1.Significance and necessity of developing quality of life questionnaire for cancer patients adapting to traditional Chinese medicine

The clinical evaluation system for therapeutic effects of tumor, which focuses on survival period and tumor response rate, has been proved not very proper for evaluating the therapeutic effects of traditional Chinese medicine (TCM). Quality of life (QOL) has been introduced into the effective evaluation system of TCM and integrative medicine because of its similarity in concept and extension with TCM at theory basis, syndrome differentiation-treatment, and evaluation of therapeutic effects. But there are no QOL questionnaires which can evaluate the therapeutic effects of TCM and integrative medicine precisely. This article discussed the necessity and significance of developing QOL questionnaire special for TCM.
2.Inhibitory effect of receptor of advanced glycation endproducts,nuclear factor-?B double gene antisense RNA on productions of inflammatory factor treated by advanced glycation endproducts

AIM:To observe the effects of receptor of advanced glycation endproducts(RAGE),NF-?B double gene antisense RNA on the productions of TNF-? and IL-6 treated by advanced glycation endproducts(AGEs).METHODS:The levels of TNF-? and IL-6 in the supernatants were measured by enzyme linked immunosorbent assay(ELISA) in ECV304 cells treated by AGEs.The RAGE,NF-?B single/double gene antisense RNA were transfected into ECV304 cell.The expressions of RAGE and NF-?B were detected by flow cytometry and RT-PCR. In different clone cells,the effects of antisense RNA on the productions of TNF-?、IL-6 were determined by ELISA.RESULTS:AGEs,instead of human serum albumin(HSA),stimulated ECV304 cell to produce TNF-? and IL-6 with a time-and dose-dependent manner.The RAGE,NF-?B single/double gene antisense RNA were transfected into ECV304 cell.Induced by AGEs,the expressions of RAGE and NF-?B in double gene cotransfected cell were inhibited by((62.2)?(8.7))% and((37.2)?(7.1))%,respectively.Induced by AGEs,the amount of TNF-?、IL-6 in the medium were lower in single gene transfected cells ECV-asRAGE,ECV-asP65 than ECV-Vector(P
3.Treating Cancer-related Fatigue after Chemotherapy in Lung Cancer Patients by Yiqi Jianpi Recipe: a Clinical Study.

OBJECTIVETo evaluate the clinical efficacy of Yiqi Jianpi Recipe (YJR) in treating cancer-related fatigue (CRF) after chemotherapy in lung cancer patients. Methods Totally 124 lung cancer patients were assigned to the treatment group (63 cases) and the control group (61 cases) according to random digit table. All patients received pre-set chemotherapy regimens according their conditions. Patients in the treatment group took YJR Decoction from the 1st day of chemotherapy, one dose per day for 2 successive weeks, while those in the control group took no Chinese medical decoction. Changes of each dimensional scoring and the total scoring were observed by cancer fatigue scale (CFS) in the two groups one day before chemotherapy, on the 3rd day and the 14th day after chemotherapy. The degree of bone marrow depression and recombinant human granulocyte colony-stimulating factor (rhG-CFS) doses were also observed after chemotherapy.
RESULTSCompared with one day before chemotherapy in the same group, scorings for emotional, perceptional, and somatologic dimensions, and the total scoring significantly increased on the 3rd day in the treatment group, as well as on the 3rd and the 14th day in the control group (P <0. 01). Scorings for emotional, perceptional, and somatologic dimensions, and the total scoring significantly decreased more at the fourteenth day after chemotherapy than at the 3rd day after chemotherapy (P <0. 01). There was statistical difference in the scoring for perceptional dimension at day 3 between the treatment group and the control group (P <0.01). However, scorings for emotional, perceptional, and somatologic dimensions, and the total scoring significantly decreased more at the fourteenth day after chemotherapy in the treatment group, with statistical difference when compared with those in the control group (P <0. 01). The occurrence rate of bone marrow depression was 70. 97% (44/62) in the treatment group and 68. 85% (42/61) in the control group. There was no significant difference in the occurrence rate between the two groups (χ2=0. 540, P >0.05). There was no statistical difference in rhG-CFS doses between the two groups (χ2=0. 696, P >0. 05).
CONCLUSIONYJD could significantly relieve CRF after chemotherapy in lung cancer patients, and obviously attenuate the effect of CRF on their daily life.

4.Effects of hyperbaric oxygen on choline acetyltransferase in neurons after traumatic brain injury

Objective To observe the effect of hyperbaric oxygen on choline acetyltransferase ChAT levels in the neurons of rats with traumatic brain injury. Methods Traumatic brain injuries were modeled in rats using Feeney's method. Thirty male rats were randomly divided into 3 groups: a traumatic brain injury group (group A) , a traumatic brain injury plus hyperbaric oxygen ( HBO) group ( group B) , and a sham operation group (group C). Samples of each rat's brain were tested for ChAT using immunohistochemical staining and light microscopy. Results ChAT-positive fibers in the hippocampus almost vanished after traumatic brain injury, but positive fibers were markedly increased after HBO treatment. Conclusion HBO treatment can activate neurons in the hippocampus and inhibit regression of cholinergic neurons, which may be one of the mechanisms by which HBO therapy improves learning ability in injured rats.

7.An efficacy evaluation of the PBL teaching in neurosurgical clinical training

Objective To explore the effect and experience of problem-based (PBL) teaching in neurosurgical clinical training and improve the quality of the neurosurgical clinical training. Methods 600 clinical medical students of Grade 2011 in their probation in our department of neurosurgery were chosen as the subjects of study and were randomly divided into the experimental (PBL, 15 groups, 20 people/group, over 300 people) and control groups (CBL+ LBL, 15 groups, 20 people/group, over 300 people). In experimental group, PBL which is a problem-based, student-centered and teacher-directed pedagogy was adopted. In control group, CBL (case-based learning) + LBL (lecture-based learning) were adopted. After probation, two groups of students had the unified test and assessed their teaching methods by questionnaire survey and at the same time, the questionnaire survey about PBL' impact on the various aspects was done only to experimental group students. SPSS 15.0 software was used to do statistical analysis and data were processed by description analysis, t test, Mann-Whitney U test and correlation analysis. Results The scores of theoretical knowledge and general ability in PBL group [(40.57 ±3.24), (41.23 ±4.56) individually] were higher than these in control group [(37.21 ±4.05), (36.51±4.11) individually]. The differences demonstrated statistically meanings. Through the surveys, the students in experimental group showed the high satisfaction (97.0%, n=291) in PBL teaching. Most of the students considered that their abilities of team cooperation (96.7%, n=290), and self-directed learning (92.7%, n=278) were improved by PBL teaching. The students in experimental group illustrated their attitudes in satisfaction of teaching; interests of clinical training, participation of teaching and understanding of knowledge were superior to the control group. The differences demonstrated statisti-cally meanings. Conclusion PBL teaching got a good effect in neurosurgical clinical training. This method, which improved students' participation and motivation of study and contributed to the training of medical students' comprehensive abilities, is suitable for modern neurosurgical clinical training.
8.Effect of Guizhi Gancao decoction on cardiac function and serum VCAM-1 and sE-selectin levels in patients with coronary heart disease

Objective To analyze the effect of guizhi gancao decoction on dardiac function and serum VCAM-1 and sE-selectin levels in patients with coronary heart diseaser. Methods Fifty-eight patients with coronary heart disease were enrolled in this study from September 2014 to June 2015. According to the different treatment methods, 40 patients were treated with conventional western medicine. The observation group of 40 cases were treated with conventional western medicine and Guizhi licorice soup. The effects of cardiac function, 6-minute walking test, cardiac parameters and serum sE-Selectin and ICAM-1 levels were observed. Results Observation group of patients with cardiac ultrasound parameters LVEF (43.27 ± 5.21)%, LVEDd (59.34 ± 5.43) mm, FS (26.32 ± 1.63)%, E / A (1.23 ± 0.22) and 6 min walking distance were better than those of the control group (t=2.3628,P =0.0206;t=2.4643,P=0.0159;t=2.7831,P =0.0068;t=2.5023,P =0.0144;t=3.7571,P =0.0006). The markedly effective rate was 47.50%, which was higher than that of the control group (25.00%), and there was significant difference (χ2 = 4.3813, P= 0.0363). In the observation group, 4 patients were ineffective and the inefficiency rate was 10.00%, which was significantly lower than that of the control group (χ2 = 4.0205,P= 0.0450). The levels of TC (5.21 ± 0.82) mmol / L, TG (1.22 ± 0.32) mmol / L and LDL-C (2.44 ± 0.52) mmol / L were significantly lower in the observation group than those in the control group (t= 3.2729, P = 0.0016; t= 3.5119, P = 0.0007; t= 3.5136, P= 0.0007). While the HDL-C (1.21 ± 0.17) mmol / L index was higher than that of the control group (t = 5.4575, P = 0.0000). There was no significant difference in serum index between the two groups before treatment . After treatment, the serum sE-Selectin (75.58 ± 9.97) ng / mL and VCAM-1 (662.43 ± 65.78) ng / mL were lower than those in the control group (t = 3.4082, P = 0.0010;t = 3.3088, P = 0.0014). Conclusion Treatment of CHF patients with CRT / CRT-D pacemaker implantation in the heart after surgery can effectively improve its clinical condition, and the effect is outstanding, side effects, and CRT-D better effect.
9.The Routine Use of Drainage after Thyroid and Parathyroid Surgery:a meta-analysis

Objective A meta-analysis was performed to assess the routine use of drainage after thyroid and parathyroid surgery with respect to the postoperative hematoma、haemorrhage、wound infection、seroma and hospital stay.Methods Medline、EMBASE、Cochrane Controlled Trials Register and China Biological Medicine Disk from Jan 1966 to Apr 2007 were searched for the randomized controlled trials of the routine use of drainage after thyroid and parathyroid surgery.Eleven studies were enrolled into the analysis.The details about the trial design,characters of the subjects,results of the studies were reviewed by two independent authors and analysed by using Revman4.2 software.Results Compared the drain group with non drain group,the drain group was not associated with significantly lower incidence of hematoma(OR=1.31,95%CI:0.71~2.42,P=0.38) 、wound infection(OR=1.51,95%CI:0.53~4.28,P=0.44)、hemorrhage(OR=1.21,95%CI:0.35~4.24,P=0.77) and seroma(OR=0.50,95%CI:0.22~1.15,P=0.10).Length of hospital stay between the two groups was significantly different(OR=1.29,95%CI:0.69~1.88,P
10.The comparision between two international QOL questionnaire of lung cancer:EORTC QLQ-LC43 and FACT-L

Background and purpose:Quality of life (QOL) questionnaires are tools to assess the quality of life of patients. How to choose a proper questionnaire is the fi rst problem of QOL assessment in clinic research. However, there is few report on it. Our current research compares the data obtained from two world-wide used QOL questionnaire of lung cancer patients EORTC QLQ-LC43 (European Organisation for the Research and Treatment of Cancer Quality of Life Questionnaire-Lung Cancer43) and FACT-L (Functional Assessment of Cancer Treatment-Lung) by various statistic methods in order to show differences between the two questionnaires and to provide some suggestions for choosing questionnaires to assess patients’ QOL in clinic research. Methods:The clinic data were obtained from 125 lung cancer patients by two different international QOL questionnaires of lung cancer patients EORTC QLQ-LC43 and FACT-L, and analyzed by various statistic methods such as Pearson correlation analysis, canonical correlation analysis and multiple regression. In addition, we also compare the general modality such as the frame, item numbers and the contents of the subscales, etc. Results:Of the two QOL there are some parts similar in length, ranking type and time construction, even in items. They all contain items in measuring the physical domain, emotion domain, function domain, society domain and lung domain. Correlations between corresponding subscales of the FACT-L and the EORTC QLQ-LC43 are all signifi cant ranging from r=0.331 for the social domain to r=0.664 for the emotional domain. Canonical correlation analysis for the two sets of subscales revealed there are four signifi cant canonical variables (canonical correlation coeffi cient r=0.87 to 0.26 and overall redundancy about 41% ). The eight EORTC QLQ-LC43 subscales are well represented by the fi ve FACT-L subscales (multiple linear regression, R 2=0.531 to 0.766) and the fi ve FACT-L subscales (except relationship with doctors) are also well represented by EORTC QLQ- LC43 subscales (R 2=0.537 to 0.823). The proportion of the corresponding subscale in the total explanation is 50% to 90%. Conclusion:These two questionnaires cover some common parts, but they measure different contents of QOL. Though have the same name, some domains cann’t be compared directly. The two questionnaires cann’t be alternative and the results of them shouldn’t be compaired directly. Both questionnaires are suited for clinic study (e.g compare the effect of two different theropies). But each has its own characters and should be chosen according to reseach goals.
