eicosapentaenoic acid (EPA) low values group and the docosahexaenoic acid (DHA) low and medium values groups was higher than that in the high values groups ;the incidence rate of postpartum depression in the arachidonic acid (AA) high value group was the higher than that in the low value group .Conclusion The lower the EPA and DHA levels ,the easier to develop depression .
4.An overview of epidemics in Liberia and its revelation for epidemic prevention during peacekeeping operations

Communicable diseases are the major threats to the health and security of all the people living in Liberia, including UN peacekeepers from China. The most prevalent communicable diseases in Liberia include malaria, HIV/AIDS, acute respiratory infection, sexual transmitted disease, schistosomiasis, onchocerciasis, tuberculosis, cholera, pertussis, hepatitis, meningococcal disease, typhoid fever, Lassa fever and yellow fever. An insight into the profiles and epidemiology of the above epidemics in Liberia would greatly help peacekeepers with disease diagnosis and epidemic prevention. According to the profiles of epidemics in Liberia and the authors' experience in epidemic prevention in Liberia, the authors recommend that peacekeepers strengthen their epidemic prevention as follows. Firstly, a combined vector control strategy is suggested for the prevention and control of vector-borne diseases. Secondly, water safety should be highlighted by water disinfection and regular water quality testing. Thirdly, vaccines, diagnostic reagents and medications should be accordingly outfitted. Then, the awareness of epidemic prevention and individual hygiene should be improved by education and strict management. Finally, the daily life management for peacekeepers is also very important. The epidemic overviews and strategies for epidemic prevention described in this paper are also useful for all the other peacekeepers deployed in Africa.

6.The cut-point for glycosylated hemoglobin in different populations in the plateau region

Objective To explore the optimal HbAlc diagnostic cutpoint in different glucose tolerance populations in the plateau region.Methods (1) 472 diabetes mellitus (DM) patients and highrisk groups accepting diabetes screening in the First Affiliated Hospital of Kunming Medical College (217 males and 255 females,≥20 years old,median age 54 years old) were collected,oral glucose tolerance test (OGTT) and HbAlc were tested.(2) the research subjects were divided into normal glucose adjustment group (NGT),Impaired fasting glucose group (IFG) and (or) Impaired glucose tolerance IGT group and diabetes mellitus (DM) group.The receiver-operating characteristic curve (ROC) was explored to determine the optimal HbA1c diagnostic cut point for IFG,IGT and DM status respectively.Results The average HbA1 c values of NGT,IFG and (or) IGT,DM groups were (6.06 ± 0.11) %,(6.63 ± 0.11) %,(8.70 ± 2.08)% respectively,for IFG and IGT groups,the optimal HbA1c diagnostic cut points were 6.7% and 6.6%,respectively; If use either FBG or 2 h PG to diagnose DM,the corresponding optimal HbA1 c diagnostic cut point was 7.1% ; If use anyone of FBG or 2hPG to diagnose DM,the corresponding optimal HbA1c diagnostic cut point was 7.0% ; If both FBG and 2hPG were used to diagnose DM,the corresponding optimal HbA1 c diagnostic cut point was 7.1%.Conclusion Preliminarily confirm the optimal HbA1c diagnostic cut point in different glucose tolerance populations in the plateau region of Kunming,and provide the evidence for further clinical application of HbA1c.
7.Research on correlation between E-cadherin mRNA expression and lung cancer

Objective To explore the correlation of E-cadherin(E-cad) mRNA expression with tumorigenesis and development of lung cancer.Methods The relative quantitative nested RT-PCR was used to detect the expression of E-cad mRNA in cancerous tissues,adjacent and distal tissues of tumor from 50 lung cancer patients,and non-cancerous benign lung tissues from 5 lung disorder patients.Results The means of expression rate of E-cad mRNA in cancerous tissue,adjacent and distal tissues of tumor,and non-cancerous tissues were 1.33?0.53,1.65?0.60,2.01?0.46 and 2.09?0.09 respectively.The data were evaluated by analysis of variance.There were significant difference among cancerous tissue,adjacent and distal tissue(F=18.810,P0.05).Also,there were no significant differences among squamous cell carcinoma,adenocarcinoma and adenosquamous carcinoma in the group of cancerous,adjacent and distal tissue.Conclusion The reduced expression of E-cadherin mRNA may play important role for tumorigenesis and development of lung cancer,and there was no evident relationship of E-cad mRNA with different pathological types of lung cancer.
8.Clinical observation on herb-partitioned spreading moxibustion at Baliao points plus climen for diminished ovarian reserve

Objective: To observe the clinical efficacy of herb-partitioned spreading moxibustion at Baliao points plus climen for diminished ovarian reserve (DOR). Methods: A total of 60 patients with DOR were randomized into a spreading moxibustion group and a Western medicine group by the random number table method, with 30 cases in each group. The Western medicine group was treated with climen, starting from the 5th day of the menstrual cycle for 21 d. The spreading moxibustion group was treated with herb-partitioned spreading moxibustion at Baliao points on the basis of the medication in the Western medicine group, 1 h per time, once a week. The treatment was performed for 1 month as one treatment course in both groups, for 3 courses in total. The serum follicle-stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2) in the patients were measured before and after treatment. The peak systolic velocity (PSV) and resistance index (RI) were also detected. The traditional Chinese medicine (TCM) symptom score was evaluated. The clinical efficacy was evaluated after treatment. Results: The total effective rate in the spreading moxibustion group was 93.3%, which was significantly higher than 80.0% in the Western medicine group, and the difference between the groups was statistically significant (P<0.05). After treatment, the TCM symptom scores, the serum FSH levels, FSH/LH ratios and RI in both groups decreased, and the intra-group differences were all statistically significant (all P<0.05). The serum E2 level and PSV increased compared with those in the same group before treatment, and the intra-group differences were statistically significant (all P<0.05). After treatment, the TCM symptom score, the serum FSH level, FSH/LH ratio and RI in the spreading moxibustion group were lower than those in the Western medicine group, while the serum E2 level and PSV were higher than those in the Western medicine group, and the differences between the groups were statistically significant (all P<0.05). Conclusion: Herb-partitioned spreading moxibustion at Baliao points plus climen can produce valid therapeutic efficacy for DOR. It can improve the clinical symptoms, regulate serum hormone levels and increase ovarian blood perfusion, thus improving ovarian reserve function, producing more significant efficacy than climen alone.
10.Analysis of Th1/Th2 response pattern for erythrodermic psoriasis.

As one of the most serious types of psoriasis, pathogenesis of erythrodermic psoriasis (EP) is unclear so far. In this study, we aimed to detect the levels of Th1/Th2 cytokine-associated transcription factors and T-lymphocyte clone in peripheral blood mononuclear cells (PBMCs) derived from EP patients, and gene expression level of T-bet/GATA-3 in skin lesion. The potential role of Th1/Th2 reaction pattern played in the pathogenesis of EP was also discussed. Serum levels of IFN-γ, IL-2, IL-4 and IL-10 were quantified by ELISA among 16 EP patients, 20 psoriasis vulgaris (PV) patients and 15 healthy controls. The expression levels of T-bet/GATA-3 in the skin lesion and PBMCs were examined by real-time qPCR. The ratio of Th1/Th2 was measured by flow cytometry. The levels of IFN-γ, IL-2, IL-4 and IL-10 were higher in EP patients than in the healthy controls. The levels of IL-4 and IL-10 were 69.44±11.45 and 12.62±4.57 pg/mL, respectively, in EP patients, significantly higher than those in PV patients and healthy controls (P<0.05). Flow cytometry revealed the levels of both Th1 and Th2 in PBMCs from EP patients were higher than those in healthy controls, and the Th1/Th2 ratio was dramatically lower than in PV patients (P<0.01). The ratios of IFN-γ/IL-4 and T-bet/GATA-3 in EP patients were both less than 1.0, suggesting a reversal when compared with the other two groups. Our study indicated that the EP patients exerted a Th1/Th2 bidirectional response pattern, and the balance of Th cell subsets inclines to Th2, which might be one of the important mechanisms of EP pathogenesis.
