1.Treatment of Recurrent Oral Ulceration (Yin Deficiency Fire Excess Type) by Qianjin Kouchuang Jiawei Granule: a Clinical Observation.

OBJECTIVETo observe the curative effect of Qianjin Kouchuang Jiawei Granule (QKJG) on recurrent oral ulceration [yin deficiency fire excess type (YDFET)].
METHODSTotally 120 patients who suffered from recurrent oral ulceration (YDFET) were randomly assigned to two groups, the experiment group and the control group, 60 in each group. Patients in the experiment group took QKJG, 20 g each time, twice per day, while those in the control group took Kouyanqing Granule (KG) , 20 g each time, twice per day. Fourteen days consisted of one therapeutic course, two for all. Scores for patients' symptoms and signs (ulcer area, exudation, hyperaemia, edema, the number of ulceration, burning sensation, and pain degrees) were assessed before treatment, at day 3 and 7 after treatment. Short-term efficacy was assessed by visual analogue scale (VAS). The total paralysis time and the total number of ulceration at month 12 after treatment were taken as judgment for long-term efficacy. Results Compared with before treatment in the same group, symptoms and signs were obviously improved at day 3 and 7 after treatment in the two groups (P < 0.05). Compared with the control group at day 3 after treatment, the improvement of edema, exudation, pain degree, and burning sensation was more obvious in the experiment group (P < 0.05, P < 0.01). The improvement of edema, pain degree, and burning sensation at day 7 after treatment was more obvious in the experiment group than in the control group (P < 0.05). As for short-term efficacy, the total effective rate was 86.67% (52/60 cases) in the experiment group and 83.33% (50/60 cases) in the control group, with no statistical difference between the two groups (P > 0.05). As for long-term efficacy, the total effective rate was 90.00% (54/60 cases) in the experiment group, significantly higher than that of the control group with statistical difference [81.67% (49/60 cases), P < 0.05]. At month 12 after treatment, the total number of ulceration was reduced and the paralysis time of ulcer attack prolonged in the experiment group, with statistical difference when compared with the control group (P < 0.05).
CONCLUSIONQKJG showed better long-term efficacy than that of KG in treating recurrent oral ulceration (YDFET).

2.Visualization and analysis of flow patterns of human carotid bifurcation by computational fluid dynamics

Objective To investigate flow patterns at carotid bifurcation in vivo by combining computational fluid dynamics(CFD)and MR angiography imaging.Methods Seven subjects underwent contrast-enhanced MR angiography of carotid artery in Siemens 3.0 T MR.Flow patterns of the carotid artery bifurcation were calculated and visualized by combining MR vascular imaging post-processing and CFD. Results The flow patterns of the carotid bifurcations in 7 subjects were varied with different phases of a cardiac cycle.The turbulent flow and back flow occurred at bifurcation and proximal of internal carotid artery (ICA)and external carotid artery(ECA),their occurrence and conformation were varied with different phase of a cardiac cycle.The turbulent flow and back flow faded out quickly when the blood flow to the distal of ICA and ECA.Conclusion CFD combined with MR angiography can be utilized to visualize the cyclical change of flow patterns of carotid bifurcation with different phases of a cardiac cycle.
3.Experience of PEI Xue-yi in treating children's nephrapiathy

PEI Xue-yi has 60 years,experience in treating pediatric diseases with traditional Chinese medicine,especially obtains good effect in children's nephrapiathy.Based on the regulation and characteristics of children's zang-fu,yin-yang,blood-qi and children's nephrapiathy,he makes different therapeutic plans in different phases.In clinical treatment,he emphasizes the lung,the spleen and the kidney and combines syndrome differentiation with disease diagnosis.Eliminaing pathogens,excessive damp-heat is used in acute phase and strengthening healthy qi,securing the spleen and kidney is used in recovery phase.Children's nephrapiathy is treated by three phases as the edema,protein urine and rescovery stage.He adopts the methods of dispersing lung qi and invigorating spleen for diuresis,clearing heat-damp,nourishing spleen and kidney,nourishing yin for protecting lower jiao.
4.Nipple-areola complex sparing mastectomy with immediate breast reconstruction using subpectoral prosthesis implantation

Objective To explore the feasibility of immediate breast reconstruction with subpectoral implantation of silicon gel prosthesis after nipple-areola complex (NAC) sparing modified radical mastectomy. Methods A total of 28 patients of 0,I,II stage breast cancer were implanted with silicon gel prosthesis immediately after they underwent skin-sparing modified radical mastectomy. NAC conservation depended on frozen section result. Results NAC of all the 28 patients were conserved. During 2-18 months (median15 months) follow-up, bilateral breasts were symmetrical and in good appearance with a 96.5% excellent rate. No local recurrence, distant metastasis or obvious complications occurred. Conclusions Immediate breast reconstruction with subpectoral implantation of silicon gel prosthesis after NAC sparing modified radical mastectomy is proven to be an effective and safe method with the advantage of good appearance and shorter recovery time in early-stage breast cancer.
5.Tumor necrosis factor-alpha enhances the effect of endothelin on renal vasoconstriction in isolated perfused rat kidney.

OBJECTIVETo explore the role of tumor necrosis factor-alpha (TNF-alpha) in the pathogenesis of hepatorenal syndrome.
METHODSBy isolated perfused kidney technique, rat kidneys were perfused at a constant flow. Changes in perfusion pressure (mmHg) were consecutively measured with multi-functional physiology recorder. After TNF-alpha or heparin treated 90 minutes, the perfusion pressure stimulated by endothelin-1 (ET-1) was detected.
RESULTSTNF-alpha and heparin didn't modify the baseline perfusion pressure. When ET-1 was added at 2 nmol/L, the perfusion pressures increased to (47+/-9) mmHg, (97+/-36) mm Hg and (11+/-6) mm Hg in control, TNF-alpha and heparin (10mg/L) treated group, respectively, which were different among the three groups (t>or=3.811, P<0.01). No pathological damages were found in kidney tissues from all the groups after being stained with hematoxylin/eosin.
CONCLUSIONTNF-alpha plays an important role in the pathogenesis of hepatorenal syndrome by promoting renal vasoconstriction.

6.Effect of extracts of astralus on learning,memory and immune function in mice

Aim The effect of extracts of astralus(EA) on learning, memory and immune function of CTX treated mice was observed.Methods The step-down test was adopted to investigate learning and memory in mice; the method of enveloping CRBC was adopted to explore the function of macrophage in abdominal cavity; 3H-TdR marking method was used to observe the prolifernation of T lymphocytes and the method of ELISA was applied to measure the concentration of IFN-? in blood. Results EA (25 mg?kg-1, 50 mg?kg-1,ig, 21 d ) could boost learning and memory , enhance the phagocytic activity of monocytes and the proliferation of T lymphocytes of CTX treated mice, and increase the concentration of IFN-? in blood of CTX treated mice. Conclusion EA not only promote learning and memory, but regulate immune function.
7.Changes of Expressions of CD_(40) and CD_(40) Ligand to Graft-Versus-Host Disease after Allogeneic Hematopoietic Stem Cell Transplantation

Objective To explore the changes of CD40 and CD40 ligand(CD40L) levels and investigate their significances in children with graft-versus-host disease(GVHD)after related-donor human leukocyte antigen(HLA) matching allogeneic hematopoietic stem cell transplantation(HSCT).Methods Nineteen patients with ?-thalassemia major and 1 patient with congenital inherent hemolytic anemia accepted umbilical cord blood transplantation(UCBT) and allogeneic peripheral blood stem cell transplantation(allo-PBSCT),respectively,and all cases were received successfully related-donor human leukocyte antigen matching allogeneic HSCT.Peripheral blood samples were obtained before and after transplantation,the time when GVHD happened,the expressions of CD40 and CD40L were measured by using immunofluresence asssy.Results Four UCBT children and 3 allo-PBSCT children had no acute GVHD.Thirteen children had acute GVHD(degreeⅠ-Ⅳ),the expressions of CD40L on CD4+ and CD8+ T cells in the patients with acute GVHD increased,especially in allo-PBSCT.Five cases of UCBT and 12 cases of allo-PBSCT patients had chronic GVHD,the expressions of CD40L+,CD25+ and CD69+ on CD4+ and CD8+ T cells in patients with chronic GVHD increased obviously.The expression of CD19+CD40+ was lower than normal within 3 months after transplantation.Conclusions The high expression of CD40L+T cells in periferal blood after HSCT was related to the activation and proliferation of T cells in the development of GVHD in HSCT.
8.Clinical experience of Professor Shao Jing-ming: a master of acupuncture-moxibustion

Professor Shao Jing-ming had practiced Chinese medicine for more than 80 years with rich clinical experience and exquisite acupuncture techniques. From his clinical experience, Professor Shao's clinical features can be summarized as the followings: attaching importance to the theory of meridians and collaterals, combining pattern identification and disease identification, using fewer acupoints for treatment, and using Ashi point and Hegu (LI 4) to treat goiter. He emphasized the priority and sequence in acupoint-selection and manipulation. In treatment of epilepsy, he proposed to treat it according to the situation and paid great attention to the special function of the extraordinary acupoints. During the onset, it should be managed by calming the mind and controlling the symptoms; during the remission period, acupuncture and drugs should be applied simultaneously to regulate qi-blood and yin-yang, so as to reduce the frequency of attacks. He believed that acupuncture manipulation be one of the key factors in achieving the efficacy. In treatment of the motive diseases, such as convulsions and cramps, acupuncture with static and longer needle-retaining time should be adopted to control the limb movement, to strengthen the stimulation and to obtain a long-term efficacy. Professor Shao Jing-ming's medical records range from internal medicine, external medicine, gynecology and pediatrics, listing various refractory diseases effectively treated by him. Professor Shao was a moral and erudite Chinese medicine master, and his clinical experience is worthy of inheritance and development.
9.The application of breast blood oxygen functional imaging combined with mammography examination in breast cancer screening

Objective To evaluate the effect of breast blood oxygen functional imaging combined with mammography on breast cancer screening in women aged over 40 years.Methods A total of 10 076 cases of women in our hospital underwent breast blood oxygen functional imaging and mammography X -ray examination.The breast imaging report and data system (BI -RADS)III level of women performed on the biopsy pathology were used as the gold standard.The effect of different inspection methods were compared and analyzed.Results Among 10 076 cases,235 cases were diagnosed as BI -RADS,which was diagnosed by blood oxygen functional imaging system and a molybdenum target X -ray diagnosis of III.Thirteen cases with early breast cancer were diagnosed by needle biopsy.And 222 cases were diagnosed benign lesions.The sensitivity of the blood oxygen functional imaging system was 69.23% (9 /13)and 85.58% (190 /222), respectively.The sensitivity of molybdenum target X -ray was 76.92% (10 /13),and the specificity was 82.88% (184 /222).The sensitivity of combined detection was 100.0% (13 /13)and the specificity was 82.88% (184 /222).The accuracy index of combined molybdenum target X -ray (0.829)was significantly higher than that of single detection (0.548 and 0.598)(U =2.117,2.501,P <0.05).Conclusion Breast blood oxygen functional imaging instrument combined with molybdenum target examination could effectively improve the accuracy of diagnosis in breast cancer screening among women over 40 years.
10.Expression of type I inositol 1,4,5-triphosphate receptor on rat glomerular and afferent arterioles in a model of liver cirrhosis.

OBJECTIVETo study the expression of type I inositol 1,4,5-triphosphate receptor in rat glomerular and afferent arterioles in a model of liver cirrhosis and study the role of cross-membrane message transduction in the pathogenesis of hepatorenal syndrome.
METHODSIn a rat model of carbontetrachloride liver cirrhosis, the expression of type I inositol 1,4,5-triphosphate receptor (IP3R) on glomerular and afferent arterioles was measured by immunohistochemical method.
RESULTSIn the experimental group, 30 rats were used to make a model of liver cirrhosis. 11 rats survived during the experiment. The expression of type I IP3R on glomerular and afferent arterioles was 4.97+/-1.34 and 4.09+/-1.14 in the liver cirrhosis group, and it was 2.43+/-1.67 and 1.83+/-1.32 in the normal control rats. The differences between these two groups are statistically significant (t = 2.28, P = 0.0458).
CONCLUSIONExpression of type I IP3 receptor on rat glomerular and afferent arterioles in a model of liver cirrhosis indicated that the mechanism of cross-membrane message transduction plays a very important role in the pathogenesis of hepatorenal syndrome.
