1.Advance in Non-drug Therapy of Insomnia (review)

At present, insomnia treatments mainly contain drug therapy and non-drug therapy. The non-drug therapy included sleep hygiene instruction, cognitive behavioral therapy, psychological therapy, physical therapy, relaxation therapy, and music therapy, etc. The non-drug therapy is a safe and effective treatment, and it plays a good effect whether using with drug or alone.

Objective To compare the effect of one-site and two-site incision in glaucoma with cataract combined surgery.Methods Forty-two cases (58 eyes) of patients of glaucoma with cataract combined surgery were selected.One-site incision were 22 cases with 30 eyes (one-site incision group),and two-site incision were 20 cases with 28 eyes (two-site incision group).The postoperative intraocular pressure,ultrasound biomicroscope (UBM),filtering bleb and complications were compared in two groups.Results The postoperative visual acuity and intraocular pressure in two groups were better than preoperative [one-site incision group:0.82±0.11 vs.0.18±0.09,(17.32±4.52) mm Hg (1 mm Hg=0.133 kPa) vs.(31.98± 5.18) mm Hg,two-site incision group:0.79 ±0.20 vs.0.20 ±0.03,(16.98 ±4.65) mm Hg vs.(33.16 ±4.61) mm Hg],there were statistically significant differences (P < 0.05),but there was no significant difference between two groups on postoperative (P> 0.05).There was no statistically significant difference in visibility of filtering routes under the scleral flap and reflectivity inside the blebs in two groups (P > 0.05).Conclusions Ohe-site and two-site incision in glaucoma with cataract combined surgery has good intraoeular pressure control,can maintain a good filtering bleb function at the same time.The effects are similar between the two operative methods.
4.Comparison of the efficacy of IMRT and conventional radiation therapy combined with chemotherapy in ad-vanced nasopharyngeal carcinoma

Objective To observe the curative effect and adverse reactions of IMRT and conventional radiotherapy combined with chem-otherapy in treating patients with advanced nasopharyngeal carcinoma. Methods The patients were divided into the IMRT group (46 cases) and conventional radiotherapy combined with chemotherapy group ( 50 cases ) . Making the IMRT and conventional radiotherapy combined with chemotherapy treatment plan, and analyze the curative effect and adverse reaction of the two groups. Results Compared with the con-ventional radiotherapy combined with chemotherapy group, IMRT group has a significantly increase in complete remission rate and overall re-sponse rate, but there is no significant difference in long-term efficacy. The skin, dry mouth ( parotid) ,and oral mucositis response is lower in IMRT group, and the neutropenia incidence, prevalence of anemia, liver and kidney function damage rate is also significantly lower than conventional radiotherapy combined with chemotherapy. Conclusion The IMRT can significantly improve the curative effect in treatment of advanced nasopharyngeal carcinoma and reduce the adverse reactions compared with the conventional radiotherapy combined with chemotherapy.
5.Incidence rate and related factors of intraocular pressure after vitro-retinal surgery for patients with diabetic retinopathy

Objective To explore the incidence rate and related factors of high intraocular pressure after vitro-retinal surgery for patients with diabetic retinopathy(DRP). Methods The 986 patients(1 326 eyes) with DRP arranged vitro-retinal surgery were studied. The inci-dence rate of intraocular pressure was analyzed between different types,grades,complicated with macular detachment,filled with silicone oil/C3F8,total/supplementary retina photo-coagulation and crystalline lens excision. Results The incidence rate of high intraocular pressure was 27. 8%(368/1 326). The incidence rate of high intraocular pressure of proliferative DRP (34. 0%) was obviously higher than non-pro-liferative DRP (17. 2%)(χ2 =30. 648,P=0. 000),and higher in DRP D grade than B grade(34. 1%,20. 8%)(χ2=8. 834,P=0. 003). The incidence rate of high intraocular pressure of proliferative DRP (34. 0%) was obviously higher in patients complicated with macular de-tachment(30. 4%,22. 7%)(χ2 =8. 834,P=0. 0030). The incidence rate of patients filled with silicone oil/C3F8(39. 1%,24. 0%) (χ2 =28. 088,P=0. 000),total/supplementary retina photo-coagulation(32. 8%,16. 8%) (χ2 =21. 159,P=0. 000),with/without crystal-line lens excision (40. 5%,25. 9%)(χ2=15. 532,P=0. 0001)was different obviously. Conclusion There is a high incidence rate and re-lated factors of high intraocular pressure after vitro-retinal surgery for patients with diabetic retinopathy,and it is related to different DRP types,grades,complicated with macular detachment,filled with silicone oil/C3F8,total/supplementary retina photo-coagulation,crystalline lens excision.
6.Detection and clinical observation of viral infection in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To analyze the viral infection in patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)and its clinical significance.Methods 350 patients with acute exacerbation of COPD were collected.The viral infection and clinical manifestations,pulmonary function and imaging features were detected and analyzed.Results A total of 104 patients with viral infection,the proportion was 29.7%.Single virus infection in 74 cases(71.2%),the highest proportion was FluB(19.2%)in pathogen type,followed by FluA(15.4%).Multiple virus infection in 30 cases(28.8%),the highest proportion was RSV+FluB(11.5%).The virus infection in patients with sore throat(20.2% vs.6.9%),lung function grade(32.7% vs.20.3%),presence of pulmonary inflammatory lesions(57.7% vs.40.7)and the death rate(12.5% vs.0.8%)were significantly higher than those of the combined group of virus infection,the proportion of lung function was significantly lower than patients without viral infection,the differences were statistically significant(x2=6.546,5.477,4.987,4.124,3.997,all P<0.05).The proportion of respiratory failure in patients with acute COPD infection was 86.5%,which was higher than 62.6% of patients without viral infection,the difference was statistically significant(x2=19.837,P<0.05).Conclusion Acute exacerbation of COPD has a high proportion of virus infection,and the prognosis of patients is poor.

8.Analysis of statistical problems in clinical research articles of oncology

Objective To investigate the application of statistical methods in clinical research articles of oncology in China. Methods The core journals in Chinese oncology journals were retrieved in "Wanfang Data", and the application of statistical methods in the clinical research articles published in the first issue of 2017 were analyzed,including the selection of statistical indicators,the selection of statistical methods and the interpretation of statistical results. Results A total of 176 clinical research articles from 24 journals were taken into the study.In the selection of statistical indicators, 12.6 %(16 articles) used the description method of normal distribution data for the skewness distribution measurement data; 37.9 % (47 articles) described the count data with samples<20 as a percentage; 14.5 % (9 articles) replaced the constituent percentages with composition ratios. In the selection of statistical methods, 10.3 % (12 articles) did not perform analysis of variance when comparing among multiple groups, and the t test was used to compare each other directly. 15.1 % (8 articles) disregarded the preconditions of the use of the χ 2test. In the interpretation of statistical results, 18.4 % (27 articles) confused the statistical significance with biological significance. Conclusions The statistical problems are widespread in Chinese oncology clinical research articles,involving the entire process from study design to interpretation of results. Scientific research management departments, clinical researchers, and journal editorial board need to work together to improve the scientificity of clinical scientific research results of oncology in China.

Methods CTfindingsof36patientswithluminaltumorGlikedilationCAFprovedbysurgerywereanalyzedretrospectively.Bymeans ofMIP,CPR,MPRandVRreconstruction,wearrangedtheabnormalvesselsintogroupsaccordingtoitscharacteristics,andmeasured thediameterofdilatation.ThediagnosticaccuracyofMSCTforCAFwascomparedwithtransthoracicechocardiography.Results Among the36cases,drainagesitesof8caseswererightatrium,14casesrightventricle,2casespulmonaryartery,1caseleftatrium,9cases leftventricle,and2casescoronarysinus.Amongallcases,4casesoriginatedfromleftmainartery,2casesfromleftanteriordescending, 5casesfromcircumflexartery,and25casesfromrightCAF.Ofwhich,24casesunderwentfistulaligationwithextracorporealcirculation. 12casesweretreatedwithminimallyinvasivesurgery.ThediagnosticaccuracyofMSCTbeforesurgerywas100%(36/36),andthat ofechocardiographywasabout80.6%(29/36).Conclusion ReconstructiontechniqueofMSCTcanprovidetheinformationofthe CAFvesselsanditsorigin,courseandshapeoffistulaorificium.MSCTcanaccuratelylocateandguidethesurgicalapproach,especially forpreGinvasivesurgery,providingimportantinformationfortreatment.MSCTshouldbethebestimagingmethodforCAF.
10.Diversity research about facial morphology of unilateral complete cleft lip and palate affecting by different repair periods of palate repair.

OBJECTIVETo evaluate facial morphology characteristics of patients with unilateral complete cleft lip and palate (UCCLP) after cleft palate repair in different periods.
METHODS46 nonsyndromic UCCLP patients were chosen as test group and divided into three kinds (under 4-year-old, 4-7-year-old, over 7-year-old) according to periods of palate repair. 38 age and gender matched non-clefts children were involved as control group. 26 cephalometric measurements were measured to evaluate facial morphology of four groups.
RESULTSCompared with patients who received palate repair before 4-year-old, patients who received repair after 4-year-old had more protrusive ANS point (Ba-N-ANS, Ba-ANS) and less Y-axis angle. Patients who received palate repair after 7-yesr-old had greater A-PMP and ANS-PMP than patients who received palate repair before 4-year-old, patients who received palate repair before 7-year-old had smaller ANS-Me but after had no significant difference when compared with non-clefts.
CONCLUSIONWhenever palate repair is undertaken in the period of maxillary growth and development, patients' maxillary growth would be restrained, the delayed repair sample have better lower face height, maxillary protrusion, maxillary sagittal length and mandible growing direction when compared with the early repair sample, that is to say, the delayed repair sample have preferable head-face morphology.
