2.Application of long-time EEG monitoring in comatose patients

Objective To explore the prognostic value of long-time EEG monitoring and the clinical examination index in comatose patients.Methods The long-time EEG monitoring and the modified Glasgow coma scale were done in 26 comatose patients(15 patients were male;11 patients were female;17—40 years old)in acute phase.The complexity of cerebral hemisphere was analyzed by computer.The patients were followed-up for 2 months and evaluated by Glasgow outcome scale.The correlation between the long-time EEG monitoring,clinical examination index and the prognosis of the patients was analyzed.The time- variation of hemisphere complexity and the relationship between prognosis and entropy were observed. Results The specificity(77.8%)and accuracy rate(92.3%)of long-time EEG monitoring in predicting the outcome of the acute comatose patients were higher than those of clinical examination.The EEG index appeared more closed-up with prognosis than clinical examination.The correlation coefficient between the EEG index and the modified Glasgow prognosis scoring was r=0.81(P

4.Diagnosis and treatment of bladder outlet obstruction caused by bladder neck fibrosis (report of 30 cases)

Objective To present experience on the diagnosis and treatment of bladder outlet obstruction (BOO) caused by bladder neck fibrosis. Methods 30 cases of BOO caused by bladder neck fibrosis were studied. All the patients were treated by transurethral partial resection or incision of the cervix vesicae combined with alpha 1 adrenoceptor antagonists such as tamsulosin and terazosin or parazosin. Results Urination has been improved in all.The patients have been followed up for 3 to 30 months.On repeated uroflowmetry,the 30 patients showed the urine flow rates have been more than 15ml/s. IPSS scores was 5.3?1.7(25.4?4.2 preoperative) and quality of life scores was 1.4?0.6(4.1?0.8 preoperative).Fibroblast proliferation was noted in all the 30 and obvious chronic inflammation in 18. Conclusions Clinical symptoms, pressure rate measurement and cystoscopy are the reliable diagnostic methods.Transurethral partial resection or incision of cervix vesicae combined with alpha 1 adrenoceptor antagonists is effective.
5.Study on material base of corydalis rhizoma.

Corydalis Rhizoma, the dried tuber of Corydalis yanhusuo (Papaveraceae) distributed traditionally mainly in south-eastern and now cultivated in northwestern and other district in China, is one of the commonly used and well-known traditional Chinese medicine. It has been widely used to treat spastic pain, abdominal pain, pain due to injury, and promote blood circulation. Its main chemical constituents were alkaloids, which were divided into the two types of protoberberines and aporphines. Among them, some alkaloids were found to elicit profound effects on the dopaminergic system in the central nervous system, which plays an important role in regulating nociception. In this article, the chemical composition and structure-types, new methods of qualitative and quantitative analysis as well as characteristics of biotransformation, absorption, distribution, metabolism, excretion, pharmacokinetic, and drug-drug interaction for the alkaloids were revealed. These results would greatly contribute to the establishment of bioactive material base of Corydalis Rhizoma.

6.Effects of RNA interference-mediated adiponectin gene silence on glucose-lipid metabolism in ApoE gene knock-out mice

The effect of RNA interference-mediated adiponectin gene silence on glucose-lipid metabolism in apolipoprotein E gene knock-out(ApoE~(-/-)) mice was studied. The plasma adiponectin level in ApoE~(-/-) mice with high-fat diet and adiponectin short hairpin RNA (shRNA) adenovirus injection (ADI group) was significantly lower than that in ApoE~(-/-)mice with high-fat diet(HF group), normal-chow(NF group)and naked adenovirus injection (GF, both P<0.01). Fasting blood glucose, fasting plasma insulin, free fatty acids, total cholesterol, triglycerides, low-density lipoprotein-cholesterol (LDL-C), high-density lipoprotein-cholesterol (HDL-C), and body weight were significandy elevated in three high-fat diet groups compared with in NF group, (P<0.01). However, fasting plasma insulin, free fatty acids, total cholesterol, triglycerides, LDL-C, and HDL-C in ADI group were significantly higher than those in HF and GF groups(P<0.01 or P<0. 05). A typical clinical phenotype of glucose-lipid metabolism and insulin resistance in ApoE~(-/-) mice can be induced by high-fat feeding and adiponecting gene silencing.
7.A study of the relationship between airway responsivenss and ICS/LABA inhalation therapy for bronchical asthma

Objective To explore the relationship between airway responsiveness and bronchial asthma inhalation therapy .Meth-ods Select 50 asthmatic patients as research subjects ,Fluorine given inhaled salmeterol 50 mcg/fluticasone propionate 250 micro-grams ,1-2 times a day ,Respectively before treatment ,after treatment ,3 months ,6 months ,12 months ,conventional pulmonary function tests and bronchial provocation test ,Determination of peak expiratory flow (PEF) ,and a second forced expiratory volume (FEV1 ) ,maximum mid-expiratory flow (MMEF) and specific airway conductance decreased 35% or above to inhaled methacholine the concentration(PC35sGaw) .Results Three months after the majority of patients with clinical symptoms before treatment ,after treatment ,significant improvement ,PEF ,FEV1 increased significantly in all cases of bronchial provocation test is still positive ,six months after treatment ,more than 80% of patients with asthma ,complete control ,lung function returned to normal follow-up prov-ocation test positive cases of 38 cases ,up to 76% after 12 months of treatment ,more than 90% of patients with lung function re-turned to normal .29 cases(58% ) stimulation test positive .Conclusion Asthma clinical indicators have reached complete control to achieve the desired level of time earlier than the airway responsiveness ,airway responsiveness index value in the long-term follow-up of the combination therapy ,and adjust the treatment plan is superior to clinical symptoms and lung function ,is a serious assessment of asthmadegree of judgment and withdrawal of drug treatment of one of the indicators .
8.Localizing value of epileptic auras

ObjectiveTo study the value of localizing different types of auras. MethodsThe 31cases with epilepsy were divided into a group with auras (n =19) and a group without auras (n =12).Student t-test was used to analyze difference in onset age in the two groups. The chi-square test was used to analyze the differences in the rates of abnormal brain MRI in two groups and in the efficiency of epileptic surgery between the two groups.ResultsThe 19 patients in aura group had psychiatric symptoms,autonomic symptoms, somatosensory, visual, hearing, and dizziness auras. These auras and EEG and other examines presumebly localized the epilepsy lesions in temporal lobe, temporal lobe, contralateral parietal or temporal lobe, occipital or temporal lobe, contralateral temporal lobe, and temporal-parietal lobe. After the surgery, auras diminished in all these patients. There was no significant differences in onset age and in presence of abnormal brain MRI between the two groups. The percentage of efficient epileptic surgery in the group with aura (15/19) was significantly higher than that in the other group( 5/12, x2 =4. 456, P =0. 035). ConclusionsEpileptic aura has significant value in localization of lesion. It can help determine the origins of the epilepsy focal and guide epilepsy surgery.
9.Clinical study of epileptic aura

Objective To study the prevalence and clinical features of auras in epilepsy patients,and to evaluate the diagnosis criteria of epileptic auras.Methods All 1028 patients with epilepsy who were treated in the First Hospital of PekiIIg University from January 2007 to January 2008 were enrolled in this retrospective study.Seizure types were diagnosed in accordance with the seizure classification scheme proposed by the International League Against Epilepsy(1981).We studied the incidence rate,clinical features and presentations of electroencephalogram(EEG)and neuroimaging of epileptic auras.Student ttest was used to assess the statistic difference of the onset age of partial seizure in patients with and without auras.The chi-squaretestWas usedto statistically analyze the EEGand neuroimaging results in patients with partial seizure and the occurrence percentages of abdominal aura in lateral temporal lobe epilepsy(LTLE)and mesial temporal lobe epilepsy(MTLE).Results Four hundred and eighty-four patients(66.8 % of 725 patients)with partial seizures reported epileptic auras.None of the 303 patients with generalized seizures reported auras.Sixty-four patients experienced two or more types of auras.Fourteen patients had aura status. Of all enrolled patients,53.2 % and 21.1 % showed abnormal EEG and neuroimaging,respectively.Of 484 partial epileptic patients with auras,59.1 % and 26.0 % presented abnormal EEG and neuroimagings,respectively.The percentage of abdomen aura in patients with MTLE was significantly higher than those with LTLE(x2=170.877,P<0.01).Condusion Analysis of auras may help in classifying and localizing epilepsy and also in planning treatment.
10.The development of the knee osteoarthritis TCM syndrome evaluation scale as the Patient-Reported Outcome

Objective To develop the knee osteoarthritis (KOA) TCM syndrome evaluation scale. Methods Resrarch team collected subjective symptoms from all kinds of syndrome types of KOA and classified the subjective symptoms, which formed the first version of item pool containing 24 subjective symptoms. Expert questionnaires were conducted to calculate the weight coefficient of items, and formed the item pool of the Knee Osteoarthritis TCM Syndrome Evaluation Scale. According to the patient's attention to the 11 items, the items were weighted.Results The Knee Osteoarthritis TCM Syndrome Evaluation Scale contained 11 items, including pain, swelling and stiffness with different weighted scores.Conclusions The Knee Osteoarthritis TCM Syndrome Evaluation Scale were introduced to assess the KOA outcomes on the basis of TCM syndromes. It showed the characteristic of TCM and TCM syndrome.It could provide an objective and standardized measurement for TCM treating KOA.
