2.Necessary and Feasibility of Fistulous Tissue-Saving Anorectoplasty for Low Anorectal Malformation with Fistula in Children

Objective To investigate the necessary and feasibility of fistulous tissue-saving anorectoplasty for low anorectal malformation with fistula in children.Methods The clinical data of 67 cases of low anorectal malformation with fistula were analyzed retrospectively, there were 59 girls with rectovestibular fistula, 8 boys with rectoperineal fistula. The patients′ average age were 10 months, ran -ging from 3 months to 16 years. The clinical manifestations of all patients were intractable constipation, or out of position of anus. Cut-back procedure had been performed on 4 patients in infancy. The anterior sagittal anorectoplasty was performed in all cases.Results In all cases returned for follow-up at 3 and 6 months after operation, the longest was 8 years. The postoperative course of 65 patiens was uneventful without wound infection, and had a normal-looking perineum, other 2 cases with anal rupture in operation were with sagittal wound infection resulted from perforation in repairing anus, and formed finally rectoperineal fistula with a normal-looking anus. Sixty-four cases (95.5%) had good continence by 5-6 score, another 3 cases (1 boy, 2 girls) with intractable constipation defecated with help of Glycerinum.Conclusions The fistulous tissue-saving anterior sagittal anorectoplasty is a reliable and feasible procedure for low anorectal malformation with fistula. This operation provides good results.
4.Modification,Expression and Purification of Human Endotoxin Binding Peptide Gene

Objective: To modify EBP(endotoxin binding peptide), clone and express the mutate of EBP gene and gain purified mEBP.Method: mEBPgene was cloned by PCR site-directed mutagenesis. PinpointXa-3/mEBP expression vector was designed to express human mEBP as a fusion protein in BL21 (DE3) pLysS. Digested engineering bacteria by lysozyme and collected inclusion bodies.Fusion protein was purified by Pinpoint TM Xa purification system and cleaved by factorXa,mEBP was purified by RP-HPLC. Results: Mutations at residues 5 and 18(Gln→Lys) was obtained by PCR site-directed mutagenesis, expressed and purified mEBP successfully.Conclusions: Obtaining of purified mEBP lay a foundation for its biological activity research.
5.Delayed gastrointestinal transit time and changes of ileum myenteric plexus in diabetic rats

Objective To investigate the morphologic abnormalities of myenteric plexus in diabetic rats and to explore the mechanism of its effect on gastrointestinal motility.Methods Thirty rats were randomly divided into diabetic group and control group.Gastrointestinal transit time was measured and histologic changes of cholinergic and nitriergic nerves in ileum myenteric plexus were observed with enzy- matic histochemistry.Results Four months after the establishment of the diabetic rats model,gastroin- testinal transit time was found delayed comparing with control group,the density of cholinergic neurons in the ileum myenteric plexus was decreased (P<0.01) and the densities of nitriergic ganglions and neurons were significantly increased comparing with control group (P<0.05 and P<0.01).Conclu- sion Decrease of cholinergic nerves and increase of nitriergic nerves in the myenteric plexus of intestine is one of the mechanisms of delay gastrointestinal transit time in diabetic rats.
6.Effect of acute exhaustive exercise on gastrointestinal transit rate and nitrergic nerves in myenteric plexus in rats ileum.

AIMTo investigate the effect of acute exhaustive exercise on gastrointestinal motility and its enteric nervous mechanisms.
METHODS24 rats were randomly divided into control group (C) and acute exhaustive exercise group (AEE). The rate of gastrointestinal transit was measured and histologic changes of nitriergic nerves in ileum myenteric plexus were observed with enzymatic histochemical and image analytic technique.
RESULTSIn the rats of AEE group, the rate of gastrointestinal transit was delayed comparing with C group (P < 0.05), the numbers of nitrergic neurons and expression levels of nitric oxide synthase (NOS) in the ileum myenteric plexus significantly increased comparing with C group (P < 0.01).
CONCLUSIONIt is possible that increase of nitrergic neurons and expression levels of NOS in the myenteric plexus of small intestine are one of the mechanisms of delay of gastrointestinal transit rate in acute exhaustive exercise rats.

7.Treatment of choroidal neovascularization with Chinese medicine huangban bianxing recipe.

OBJECTIVETo investigate the efficacy of Chinese medicine Huangban Bianxing Recipe (HBR) for treatment of choroidal neovascularization (CNV).
METHODSEighty patients (97 eyes) suffered from CNV were assigned to two groups, the treated group treated by HBR orally taken one dose every day and the control group treated with ginkgo leaf tablet 1 tablet (19.2 mg) thrice a day, all for 3 months. The best corrected visual acuity (BCVA), fundus hemorrhagic area were measured; the neo-genetic vascular leakage area determined by fluorescent fundus angiography (FFA); and the out-layer high reflective band thickness (OHRBT) estimated by optical coherent tomography (OCT) were recorded before and after treatment.
RESULTSAfter treatment, in the treated group, BCVA was improved, with an effective rate of 67.12%, while in the control group, it was 33.33%, the former was better than the latter significantly (P < 0.05). The effective rate in reducing fundus hemorrhagic area in the two groups (85.11% vs 47.62%) and that in the CNV closure showed by FFA (67.50% vs 37.50%) were also superior in the treated group (P < 0.05). Besides, OCT showed that the OHRBT reduced significantly after treatment in the treated group (P < 0.01).
CONCLUSIONHBR can improve the vision, reduce the CNV leakage in the macular region, and advance the absorption of fundus hemorrhage in CNV patients.

8.Advanced glycation end products inhibit testosterone production in rat Leydig cells.

OBJECTIVETo study the expression of the receptor for advanced glycation end products (RAGE) and the inhibitory effect of advanced glycation end products (AGEs) on testosterone production in rat Leydig cells.
METHODSRat Leydig cells were primarily cultured and the expression of RAGE in the Leydig cells was detected by RT-PCR and immunofluorescence staining. The Leydig cells were treated with AGEs at the concentrations of 25, 50, 100 and 200 microg/ml, respectively, and the testosterone content was determined by ELISA.
RESULTSRT-PCR and immunofluorescence staining exhibited the expression of RAGE in the rat Leydig cells. AGEs remarkably suppressed hCG-induced testosterone production in the Leydig cells in a concentration-dependent manner in the 50, 100 and 200 microg/ml groups as compared with the control (P < 0.01).
CONCLUSIONRAGE exists in rat Leydig cells and AGEs can significantly inhibit the secretion of testosterone in primarily cultured rat Leydig cells.
