1.Effect and regulation of metformin on endometrial carcinoma subline of progestin-resistance

Objective To investigate the effect and mechanism of metformin on endometrial carcinoma subline of progestin-resistance and find whether metformin could regulate progestin-resistance in endometrial carcinoma. Methods Ishikawa endometrial carcinoma cells were cultured for a long period in the presence of the synthetic medroxyprogesterone 17-acetate (MPA) to generate a subline refractory to the growth-suppressive effects of MPA,named Ishikawa/MPA cells. The effect of MPA (1, 5, 10, 20, 40 and 60 μmol/L) or metformin (1, 10, 20, 40, 60, 70 and 80 mmol/L) on proliferation of the Ishikawa/MPA and parental Ishikawa cells was detected by cell counting kit-8 (CCK-8) method. Western blot was used to detect the effect of metformin and(or)MPA on the expression of PR-B in the Ishikawa/MPA and Ishikawa cells. Results The Ishikawa/MPA showed that growth stimulation rather than inhibition in the Ishikawa cells after low MPA concentration treatment. The doubling time of Ishikawa/MPA cell lines were (43±4) hours and that of Ishikawa cell line were (47 ± 3) hours. No changes in doubling time were observed (t=0.349,P=0.572). Low concentration (1 and 5μmol/L) of MPA could promote the growth of Ishikawa/MPA cells (by 3% and 13%). High concentration (10, 20, 40 and 60 μmol/L) of MPA could inhibited the growth of Ishikawa/MPA cells (by 4%, 3%, 9%and 40%) and the growth of Ishikawa cells (by 41%, 55%, 65%and 66%). At the same concentration, the difference of inhibition rates between the two cell lines were statistically significant (P<0.01). Metformin (1, 10, 20, 40, 60, 70 and 80 mmol/L) could inhibite the growth of Ishikawa/MPA (by-10%, 20%, 56%, 89%, 97%, 98%and 99%) greater than those in parental Ishikawa cells (by-6%, 19%, 37%, 54%, 70%, 72%and 83%), and the inhibitory effect was dose-dependent manner. Western blot assay showed that for Ishikawa cells, the protein expression levels of PR-B in metformin group and MPA+metformin group were respectively (53.5±4.0)%and (37.7±5.2)%, which were higher than that in the control group [(23.4 ± 3.0)%], and there were significant the differences (P<0.01). For Ishikawa/MPA cells, the protein expression levels of PR-B in metformin group and MPA+metformin group were respectively (38.6 ± 1.7)%,(36.3 ± 2.5)%,which were higher than those in the control group [(6.4 ± 1.6)%], and there were also significant differences (P<0.01). Conclusion Metformin may regulate the progestin-resistance in endometrial carcinoma by increasing the expression of PR-B.
2.Analysis of the Changes of Burden Factor Loading for Chronic Disease Inpatients before and after the Im-plementation of Zero Price Policy by Establishing Structure Equation Model

OBJECTIVE:To construct the structure equation model for the burden of chronic disease inpatients in primary hos-pital before and after the implementation of zero price policy,and analyze the changes of burden factor loading. METHODS:6 pri-mary hospitals were randomly selected in Danyang,Jiangsu province. The data of hospitalization expenses for chronic diseases(hy-pertension,diabetes,bronchitis)were collected before and after the implementation of zero price policy. Using drug cost,nursing fees,inspection fees and treatment fees as independent variables,hospitalization burden as latent variable,SPSS and AMOS 24.0 software were adopted to establish the model. RESULTS:The burden factor loading of inpatients changed greatly before and after the implementation of zero price policy. χ2=24.586,χ2/df=1.446, RMSEA=0.019,GFI=0.995,AGFI=0.989,CFI=0.988,NFI=0.963 manifested good model fitting. Factor loading of drug cost increased greatly,indicating the burden of inpatients was reduced after the implementation of zero price policy. Factor loading of inspection fees and treatment fees increased significantly,the charac-terization effects of them to the burden of inpatients were enhanced. Factor loading of nursing fees was the lowest,and it had the weakest effects on the burden of inpatients,of which the government should enhance the regulation. CONCLUSIONS:Established model has good reliability and validity. It can reflect the change of burden factor loading of inpatients before and after the implemen-tation of zero price policy.
3.Clinical effect of creatine phosphate on infantile with rotaviral enteritis and myocardial damage

Objective To explore the clinical effect of creatine phosphate on infantile with rotaviral enteritis and myocardial damage.Methods One hundred and twenty children with rotaviral enteritis and myocardial damage were randomly divided into the control group (n =60) and observation group (n =60).Two groups were given antiviral,rehydration,nutritional support,redressing acid-base imbalance and electrolyte disorders and other conventional treatment.The patients of the control group were given 160 mg/(kg · d) 1,6Fructose Diphosphate(1 times/d) on the basis of routine treatment,while those in the observation group were given 1.0 g creatine phosphate + 150 ml 5% glucose injection(1 times/d) on the basis of routine treatment,both courses of two groups were 7 days.The levels of myocardial enzymes were measured and the clinical effect and adverse reactions were recorded.Results The total effective rate in observation group was 95.0% (57/60),obviously higher than that of control group (80.0% (48/60)) and the differences were statistically significant (x2 =6.268,P < 0.05).There were no statistical significant difference in terms of myocardial enzyme between the two groups before treatment(P >0.05).The level of lactate dehydmgenase(LDH),creatine kinase (CK),creatine kinase isoenzyme (CK-MB) and aspartate aminotransferase (AST) of observation group were (163.8 ±18.5) U/L,(152.8±26.4) U/L,(19.4±7.8) U/Land (24.4 ±6.8) U/Lafter treatment,and (193.4 ± 22.8) U/L,(168.6 ± 32.6) U/L,(27.4 ± 9.2) U/L and (35.4 ± 9.8) U/L of control group,and the differences between the two groups were significant (t =3.484,3.228,4.278 and 4.729; P < 0.05).There was no obvious adverse reaction during the treatment course.Conclusion The clinical effect of creatine phosphate on infantile with rotaviral enteritis and myocardial damage is remarkable and safe,and it is worth of clinical application.
4.Pancreatitis in patients with primary hyperparathyroidism

Objective To investigate the clinical characters of pancreatitis in patients with primary hyperparathyroidism (PHPT).Methods The clinical data of patients with PHPT undergoing parathyroidectomy from Jan.2009 to May.2014 in Peking Union Medical College Hospital were retrospectively analyzed for coexistent pancreatitis.Results 571 patients received parathyroidectomy due to PHPT during this period.Thirteen patients (2.3%)with PHPT were confirmed with coexistent pancreatitis by clinical manifestation and abdominal imaging evaluation,including acute pancreatitis in 9 patients and chronic pancreatitis in 4 patients.PHPT with pancreatitis was associated with preoperative serum intact parathyroid hormone(iPTH)higher than 500 pg/ml (P=0.025)and occurrence of hypercalcemic crisis (P=0.013).The age and sex of patients,preoperative serum calcium,gallstones,urinary calculi and bone fracture were not related with pancreatitis in this group.The average follow-up period for these patients was 34(ranging from 13 to 68)months.All patients were free from hyperparathyroidism recurrence.Only one young lady with chronic pancreatitis suffered from repeated pancreatitis after surgery.Conclusions Serum iPTH higher than 500 pg/ml and occurrence of hypercalcemic crisis were possible risk factor of pancreatitis in patients with PHPT.Parathyroidectomy can help to prevent the recurrence of pancreatitis in these patients.
6.The diagnosis and treatment of non-functional parathyroid cysts

Objective To investigate the strategy of diagnosis and treatment for non-functional parathyroid cysts.Methods The clinical data of non-functional parathyroid cysts patients undergoing resection between Jan.1991 and Jan.2013 in Peking Union Medical College Hospital were retrospectively analyzed.Results Non-functional parathyroid cysts were confirmed by intra-operation exploration and post-operation pathology in 22 patients.Ultrasound scan revealed cervical cystic lesions in all patients before operations,and concomitant lesions in thyroid were found in 10 patients.The serum calcium and phosphate level were in normal range in all patients.Local excisions were performed in 11 cases for isolated cyst.Subtotal thyroidectomy were performed in 10 cases for concomitant nodular goiter or close anatomy association cyst and thyroid.Radical resection of thyroid and cervical lymph node dissection were performed in one patient for concomitant thyroid carcinoma.No operation related complications was recorded in these series.No relapse presented in these patients after more than 7 months of follow-up.Conclusion Non-functional parathyroid cysts should be considered in the differential diagnosis of cervical mass even though it rarely occurs.Surgical resection was one promising treatment for non-functional parathyroid cysts.
7.Recent advances in natural product induced DNA damage response in cancer cells.

The DNA structures could be altered or even damaged by exogeous or endogenous factors during cell proliferation. Failure of effective and timely repair will lead to cell cycle arrest or apoptosis. By taking the advantage of the quick proliferation of cancer cells, DNA damage induction, cell cycle arrest and apoptosis promotion have become important strategies for ant-cancer chemotherapy. Previous reports showed that an array of natural compounds inhibit cancer cell proliferation by inducing DNA damage, which have therapeutic potentials for anti-cancer drug research and development.

8.Factors Affecting Sterilization of HBV on Dental Hand Piece

OBJECTIVE To investigate the factors influencing the effects of disinfection of HBV on dental hand piece. METHODS After polluted with HBV positive serum,the dental hand piece was disinfected with sterilizer at the condition of 134℃ for 5 minutes according different treatment procedures.The samples were then taken and detected HBsAg by the automatic enzyme immune analyzer using the third generation EIA agent.The efficacy of disinfection of HBV was presented by the reaction of HBsAg detection. RESULTS If the dental hand piece was sterilized directly not cleansed using clean water after pollution and dried in room temperature,there were 96.87% of the samples being HBsAg positive,and the polluted but dried hand piece,even cleansed there were 87.5% of positive result, even if prolonged the time of sterilization and the frequencies of vacuumization,the HBsAg positive rate of the sample was still up to 56.25%.But if the hand piece was irrigated and cleansed immediately after pollution and sterilized,all samples were HBsAg negative. CONCLUSIONS Whether or not cleansed immediately is the most important factor influencing the efficacy of hand piece sterilization after pollution.
9.The Substrate Specificity of Cyclic Imide Hydrolase Mutants

The effect of C-terminal region residues on the substrate specificity of a novel cyclic imide hydrolase (CIH), a recombinant cyclic imide hydrolase (CIH293), and its mutants deleted or substituted at C-terminus (CIH291, CIH290, KK292-293EE) was reported. The substrate specificity and kinetic parameters of the mutants were analyzed by both the spectrophotometric assay and high-performance liquid chromatography. Results show that the substrate specificity of mutants was not obviously changed, but slightly low for the affinity between the substrate and enzyme, compared with the wild-type enzyme, CIH293. In conclusion, the last three residues of CIH293 play an important role for the enzyme activity.
