1. Prevention and treatment of urethrovesical anastomotic leakage following laparoscopic radical prostatectomy

Objective To search for method for treatment and prevention of urethrovesical anastomotic leakage (PUAL) follownng laparoscopic radical prostatectomy (LRP). Methods Eight (5. 3%) of the 151 laparoscopic radical prostatectomies performed between Jan. 2006 and Jan. 2011 developed PUAL. Running urethrovesical anastomosis was used for 58 patients and traditional suture was used in 93 during LRP. The mean age of the 8 patients was 66 years (ranging 53 to 78 years), the mean preoperative total prostate specific antigen(T-PSA) was 34. 3 ng/ml (ranging 1. 1 to 165. 0 ng/ml). The preoperative Gleason sum was <7 in 4 patients, 7 in2, and > 7 in 2 patients. The mean preoperative Gleason score was 7 (ranging 5 to 9). Pathological stage was T1c nn 5, T2a in 2, and T2c in 1 patient. The mean operation time was 202 min (ranging 60 to 360 min). Several treatments were used, including prolonged retropubic drainage, bladder catheter traction, drain position adjustment, reduction of fluid intake, delayed bladder catheter removal and enhancing nutrition for patients. Patients underwent reoperation using a laparoscopic approach after failure of the initial management. Results PUAL was not observed in LRP cases with running laparoscopic suture technique, which was significantly better than cases with traditional suture method (0 vs 8. 6% [8/93], P<0. 05). Six patients were cured by conservative treatments, 2 patients required reintervention via a laparoscopic approach. The catheters of all patients were removed after a mean of 33 d (ranging 21-43 d). Urethral stricture was not found during a 39-month follow-up(ranging 22to 60 months). Concision Urethrovesical anastomotic leakage can be effectively avoided by running urethrovesical anastomosis; conservative treatment and laparoscopic repair are effective managements to treat PUAL.

AIM:To study the anti-tumor effect and mechanism of fusion vaccine on DCs and C6 glioma cells.METHODS: PEG was used to fuse DCs with C6 glioma cells.Immunofluorescence with GFAP-FITC was used to identify the DC/C6 fusion cells.Rat brain glioma models were made by stereotactic technique.After 5 days of inoculation of C6,107 fusion cells were injected through tail vein in group A.The same number of DCs and the same volume of PBS were used in group B and group C.The survival time of rats in these three groups was analyzed by Log-rank survival analysis.Tumor samples were checked by HE staining and immunohistochemical staining with CD8Mcab.RESULTS: Positive result of GFAP-FITC immunofluorescence was observed in DC/C6 fusion cells.The Log-rank survival analysis showed that statistically significant difference in group A was observed compared to that in group B and group C(P
3.Endoscopic incision in the treatment of ureterovesical anastomosis site obstruction in transplanted kidneys

Objective To report the clinical outcomes of applying endoscopic incision in the treatment of ureterovesical anastomosis site obstruction in transplanted kidneys. Methods Between February 2001 and April 2008, 13 men and 5 women with ureterovesical anastomotic site obstruction in their transplanted kidneys were treated by endoscopic incision with electrocautery or holmium: YAG laser. After the anastomosis was completely resected, two Double-J stents were placed in the ureter for 6-8 weeks. During follow-up, renal function, ultrasound examination and wash-out renal scintig-raphy were performed every month for the first 6 months, then every 3 months. Results Total 25 procedures of endoureterotomy were performed and all procedures resulted in successful incision of the obstruction. No complication was recorded during or after the procedure. At the mean follow-up of 51 months (range 4-90 months), 5 patients presented with recurred obstructive uropathy immediately after the Double-J stent removal and finally underwent open surgical correction. Conclusions Endo-scopic incision is safe and effective in the treatment of ureterovesical anastomosis site obstruction in transplanted kidney. However, open surgical reconstruction should be considered if endoscopic inci-sion procedure has failed.
4.Minimally invasive nercutaneous nephrolithotomy combined with negative pressure system in one-stage treatment of calculus pyonephrosis

Objective To evaluate the efficacy and safety of minimally invasive percutaneous nephrolithotomy combined with negative pressure system in one-stage treatment of calculus pyonephrosis.Methods Eighty-three cases of calculus pyonephrosis,including 15 upper ureteral calculus cases,9 renal pelvis calculus cases,28 multiple calculus cases and 31 staghorn calculus cases,were retrospectively analysed.The diameter of the stone was from 1.2 to 6.3 cm.All the patients were punctured under X-ray or ultrasound guidance and established an access of 20 F.A 12 F nephroscope,combined with negative pressure system,was inserted to the collecting system to suck off the liquor pus.The stone was fragmented by pneumatic lithotripsy or holmium laser lithotripsy at one-stage.Negative pressure system was used to reduce the intrapelvic pressure during the operation.Results All the patients were treated successfully.The average operative time was 34 ± 19 min.The upper ureteral calculus and renal pelvis calculus cases were all stonefree at one-stage treatment.Of the other 59 cases,33 cases were stone-free and 26 cases need a secondlook.The total stone free rate was 68.7%(57/83)at one-stage and 91.6%(76/83)at second-look.Only 7 patients had fever after operation and no patient had sepsis or shock.Conclusion Combined with negative pressure system,minimally invasive percutaueous nephrolithotomy via a 20 F tract is safe and effective for one-stage treatment of calculus pyonephrosis.
5.A high-fidelity DNA unidirection synthesis method:preliminary exploration

Objective To establish a simple but quick method to improve the high fidelity of the synthesis of DNA frag -ments.Methods High fidelity DNA unidirection synthesis method (HFUS) was presented and used that involved Phusion DNA polymerase, BsrDⅠrestriction enzymes and λexonuclease.Using the same system at different temperatures , HFUS method synthesized one positive single-strand DNA and several reverse single -stranded DNA one by one into the target DNA fragment.Results Two random sequences DNA fragments of 340 bp and 450 bp were synthesized using HFUS meth-od.Conclusion This article explores a new method for the synthesis of genes .Through the harvest of 450 bp DNA, HFUS may promise to be a new approach to the synthesis of DNA .
6.Effects observation of Fexofenadine hydrochloride tablets combined with BCG polysaccharide nucleic acid in the treatment of chronic urticaria

Objective To study clinical effects of the fexofenadine hydrochloride Tablets and BCG-PSN in the treatment of chronic urticaria. Methods 122 patients with chronic urticaria treated in Yiwu skin disease hospital from January 2016 to March 2017 were selected and randomly divided into the control group and the experimental group, 61 patients for each group. The control group was given Fexofenadine Hydrochloride Tablets treatment, the experimental group was given BCG-PSN on the basic treatment of the control group. The patients in the experimental group and the control group were treated for 3 months. The treatment effect and adverse reactions were compared between two groups. Results After the corresponding treatment, the number of invalid cases in the experimental group was 10. In the control group, 21 cases were ineffective. The effective rate of the experimental group was 63.93%, which was significantly higher than 32.97% of the control group with statistical significance (P<0.05). There were no obvious adverse reactions in the two groups, and there was no significant difference in the incidence of adverse reactions. After treatment, the integral score of the experimental group was (3.20±0.42), significantly lower than that of the control group (3.98±0.37), with statistical significance (P<0.05). Conclusion fexofenadine hydrochloride Tablets combined with BCG-PSN for the treatment of chronic urticaria could significantly improve clinical symptoms of patients with good efficacy, high safety and clinical application significance.
7.Adrenocorticotropic hormone-producing pheochromocytoma: a case report and review of the literature.

Ectopic Cushing’s syndrome caused by pheochromocytoma is rare. We reported a 15-year-old female patient who was admitted to hospital with typical Cushing’s syndrome. She had not started menstruation. Her plasma adrenocorticotropic hormone (ACTH) and 24-hour urinary free cortisol levels were extremely high. Gonadal and progestational hormone levels were also abnormal. Abdominal computed tomography scans and enhanced scans revealed multiple irregular tumors in the right adrenal. Pelvic echogram showed an infantile uterus, while the ovaries were at an immature stage of development. Retroperitoneal laparoscopic right adrenalectomy was performed without intraoperative complications. Histology and immunohistochemistry of the tumor were consistent with pheochromocytoma. Retroperitoneal laparoscopic adrenalectomy is a safe procedure with satisfactory outcomes and allows for rapid recovery.

8.Effect of beta-Fibrinogen-455 Gene Polymorphism on Plasma Fibrinogen Levels in Patients with Ischemic Stroke

In large prospective studies, plasma fibrinogen levels have been shown to be an independent risk factor of vascular disease, including ischemic stroke. Elevated plasma fibrinogen in an individual could be due to the presence of predisposing genetic and/or environmental factors, such as smoking. Of the polymorphisms studies to date, the beta-fibrinogen-455 (beta-Fg-455) G-->A substitution in the 5' flanking region is associated with the most consistent difference in plasma fibrinogen levels in both case-control studies and in selected groups of healthy individuals. In order to further elucidate the role of the beta-Fg-455 G-->A substitution in determining fibrinogen levels and susceptibility to ischemic stroke in case-control population, including 104 individuals with verified ischemic stroke and 156 healthy individuals. Turbidimetriy assays were used to measure plasma fibrinogen levels of all samples. The beta-Fg-455 G-->A mutation was identified by the polymerase chain reaction followed by restriction enzyme digestion of the amplified DNA with HaeIII. The plasma fibrinogen level in patients with ischemic stroke [(3.51 +/- 1.09) g/L] was significantly higher than that in the control [(3.08 +/- 0.71) g/L] (P < 0.01). The A-allele is associated with elevated fibrinogen levels in both patients and controls. The plasma fibrinogen levels in controls with A-allele in elder people were higher than in younger people (P < 0.05). Those with A allele in males of ischemic stroke had significantly higher plasma fibrinogen levels in smokers than in non-smokers and ex-smokers (P < 0.05), but it was not significantly difference in subjects of GG genotype (P > 0.05). Our data demonstrates an association of the beta-Fg promoter A-455 allele with higher fibrinogen levels in the general population, and suggests that the A-allele may be a susceptible predictor of ischemic stroke, particularly in aging and smoking.
9.Anti-tumor effects of 10-hydroxycamptothecinc-treated DC-Hepa1-6 fusion vaccines.

OBJECTIVETo investigate the induction of antitumor immune responses and therapeutic effects of 10-hydroxycamptothecinc-treated (HCPT) DC-Hepa fusion vaccines by DC fused with hepal-6 cell from hepatoma.
METHODSThe fused cells were isolated by magnetic cell sorting and adherent culture. Cell apoptosis was detected by Rhodamine123/PI double-labeled assay, CTL activity by 4 h (51)Cr releasing assay. Protective and therapeutic effects of the fusion vaccine to the tumor-bearing mice was also observed.
RESULTSThe apoptosis rate was 29.7%+/-4.1% when DC-Hepa fusion vaccine was treated with 50 microg/ml HCPT for 24 h. After treatment with the HCPT-DC-Hepa fusion vaccine, the tumor grew obviously slowly, survival period of the mice was prolonged, induced more potent CTL cytotoxicity, and resisted against the rechallenge of Hepal-6 cells.
CONCLUSIONVaccination with HCPT-DC-Hepa fusion vaccine could elicit potent antitumor responses, which will provide a new approach to the DC-mediated therapeutic antitumor immunity.
