ObjectiveTo study the effects of Tanreqing injectio (a Chinese herb preparation acts as an anti-inflammatory agent to eliminate the pulmonary infection) on inflammatory cytokines of rats with acute lung injury (ALI). MethodsFifty-six clean grade healthy male SD rats were randomly (random number) divided into three groups: normal group, model group and treatment group. Rats in the model group and treatment group were injected with lipopolysaccharide (LPS) into tail vein. Rats in treatment group were treated with Tanreqing injection one hour after LPS injection. The observing intervals were respectively set in 2 h, 4 h and 6 h. At each observing interval, TNF-α, IL-1 β and IL-8 in brochoalveolar lavage fluid (BALF) were detected by radioimmunoassay, and the ratio of polymorphonuclear neutrophiles (ωPMN) in BALF were calculated by Wright-Giermsa staining, as well as to observe the pathological changes of lungs and to examine the lung wet/dry weight ratio (W/D). Data were analyzed with SPSS version 17.0 software. ResultsAt 2, 4 and 6 h intervals, TNF-α, IL-1β, IL-8 and ωPMN in BALF were significantly higher in model group than those in normal group (P ＜0. 05 or P ＜0. 01 ), the lung W/D in model group were obviously higher than those in normal group ( P ＜ 0. 01 ), and the pathological injury of lung tissue in model group were severe. At each observing interval, compared with model group,TNF-o, IL-1 β, IL-8 and oPMN in BALF were significantly reduced in treatment group ( P ＜ 0. 05 or P ＜0.01 ), the lung W/D in treatment group were obviously decreased ( P ＜0.01 ), and the lung injury were attenuated in treatment group. ConclusionsTanreqing injection could provide partly protection in rats with ALI by inhibiting inflammatory factor.

Objective: To investigate the prevalence and types of maternal near miss in Hangzhou City from 2015 to 2020, so as to provide insights into early identification and interventions for maternal near miss. Methods: The data of maternal near miss in 5 hospitals of Hangzhou City from 2015 to 2020 were collected from China's National Maternal Near Miss Obstetrics Surveillance System, and the incidence and mortality of maternal near miss, types of pregnancy-related complications/comorbidities and types of critical illness were descriptively analyzed. Results: Totally 230 490 pregnant and lying-in women were recruited in 5 hospitals in Hangzhou City from 2015 to 2020, and there were 182 514 live births, 177 428 women with pregnancy-related complications/comorbidities, and 1 790 cases of maternal near miss with a 0.98% annual incidence rate. The incidence of maternal near miss was high in 2019 (1.18%). The hospital mortality and death index were respectively 3.29/105 and 0.33% among pregnant and lying-in women from 2015 to 2020, and there was no death found among pregnant and lying-in women from 2018 to 2020. The main pregnancy-related complications/comorbidities included anemia (79.33%), obstetric hemorrhagic disorders (69.83%), and gestational hypertension (16.65%). The main critical illness among maternal near miss included coagulation dysfunction (87.23/104), cardiovascular dysfunction (20.44/104), and respiratory dysfunction (9.70/104). Conclusions The incidence of maternal near miss was high and the hospital mortality appeared a reduction tendency among pregnant and lying-in women in Hangzhou City from 2015 to 2020. Coagulation dysfunction is the primary critical illness among maternal near miss, and the management of anemia and hemorrhagic diseases requires to be improved.

Objective To analyze the bacterial distribution and resistance of clinical isolates from exacerbation of COPD patients in C and D classification in our emergency department.Methods The sputums were taken to culture and antimicrobial susceptibility test.VITEK-2 compact automicrobe system was used to identify bacteria.Disc diffusion test was used to determine the antimicrobial resistance.Results Of 222 clinical isolates,27.0％ were Gram positive bacteria,73.0％ were Gram negative bacteria.The Gram positive bacteria mainly was Staphylococcus aureus and Streptococcus pneumoniae.The detection rates of methicillin-resistant Staphylococcus aureus (MRSA) were 50.00％.The MRSA were more resistant than methicillin-sensitive Staphylococcus aureus (MSSA),and they were all sensitive to vancomycin,linezolid or tigecycline.The Gram negative bacteria mainly was Pseudomonas aeruginosa,Escherichia coli,Klebsiella spp and Acinetobacter baumannii.Pseudomonas aeruginosa and Acinetobacter baumannii had high resistant rates.The extended spectrum β-lactamases (ESBLs) producing strains in Escherichia coli were higher than in Klebsiella spp,which had multiple drug resistance.Conclusions The bacterial resistance situation is very severely in exacerbation of COPD patients in C and D classification in our emergency department.The first isolating rate was Pseudomonas aeruginosa.Surveillance of bacterial resistance is very important and effective control measures to reduce resistance is very necessary.

BACKGROUND: The marginal division, a new sub-region in rat brain striatum discovered in recent decades, has been found to closely relate to learning and memory function of the brain. The immediate-early genes such as c-fos and c-jun participate in the signal transduction of learning and memory in the marginal division. But what other intermediate events are initiated in the marginal division in the process of learning and memory?Cyclic adenosine monophosphate (cAMP) response element-binding protein (CREBP) is an molecule essential for the formation of long-term memory,and investigation of the expression and distribution of phosphorylated CREBP in the striatum may help understand the signal transduction mechanism in the striatum during learning and memory at the molecular level.OBJECTIVE: To investigate the expression of phosphorylated CREBP in rat brain stratum during learning and memory process.DESIGN: Completely randomized controlled study.SETTING: Institute of Neurosciences, Zhujiang Hospital of the First Military Medical University of Chinese PLA.MATERIALS: This experiment was conducted in the Institute of Neurosciences, Zhujiang Hospital of the First Military Medical University of Chinese PLA between April and August 2003. Totally 48 normal male adult SD rats were provided from the Experimental Animal Center of First Military Medical University, and after two Y-maze tests, 40 rats were selected for this study (MG-2 type, Sanshengxing electricity company).METHODS: The 40 SD rats were randomized into 4 equal groups. The rats in the first group were subjected to training to acquire dark avoidance reflex in a Y maze, those in the second group underwent sham training with only light stimulation in the Y maze without electricity on the floor.The rats in the third group were trained in the Y maze with electricity on the floor but not light stimulations, with the rest 10 rats serving as the control group subjected to sham training in the Y maze without either electric or light stimulations. After the training in the Y maze, the rats were sacrificed immunohistochemical detection of phosphorylated CREBP expression in the brain striatum.MAIN OUTCOME MEASURES: Expression of phosphorylated CREBPin rat brain striatum.RESULTS: All the 40 rats enrolled in this study were examined for phosphorylated CREBP expression. Positive expression of phosphorylated CREBP was observed in the medial marginal division of the brain striatum after the rats were trained in the Y maze with electric stimulation, but no obvious expression was seen in rats in the sham training or control groups.Massive expression of phosphorylated CREBP could be observed, typically,in the hippocampus, front prefrontal lobe cortex and cingulate gyrus of the rat brain.CONCLUSION: The transcriptional factor phosphorylated CREBP in the marginal division of the striatum participates in the signal transduction for learning and memory in rats receiving Y maze training to acquire dark avoidance reflex.

Objective To study the antibacterial property of silver loaded titanium dioxide (TiO2) antibacterial coated endotracheal intubation tube,and to determine the minimum effective antibacterial concentration.Methods Intubation tubes coated with different concentrations of antibacterial agents were prepared with sol gel method.Polyethylene endotracheal intubation tubes were used as substrate,and silver loaded TiO2 was used as the antibacterial agent.According to the different antibacterial concentrations of the antibacterial agent,the tubes were divided into nine groups:10.0％ group,5.0％ group,2.0％ group,1.5％ group,1.0％ group,0.8％ group,0.6％ group,0.2％ group,and conrol group.They were respectively immersed in three standard bacteria suspensions with 1.0 × l05 cfu/mL:Pseudomonas aeruginosa,Staphylococcus aureus,and Escherichia coll Together with standard bacteria liquid group,there were 10 experimental groups.They were kept overnight for 24 hours.10 μL of respective culture medium was smeared on blood agar culture medium.After being cultured overnight in 35 ℃,the number of bacteria colonies was respectively counted.Results In 1.0 × 105 cfu/mL of three standard bacteria liquids with antibacterial agent concentration≥ 1.0％,three bacterial colonies had un-obviously growth rate.Almost the same strong antibacterial effects to achieve sterilizing rates of more than 98％ was shown in each group of the antibacterial coating endotracheal intubation tubes (all P＞0.05).As the antibacterial agent concentration decreased,three bacterial colonies were increasing gradually.Intermediate antibacterial effects were shown in tubes of 0.8％ group,with significant statistic difference as compared with 1.0％ and 0.6％ groups [Pseudomonas aeruginosa:7.300 (4.050,8.350) vs.0.200 (0.050,1.200),9.700 (9.000,10.000); Staphylococcus aureus:4.100 (3.300,4.650) vs.0.000 (0.000,0.150),5.800 (5.350,7.650); Escherichia coli:1.400 (0.750,3.750) vs.0.050 (0.025,0.050),9.500 (8.500,9.800),all P＜0.01].Conclusions Silver loaded TiO2 antibacterial coated endotracheal intubation tube had definite antibacterial properties,which were related to the antibacterial concentration.Strong antibacterial effects were shown when antibacterial concentration was above 1.0％,with bacteria almost completely killed in the immersing liquid.

Objective : To explore the influencing factors for maternal anemia during the third trimester of pregnancy, so as to provide the evidence for the effective prevention of anemia. Methods : Data of parturients, who received prenatal care and delivered at Hangzhou Women＇s Hospital from October to December of 2019 were retrospectively collected and analyzed. The obstetric electronic medical record system of this hospital was used to collect general information of the subjects. Serum ferritin, serum vitamin B12, serum folic acid and hemoglobin were determined at the second and third trimester. A multivariate logistic regression model was used to analyze the influencing factors for anemia in the third trimester. Results : A total of 1 143 parturients were enrolled, with a median age of 29 ( interquartile range: 4 ) years. Among them, 1 050 cases had normal weight before pregnancy, accounting for 91.86%; 267 cases were multiparas, accounting for 23.36%; 74 cases of anemia occurred during the third trimester of pregnancy, accounting for 6.92%.Multivariate logistic regression analysis indicated that the multipara（OR=1.714,95%CI: 1.032-2.848）, iron deficiency duiring the second trimester（OR=3.301,95%CI: 1.401-7.781）, iron deficiency during the third trimester （OR=14.134,95%CI: 1.944-102.774）and folate deficiency during the third trimester（OR=2.960,95%CI: 1.295-6.766）were influencing factors for anemia during the third trimester. Conclusion Anemia during the third trimester of pregnancy is related to iron deficiency during the second and third trimesters, folic acid deficiency during the third trimester, and multipara.

Objective To observe the tendency of the plasma concentration of plasminogen activator inhibitor type-1 (PAI-1) and thrombin activatable fibrinolysis inhibitor (TAFI) before and after thrombolytic treatment of acute ST elevation myocardial infarction (STEMI) and to explore their recanalization predictive value of PAI-1 and TAFI for acute myocardial infarction patients with thrombolytic treatment.Methods Sixty patients,who received thromobolytic treatment from January 2007 to March 2009,were prospectively recruited.The blood sample were collected within 2 hours of thromobolytic treatment ( 0,0.5 h,1 h,1.5 h and 2 h).The plasma concentration of TAFI and PAI-1 were test by ELISA.16 healthy people were recruited as control group.Results The plasma levels of PAI-1 in STEMI patients before thrombolytic treatment were higher than those of Control group ( P ＜0.01 ),however the same significant change of TAFI level was not seen.The levels of TAFI were no significant difference before and after thrombolytic therapy during whole observation periods.However,the level of PAI-1 increased at 1.5 h and 2 h after thrombolytic therapy (P ＜ 0.01 ).The plasma PAI-1 levels of no - revascularigation group at 2 h after thrombolytic therapy were significant higher than that in revascularization group ( P ＜ 0.05 ).The levels of TAFI were not significantly different between two groups (P ＞ 0.05).Conclusions The decrease of plasma PAI-1 from high level within 2 hours after thrombolysis treament may be exploring the predictive value for revascularization.The tendency of TAFI can' t forecast the result of revascularization.

OBJECTIVE: To investigate the expression of FLIP and PTEN in laryngeal Squamous cell carcinoma (LSCC), and the relationship between FLIP and PTEN. METHOD: The protein expression of FLIP and PTEN were examined by using immunohistochemical method in 45 cases of LSCC and 15 cases of para-carcinoma tissues. RESULT: FLIP protein positive expressive rate in the laryngeal squamous cell carcinoma cases (77.8%) was higher than that in the para-carcinoma tissues cases (33.3%, P < 0.05). The protein expression of FLIP was correlated with cervical lymph node metastasis, clinical stage and prognosis. On the other hand, PTEN positive expressive rate in the laryngeal squamous cell carcinoma (65.0%) was higher than that in the para-carcinoma tissues (0%, P < 0.01). The protein expression of PTEN was associated with tumor differentiation grade, clinical stage, cervical lymph node metastasis and prognosis. There was a negative relationship between the expression of FLIP and PTEN in LSCC. CONCLUSION The protein expression of FLIP may be an important prognostic marker for LSCC. The protein expression of PTEN was correlated with clinical stage, tumor differentiation grade, and cervical lymph node metastasis . Consequently, it could be used as a valuable marker for the prognosis of LSCC.

[Objective]To summarize the research and understanding on the role of inflammatory factors in the onset of type-2 diabetes(T2DM)at home and abroad,the correlation between damp-heat syndrome and inflammation in T2DM,and the experience of traditional Chinese medicine(TCM)in treating T2DM from damp-heat syndrome,so as to provide new ideas for the treatment of T2DM.[Methods]Reviewing relevant information on T2DM,reading relevant literature,summarizing the research results on the role of inflammatory factors in the pathogenesis of T2DM at home and abroad,summarizing the correlation between damp-heat syndrome and inflammation in TCM,and summarizing the experience of treating T2DM from the perspective of damp-heat in TCM.[Results]The onset of T2DM is closely related to inflammatory factors,inflammatory factors such as tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)in the serum of T2DM patients are higher than normal levels.The role of inflammatory factors in the pathogenesis of T2DM includes hindering normal tyrosine phosphorylation in the insulin receptor substrate(IRS),accelerating the dissociation and degradation of IRS and phosphatidylinositol 3-kinases(PI3K),affecting downstream PI3K phosphorylation,interfering with insulin signaling through the pathway,and triggering insulin resistance(IR);promoting nuclear factor-κB(NF-κB)activation continuously exacerbates the inflammatory state through a positive feedback mechanism,promoting IR;directly or indirectly causing pancreatic islets β cellular destruction and decreasing insulin secretion function.TCM syndrome differentiation of T2DM patients is mostly characterized by damp-heat syndrome,the levels of IL-6 and hypersensitive-C reactive protein(hs-CRP)in T2DM damp heat syndrome are higher.Lipid metabolism disorders,IR and inflammation are more severe.There is a strong correlation between T2DM damp-heat syndrome and the inflammatory state of the body.TCM can reduce the level of inflammatory factors in the body by using various TCM formulas and single drugs to treat T2DM damp-heat syndrome.[Conclusion]There is a correlation among T2DM,inflammation and damp-heat syndrome.T2DM mostly belongs to damp-heat syndrome.The use of TCM formulas for clearing heat and removing dampness and single drugs is effective in treating T2DM damp-heat syndrome,which is a new approach to treat T2DM from the perspective of damp-heat theory.

This study was to assess the chronic morbidity and metabolic disordelters in 262 patients with type 2 diabetes.Of all participants,64(24.4%)coexisted with peripheral neuropathy,34(13.0%)combined with peripheral vascular disease.41(15.6%)were diagnosed as diabetic retinopathy,and 46 (17.6%)had concurrent diabetic nephropathy.In comparison with diabetic patients without these complications,those with the chronic conditions generally had higher plasma glucose,blood pressure or body mass index.
