1.Analysis of common pathogenic bacteria distribution and drug resistance in children with lower respiratory tract infection

Objective To investigate the common pathogenic bacteria distribution and drug resistance in children with lower respiratory tract infection,which guide how to use antibiotics and decrease the progress of drug resistance.Methods Nine hundred and ninety-seven cases of lower respiartory tract infection were analyzed retrospectively in the Department of Pediatrics of the First Hospital of Lanzhou University from Jan.2008to Dec.2010.The sputum samples were collected for routine bacterial cultivation.The antibiotic sentivity test were used for pathgen.Results The phelm cultivition amount is 997,bred and separated 498 bacteria of 425samples,the check rate is 42.73%.There were 151 Gram Positive Bacteria,which had a rate of 30.32%,and 321 Gram Negative Bacteria,which had a rate of 64.46%,and 26 fungi whose rate is 5.62%.The main bacteria was changed during the three years.There were significant differences in detection rate of pseudomonas aeruginosa(PA) and haemophilus parainfluenzae(HPI) during the three years.PA was declining while HPI was rising.There were no significant differences in detection rate of streptococcus pneumoniae( SP),staphylococcus aureus ( SA),escherichia coli( E.coli ),klebsiella pneumoniae ( K.pn ),haemophilus influenzae ( HI ) and fungi.There were no significant differences in detection rate of PA between different age groups.With the increasing of age,the detection rate of SP,HI and HPI trended to rise,whereas the detection rate of SA,E.coli and K.pn trended todecline.The resistance phenotype of SA,E.coli and HPI in detection rate had significant difference in each year.SP and SA were most sensitive to linezolid and vancomycin.Ecoli and K.pn were most sensitive to carbapenems.HI and HPI were relatively sensitive to common antibiotics.Conclution In the past three years,the main pathogen in children with lower respiartory tract infection were Gram Negative Bacteria.Among the bacteria,SP and Ecoli was the most,followed by SA,HI,K.pn,and HPI.These bacteria had signifeicant difference in dectection rate between different age groups.SP and SA were most sensitive to vancomnycin and linezolid.E.coli and K.pn were most sensitive to carbapenems.HI and HPI were relatively sensitive to common antibiotics.So clinical doctors should use antibiotics based on the microorganism cultivation and sensitivity test m order to decrease the drugresistance strain.
2.Effect of rotating magnetic field on bone calcium metabolism in rat

BACKGROUND: Studies have proved that rotating magnetic field(RMF) can significantly increase bone mineral density(BMD) in rats, and such high BMD maintains for a long time which is independent of the actions of hormone.OBJECTIVE: To investigative the effect of RMF on bone calcium content and the correlation between bone-specific alkaline phosphatase(BAP) and deoxypyridinoline crosslinks (DPD) in rats.DESIGN: A randomized controlled study in rats.SETTING: A college of life sciences of a university.MATERIALS: The experiment was conducted in Shenzhen Key Lab of Microbiological Genetic Engineering from March 2004 to October 2004, using 90 healthy adult SD rats including 60 females weighing(259 ± 70) g and 30 males weighing (351 ± 104)g. The rats were randomly divided into 9 groups including 6 ovariotomy groups, namely female sham-operation group, normal calcium control group, low calcium control group, low calcium experimental group, normal calcium experimental group, and low calcium group with traditional Chinese medicine(TCM) treatment, besides a male control group,male low calcium experimental group, and male normal calcium experimental group, with 10 rats in each group.INTERVENTIONS: The female rats in each group, except for those in the sham-operation group, received ovariotomy. The rats in the ovariotomy control group had only ovariotomy. After metabolic exhaustion of the remnant estrogen on the 15th day, the rats in the ovariotomy experimental group were treated with RMF for 2 hours once a day for 15 days. The male rats, except for those in the control group, all bad RMF in identical manner. The rats were given food with normal calcium(containing 0.26% calcium), lower calcium (containing 0.1% calcium) and traditional Chinese medicine(powder of Fructus Psoraleae, Radix Astragali, epimedium herb, and herba cistanchis) .After RME treatment, the rats were fed for another 15 days before the femur was collected to measure the bone calcium content. Serum samples were also obtained for measuring the content of BAP and urine samples collected to measure the content of DPD.MAIN OUTCOME MEASURES: The bone calcium content of each group and serum BAP and urine DPD.RESULTS: Of the 90 rats used, 4 died during the experiment and 86 entered the result analysis. The bone calcium content was increased after RMF treatment, and the content in rats in the low calcium group with ovariotomy and TCM treatment and lower calcium control group with ovariotomy was(0.226± 0. 015) and(0. 206 ± 0. 015) g/g, respectively( t =4.63, P < 0. 05). The bone calcium content in the male normal calcium experimental group and male control group was(0. 206 ±0. 031) and(0. 199 ±0. 014) g/g respectively ( t = 4.21, P < 0.05 ) . After treatment with RMF, serum BAP content was increased and was(20. 52 ± 1.78) and(15.68 ±3.68) U/L in normal calcium experimental and control groups with ovariotomy, respectively( t=4.76, P <0.05), and(17.69±3.78) and (8.53±2.54) U/Lin the male low calcium experimental and male control groups, respectively( t= 4. 59, P < 0.05). Urine DPD after RMF treatment was (86.97 ± 37.19)and(401.57 ±79. 34) nmol/L in the low calcium experimental group with TCM treatment and ovariotomy and the low calcium control group with ovariotomy, respectively( t =7.45, P < 0.01), and(97.87 ±31.97) and (168.71 ±53.19) nmol/L in the male normal calcium experimental and male control groups respectively( t = 8.31, P < 0.01 ).CONCLUSION: RMF can effectively increase bone calcium content in a short time( 15 days), and the effect was positively correlated with increased blood BAP and decreased urine DPD.
3.Constructing a competency-based indicator system for emergency physicians in their career stages

Objective To build a competency-based indicator system for emergency physicians in their career stages.Methods We conducted expert consultations which were guided by Delphi method,and analyze the data collected from the experts using professional statistic software such as Excel and SPSS.Results The evaluating indicator system for emergency specialists at primary,intermediate and senior level was constructed base on experts' view.Conclusion The indicators illustrate the required competencies of emergency specialists in different stages comprehensively,which help to evaluate and classify the career stages of emergency physicians effectively.
4.Effects of Exogenous Methionine on Arsenical Distribution in Female Mice Exposed to Sodium Arsenite through Drinking Water

Objective To explore the effects of exogenous methionine on arsenical distribution in liver,blood and brain of mice exposed to sodium arsenite through drinking water. Methods The female Kunming mice were randomly divided into control group, the alone arsenic exposure group , the low level methionine intervention group, the moderate level methionine intervention group and the high level methionine intervention group, eight mice in each group . The mice in the experimental groups were exposed to sodium arsenite through drinking water at 50 mg/L arsenic for four consecutive weeks. And at the fourth week the 5 groups were treated intraperitoneally with saline solution (control and As group),100 mg/kg b.w,200 mg/kg b.w or 400 mg/kg b.w methionine,respectively . Twenty-four hours after cessation of methionine administration,mice were anaesthetized and rapidly dissected. The samples of blood,liver and brain were removed immediately for arsenic species analysis. Levels of inorganic arsenic (iAs),monomethylarsonic acid (MMA) and dimethylarsenic acid (DMA) were determined by HG-AAS method. Then total arsenic speciation( TAs), primary methylation ratio( PMR)and secondary methylation ratio( SMR)in each tissue were calculated. 5.Polyvinyl alcohol hydration algae salt bubble wound dressings with sealing negative pressure drainage repair diabetic foot ulcers

BACKGROUND:Biological dressings with sealing negative pressure drainage can promote the healing of acute and chronic wounds. OBJECTIVE:To observe the clinical effects of biological dressings with sealing negative pressure drainage in repair of diabetic foot ulcer. METHODS: Totaly 40 patients with diabetic foot ulcer were enroled and al underwent debridement or debridement plus skin grafting treatment. Wound treatment methods were selected according to the wiling of patients. Patients in the test group (n=20) underwent polyvinyl alcohol hydration algae salt bubble wound dressings with sealing negative pressure drainage. Patients in the control group (n=20) underwent regular wound dressing treatment. At the 7th RESULTS AND CONCLUSION: The visual analog scale scores, blood sedimentation, C-reactive protein levels in the test group were lower than those in the control group (P < 0.05). The wound healing time in the test group was shorter than that in the control group (P < 0.05), and the patient satisfaction was higher in the test group than the control group (P < 0.05). There was no significant difference in economic costs between the two groups. There was no local or systemic toxicity, alergy and other adverse reactions in the test group. These results day of repair, visual analog scale scores, blood sedimentation, C-reactive protein levels, wound healing time, patient satisfaction and economic costs at the termination of the treatment in these two groups were compared. demonstrate that polyvinyl alcohol hydration algae salt bubble wound dressings with sealing negative pressure drainage in repair of diabetic foot ulcer is safe and effective to promote wound healing and reduce local inflammatory response.
6.Distinguishing value of serum aldosterone level for patients with different types of atrial fibrillation

Objective:To explore distinguishing value of serum aldosterone level for different types of atrial fibrilla-tion (AF) .Methods:A total of 153 AF patients were selected ,including 99 patients with paroxysmal AF (paroxys-mal AF group) and 54 patients with persistent AF (persistent AF group) .Serum aldosterone concentration was measured ,and its correlation with two types of AF was analyzed .Results:Compared with patients with paroxysmal AF ,there was significant rise in serum aldosterone level [ (234.3 ± 69.6) pg/ml vs .(302.7 ± 78.3) pg/ml , P<0.01] in patients with persistent AF .Correlation analysis indicated that aldosterone level was positively correlated with atrial diameter and left ventricular ejection fraction (r=0.896 ,0.831 ,P<0.01 both) .Conclusion:Serum al-dosterone level is helpful to distinguishing paroxysmal atrial fibrillation and persistent atrial fibrillation ,and posses-ses certain guiding significance for outcome .
7.Effectiveness of Microresection Combined with Radiofrequency Ablation under Laryngoscope and Rigid Telescope for Laryngeal Precancerosis

Objective To investigate the effectiveness of microresection combined with radiofrequency ablation under laryngoscope and rigid telescope for laryngeal precancerosis. Methods Fifty-one patients with laryngeal precancerosis, which was diagnosed pathologically, were treated with microresection combined with radiofrequency ablation under laryngoscope and rigid telescope. The patients were followed up for over 6 months. AIM: To study the protective effect of hyperpolarized cardioplegic arrest on reperfused rat heart performance and to investigate the role of mitochondrial ATP-sensitive K+ channels(mitoKATP) opening in the protection of hyperpolarized cardioplegia against ischemia/reperfusion damage.METHODS: Forty Sprague-Dawley rats were randomized into five groups(n=8 in each group): control group(Con);depolarized arrest group(D);hyperpolarized arrest group(H);depolarized cardioplegia with 5-hydroxydecanoate(5-HD) group(5HD+D);hyperpolarized cardioplegia with 5-HD group(5HD+H).The rat hearts were quickly removed to Langendorff apparatus.The heart perfusion was performed for 20 min with 37 ℃ Krebs-Henseleit buffer balanced with gas mixture(O2∶CO2=95%∶5%) at 5.8 kPa perfusion pressure,then cardial arrest was induced by different cardioplegic solution.Hearts were subjected to ischemia at 37 ℃ for 40 min followed by 30 min reperfusion.(1) The hemodynamics was detected at recovery after 30 min reperfusion.(2) Before ischemia and at the end-reperfusion,tissue was harvested for mitochondrial isolation and ultrastructure was observed by transmission electron microscopy(TEM).(3) Production of reactive oxygen species(ROS) was also determined at different time points.RESULTS:(1) Compared with end-equilibration,30 min reperfusion caused significant differences in left ventricular developed pressure(LADP),left ventricular end-diastolic pressure(LVEDP),double product(DP),heart rate (HR),coronary flow(CF)(P
9.Vasodilation reduction and insulin resistance in rats induced by high sucrose, high saturated fatty acid and high unsaturated fatty acid diets

Objective To observe the effects of high sucrose,high saturated fatty acid and high unsaturated fatty acid(diets) on insulin resistance and endothelium-dependent vasodilation function.Methods Adult Wistar rats were divided into normal control(NC) group,high sucrose(HS) group and high saturated fatty acid(HSF) group,high unsaturated fatty acid(HUF) groups.Insulin sensitivity was tested by hyperinsulinemic-euglucemic clamp after 24 weeks.Acetylcholine-induced(or sodium nitroprussideinduced) relaxation of preconstricted isolated renal arteries was measured by Mulvany myograph.Results GIR was obviously lower in experimental groups than that in NC group.GIR was negatively correlated with triglyceride (TG),free fatty acid(FFA).Acetylcholine-induced relaxation was markedly decreased in all experimental groups compared with that in NC group and the maximal response was decreased 37.4% in HSF group,32.7% in HUF group,27.7% in HS group.Acetylcholine-induced relaxation was enhanced by incubation with L-Arg and decreased incubated with L-NNA,MB in all experimental groups.Vasodilation response was negatively correlated with TG,INS and well positively correlated with NO,GIR.There was significantly negative correlation between FFA andNO.Conclusions: The rats fed high sucrose,high saturated fatty acid and high unsaturated fatty acid diets developed insulin resistance with reduced endothelium-dependent vasodilation function.
10.Sex differences in the length ratios of metapodials in Macaca mulatta from the Taihang mountains

Objective Sex difference of the length ratios of metacarpals and metatarsals in Macaca mulatta from the Taihang mountains was studied in our laboratory. Methods The lengths of 27 metacarpals (10 males, 17 females) and 30 metatarsals(12 males, 18 females) were measured from the skeletons of 30 adult Macaca mulatta. Length ratios were constructed for all possible pairings of the five bones in each individual hand and foot. One-Way ANOVA adopting SPSS13.0 for windows was used to study the sex differences of length ratios of metacarpals and metatarsals. Results For Macaca mulatta, several of these lengths ratios exhibited substantial differences between the sexes. The metacarpal(Mc) length ratios showing the largest sex differences were 2Mc∶5Mc and 4Mc∶5Mc in both hands (P<0.01), and the metatarsal(Mt) length ratios showing the largest sex difference was 1Mt:3Mt in both feet (P<0.05). Conclusion The sex differences of metacarpals and metatarsals remained when specimens of similar size were compared. It showed that body size was not the basis for these sex differences. Various facts suggested that the sex difference of length ratios in primate metapodials was associated with sex hormones exposure, possibly during prenatal development.
