2.Bone substitutes augmentation combined with internal fixation versus internal fixation alone in treating proximal femoral fractures in the elderly: a meta-analysis.

OBJECTIVETo systematically review the effectiveness of bone substitute augmentation combined with internal fixation versus internal fixation alone in treating proximal femoral fractures in the elderly.
METHODSSubject term and keywords were searched from Pubmed, Cochrane databases and CNKI from database foundation to August 2015. Randomized controlled studies and qusi-randomized controlled studies on bone substitutes augmentation combined with internal fixation versus internal fixation alone for the treatment of proximal femoral fractures in the elderly were chosen. Postoperative re-displacement, re-operation rate, complications (infection and bone ununion), functional outcome, quality of life scores and muscle strength were seen as outcome indicators. Enumeration data were statistical analyzed by risk difference and 95% confidence interval. Measurement data were analyzed by standardized mean difference and 95% confidence interval. If the same measurement data were evaluated by different standards in different studies, standardized mean differences and 95% confidence interval were used. The methods of statistical analysis were used by Cochrane databases.
RESULTSEleven RCTs (677 patients) were included. Meta-analysis results indicated that bone substitutes augmentation combined with internal fixation occurred fewer re-displacement [SMD = -0.75, 95% CI (-1.03, -0.47)] and obtained better function [SMD = 0.40, 95% CI (0.20, 0.59)]. While there were no significant differences in reoperation rate [RD = 0.02, 95% CI (-0.05, -0.09)], pain at 1 week after operation [MD = -1.79, 95% CI (-13.55, -9.96)], pain ranged from 6 to 8 weeks [MD = -7.24, 95% CI (-20.07, -5.59)], postoperative pain at 12 weeks [MD = -0.32, 95% CI (-4.9, -3.55)], muscle strength [MD = 1.25, 95% CI (-6.98, -9.48)], bone ununion [RD = 0.02, 95% CI (-0.01, -0.05)] and postoperative complications [MD = 0.01, 95% CI (-0.03, -0.04)].
CONCLUSIONCompared with single internal fixation, bone substitutes augmentation combined with internal fixation for the treatment of proximal femoral fractures in the elderly less occur re-displacement and could obtain better functional recovery.

3.Design of a noninvasive ventilator's turbine.

4.Effect of ethyl acetate extract of sappan wood on expression of myocardial GrB mRNA in rat model of allogeneic ectopic cardiac transplantation.

OBJECTIVETo explore the action mechanism of the immune active components of sappon wood (SWE) for antagonizing reject reaction by observing the influence of its ethyl acetate extract on mRNA expression of myocardial GrB in rat model of allogenic ectopic cardiac transplantation.
METHODSAnimal model of abdominal cardiac ectopic transplantation was established taking Wistar rat as the donor and SD rat as the receptor. The successfully modeled rats were randomly divided into the model group, the SWE group and the CsA group. GrB mRNA expression was detected by RT-PCR method and myocardial pathomorphologic picture was observed in routine.
RESULTSThe pathologic changes in the SWE group (23 scores) and the CsA group (14 scores) were milder than in the model group (31 scores), the former two could markedly alleviate the myocardial pathologic injury (P<0.05, P<0.01). The GrB mRNA expression in the model group was 1.3000 +/- 0.1207, the SWE group 0. 7070 +/- 0.1215, and the CsA group 0.6700 +/- 0.0997, respectively; compared with the model group, the latter two could obviously down-regulate the expression of GrB mRNA (P<0.01) and no significant difference was found between the latter two groups (P>0.05).
CONCLUSIONSWE could alleviate the pathologic change, down-regulate the mRNA expression of myocardial GrB in allogenic ectopic transplanted myocardium of rats, it is possibly one of the factors for its antagonizing effect against reject reaction.

5.Chemical constituents from the tuber of Cremastra appendiculata.

To study the chemical constituents of "Shan-Ci-Gu" (the tuber of Cremastra appendiculata (D. Don) Makino), the compounds were isolated with silica gel and reverse phase silica gel as well as Sephadex column chromatographic method. Their structures were elucidated on the basis of modern spectra technology. Seven compounds were isolated and identified as 5-methoxybibenzyl-3, 3'-di-O-beta-D-glucopyranoside (1), militarine (2), loroglossin (3), protocatechuic acid (4), succinic acid (5), gastrodin (6), and daucosterol (7). Compound 1 is a new compound. Compounds 2 -6 were isolated from this plant for the first time.

OBJECTIVETo observe the effect of Sappan wood (SW) on the expression of perforin mRNA in myocardium of rats after allogeneic cardiac transplantation.
METHODSThe animal model of allogeneic (abdominal) cardiac transplantation was established by taking Wistar rat as provider and SD rat as receptor, perforin mRNA expression in the model's myocardium was detected by RT-PCR.
RESULTSSW could obviously reduce the perforin mRNA expression, it also could alleviate the pathological morphology and ultrastructural damage of myocardial cells.
CONCLUSIONSW has obvious effect in antagonizing immune rejection after transplantation, the mechanism of its immunosuppression could be through lowering the perforin mRNA expression.

8.Relationship between macular retinal thickness and visual acuity in high myopia

Background It is well known that the diminution of visual acuity appears before notable complications in some high myopic eyes.However,whether the impaired vision is associated with the change of retinal thickness at macula area is still under investigation.Objective This study was to investigate the relationship of macular retinal thickness with the change of visual acuity in high myopic eyes.Methods A consecutive caseobservational study was performed.Two hundred and forty-five eyes of 132 patients with the diopter of-6.00~-20.00 D were enrolled in this study during the January 2011 to January 2012 in Beijing Tongren Eye Center.All of the patients received the measurement of retinal thickness with Fourier Domain Optical Coherence Tomography (FDOCT),and the scan mode was MM6.The eyes were divided into the corrected vision ≥0.9 group and the corrected vision ≤0.8 group.In addition,the eyes were assigned to the non-posterior staphyloma group,posterior staphyloma Ⅰ group (macular symmetry) and posterior staphyloma Ⅱ group (macular gradient).The retinal thicknesses in different quadrants at the macular zone were measured and calculated by OCT software.Results The demography was matched in different groups.Corrected visual acuity was significantly increased in the corrected vision ≥ 0.9 group than that in the corrected vision ≤0.8 group (1.02±0.16 vs.0.62±0.08) (t=3.233,P=0.001).Retinal thickness value at fovea was (256.28±13.19) μm in the corrected vision ≥0.9 group,and that in the corrected vision ≤0.8 group was (231.17 ± 10.96) μm,with a significant difference between the two groups (t =2.134,P =0.031).The corrected visual acuity was 1.00±0.27,0.78±0.21 and 0.90±0.13 in the non-posterior staphyloma group,posterior staphyloma Ⅰ group and posterior staphyloma Ⅱ group,respectively,showing significant difference among the three groups (F=15.760,P=0.015),and the corrected visual acuity of the non-posterior staphyloma group and posterior staphyloma Ⅱ group were significantly higher than that of posterior staphyloma Ⅰ group (q =16.131,P =0.006 ; q =-10.831,P=0.008).A significant difference also was seen in the mean retinal thickness among the three groups (F=2.316,P =0.025).The mean retinal thickness in the posterior staphyloma Ⅰ group was (234.21 ± 15.69) μm,which was significantly smaller than (252.25± 15.31) μm of the posterior staphyloma Ⅱ group (q =12.977,P =0.023).There were no significant difference in the retinal thickness at para-fovea area among the three groups (F=0.318,P =0.078).However,significant difference was found at peri-fovea area in different groups (F=1.925,P =0.013).The mean retinal thicknesses at peri-fovea area was (273.26 ± 16.37) μm in the posterior staphyloma Ⅱ group and was significantly smaller than (289.11 ± 19.30) μm of the posterior staphyloma Ⅰ group and (290.33 ± 17.12) μm of the non-posterior staphyloma group (q =-8.305,P =0.023 ; q =-7.011,P =0.012).Conclusions The retinal thickness at fovea is associated with the corrected visual acuity in high myopic eyes.The thinning of retinal thickness at the vertex of posterior staphyloma is one of causes of visual function impairment.
9.Relationship between collagen Ⅰ,MMP-2 and TIMP-2 gene expression and atrial fibrosis and fibrillation during heart failure in dogs.

Objective To study the relationship between Couagen Ⅰ,MMP-2,TIMP-2 gene expression and atrial fibrosis during heart failure(HF)in dog.Methods Fourteen dogs were used and randomized into HF induced by ventricular tachypacing and control group.Burst atrial pacing was used to induce atrial fibrillation(AF).And the mRNA and protein level of collagen Ⅰ,MMP-2 and TIMP-2 were detected by RT-PCR and immunohistochemical technique.Tissue samples were stained with Mallory trichrome.Results Left ventricular ejection fraction (LVEF) decreased from (67.4? 6.0)% to (29.2?7.8)%,the inducible rate of AF(7/7 vs 2/7) and sustained AF(5/7 vs 0/7) increased and duration of AF stabeatrial fibrillation(SAF) [(462.12?181.43)s vs(0.57?0.57) s] prolonged significantly in HF group.Atrial fibrous tissue content and atrial size of HF group were significantly greater than the controls dogs(268.8% in lefe atria and 190.3% in right atria).The mRNA and protein level of collagen Ⅰ(56.2% and 132.2% in lefe atria,37.4% and 78.0% in right atria)and MMP-2 (100.0% and 115.7% in lefe atria,65.7% and 96.8% in right atria) increased evidently in both lefe atria and right atria,TIMP-2 mRNA decreased 46.3% in lefe atria and had no change in right atria and that its protein had no change in both atrium,whereas the ratio of MMP-2/ TIMP-2 of mRNA and protein increased markedly in both lefe atria (285.3% and 148.8%)and right atria (106.1% and 134.7%)of HF group.SAF had a positive correlation with fibrosis and the gene level of collagen Ⅰ in lefe atria,the ratio of MMP-2/TIMP-2 had a positive correlation with fibrosis and collagen Ⅰ gene level in lefe atria during HF.Conclusions The changes of collagen Ⅰ,MMP-2 and TIMP-2 gene expression appear to be a molecular mechanism of AF, and the molecular remodeling of collagen Ⅰ induced by regulation unbalance of MMP-2/TIMP-2 appears to be an important mechanism of atrial fibrosis during HF.
10.Effects of yangxin decoction on metabolomics of patients with unstable angina.

OBJECTIVETo primarily seek for unstable angina (UA) correlated metabolic markers and to observe the effects of Yangxin Decoction on the plasma metabolism of UA patients.
METHODSTen UA patients from clinics of No. 2 Circulation Department, First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from November 2009 to December 2010 were recruited as the treatment group. Another 10 healthy volunteers were recruited as the control group (Group D). The changes of plasma metabolic components were observed in UA patients after they took Yangxin Decoction for 28 days, using liquid chromatography mass spectrometry (LC-MS). Analysis was conducted using principle component analysis (PCA) and partial least squares (PLS) to seek for UA correlated metabolic markers.
RESULTSResults of PCA among Group D (the healthy group), Group M (UA group before medication), and Group G (UA group after medication): all samples of the three groups were centrally distributed in the four score plot of the oval regions (95% confidence interval). In Group M, the relative dispersion of the sample 8, 9, and 10 was near to Group D. The samples of the three groups could be separated. The samples of Group M (before medication) and Group G (after medication) could be basically separated. Comparison of metabolic end products between Group D and Group M: In the metabolites of UA group, ceramide, glycocholic acid, allocholic acid, lithocholic acid, leukotriene B4 obviously increased.
CONCLUSIONSCeramide, glycocholic acid, allocholic acid, lithocholic acid, leukotriene B4 might be metabolic markers of UA. Yangxin Decoction might have effects on some plasma metabolites or metabolic pathways of UA patients.
