1.Clinical application of endobronchial ultrasound-guided transbronchial needle aspiration in the diagnosis mediastinal lymphadenopathy

Objective To evaluate the diagnostic value of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in the diagnosis of mediastinal lymphadenopathy.Methods EBUS-TBNA were performed in 148 mediastinal lymphadenopathy previously by CT were selected,and 129 malignant tumor were confirmed by surgery.The advantages of EBUS-TBNA were evaluated,including superiority,success rate of puncture and diagnostic accurary.Results In 129 cases,mediastinal lymphadenopathy were detected clearly and were guided accurately by EBUS-TBNA.The positive results were obtained in 118 cases,and the false negative results in 11 cases.The success rate of biopsy and sensitivity of diagnosis by EBUS-TBNA in malignant tumor were 100% and 91.47% respectively.There were no complications in this group.Conclusions EBUS-TBNA is a safe and useful technique in diagnosis of mediastinal lymphadenopathy and evaluate the response to neoadjuvant chemotherapy.
2.Ultrasonic Analysis for Resected Specimens with Esophageal Carcinoma

The macroscopy and ultrasondgraphy analysis for thirty reseeted specimens of esophageal carcinoma proved by pathology was reported in this paper.The ultrasonic features of esophageal carcinoma as follows:the local wails became thicker,their layers were unclear or disap-peared,the normal walls were insteaded of irregular masses with hypoechoic patterns, the borders of the masses were definite or indefinite,and the posterior echoes of the masses had little change.In addi-tion,the depth of tumor invasion Was compared by the ultrasonography and pathology.The ultrasonic features of the esophageal walls without tumor were also described.
3.Cause analysis and countermeasure of the implementation of basic nursing by practice nursing students

Objective To explore the factors affecting implementation of basic nursing by practice nursing students and countermeasures.Methods By cluster sampling, from November to December 2015, 119 nursing students from Xiangtan Medicine & Health Vocational College who had completed their 10-month practice in Hunan Provincial People's Hospital, were selected as objects and were surveyed by a self-designed questionnaire about causes and countermeasures of implementation of basic nursing among practice nursing students.Results The nursing students' negative attitude was the most important cause affecting implementation of basic nursing, which accounted for 0.861 of the original variable; allocating nursing staff more reasonably, establishing an incentive mechanism to overcome occupation burnout, and promoting humanistic care by concerning psychological needs were significant in promoting implementation of basic nursing, each accounting for 0.719, 0.660 and 0.613 of the original variables.Conclusions Nursing educators should strengthen guidance to the nursing students, help them to be more aware of basic nursing, change their negative attitude, allocate nursing staff more reasonably, establish incentive mechanism and pay more attention to their psychological needs.
4.Study on the motivation and path of the formation of breastfeeding education behavior of nurses in NICU based on grounded theory

Objective:To explore the motivation of the formation of the breastfeeding mission and education behavior of nurses in NICU, analyze the deep reasons, and construct the motivation and path theoretical model of the formation of the breastfeeding mission and education behavior of nurses in NICU.Methods:Through semi-structured interviews, the qualitative research method of grounded theory was used to study 20 nurses in the Second Affiliated Hospital, University of South China from July to September 2020, to construct the motivation and path theoretical model of the formation of the breastfeeding mission and education behavior of nurses in NICU.Results:The research identified five main categories: attitude factor, ability factor, behavior factor, internalization and externalization. Under the influence of three dimensions of attitude level, ability level and behavior level and their cross-layer interaction effects, the NICU nurses were driven to form stable breastfeeding mission and education behaviors through internalization and externalization.Conclusions:Based on the evolutionary context of "multi-dimensional factors and their interaction drivers-internalization-externalization", the relationship between attitude factors, ability factors, behavioral factors, internalization, and externalization are clarified. It provides a theoretical reference basis for managers in managing the breastfeeding propaganda and education of nurses in the NICU, and enriches the theoretical framework of breastfeeding behavior in the NICU.
5.Effect of different sleep positions on the apnea hypopnea index of OSAHS patients

Objective To explore the effects of different sleep positions on the apnea hypopnea index (AHI) of obstructive sleep apnea hypopnea syndrome (OSAHS) patients under the state of sleep. Methods Totally 98 OSAHS patients from the ENT clinic and the inpatient department of a tertiary hospital of Changsha from May to July 2015 were selected as the research objects. By using polysomnography detection instrument, the AHI index of patients under different sleeping postures were observed and compared between two groups. Results For 98 patients, the AHI index was (16.51±42.08) in their right lateral position, the AHI index was (20.05±66.11) in left lateral position, AHI index was (22.04±86.39) in supine position, and the AHI index was (53.31±122.88) in prone position. The differences of AHI index in prone position were statistically significant compared with the right lateral position, the left lateral position and supine position (P < 0.05). Conclusions The right lateral position is the best position for OSAHS patients under the state of sleep, and it can decrease upper airway pressure and reduce the sleep apnea and hypopnea during sleep.
6.Single-cell Transcriptome Analysis of Sertoli Cells under High-altitude Environment Reveals Reproductive Toxicity Mechanisms

To construct a single-cell transcriptomic map of testicular tissue under hypobaric hypoxia exposure and perform diversity analysis of supportive cells, aiming to provide new insights into the mechanisms of reproductive toxicity for future research. Twenty healthy male mice were randomly divided into a control group ( A single-cell transcriptomic map of testicular tissue was successfully constructed, including 6 samples with a total of approximately 49 027 cells covering 11 cell types. Supportive cells were clustered into 4 subgroups using non-negative matrix factorization algorithm. Subgroup 3 may be more sensitive to hypoxic stimulation and could reduce reproductive cell function damage caused by hypobaric hypoxia by affecting Based on single-cell sequencing technology, the molecular basis and regulatory signals of supportive cells under hypobaric hypoxia exposure is revealed for the first time. It provides an in-depth exploration of the mechanisms of reproductive toxicity induced by hypobaric hypoxia at the single-cell level, offering a new perspective for future clinical research in this field.
