The purpose of this study was to determine whether body composition alterations induced by aerobic exercise training show seasonal varition. Ninety-five middle-aged Japanese obese women volunteered as subjects. All subjects participated in our “exercise+diet” program for 3 months. The subjects were categorized into two groups (Summer group : S, and Winter group : W) based on the season when they participated in the program. Body composition, cardiorespiratory variables and dietary habituation were evaluated before and after the programs. More desirable changes in body composition were observed in Group W than in Group S. Group W showed a significant decrease in body mass from 62.2±8.7 kg to 60.3±8.6 kg, and in percentage body fat from 34.0±7.0% to 30.5±6.0%, while fat-free mass increased significantly from 41.0±3.9 kg 41.9±3.6 kg. However, alterations in cardiorespiratory variables were not associated with body composition alterations. It is concluded that season may have an effect on body composition alterations, but has no effect on cardiorespiratory variables, and that body composition may be improved by an exercise+diet program more effectively in winter than in summer.
2.Role of the Duodenum on Sphincter of Oddi Motility in Conscious Dogs

Objective The aim of the present study was to determine the role of the duodenum in controlling sphincter of Oddi(SO) motility using conscious dogs after total duodenectomy. Methods In a control group (n=6), a cannula was implanted into the duodenum opposite to the papilla to allow sphincter of Oddi manometry. In a duodenectomy group (n=6), the papillae were preserved at total duodenectomy and sutured to the jejunum anastomosed to the stomach (neoduodenum). The cannula was implanted opposite to the implanted papillae. Interdigestive and postprandial sphincter of Oddi and duodenal or neoduodenal motility were recorded by manometric and myoelectric methods. Results Duodenectomy disrupted SO cyclic motility. Conclusions During the interdigestive period, the duodenum has a distinct role in regulating sphincter of Oddi cyclic motility.
3.DIFFERENTIAL DIGITAL PHOTOPLETHYSMOGRAPHIC ANALYSIS OF MICROCIRCULATORY RESPONSES INDUCED BY ARM POSITION CHANGE AND IMAGE LOADING IN KENDO MASTERS

The purpose of this study was to investigate the differences and effects of positional loading (arm position change) and image loading in Budo (martial arts) competitors, mainly kendo (Japanese fencing) masters. The microcirculatory responses were measured by differential digital photoplethys-mography (ΔDPG) using the characteristics of Budo mental attitude, experience and skill as para-meters.
The subjects were 7 kendo masters of the fifth grade or higher with mean experience of 29 years, 7 kendo athletes, 7 kyudo (Japanese archery) athletes, 5 karate athletes in university sports clubs and 9 middle-aged controls. Positional loading induced myogenic responses and image loading for examining sympathetic effects via the central nervous system on peripheral circulation were mea-sured in the subjects at rest in a sitting position. The following results were obtained
The increase in the . ΔPG-P wave height in response to positional loading was markedly larger in the kendo masters. In particular, the increase when raising the right arm was significantly higher in the kendo masters than in the kyudo and karate athletes (P<0.05) . The decrease in the ΔDPG-P wave height when lowering the right arm was smallest in the kendo masters. There were differences in the peripheral vascular responses between kinds of Budo and levels of skill, which were induced by differences at the time of arm raising.
The DPG-P wave height was significantly lower during stress imaging than before loading in all subjects. The effect of stress imaging was smallest in the kendo masters, and markedly large in the kyudo athletes (P<0.05) . During relaxation imaging, an increase in the ΔDPG-P wave height was observed in the kendo masters alone, and the occurrence of vascular dilation was confirmed.
These results suggest that arteriolar responses of kendo masters are flexible and stable despite their long training period ; and the control and regulation of muscle vascular tone is well-maintained via the central nerve-sympathetic and myogenic coupling.
4.THE EFFECT OF DIRECT KICKING FORCE ON ANTERIOR TIBIAL TRANSLATION FROM SOCCER BALL IN HEALTHY KNEES

Anterior cruciate ligament (ACL) is the first restraint against anterior tibial translation (ATT) in the knee joint. Previous studies suggested that knee joint laxity might be increased by the repeated stress imposed on the knee joint. The purpose of this study was to demonstrate the effect of the repeated soccer ball kicking actions on the ATT in healthy knees. Forty female collegiate students volunteered for this study. KT-2000 knee arthrometer was used to measure ATT before and after the task. The findings demonstrated that the repeated kicking actions had no significant effect on the ATT in healthy knees. The maximum knee extension angle on the striking side was greater than the supporting side (p<0.05). Significant correlations between mean anterior translation and maximum knee extension angle were observed on both sides (p<0.05). The kicking force on a soccer ball resulted in no increase in the knee laxity.
5.Quantative determination of free silica in pesticides.

Silicate compounds are added as extenders to dusts, flow dusts (FDs) and wettable powders, meaning that it can be predicted to contain free silica, which in turn affects the incidence of pneumoconiosis. The percentage of free silica content was measured by X-ray diffraction in 18 pesticides consisting of 3 types of dusts, 4 types of FDs and 11 types of wettable powders. As a results, free silica was identified in 14 of pesticides as follows: 22-34% in dusts, 3-11% in FDs and less than 1% to 20% in wettable powders. The percentages contained in the FDs were lower than in the dusts, probably because crystal structures became amorphous during the processing of FDs into super fine gradules with a mean particule size of 2μm.
The levels of dust exposure during spraying were measured with respect to Sumithion dust, Morestan FD and TPN FD by classifying them into respirable dust of less than 7.07μm in aerodynamic diameter (which is liable to be deposited in the lung) and total dust. When the concentration of a dust to which sprayers were exposed was compared with the TLV established taking into free silica content based on recommendation of the Japan Association of Industrial Health, the dust spray levels were 75-fold in the case of respirable dust and 53-fold for total dust, those for the spraying of FDs were 7-fold for respirable dust and 3-fold for total dust.
These results suggest the necessity of using pesticides with low free silica content. It is also considered necessary for sprayers to wear dust respirators.
6.An Attempt at Objective Evaluation of the Current Situation of Concomitant Drug Use for Dementia Outpatients at Community Pharmacies

Objectives: The objectives of this study are to evaluate the current situation of concomitant drug use by community-dwelling elderly dementia patients, and to extract factors influencing the presence of potentially inappropriate medications (PIMs).
Method: The research subjects were patients 65 years of age or older for whom concomitant drugs were prescribed on the same prescription form as donepezil hydrochloride. One hundred and twenty community pharmacies randomly selected from 28 of the 47 prefectures throughout Japan. Main research topics were sex, age, the clinical department to which the prescribing physician belonged, daily dosage of donepezil, and concomitant drugs. Concomitant drugs were evaluated using the Beers Criteria: the Japanese Version. Furthermore, to search factors influencing the presence of PIMs, logistic regression analysis was used.
Results: Data for 335 patients were extracted. The average number of concomitant drug cases per patient were 4.2 (SD 2.6), and 109 patients (32.5%) were prescribed PIMs. A logistic regression analysis confirmed that significant factors influencing the presence of PIMs were “the clinical department to which the prescribing physician belonged” (OR 4.80, 95%CI 1.01-22.72), “concomitant drug cases” (OR 1.26, 95%CI 1.13-1.41) and “the presence of concomitant use of central nervous system agents” (OR 5.82, 95%CI 3.35-10.11).
Conclusion: This study revealed that more than a few cases of community-dwelling elderly patients of dementia were using concomitant drugs with potential risks. It suggested that influencing factors were “the specialty of the prescribing physician” and “the presence of concomitant use of central nervous system agents.”
7.A Preliminary Study about the Relationship between Workload and the Outcomes of Community Pharmacists’ Home Visiting Service

Aim:To examine any relations of workload and outcomes of pharmacists’ home visiting service for medication management and guidance. Survey Target:Managing pharmacists and home-visiting pharmacists from community pharmacies of the Osaka Pharmaceutical Association’s Yao and Toyonaka branches as of March, 2012. Methods:Survey forms regarding drug management and guidance at patients’ homes were mailed, requesting mail or online response. Workload indexes were visit frequency and work time on site. The relation of workload and its outcomes was examined using univariate analysis regarding three items:change in unused medication amount;detection of side effects during visits;change in prescription. SPSS ver. 20 for Windows was used for statistical analysis. Results:90 of 201 pharmacies responded (collection rate 44.8%), and 110 home patients’ data were analyzed. 5-to-15-minute actual work time scored highest (57.4%), followed by less than 5 minutes and 15 to 30 minutes, both at 21.3%. Visit frequency of twice a month scored highest (70.4%), followed by once a week (19.4%), once a month (7.4%), and once in more than one month (2.8%). 5-minute or longer patient visits had a tendency of higher percentage of patients whose unused medication decreased after visits started than less-than-5-minute visits (P=0.072). “Once a week” visits had a tendency of higher percentage of pharmacists detecting side effects than less frequent visits (P=0.061) and changing in prescription (P=0.085). Conclusion:The results above implied the relationship between workload and outcomes incurred by pharmacists’ home visits for medication safety management and guidance.
8.Consumer Opinions on the Online Sales of Over-The-Counter Drugs

Objective: To identify consumer opinions on the online sales of over-the-counter drugs (“OTCs”) and related factors.
Methods: A Web survey of consumers was conducted. The main questions were: (1) Respondent attributes; (2) Frequency of purchasing OTCs; (3) Behaviors while in poor physical condition or while ill; (4) Experience of side effects; (5) Experience of purchasing health foods and OTCs online; (6) Opinion on online sales, and (7) The reasons for their opinion. Respondents were grouped into two categories based on whether they were for or against online sales in (6). The distributions of the answers to (1) through (5) from these groups were verified using the χ2 test. Text mining was used to closely examine the answers to (7).
Results: 68.5% of the 2,609 respondents were in favor of online sales, with 31.5% against. Females and elderly respondents had higher rates of opposition. Consumers who frequently purchased OTCs, used drugs while in poor physical condition or while ill, and had experience purchasing health foods and OTCs online had higher rates of favoring it. The top reason for favoring online sales was “convenience,” while the main reasons for opposing it were “safety,” “difficulty in selection,” and “liability.”
Conclusion: Pharmacists will need to respond to consumer concerns when selling OTCs by developing the “ability to observe consumers’ condition and understand their concerns,” thereby improving their communication ability in face-to-face sales.

10.Creation of a Guidebook for the Use of Antimicrobials In-Hospital and Its Assessment

Objective: At Kitasato Institute Hospital, Kitasato University (henceforth, the hospital), the first edition of an antimicrobial use guidebook (henceforth, the guidebook) was produced in 2000. This time, the Pharmaceutical Department’s members on the hospital’s Infection Control Committee (ICC) took the lead and revised the guidebook so that it better conforms to clinical conditions and assessed the guidebook they created.
Methods: A Working Group (WG) consisting of two ICC Pharmaceutical Department members and two members of the Medical Products Information Office staff examined the items to be listed in the guidebook. These items were approved by the ICC and then assigned to physicians and pharmacists to be written up. A questionnaire survey was conducted on the physicians and pharmacists in the hospital as to their assessment of the content and the conditions of use of the guidebook created.
Results: In the revised edition, “Antimicrobials PK/PD,” “TDM,” “Empiric Therapy for Infections in Each Department,” etc. have been added, the physicians wrote “Empiric Therapy for Infections in Each Department” and everything else was written by the WG. The return rate of the questionnaire was 65.7% (N=99), and the guidebook utilization rate was 75.4%. Frequently viewed items were “List of Antimicrobials Used In-Hospital,” “How to Administer Medication during Decreased Renal Function,” “Empiric Therapy for Infections in Each Department,” etc. by the internal medicine physicians, “List of Antimicrobials Used In-Hospital” and “How to Administer Medication during Decreased Renal Function,” etc. by the surgeons, and “Classification of Antimicrobials,” “List of Antimicrobials Used In-Hospital,” “When Renal Function is Decreased,” “TDM,” and “Empiric Therapy for Infections in Each Department,” etc. by the residents. Furthermore, there were no items deemed “not necessary for inclusion” for a clear reason. As to be expected in this guidebook, 72.3% of all respondents mentioned it was “a source of information on basic antimicrobial use” and next, “a source of information for treating infections outside one’s area of specialty.”
Conclusion: The items listed in the guidebook are thought to be appropriate, and its usage conditions are also good. As they are specialists on antimicrobials, the pharmacists playing a central role while cooperating with the physicians on the ICC led to the creation of a highly useful guidebook.
