2.Relationship between inhaling oxygen's flowing rate and respiratory depression in patients with chronic obstructive pulmonary disease plus carbon dioxide retention

We observed 38 patients' breath frequency after different oxygen therapy according to their visit sequences and tolerance conditions.All patients with chronic obstructive pulmonary.disease (COPD) plus carbon dioxide retention from April 2008 to June 2011 were recruited at our hospital.Twentythree patients who could maintain SpO2≥92% through inspiring O2 at ≤3.5 L/min were assigned into the first group.And another 15 subjects whose SpO2 could not reach 92% at the same oxygen flow for 5 hours were assigned into the second group.The second group received an elevated oxygen flow until their SpO2 attained 92% or so.But the maximal rate did not exceed 8 L/min even if SpO2 was below 92%.The state was maintained for 5 hours.Paired t-test showed PaO2 became elevated and PaCO2 decreased significantly on onset and termination of low oxygen flow in both groups.But the frequency of the second group's breaths per minute (BPM) had no significant differences.After elevating oxygen flow,the second group's BPM and PaCO2 on onset and termination were (19.4 ± 3.4) and (18.1 ± 2.6) perminute (P =0.038),(55.0 ±8.1) and (52.0 ±7.9) mm Hg (1 mm Hg =0.133 kPa) (P =0.030) respectively and the results showed significant decreases.But SpO2 was elevated [(85.8 ± 4.1) and (90.7 ± 2.4) mm Hg (P =0.000)].
3.Effect Observation and Mechanism Exploration of Huiyang Shengji Ointment in Treating Chronic Refractory Skin Ulcer

Objective To research the curative effect of the chronic refractory skin ulcer,which belong to yang deficiency of both spleen and kidney type with the treatment of Huiyang Shengji ointment,and discuss the mechanism at cell and molecular level.Methods According to the diagnostic criteria,102 patients were divided into the treatment group(treated with topical Huiyang Shengji ointment) and the control group(treated with topical chlorhexidine).The effects were summarized six weeks later.The marginal tissue of the patients with the chronic skin ulcer was taken to analyze collagen Ⅰ/Ⅲ with the software and the expression of cytokine FGF-2,VEGF,Fn were observed during the wound heal.Results The effective rate was 50% in the treatment group,the control group was 33.33%,with statistical differences.Huiyang Shengji ointment can change the status of the chronic wound,promote growth of new blood vessels and improve the collagen synthesis,especially collagenⅠ.The expression of the FGF-2 and VEGF in the marginal tissue of the chronic skin ulcer showed a rising trend.Conclusions Huiyang Shengji ointment can accelerate the wound heal of chronic refractory skin ulcer.One of the mechanisms is related to promoting growth of new blood vessels and improving the collagen synthesis.

Aim To study the anti-inflammatory effects and mechanisms of SBM isolated from Scutellariae radix by high hydrostatic pressure.Methods The effect of SBM on ConA-induced splenocyte proliferation and LPS-induced IL-1? production from peritoneal macrophage was studied in vitro,and the therapeutic effects of SBM on adjuvant-induced arthritis,formalin-induced paw oedema and acetic acid-induced vascular permeability were also investigated.Results SBM(31.25~500 mg?L-1)significantly inhibited ConA-induced splenocyte proliferation,IL-1? production from peritoneal macrophages in vitro.SBM 20 mg?kg-1 inhibited primary and secondary inflammatory reaction,decreased the elevated level of IL-1? released from peritoneal macrophages,inhibited splenocyte proliferation in adjuvant-induced arthritis in rats.Moreover,SBM inhibited formalin-induced paw oedema and acetic acid-induced vascular permeability.Conclusions SBM could inhibit inflammatory reaction.Its mechanisms might be related to the suppression of immune function and inhibition on proinflammatory cytokines production.

Aim To study the anti-inflammatory effects and mechanisms of SBM isolated from Scutellariae radix by high hydrostatic pressure.Methods The effect of SBM on ConA-induced splenocyte proliferation,LPS-induced PGE2 production and protein expression of COX-2 were studied in vitro,and the therapeutic effects of SBM on carragineen-induced paw oedema and acetic acid-induced twisting reaction were also investigated.Results SBM(15.63~250) mg?L-1 significantly inhibited LPS-induced PGE2 production and COX-2 enzyme activity in peritoneal macrophages in mice.SBM(125 mg?L-1)also inhibited ConA-induced protein phosphorylation of p38 MAP kinase and LPS-induced COX-2 protein expression.SBM(10,20 mg?kg-1)inhibited carragineen-induced paw oedema and acetic acid-induced twisting reaction.Conclusion SBM can inhibit inflammatory reaction.Its mechanisms may be related to the suppression of inflammatory medium production.
7.Interference effect of nerve growth factor on apoptosis of retinal cells in experimental retinal detachment

Objective To investigate the interference effect of nerve growth factor (NGF) on apoptosis of retinal cells in experimental retinal detachment (RD). Methods Twenty seven Sprague-Dawely rats were selected, and the left and right eyes were in the experimental control group and NGF group, respectively. After the RD model was set up by subretinal injection with sodium hyaluronate, 5 ?l NGF (1 ?g?l) was injected into the vitreous body of the right eyes which were in the NGF group; 5 ?l PBS was injected into vitreous body of left eyes which were in the experimental control group. The injection was performed once every 4 days till the end of the observation period. The eye balls of the 27 rats were extrafted 1.5, 3, 6, 12 hours, 1 day, 2, 4, 8, 16, and 32 days after the RD model was established. Another 2 rats were selected as the normal control, which underwent none of the injections but eyeball extraction at the end of the observation period. TUNEL and transmission electron microscopy were used to detect the apoptosis of the retinal cells. Cell counts and statistical analysis were used to assess results. Results Typical apoptosis cells were observed in the early time of RD. Apoptosis was found in each retinal layers, especially in inner and outer nuclear layers. 9.Iron-deficiency anemia complicated by cerebral thrombosis an analysis of 4 cases

Objective To investigate the relationship between cerebral thrombosis and iron-deficiency anemia, and their clinical manifestations and therapy. Methods The clinical data of 4 patients with iron deficiency anemia complicated by cerebral thrombosis were analyzed retrospectively. Results Four patients were moderate-to-sexere anemia with thrombocytosis, and their blood pressure was relatively low. Two had chronic infection foci. All patients had cerebral infarction in the internal carotid system, of those, 2 were weatershed infarctions, and 2 were basal ganglia infarctions. Oxygen inhalation, correcting anemia, adjusting blood presssure to an appropriate level, and other conventional treatment were effective. Conclusions The patients with moderate-to-severe anemia, such as accompanied by thrombocytosis and lower blood pressure, are easy to came thrombosis. While in the routine treatment of cerebral thrombosis, attention should be paid to finding the causes of anemia and correcting them.
10.Observation of the effect of bilevel positive airway pressure on patients with obstructive sleep apnea hypopnea syndrome complicated with stroke

Objective To compare the effect between bilevel positive airway pressure (BiPAP) and routine drugs treatment to patients with obstructive sleep apnea hypopnea syndrome ( OSAHS ) complicated with stroke. Methods Twenty-nine patients with OSAHS complicated with stroke were randomly divided into two groups, 15 cases in the treatment group and 14 cases in the control group. Patients in the treatment group were applied BiPAP and routine drugs including degrading intracranial pressure, promoting blood flow, improving microcirculation. BiPAP were maintained for two months and over 10 hours daily. The control group was executed only routine drugs treatment. Results The difference of apnea hyponea index( AHI) and frequency of below 90% O2 saturation during sleep time between pre-therapy and post treatment was significantly higher in the treatment group(36. 8 ± 10. 7 and 105. 3 ± 17. 9) than in the control group (20. 8 ± 8. 1 and 44. 4 ± 24. 3 ) ( P < 0. 01 ) . The difference of lowest nighttime SaO2 between pre- and post-therapy was significantly higher in the treatment group( - 11. 7 ±8. 1)% than in control group(-2. 0 ± 8. 9) % ( P <0. 01). The difference of neurologic impairment score was significantly higher in the treatment group ( 16. 0 ± 2. 6) than in the control group ( 10. 1 ± 3. 6) ( P < 0. 01 ) . As for whole blood viscosity and red blood cell aggregation index,there was no significant difference between pre- and post-therapy(P> 0. 05). Conclusions BiPAP combined with routine drugs can significantly improve sleep anapnea indexs and neurologic impairment scores in patients with OSAHS complicated with stroke than only drug treatment. Blood rheology's indexs could not obtain amelioration in short period.
