1.The influence of different dosage of protamine neutralization heparin on perioperation of on-pump coronary artery bypass grafting

Objective To investigate the effect of different doses of protamine neutralizing heparin on perioperation of on-pump coronary artery bypass graftting (CABG).Methods A total of 180 on-pump CABG patients hospitalized from January 2015 to November 2016 were randomly divided into three groups,the protamine group l,protamine group 2 and protamine group 3,60 patients in each group.Heparin (3 mg/kg) was used before extracorporeal circulation.After intracardiac operation was over,protamine was used to neutralize the heparin to adjust the activated clotting time (ACT) in protamine group 1,which was 10% higher than that of intubation.Meanwhile,protamine group 2 was neutralized to equal to the ACT before intubation,and protamine group 3 was 10% lower than that before the intubation.The differences of intraoperative and postoperative parameters were compared between the three groups.Results No death was found in the three groups during hospitalization.Comparing with protamine group 1 and protamine group 2,the time of operation,the ACT before the leaving operation room,the ACT of the first hour after returning to ICU,the amount of bleeding during operation,the time of closing and the amount of red blood for transfusion were decreased in protamine group 3 (P > 0.05).The total amount of protamine for neutralizing and the ratio of protamine and heparin were significantly increased in protamine group 3 (P < 0.05).The heart dysfunction after operation,perioperative myocardial infarction,pulmonary edema,pulmonary infection,renal dysfunction,poor wound healing,neurological complications,and time of in hospital stay showed no significant differences between three groups (P>0.05).Conclusion ACT below 10% of preoperation is safe,after neutralization of heparin by protamine,which can obviously reduce the bleeding,the time of sternal closure and the amount of red blood cell transfusion,showing a positive clinical significance.
2. Comparison on imaging and pathological features of adenosquamous carcinoma of pancreas and invasive ductal adenocarcinoma of pancreas

Objective: To comparatively analyze CT, MRI and pathological findings of adenosquamous carcinoma of pancreas (ASqC) and invasive ductal adenocarcinoma of pancreas (IDAC). Methods: Clinical, imaging and pathological data of 11 patients with ASqC (ASqC group) and 22 with IDAC (IDAC group) confirmed with pathology were retrospectively analyzed. The differences of imaging findings (lesion location, size, morphology, central necrosis, peak enhancement time, bile duct dilatation, main pancreatic duct dilatation) and pathological features (nerve tract invasion, lymph node metastasis, vascular tumor emboli, Ki-67) were compared between 2 groups. Results: CT showed significant differences in lesions' diameter, morphology, central necrosis and peak enhancement time between 2 groups (χ2=7.34, 6.82, 10.25, 5.57, all P<0.05). MRI showed that there was no significant difference of lesion's signal on T1WI and dynamic enhancement between 2 groups, while IDAC group had lower T2WI signals than ASqC group. Pathological results showed that the incidence of vascular tumor emboli in ASqC group (5/11, 45.45%) was higher than that in IDAC group (2/22, 9.09%, χ2=5.80, P=0.02). Conclusion: ASqC and IDAC have certain pathological and imaging characteristics. Combination with clinical data is helpful to differential diagnosis.
3.Effect on colorectal cleansing of CT colonography with gulping down bisacodyl before or after oral taking polyethylene glycol

Objective To comparative study the effect on colorectal cleansing of CT colonography with gulping down 10 mg bisacodyl before or 1 h after oral taking 2 liter polyethylene glycol .Methods Forty participants with informed consent were appor‐tioned to group A ,group B randomly ,20 cases in each group .On the day before CT colonography ,participants in group A oral took 20 mL of 40% W/V barium sulfate prior to 3 mealtime ,and 20 mL of 60% diatrizoate meglumine diluted in 250 mL of water after supper ,then gulped down 10 mg bisacodyl enteric‐coated tablets 1 hour before oral taking 2 liter polyethylene glycol electrolyte so‐lution .Participants in group B were the same as that in group A ,with the exception of gulping down 10 mg bisacodyl enteric‐coated tablets 1 hour after oral taking 2 liter polyethylene glycol electrolyte solution .Cleansing efficacy of stool and fluid ,and attenuation value of remainder fluid between the two groups were analyzed statistically .Results In group A ,score of cleansing efficacy of stool (1 .96 ± 0 .11) was lower than that in group B (2 .01 ± 0 .12) ,segments with good cleansing efficacy of stool (87/120 segments , 72 .50% ) was higher than that in group B (83/120 segments ,69 .17% ) ,the difference was not statistically significant (P>0 .05) .In group A ,score of cleansing efficacy of fluid (1 .50 ± 0 .06) was lower than that in group B (1 .53 ± 0 .06) ,segments with good cleansing efficacy of fluid(113/120 segments ,94 .17% ) was higher than that in group B (111/120 segments ,92 .50% ) ,the differ‐ence was not statistically significant (P>0 .05) .Attenuation value of remainder fluid [(729 ± 29)HU ] in group A was higher than that in group B[(653 ± 25)HU] ,the difference was statistically significant(P<0 .05) .Conclusion Gulping down 10 mg Bisacodyl before or after oral taking 2 liter polyethylene glycol has no effect on cleansing of stool and fluid ,with good cleansing efficacy .The former has better cleansing efficacy of fluid ,is beneficial to detecting polyps for CT colonography .
