Objective To observe morphological character of posterior arch of Atlas and lamina of axis,as well as provide anatomical basis to design special guiding device for the operation of upper cervical posterior fixation with Atlas titanium cable so as to make the operation safe and easy.Methods The posterior arch of Atlas and lami- na of axis were observed and measured in 100 adult dry Atlas and axis specimens,then special guiding devices were designed based on the anatomical data.Results (1)The posterior arch of Atlas was thin with a fan-shape section, whose inner plane descended from back to inside and the lamina of axis was stronger and thicker than posterior arch of Atlas,with a vertical inner plane and a ridge near the lower edge.(2)The special guiding device for posterior arch of Atlas should be designed with a bend head as well that for lamina of axis should be designed with a round head. They were used in 61 operations which became safer and easier.Conclusion The operation of upper cervical posteri- or fixation with Atlas titanium cable would be safer with the help of special guiding devices which were designed based on the anatomical character.

OBJECTIVE: To investigate the correlative factors for organic psychotic symptoms following traumatic brain injury (TBI). METHODS: In the current study, 391 subjects who had undergone forensic identification of the organic mental disorders due to TBI were included, both the demographic and post-traumatic information collected. The relevant data were statistically analyzed in those confirmed as organic psychotic syndrome according to Chinese Classification of Mental Disorders 3rd version (CCMD-3). RESULTS: Fifty-two subjects (13.3%) were identified as organic psychotic symptoms. The chi-square test showed that the detectable organic psychotic symptoms were associated with the marriage status, damage nature, injury severity and treatment, and the multivariate logistic regression analysis revealed good fitness of treatment and injury severity with the regression model (OR = 0.044, 95% CI: 0.017-0.114; OR = 2.145, 95% CI: 1.201-3.832, respectively). CONCLUSION The risks of organic psychotic symptoms following TBI can be involved in the alternative of craniotomy for the cases with trauma and moderate brain injury.

OBJECTIVETo discuss the therapeutic effect of thenar perforator flaps for the narrow cicatricial contracture at thumb and the first web.

METHODSFrom Aug. 2010 to Jan. 2012, 9 cases with narrow cicatricial contracture at thumb and the first web, were treated. The defect size after releasing the contracture ranged from 8 mm x 20 mm to 15 cm x 30 mm. The bilateral thenar perforator flaps beside the wound were designed which size was 10 mm x 25 mm to 15 mm x 35 mm. The wounds at donor sites were closed directly.

RESULTSAll the 9 flaps survived completely with primary healing. The patients were followed up for 6-18 months. The flaps had soft texture and good appearance. The shape of flaps and function of the fingers were satisfied after 6-18 months of follow-up. There was no scar contracture at incisions in thenar. The thumb motion was really normal. The abduction of first web was 70 degrees - 90 degrees degrees.

CONCLUSIONSThe thenar perforator flaps is one of the ideal methods for the treatment of narrow cicatricial contracture at thumb and the first web. The main artery is not sacrified.

Objective To analyze the epidemic situation of human brucellosis in Qinghai province,in order to provide a theoretical basis for prevention and treatment of brucellosis.Methods Five counties in Qinghai province(Ping'an county,Haiyan county,Tianjun county,Dari county and Henan county) were selected as monitoring counties in 2010,and three or four townships were selected as monitoring points in each county.People aged 7 to 60 close contacted with livestock were monitored.The methods of rose bengal plate agglutination test(RBPT) and standard tube agglutination test (SAT) were used for serological testing.Brucellosis diagnosis was based on Brucellosis Diagnostic Criteria(WS 269-2007).Results A total of 2021 high-risk subjects from 5 counties were examined for brucellosis,and 76(by RBPT) of them were serologically confirmed with a positive rate of 3.76％ (76/2021),SAT positive 15 persons,the average positive rate was 0.74％ (15/2021).The number of infections was 15 cases,average prevalence was 0.74％(15/2021 ),and 3 new eases were discovered.The seasonal peak of incidence was from February to June.Most of the eases were reported dealing with slaughtering and fur processing.Conclusions The main reasons for epidemic rise are that the source of infection is not completely clear and weak awareness of self-protection in employees.So we must strengthen the quarantine,do surveillance on occupational groups and epidemic reporting work,carry out vocational training,strengthen publicity and education,and enhance the protection awareness.Thus brucellosis epidemic will be effectively controlled.

OBJECTIVETo establish and evaluate a BA-ELISA method for the quantitative detection of cystic fibrosis transmembrane conductance regulator (CFTR) protein.

METHODSWe deliberately selected three tables of CFTR and made the synthetic peptide be expressed in E. coli, then used the antigen to immunize rabbits to obtain the anti-CFTR polyclonal serum. After that, 96 well plates were coated with the purified antibody against CFTR. The antigen CFTR which was extracted from human sperm was detected by anti-CFTR antibody labeled with biotin, horseradish peroxidase conjugated avidin, and the substrate. The concentrations of two kinds of antibodies and the experiment parameters were optimized. Thereby, the double antibody sandwich BA-ELISA method for the quantitative detection of CFTR protein was established. Furthermore, the reproducibility, specificity and so on were evaluated by clinical specimens of sperm.

RESULTSThe optimal concentration of coated anti-CFTR IgG was 4 µg/ml, while the biotin labeled anti-CFTR IgG was 10 µg/ml; the optimal blocking buffer was 1% BSA-PBST, the optimal time of the reaction between antigen and antibody was 60 min, the optimal chromogenic time was 15 min, the intra-assay and inter-assay coefficient were 2.16%-9.23% and 2.29%-11.71% respectively; The lowest detectable limit was 0.15 ng/ml; the standard curve had a good linear correlation of R2 = 0.962.

CONCLUSIONThe BA-ELISA method for the quantitative detection of CTFR protein is successfully established, and it is demonstrated that the method has strong specificity, high sensitivity and good reproducibility. It provides the basis and evidence of the further application of the method.

OBJECTIVETo observe effects of acupuncture combined with modern rehabilitation at early stage on life self-care ability and mental state of the patient of stroke.

METHODSTwo hundred and twelve cases with stable life signs and no progress of neurological signs for 48 hours were randomly divided into an acupuncture plus rehabilitation group (group I, n = 113) and a simple acupuncture group (group II, n = 99). The group I were treated by acupuncture plus psychological therapy and movement function rehabilitation therapy, and the group II by simple acupuncture. Their life self-care ability and mental state were evaluated.

RESULTSThe score of life self-care ability in the group I was up-regulated by 17.57 points (P < 0.05) and the mental state was down-regulated by 5.86 points (P < 0.05) in average as compared with that in the group II.

CONCLUSIONAcupuncture combined with modern rehabilitation technique at early stage of stroke can significantly improve life self-care ability and mental state of the patient.

Objective To determine the impact of fluorine and aluminum,and both action combined on the number of rat osteoclasts and bone resorption cultured in vitro and to explore its mechanisms.Methods The osteoclasts and bone marrow stromal cells (BMSCs) isolated from long bone of new born rats were cultured,respectively,in TC199 medium (containing 10％ fetal bovine serum) with fluoride,aluminum and fluoride combined with aluminum.The osteoclasts were inoculated in 96-well culture plate and ivory slice,BMSCs in 6-well culture plate,and culture medium was changed after 2 hours incubation.The cells were divided into control group,fluoride group,aluminum group and fluoride combined with aluminum group; the doses of sodium fluoride were 0,1.0 × 10-4,0,1.0 × 10-4 mol/L and the doses of aluminum chloride were 0,0,1.0 × 10-5,1.0 × 10-5 mol/L,respectively.Tartrate-resistant acid phosphatase (TRAP) staining positive cells were counted under light microscope after TRAP staining on the 5th day and the pit formed in ivory slices were measured by histomorphometry after staining with toludine blue.The expression of osteoprotegerin(OPG) and receptor activator of nuclear factor kappa-B ligand (RANKL) was detected by real-time fluorescence quantitative PCR in BMSCs after 8 h treatment.Results ① Fluoride,aluminum and the interactive effects of fluoride and aluminum all had impact on the numbers of osteoclasts (F =7.15,6.56 and 7.98,respectively,all P ＜ 0.05).The numbers of osteoclasts in fluoride group,aluminum group and fluoride combined with aluminum group[(136.9 ± 22.99),(135.4 ± 23.5),(163.0 ± 24.4) per well] were higher than that in the control group[(92.5 ± 22.1) per well,all P ＜ 0.05].② Fluoride,aluminum and the interactive effects of fluoride and aluminum all had impact on the resorption pit area on ivory slices(F =10.47,12.64,14.29,respectively,all P ＜ 0.05).The resorption pit area on ivory slices in fluoride group,aluminum group and fluoride combined with aluminum group[(0.242 ± 0.031),(0.293 ± 0.026),(0.333 ± 0.016)mm2 per slice] was higher than that in the control group [(0.088 ± 0.030)mm2 per slice,all P ＜ 0.05].③Fluoride,aluminum and the interactive effects of fluoride and aluminum all had impact on the expression ratios of RANKL/OPG in BMSCs (F =8.15,15.38,23.59,respectively,all P ＜ 0.05).The expression ratios of RANKL/OPG in BMSCs in fluoride group,aluminum group and fluoride combined with aluminum group [(193.98 ± 137.93)％,(326.11 ± 176.78)％,(599.84 ± 275.82)％] were higher than that in the control group[(100.00 ± 56.02)％,all P ＜ 0.05].Conclusions Both fluoride and aluminum can cause increase in the number of osteoclasts in vitro and promote cell differentiation and bone resorption activity,which may be related to increased expression ratio of RANKL/OPG mRNA in BMSCs.The stimulating effects of fluoride on osteoclasts differentiation and bone resorption is enhanced by aluminum.
