1.Effects of multiple monitoring of total intravenous anesthesia on postoperative cognitive dysfunction in ;elderly patients

Objective To observe the effect of multiple monitoring of total intravenous anes-thesia on postoperative cognitive function in elderly patients.Methods Elective 100 patients undergo-ing general anesthesia for abdominal operation,56 males,44 females,aged 65-80 years,ASA physi-cal status Ⅱ or Ⅲ.All patients were divided into multiple monitoring group (group M)and routine monitoring group (group R)by random digital table method,n =50 each.In group M,the anesthesi-ologists modulated anesthetic drugs to make NTI of 37-56 and rSO 2 higher than 50% or not lower than the baseline value by 20%,while in group R the infusion rate of propofol,remifentanil and cisa-tracurium was adjusted by anesthesiologists according to anesthesiologist’s experiences by the pa-tients’monitoring index.Cognitive function of patients in the two groups were evaluated using MMSE 1 d before surgery and 1 d,3 d,7 d,1 month and 3 months after surgery.The occurrence of cognitive dysfunction 7 d,1 month and 3 months after surgery,the postoperative recovery and the dosage of propofol,remifentanil and cisatracurium were recorded.Blood was randomly selected from each group to determine the serum content of S100βand Aβ1-42 by ELISA method at the time point of before surgery (T0 ),one hour after starting surgery (T1 ),the end of surgery (T2 )and postopera-tive 24 hours (T3 ).Results The incidence of postoperative cognitive decline in group M on 1 d (8%vs.22%),3 d (2% vs.1 6%)after surgery were significantly lower than that in group R (P <0.05). Postoperative cognitive dysfunction between the two groups 7 d and 1 month,3 months after surgery has no statistical significance.The dose of propofol [(3.3 ± 0.8)mg · kg-1 · h-1 vs.(3.7 ± 0.7 ) mg·kg-1 ·h-1 ,P < 0.05 ] and cisatracurium [(104 ± 47 )μg · kg-1 · h-1 vs.(124 ± 68 )μg·kg-1 ·h-1 ,P <0.05]in group M was less than that in group R.The time of eye-opening [(10 ±3)min vs.(1 6±6)min,P <0.01],extubation [(13±3)min vs.(22±7)min,P <0.01]and lo-cation [(1 7±4)min vs.(27 ±9)min,P <0.01 ]was shorter in group M.Compared with T0 ,the serum level of S100βprotein was significantly increased in group M at T1 ,T2 and group R at T1-T3 (P <0.05);The level of serum S100βprotein in group M was significantly lower than that in group R (P <0.05).Compared with T0 ,Aβ1-42 protein level was significantly reduced in two groups at T1 and T2 (P <0.05),but there was no significant difference between the two groups.Conclusion Total intravenous anesthesia with multiple monitoring can reduce neural injury and reduce the incidence of early postoperative cognitive decline in elderly patients with abdominal surgery,but has no significant effect on the incidence of POCD.

4.Determination of lead in workplace air by portable X-ray fluorescence spectrometry.

OBJECTIVETo investigate the permance index ofof portable X-ray fluorescence spectrometer in the determination of lead on filter membrane and to provide data for the determination of lead in workplace air.
METHODSIrradiated with X-ray, the lead would emit specific X-ray fluorescence during the process from the excited state back to the ground state. Rapid determination of lead was completed using fluorescence energy and wave length for qualitative analysis and fluorescence intensity for quantitative measurement. Under set conditions, a series of customized calibration samples were measured to create a standard curve for quantitative analysis of lead on filter membrane.
RESULTSThe regression equation obtained using a portable X-ray fluorescence spectrometer to determine the lead on filter membrane was y=0.004x-0.182 (r2= 0.9999). The linear range was 0.00 -10.40 mg/m3, the minimum detectable concentration was 0.53 µg/m3, and the minimum quantifiable concentration was 1.76µg/m3. The relative standard deviation (RSD) of within-run precision of samples with different concentrations was 0.48%-6.22%, the RSD of between-run precision was 2.51%-5.09%, and the degree of accuracy was in the calibration range of standard samples.
CONCLUSIONPortable X-ray fluorescence spectrometry is a simple, rapid, repeatable, and accurate method for the determination of lead on filter membrane.

5.Effects of Shenqi Compound Recipe on expression of COX-2 mRNA in aorta in GK rats

Objective To establish a method for the determination of geniposide in Bazheng Dispersible Tablet by HPI,C. Methods The HPLC was performed on a Hypersil ODS column(250 mm?4 mm,5?m).The mobile phase was acetoni- tril-water(11:89).The detection wavelength was at 238 nm and flow rate was 1.0 mL/min.Results Geniposide showed a good linearity in the range of 0.046~0.46?g,and r=0.999 9.The average recovery was 99.54%,and RSD was 1.42%。Conclusion This method is accurate and can be used for the quality control of Bazheng Dispersible Tablet.
7.BMP-7 counteracts TGF0-?1-induced tubular epithelial-myofibroblast transdifferentiation of cultured renal tubular epithelial cells

Objective To study the possible role of bone morphogenetic protein 7(BMP 7) in the TGF ?1 induced tubular epithelial myofibroblast transdifferentiation (TEMT) of cultured renal tubular epithelial cells. Methods The normal rat kidney tubular epithelial cell line (HK 2) was cultured for three days on plastic plates in the presence or absence of recombinant TGF ?1 and BMP 7. The alterations in the phenotype were assessed by phase contrast microscopy. Transdifferentiation of tubular cells into myofibroblasts was assessed by immunofluorescence, with monoantibodies to alpha smooth muscle actin (? SMA), vimentin and cytokeratin respectively. The expression of ? SMA of HK 2 cells was measured by flowcytometry. The expression of ? SMA mRNA of HK 2 cells was assessed with reverse transcriptase polymerase chain reaction (RT PCR). Results Treatment of HK 2 cells with BMP 7(50 and 100 ng/ml) for 24~48 hours increased cellular proliferation. The culture of HK 2 cells in the presence of TGF ?1 induced a clear fibroblast like morphology, a loss of the epithelial marker cytokeratin and de novo expression of ? SMA and vimentin. Immunofluorescence staining showed the addition of various concentrations of BMP 7 to subconfluent cells for 24 and 48 hours, and the expression of ? SMA and vimentin was decreased. Objective: To observe the clinical efficacy of acupuncture plus subtle adjustment manipulation of the cervical spine for peripheral facial paralysis.Methods: A total of 60 patients with peripheral facial paralysis who met the screening criteria were selected and divided into two groups according to the method of random number table, with 30 cases in each group. The patients in the acupuncture plus tuina group received routine treatment of acupuncture and subtle adjustment manipulation of the cervical spine, while the patients in the acupuncture group only received the same acupuncture treatment. Assessment of peripheral facial paralysis with integrated traditional Chinese and Western medicine was adopted for rehabilitation assessment. Results: The total effective rate of the acupuncture plus tuina group was 93.3%, while that of the acupuncture group was 70.0%, and there was a significant between-group difference (P<0.05). After treatment, the total scores of rehabilitation assessment in both groups were significantly higher than those before treatment (both P<0.05), and the score of the acupuncture plus tuina group was significantly higher than that of the acupuncture group (P<0.05). Conclusion: The treatment of acupuncture plus subtle adjustment manipulation of the cervical spine can improve the function of facial muscles in patients with peripheral facial paralysis, and its clinical efficacy is better than that of the acupuncture treatment alone.

Objective To investigate the effect of non-alcoholic fatty liver disease (NAFLD) on virologic response in chronic hepatitis B patients treated with nucleos(t)ide analogues.Methods Three hundred and thirty-two treatment-naive patients with hepatitis B e antigen (HBeAg)-positive chronic hepatitis B (CHB) who visited clinic or hospitalized in the First Affiliated Hospital of Wenzhou Medical College from January 2007 to December 2009 were enrolled and finally 316 patients completed the research.Among them,106 patients were diagnosed with CHB complicated with NAFLD by ultrasonography (NAFLD group),and the remaining 210 patients were diagnosed with simple CHB (CHB group).Patients in both groups were treated with combination therapy of lamivudine (LAM) plus adefovir dipivoxil (ADV) or entecavir (ETV) monotherapy for 96 weeks.Alanine aminotransferase (ALT) levels,virologic response and serological response were compared pre-and post-treatment in both groups.Enumeration data was compared using x2 test,and correlation analysis was performed using Logistic multivariate regression analysis.Results Rates of hepatitis B virus (HBV) DNA clearance in the NAFLD group at week 12,24 and 48 were 46.2%,61.3% and 74.5%,respectively,which were all significantly lower than those in CHB group (61.9%,73.8% and 83.8%,x2 =7.051,5.195,3.895,respectively; all P<0.05).However,rates of HBV DNA clearance were 85.8% in the NAFLD group and 91.9% in the CHB group at week 96.This difference did not reach the level of significance (x2 =2.838,P>0.05).Rates of HBeAg loss in the NAFLD group were 27.4% at week 48 and 37.7% at week 96,compared to 33.3% and 44.8% in the CHB group.There was no significant difference in these two groups (x2 =1.169,1.424,both P>0.05).Rate of HBeAg seroconversion in the NAFLD group was significantly lower than that in the CHB group at week 96 (17.9% vs29.5%,x2=4.972,P<0.05).Rates of ALT normalization were also found to be significantly lower in the NAFLD group than those in the CHB group at week 48 (69.8% vs 80.5%,P<0.05) and at week 96 (82.1% vs 91.0%,P<0.05).Multivariate regression analysis demonstrated that serum levels of triacylglycerol (OR=3.725,P=0.000),uric acid (OR=1.005,P=0.040) and fasting blood-glucose (OR=3.067,P=0.000) were independent risk factors associated with NAFLD.NAFLD was found to be a risk factor of virologic response at each time point throughout the treatment.(week 12,OR=2.749,P=0.001; week 24,OR=1.981,P=0.030; week48,OR=2.348,P=0.021; week 96,OR=8.053,P=0.001).Conclusions During antiviral therapy,NAFLD may affect biochemical and serological responses in CHB patients treated with nucleos(t) ide analogues,and may affect early virologic response in these patients.
10.Diffusion Tensor Imaging Study on Cervical Spinal Cord Injury without Radiographic Abnormality

Objective To apply MR diffusion tensor imaging (DTI) to quantitatively analyze cervical spinal cord injury without radiographic abnormality (CSCIWORA). Methods 15 patients with CSCIWORA and 20 healthy controls were scanned with MRI of conventional scans and DTI. The fractional anisotropy (FA) and apparent diffusion coefficient (ADC) were measured. Results FA and ADC of the patients were (0.475±0.109) and (1.438±0.252)×10-3 mm2/s, respectively. Whereas, they were (0.604±0.096) and (1.371±0.280)×10-3 mm2/s in the controls. Compared with the controls, the FA was less (P<0.05) in the patients, but the ADC was not significantly different (P=0.267). The fiber tracking (FT) showed the abnormality of white matter fiber tracts of cervical spinal cord in the patients. Conclusion DTI can detect the CSCIWORA, and FT can directly display the injuries of white matter fiber tracts of cervical spinal cord, which provide more information to evaluate the clinical severity of CSCIWORA.
