1.Thinking of Teaching Model for Endocrinology Speciality in Community Clinical Pharmacists Training Base

OBJECTIVE: To provide reference for standardization and improvement of teaching model for endocrinology speciality in community clinical pharmacists training base. METHODS: Based on the requirments of community clinical pharmacists training in Shanghai, the characteristics and contents of community clinical pharmacists training were summarized, and teaching characteriatiostics, effect evaluation and problems werer analyzed on the basis of the acutual situation in our hospital. RESULTS & CONCLUSIONS: Community clinical pharmacists training has the characteristics of training for chronic diseases, short training cycle, and support from the medical consortium platform. Its contents are theoretical learning and professional knowledge training, pharmaceutical care and medication education, case discussion, homework guidance and comment. Compared with other specility, endocrinology speciality has the characteristics of strengthening comprehensive pharmaceutical care (for example, pharmaceutical care of diabetic patients should pay attention not only to hypoglycemia, but also to hypobaric, hypolipidemic and complications), emphasizing medication treatment management (MTM) teaching (provide a series of professional services such as medication education, counseling and guidance, and medical reorganization for patients, so as to improve the safety, effectiveness and compliance of medication, and then control blood glucose, delay or avoid complications), emphasizing the cultivation of health education ability and communication ability. The evaluation of training effect included the evaluation of trainees and teachers. The former includes objective evaluation (theoretical knowledge test, bedside medication education assessment and homework assessment) and subjective evaluation (attendance, attitude, clinical practice performance). The latter is mainly scored by trainees on teachers (including professional level, teaching attitude, teaching ability and content). Through the training, it is found that the trainees’ literature retrieval ability is poor, the trainees have not clear idea to diabetic patients, the homework writing and typesetting are not standardized; the teaching teachers have less experience and teachers could not teach the students in accordance with their aptitude and provide individualized training. The training and teaching mode needed to be further improved.

3.Viral shedding in day-care children and students infected with norovirus in outbreaks

Objective:This study aimed to investigate the time duration of norovirus shedding among day-care children and students during norovirus outbreaks, as well as influencing factors affecting the viral shedding. Methods:Suspected cases of norovirus infection and their close contacts were collected from child care and school settings during norovirus outbreaks in Xuhui District, Shanghai, from 2017 through 2019. Specimens were detected using real-time RT-PCR to determine whether children had been infected with norovirus. Subsequently, further specimens were collected every 3-7 days from infected children until specimens tested negative for norovirus. Results:A total of 76 outbreaks were reported involving 1 014 suspected cases. In the 421 suspected cases, 311 confirmed cases were diagnosed after examination. Furthermore, a total of 58 confirmed cases participated in this study with informed consent, with a participation rate of 18.65%. The average time duration of norovirus shedding was (16.24±13.80) days, in which 79.31% had viral shedding more than 7 days, 37.93% more than 14 days and 17.24% more than 21 days. 4.The epidemiology and risk factors of delayed diagnosis in newly reported HIV/AIDS cases in Xuhui District, Shanghai, 2018‒2022

ObjectiveTo investigate the trends and significant determinants of delayed HIV diagnosis (DHD) among newly reported HIV/AIDS cases in Xuhui District, Shanghai between 2018 and 2022. MethodsIn the newly reported HIV/AIDS cases, patients died within one year without accident, HIV/AIDS cases with CD4 cell count <200 cells·μL-1, and AIDS cases with a CD4 cell count between 200 to 499 cells·μL-1 were defined as delayed diagnosis. Univariate and multivariate logistic analysis were employed to explore the influencing factors of DHD. ResultsAmong the 862 newly reported HIV/AIDS cases, The DHD rate was 39.79% without statistically significant difference by year(χ2=4.508, P=0.342). Patients with CD4 cell count <200 cells·μL-1 contributed the largest proportion of DHD. During 2018‒2022, the DHD rate declined among HIV/AIDS patients who were younger than 35 years old or 45‒65 years old, never married, original diagnosis from tertiary specialized hospitals. Patients who were 65 years or older, married or divorced, with heterosexual transmitted HIV/AIDS, and original diagnosis from other types of testing and tertiary metropolitan hospital, had sustainably higher DHD rates. The number of HIV screening and diagnosed from voluntary counseling and testing (VCT) decreased during the COVID-19 epidemic, while the DHD rate increased sharply. Multivariate logistic regression analysis suggested the DHD rates were higher among older age, other types of testing(OR=3.805, 95%CI: 2.260‒6.406)and pre-operative testing(OR=2.411, 95%CI: 1.424‒4.081). Patients who received CD4 test in 15 days had a higher DHD rate compared to the cases received CD4 test exceeding 90 days (OR=0.336, 95%CI: 0.216‒0.522). ConclusionThere is no significant decrease of delayed HIV diagnosis rate in Xuhui District in recent years, and the number of HIV tests has decreased in 2022. Monitoring of newly reported HIV/AIDS should be conducted continuously. Expansion of HIV antibody screening should be conducted in non-infectious departments and inpatient departments in healthcare institutions, particularly metropolitan hospitals. Assistance should be provided to clinicians and elderly patients for improving their ability to recognize and perceive the risk of HIV/AIDS, in order to enhance early diagnosis and subsequent treatment.
5.Epidemiological characteristics and trend analysis of influenza-like cases in Xuhui District of Shanghai from 2010 to 2019

ObjectiveTo analyze the epidemiological characteristics and incidence trend of influenza-like illness in Xuhui District from 2010 to 2019, in order to understand the patterns of influenza epidemic of Xuhui District,and to provide evidence for the prevention and control of influenza in the future. MethodsSurveillance data of influenza-like illness (ILI) in Xuhui District from 2010 to 2019 were collected from the National Influenza Surveillance System for statistical description and trend analysis. ResultsFrom 2010 to 2019, the overall proportion of influenza-like illness was 0.54%. The annual proportions of ILI were between 0.35%~1.14%.The majority of cases were reported in age group of 25~59, accounted for 54.50% of the total ILI. A total of 9 053 throat swab specimens from ILI patients were collected and tested, from which 2 137 specimens were positive, with a positive rate of 23.61%. The most frequent subtype of influenza virus detected was influenza A virus (accounting for 67.62%). The proportion of ILI and the positive rate of influenza virus nucleic acid detection reached the peak in summer and winter over time. 6.Consistency of Different Intensity Setting Methods of Aerobic Exercise for Coronary Heart Disease

Objective To explore the consistency of different intensity setting methods of aerobic exercise in order to provide the safe and effective strength setting method for patients with coronary heart disease (CHD).Methods From July, 2016 to July, 2017, CHD patients in our hospital were recruited. The basic data were collected. The patients received cardiopulmonary exercise test firstly to obtain anaerobic threshold, peak oxygen intake and other indicators. They underwent Six-minute Walk Test after a day off. The target heart rate was calculated using anaerobic threshold method (AT), peak oxygen uptake index method (PI), heart rate reserve method (HRR), age estimates method (AE) and quiet heart rate plus 20 method (HR20), respectively. The correlation and consistency of different methods were analyzed.Results There was no correlation (r=-0.131, P>0.05) and there was a very significant difference (P<0.001) between AE and AT on determining target heart rate. There was no correlation (r=0.372, P>0.05) between HR20 and AT. There was correlation (r=0.872, P<0.01) and there was no significant difference (P>0.05) between PI and AT on determining the target heart rate, and the Bland-Altman test showed a consistency between them. There was correlation (r=0.836, P<0.001) and there was no significant difference (P>0.05) between HRR and AT on determining the target heart rate, and Bland-Altman test demonstrated a consistency between them.Conclusion There was no correlation and consistency of AE and HR20 to AT on the target intensity of aerobic exercise. There was significant correlation and consistency between AT and HRR/PI.
7.Latent tuberculosis infection and screening method among close contacts in schools in Xuhui District,Shanghai from 2020 to 2022

ObjectiveTo investigate the latent tuberculosis infection (LTBI) of close contacts in schools of Xuhui District, and to explore the tuberculin skin test (TST)- interferon-γ release assay (IGRA) two-step method in order to discover the screening strategy of tuberculosis in Xuhui District. MethodsClose contacts of tuberculosis in schools of Xuhui District from 2020 to 2022 were selected as research subjects. Screening was conducted using symptom questionnaire, TST, chest X-rays, IGRA, and the information including the etiological results and grade of the index cases, as well as gender, age, and relationship with the index cases of the research subjects were collected. ResultsTotally 615 close contacts of 32 tuberculosis cases occurred in the schools were finally included. Of the 609 close contacts who completed tuberculosis infection screening and underwent TST testing, 153 TST(+) individuals underwent IGRA testing. The final LTBI rate was 4.6%, and the pulmonary tuberculosis detection rate was 163 per 100 000. The relationship with the index cases was an influencing factor for LTBI. The IGRA positivity rate was higher among close contacts with TST ≥15 mm than among those with 10 mm≤ TST <15 mm (χ2=14.41, P<0.05). ConclusionThe latent tuberculosis infection among close contacts of school tuberculosis cases in Xuhui District remains serious. TST-IGRA two-step method can assist in the accurate diagnosis of LTBI and pulmonary tuberculosis cases.
8.The clinical characters and acoustic rhinometry analyses of 98 cases fugal ball sinusitis.

OBJECTIVE:
To improve the cognition of fungal ball sinusitis (FB) for diagnosing and treatting it rightly, we retrospectively analyzed the hospitalization cases.
METHOD:
We analyzed 98 cases FB patients' first symptom, acoustic rhinometry, CT manifestation, operation fashion.
RESULT:
The first symptoms of FB include: 47.2% nosebleed, 38.8% snuffle, 28.6% headache and face-ache, 4.0% nasal peculiar smell, 2.0% excessive tear, 4.0% no symptom. Anterior nasoscope inspection include: 43.9% no significant abnormity, 13.3% nasoseptal deviation. 41.8% nasal cavity neoplasm (nasal polyp, nasoturbinal polyp, nasal meatus fugal ball etc), 25.5% secretion in nasal cavity. The parameters that conclude the nasal airway resistance (NAR), nasal cavity volume (NCV) and nasal minimal cross-section area (NMCA) have extremely difference (P<0.01) between FB and normal volunteers (NV): the NAR increase, but NCV and NMCA decrease; the Distance of the Minimal cross-sectional area from the nostril (DCAN) appear moving backword compared with NV. The CT manifestation: almost unilateral lesions, involved paranasal sinuses present asymmetric or symmetrical opacification: 60.2% with calcified area, 25.5% with local bone erosion, 74.5% with sinus wall sclerosis. 65.3% of the CT diagnoses as FB are accord with pathology diagnoses. The incidence of FB involve maxillary sinus most (54.1%), then sphenoid sinus (11.2%) and ethmoid sinus (5.1%), frontal sinus (2.0%), multi-sinus (32.7%). All cases operated functional endoscopy surgery: 13 cases with nasoseptal diorthosis, 4 cases with inferior meatus opening, 2 case with canine fossa opening; we clean up the fungal pathogen with different degree endoscope through different path.
CONCLUSION
The clinical symptom of FB is not representative; the acoustic rhinometry show that NAR increasing but NCV and NMCA decreasing maybe the reason of fungal infection; the CT examine is helpful for FB diagnosis; the functional endoscopy surgery is efficient therapy for FB.

9.Intervention effect of "Internet plus" postpartum health care service on postpartum depression

ObjectiveTo explore the influence of "Internet plus" postpartum health care service on postpartum depression, and to provide an evidence for reducing the incidence of postpartum depression and improving the quality of life for postnatal women. MethodsMothers who gave birth between August 1, 2021 and June 30, 2023 lived in the six streets of Xuhui District were selected as the research subjects. The puerperants were randomly divided into the intervention group and the control group according to the order of enrollment using a numerical numbering system, with 210 cases in each of the intervention group and control group. The intervention group adopted the "Internet plus" postpartum health care services, while the control group adopted the conventional postpartum health care services. The risk factors related to postpartum mental health and the incidence of postpartum depression were compared between the two groups. ResultsThere was no statistically significant difference in the scores of postpartum mental health-related risk factors between the two groups at the first 2 weeks and 1st month after delivery (t=0.736 and t=1.260, P>0.05). However, there was a statistically significant difference in the scores of postpartum mental health-related risk factors at the 3rd months after delivery and 6th months after delivery (t=2.089, P<0.05; t=2.655, P<0.05). There was no statistically significant difference in depression scores at the first 2 weeks after the birth (t=0.560, P>0.05). In the 1st month, the 3rd month and the 6th month after delivery, the intervention group adopted the "Internet plus" postpartum health care service, thus the differences in depression scores were statistically significant (t=2.616, t=2.793 and t=3.107, P<0.05). Conclusion"Internet plus" postpartum health care service is conducive to reducing postpartum depression, promoting maternal mental health, and significantly improving the well-being of postnatal women.
10.Incidence and related influencing factors of parturients with postpartum depression in Xuhui District, Shanghai

ObjectiveTo understand the occurrence of postpartum depression among puerperae in six communities in Xuhui District, Shanghai, and to analyze the related influencing factors of postpartum depression, so as to provide evidence for corresponding intervention measures. MethodsA total of 476 puerperae who gave birth from August 1, 2021 to February 28, 2022 in 6 communities in Xuhui District were selected as the survey subjects. They filled out a general situation questionnaire, Edinburgh Postpartum Depression Scale (EPDS) and a questionnaire on related influencing factors. Univariate and multivariate unconditional logistic regression analysis was conducted on the related influencing factors of postpartum depression. ResultsAccording to the EPDS score, 72 of the 476 puerperae in Xuhui District developed postpartum depression, and the incidence of postpartum depression was 15.13%. Multivariate unconditional logistic regression analysis showed that maternal introversion (OR=2.845, 95%CI:1.033‒7.842), artificial feeding (OR=11.448,95%CI:2.438‒53.767), postpartum insomnia (OR=2.559, 95%CI:1.244‒5.262), lack of postpartum parenting experience (OR=2.324,95%CI:1.121‒4.820), and family members caring about the gender of the child (OR=4.415, 95%CI:1.500‒12.994) were independent risk factors of postpartum depression, while the spouse being an only child(OR=0.434, 95%CI:0.224‒0.840) was a protective factor for postpartum depression. ConclusionPostpartum depression in Xuhui District is affected by many factors. Screening and intervention of high-risk groups should be strengthened to reduce the incidence of postpartum depression.
