3.Correlation between contrast-enhanced ultrasound features and size of clear cell renal cell carcinoma

Objetcive To analyze the contrast-enhanced ultrasound (CEUS)features in clear cell renal cell carcinoma (CCRCC) of different size and to discuss the diagnostic value of CEUS in CCRCCs. Methods The contrast-enhanced and conventional US features of 80 CCRCCs confirmed pathologically were retrospectively analyzed. Samples were divided into three groups by diameters: small CCRCCs (≤30 mm), medium CCRCCs(>30 mm) and large CCRCCs(>50 mm). The tumoral vascularity, lesion homogeneity and presence of an anechoic rim were observed on conventional US. The enhancement of echogenicity, homogeneity and pseudocapsule were evaluated on CEUS. Results Among the 80 renal masses, 32 were small CCRCCs, 28 were medium CCRCCs while the large CCRCCs were 20. On conventional US, 18.8%(6/32) of small CCRCCs, 71.4%(20/28) of medium CCRCCs and 95%(19/20) of large CCRCCs demonstrated as heterogeneous and the differences were highly statistically significant, whereas there were no differences in the tumoral vascularity and the presence of pseudocapsule sign among the three groups. On CEUS, 28.1%(9/32) of small CCRCCs, 85.7%(24/28) of medium CCRCCs and 100%(20/20) of large CCRCCs showed a homogeneous enhancement. The incidence of pseudocapsule sign in medium tumors was higher than small and large groups(71.4%,50% and 25%,respectively). There were statistically significant differences among the three groups in the enhanced homogeneity and the presence of pseudocapsule sign. However, all the three groups revealed mainly isoechoic and hyperechoic and there were no differences among them. Compared with conventional US, CEUS depicted significantly the increased tumoral vascularity (51.3% vs. 87.5%) and pseudocapsule sign(22.5% vs. 51.3%). Eight lesions demonstrated heterogeneous contrast enhancement on CEUS while homogeneous on conventional US, but there were no differences in lesion homogeneity between conventional US and CEUS findings. Conclusions Different sizes of CCRCC showed distinct CEUS features in the enhanced homogeneity and the presence of pseudocapsule sign. CEUS is more effective on improving the sonographic characteristics of tumoral visualization and may provide important information of US findings for the diagnosis of renal cell carcinoma.
4.Clinical observation on auricular point sticking for treatment of 25 children of semiluxation of circo-axis vertebrae.

OBJECTIVETo search for the best method for child semiluxation of circo-axis vertebrae.
METHODSFifty cases were randomly divided into a treatment group of 25 cases treated with auricular point sticking, and a control group of 25 cases treated with normal acupuncture.
RESULTSThe total effective rate was 92.0% in the treatment group and 64.0% in the control group with a significant difference between the two groups (P < 0.05).
CONCLUSIONAuricular point sticking has a good effect on child semiluxation of circo-axis vertebrae.

5.Clinical research on relative factors and curative effects of Yi-gan-san modified with attention deficit hyperactivity disorder

Objective To explore the main influence factors of attention deficit hyperactivity disorder ( ADHD) and evaluate the clinical effect of Yi-gan-san modified.Methods Questionnaire survey consisting of home environment, living habits and exposure to environmental toxicant was used to investigate and evaluate children in the two groups, one of them was the study group with 120 cases of ADHD children, and the other was the control group with 120 cases of healthy children.A total of 75 cases with the pi-xu-gan-kang type were found among the ADHD children, and they were randomly divided into 2 groups:the experimental group ( n =38 ) , using drug therapy of Yi -gan -san modified ( 150 mL, twice a day ) and behavioral therapy (3 times a week);the control group, using behavioral therapy alone.The treatment lasted for all 8 weeks.The data of the line integral of traditional Chinese medicine symp-tom assessment was used to evaluate the curative effect before treatment and 4, 8 weeks after treatment.Results The survey found that the family environment such as low education level of parents, parenting methods, family income less than three thousand yuan each month, parenting and feeding patterns, whether both parents were the only child in the family were significantly associated with the occurrence of ADHD.We also found the exposure to DEHP, children′s rest hobbies, sleep habits, eclipse snack times and playing phone were significantly re-lated with the occurrence of ADHD.The CR of the treatment group was 52.63% and the total effective rate was 86.84%, and there was 24.32%and 43.24%in the control group respectively.The TCM syndrome score between the two groups had significant statistical significance ( P<0.01 ) , and the curative effect of the treatment group was better than that in the control group.The TCM syndrome score were significantly lowered after 8 weeks of treatment in the two groups (P<0.01).Conclusion The family environment, lifestyle and children′exposure to environmental toxicants were susceptible to the occurrence of ADHD.Using Yi-gan-san modified intervention for the treatment of ADHD is effective.
6.Study on preparation and thermosensitive release property of composite phospholipid liposomes containing total alkaloids from Strychnos nux-vomica.

To prepare composite phospholipid liposomes containing total alkaloids of Strychnos nux-vomica with hydrogenated soybean phosphatidylcholine (HSPC) and 1, 2-dipalmitoyl-sn-glycero-3-phosphacholine (DPPC), and compare with normal DPPC thermosensitive liposomes for thermosensitive release property. Total alkaloids were extracted from S. nux-vomica with the impregnation method and further purified. Liposomes containing total alkaloids, thermosensitive liposomes and conventional thermosensitive liposomes without thermosensitive release property were prepared by ammonium sulfate transmembrane gradients and stealth liposome technique. Their encapsulation efficiency (EE), grain size, zeta potential and drug release behavior were compared. Their EEs and zeta potentials were almost identical; but the grain sizes of composite phospholipid liposomes and thermosensitive liposomes were significantly smaller than conventional liposomes. After comparing release behaviors of the three liposomes at 37, 43 degrees C, we found that the release of composite phospholipid liposomes was significantly lower than that of thermosensitive liposomes at 37 degrees C, but higher than that of thermosensitive liposomes at 43 degrees C. Meanwhile, conventional liposomes, with a very high phase-transition temperature, showed only slight release behavior at both temperatures. The study results showed that composite phospholipid liposomes had a better thermosensitive release behavior when the dosage of lysophosphatidic was reduced by 2. 5 times.

7.Application of improved new mask in noninvasive positive pressure ventilation sputum suction

Objective To explore the effect of improved new mask in noninvasive positive pressure ventilation sputum suction.Methods Among the patients with chronic obstructive pulmonary disease and type Ⅱ respiratory failure,78 patients were selected and divided randomly into observation group (n =38 ) and control group (n =40).Both groups implemented the noninvasive ventilation and symptomatic treatment.The endotracheal suction tube was left in patients for 96 hours,the end of which passes the junction part of the mask and the face.For the control group,the process of ventilation was interrupted for sputum suction,while for the observation group; the process of ventilation kept on without interruption due to the side hole on the mask for suction.In the interval of sputum suction,the same airway humidification fluid was injected into the suction tube regularly for both groups.Results When the observation group and the control group had accepted the noninvasive ventilation simultaneously for 24 hours,the oxygen and carbon dioxide partial pressure difference was not statistically significant(t =0.84,1.07;P > 0.05).While for 48 hours,the oxygen and carbon dioxide partial pressure difference was statistically significant [ (61.53 ±9.94) mm Hg versus (56.57 ± 11.19) mm Hg,t =2.07,P < 0.05 ; (64.02 ± 10.37 ) mm Hg versus ( 69.55 ± 12.73 ) mm Hg,t =2.10,P < 0.05 ].and for 96 hours,the oxygen and carbon dioxide partial pressure difference was also statistically significant [ (69.35 ±9.37) mm Hg versus (60.57 ± 10.64) mm Hg,t =3.87,P < 0.01 ; ( 55.82 ± 10.58 ) mm Hg versus ( 61.55 ±11.73 ) mm Hg,t =3.03,P < 0.01 ].The difference of the time it took for the patients to got conscious in both groups was statistically significant[ (21.21 ±7.24) h versus (25.35 ±9.28) h,t =2.17,P <0.05].The difference of the number of cases with the application of invasive ventilation in the end of two groups was statistically significant (x2 =4.78,P < 0.05 ).The difference of the total time with the application of positive ventilation of two groups was statistically.significant [ ( 152.23 ± 25.91 ) h versus ( 169.57 ± 33.49 ) h,t =2.54,P < 0.05 ].Conclusions The mask with a side hole for sputum suction and endotracheal suction tubes can be applied to the patients of the chronic obstructive pulmonary disease combined with the Ⅱ respiratory failure.Without the interruption of the noninvasive ventilation,sputum suction has significant reduction in complications,and the patients can have a shorter recovery time,better treatment effect,and smaller rate of invasive ventilation.
8.CT findings of Madelung's disease in the head and neck region

Objective To investigate the CT findings of Madelung's disease in the head and neck region,and to evaluate the value of CT in demonstrating the Madelung's disease in the head and neck region.Methods CT findings of Madelung's disease in the head and neck region in 7 cases were analyzed retrospectively.All were males,with the age from 36 to 60 years,mean 51 years.All patients were underwent CT native scan,and enhanced CT scan was performed on 3 of them.Results CT images in the neck of all patients showed accumulation of nonencapsulated fat within the subcutaneous tissue and(or) deep to the platysma,and(or)within the spaces between the muscles.The fat deposits were ill-defined and symmetrical.In most cases the fat deposits involved the anterior part of the neck(infrahyoid and suprahyoid),submandibular region,the subcutaneous tissue of the nape and deep to the stenomastoid muscles.Conclusions Madelung's disease in the head and neck region have characteristic CT findings,and CT has great value in qualitative and quantitative diagnosis in Madelung's disease.
9.Relationship of immunophenotypic features with minimal residual disease detection and gene types in 221 cases of acute promyelocytic leukemia.

This study was aimed to investigate the relationship of immunophenotypic features with minimal residual disease (MRD) detection and gene types in APL patients. Immunophenotypes were analyzed in 221 newly diagnosed APL patients by using four-color flow cytometry. Among of them, CD123 antibody was examined in 87 patients and the fused gene pml-raralpha were detected by PCR in 196 specimens simultaneously. The results of immunophenotyping demonstrated that the positive percentages of CD123, CD33 and CD9 in newly diagnosed APL patients were 100%, 99.1% and 96.0% respectively, and mean percentages of positive cells in positive patients were all around 90%. Although the positive rates of CD117, CD13, CD38 and CD64 were all above 96%, but the mean percentages of positive cells in different positive patients were diverse and average percentages of positive cells were about 70%. CD15, CD56 and CD11b were expressed in some patients, but CD34 and HLA-DR were rarely expressed in the majority of patients, and average positive percentages were all lower. Among 196 newly diagnosed APL patients, bcr1, bcr2 and bcr3 expressions were 63.3%, 4.6% and 32.1% respectively. The results showed a strong correlation of positive expression of CD34 with bcr3 isoform. When cut-off value was chosen as 20%, the proportions of CD34 positive patients in bcr3 and bcr1 cases were 15.4% (10/65) and 3.3% (4/121) separately, which had a significant difference (p < 0.05). When cut-off value was 10%, bcr3 cases had a significantly higher percentage of CD34 positive, compared with bcr1 cases (p < 0.001), which was 47.7% (31/65) and 5.8% (7/121) respectively. However, there was no statistically significant difference on the other antigens between the two groups. Bcr3 isoform was highly indicated when CD34 was positive and non- large side scatter (NL-SSC) was shown in APL cells. It is concluded that there is a unique characteristics of immunophenotyping, and antigens such as CD123, CD33 and CD9 are more applicable to the detection of MRD in APL patients. The positive expression of CD34 and NL-SSC are associated with bcr3 isoform, and the relationship between gene type and antigen expression can be suggested more accurately when the cut-off value is chosen as 10%.

10.Clinical significance of CD34(+)CD38(+) and CD34(+)CD38(low/-) subgroups in bone marrow of patients with B lymphoblastic leukemia.

This study was purpose to investigate the biological characteristics of B lymphoblastic leukemia (B-ALL) between CD34 positive CD38 positive (CD34(+)CD38(+)) and CD34(+)CD38(low/-) subgroups and their clinical significance. Immunophenotyping of B cells in bone marrow of 54 patients with newly diagnosed CD34(+)B-ALL were analyzed by 4 color multiparametric flow cytometry (FCM). According to the different expression of CD38, the newly diagnosed patients with B-ALL were divided into two groups: CD34(+)CD38(+) subgroup and CD34(+)CD38(low/-) subgroup. BCR-ABL, TEL-AML1 fusion genes and WT1 gene were detected by real time RT-PCR simultaneously. After chemotherapy, minimal residual disease (MRD) was monitored by one tube of 7 color FCM. The average follow-up time was 12 months (range 1 - 28), the average follow-up interval was 2 months (range 1 - 5). The results showed that there was no significant differences such as WBC, Plt count and Hb level between the two groups at diagnosis, the positive rate of BCR-ABL, TEL-AML1 and WT1 gene was also no significantly different. After clinical complete remission (CR), MRD positive (MDR(+)) case rates were 28.57% (10/35) in CD34(+)CD38(+) subgroup and 68.42% (13/19) in CD34(+)CD38(low/-) subgroup (P < 0.01). The relapse rate between the two groups was 5.71% (2/35) in CD34(+)CD38(+) subgroup (relapse time at 94 and 245 d respectively) and 36.84% (7/19) in CD34(+)CD38(low/-) group [median relapse time was 263 d (range 46 - 468), P < 0.01]. The age distribution was analyzed in these two subgroups (> 16 or ≤ 16 years old), there was 8 (8/35) adult patients (> 16 years old) in CD34(+)CD38(+)group and 10 (10/19) adult patients in CD34(+)CD38(low/-) group (P < 0.05). It is concluded that CD34(+)CD38(low/-) phenotype is more often presented in adult patients and the CD34(+)CD38(low/-) patients with B-ALL are more likely to have MRD(+)and relapse after treatment.
