1.Clinical efficacy and safety of leflunomide combined with irbesartan in lupus nephritis

Objective To study the clinical efficacy and safety of leflunomide combined with irbesartan in the treatment of lupus nephritis . Methods 80 cases of patients with lupus nephritis in Yueqing Hospital Affiliated to Wenzhou Medical University from May 2014 to May 2016 were selected and randomly divided into leflunomide group ( leflunomide combined with irbesartan group ) and cyclophosphamide group ( cyclophosphamide combined with irbesartan group),40 cases in each group.The urine indexes and blood indexes levels,clinical curative effect,adverse reaction of the two groups were statistically analyzed.Results The 24h urine protein,urine beta 2-MG,urine red blood cell count,blood beta beta2-MG,ESR,Cr levels of the leflunomide group were significantly lower (P<0.05),the serum albumin,C3 levels were significantly higher (P<0.05),the total treatment efficiency 97.5%was significantly higher than the cyclophosphamide group 82.5%(P<0.05),the incidence of adverse reactions 5.0%was significantly lower than the cyclophosphamide group 22.5%(P<0.05).Conclusion Leflunomide combined with irbesartan is safe and effective in the treatment of lupus nephritis.
2.Comparative Studies on the Anti- inflammatory and Analgesic Actions of Huang qi Guizhi Wuwu Decoction and Its Compositions

Objective To investigate the anti- inflammatory and analgesic a ctions of Huangqi Guizhi Wuwu Decoction (HGWD) and its compositions and to study its rule of compatibility.Methods Analgesic action of HGWD and its compositio ns were observed by hot- plate method and acetic- acid- induced body twist me thod. Anti- inflammatory action of HGWD and its compositions were observed on t he models of xylene- induced auricular swelling in mice, celiac capillary perme ability in mice, cotton- induced granuloma in rats, albumem- induced arthritis and adjuvant arthritis in rats.Results HGWD and its drug pairs could inhibit the acute inflammation induced by albumen and xylene and rat adjuvant arthriti, decrease the celiac capillary permeability, inhibit the proliferation of granu loma, increase the pain threshold in mice and reduce the frequencies of body tw ist induced by acetic acid.Conclusion HGWD has significant anti- inflammatory and analgesic actions, so does single drug; Radix Astragali alone, but when Ra dix Astragali is used with the other drugs in HGWD, its effect can be enhanced.
3.Effect of the deferoxamine mesylate on the prognosis of patients with intracerebral hemorrhage

Objective To study the effect of the deferoxamine mesylate on the prognosis of patients with intracerebral hemorrhage(ICH) after one year. Methods From February 2013 to May 2014,spontaneous ICH patients diagnosed by computed tomography ( CT ) within 18 hours of onset in Mancheng District Hospital of Baoding were evaluated. Patients were randomly divided into experimental group and control group. The treatment of the two groups was similar except that the experimental group received deferoxamine mesylate. Patients were e?valuated by CT and neurology scale( NIHSS scale,GCS scale) at the time of admission and followed up for the first year by the RANKIN( mRS) scale. All clinical data of the two groups were compared. Results Forty?two patients were included in the study, including 21 cases in the experimental group and 21 cases in the control group,there was no significant difference in baseline data between the two groups at admission. There were 6 pa?tients with mRS ≥3 in the experimental group, and 6 patients with mRS ≥3 in the control group after one year. There was no statistically significant difference in the distribution of mRS score between the two groups after one year admission( P=1. 000) . Conclusion There may be no helpful on the prognosis of patients with intrace?rebral hemorrhage by intravenous infusion of deferoxamine mesylate,the further study is needed.
4.Preparation of SiO2/Ni core-shell type nanoparticles and their magnetic properties

BACKGROUND: Inhibitory coating can prevent nanoparticle oxidation, grain growth, corrosion and agglomeration, and endow nanoparticle with special properties. ABJECTIVE: To prepare SiO2/Ni core-shell type nanoparticles and assess their magnetic properties. DESIGN, TIME AND SETTING: The observation experiment was performed between November 2005 and March 2006 at Nanometer Compound Material Research Laboratory of Dalian University of Technology, Dalian, Liaoning Province, China. MATERIALS: Nanometer nickel powder prepared by DC arc plasma jet method, Na2SiO3 produced by Bazhou Chemical Industry Branch Factory of Tianjin Quartz Clock Factory (China).METHODS: SiO2/Ni core-shell type nanoparticles were synthesized by coating a layer of SiO2 on the surface of manometer nickel powder via liquid deposition method using Na2SiO3 as the main source material. MAIN OUTCOME MEASURES: Their microstructures and material properties were investigated by X-ray diffraction, Fourier transform infrared spectrometer, transmission electron microscopy, thermo-gravimetric analysis, differential scanning calorimetry and vibrating sample magnetometer. RESULTS: The experimental results showed that SiO2 shell was in an amorphous state around Ni cores and it avoided agglomeration of the Ni nanoparticles. The oxidation temperature of nanometer nickel powder coated by SiO2 elevated from 287 ℃ to 385 ℃. The analysis result of magnetic properties indicated that the hysteresis loop of Ni had an excursion for the existence of anti-ferromagnetic NiO, the silica coating reduced the saturation magnetization and improved the coercivity. CONCLUSION: Preparation of SiO2/Ni core-shell type nanopartieles was successful; silica coating improved the oxidation resistance of nanometer nickel powder, endowed nanometer nickel powder better ferromagnetism and improved the coercivity.
5.The therapeutic effect of NeuroD on radiation-induced intestinal injury in mice

Objective To evaluate the therapeutic effect of NeuroD protein on radiation-induced intestinal injuries.Methods The expression and purification of NeuroD-enhanced green fluorescent protein (EGFP) fusion protein was performed in prokaryotic expression system.The efficiency of the fusion protein transduction into cells was monitored under fluorescence microscope.C57BL/6J mice were randomly divided into four groups with 10 mice in each group:normal control group,PBS group,EGFP group,and NeuroD-EGFP group.Besides the normal control group,the other three groups of mice received 9 Gy γ-ray total body irradiation.Intestinal tissues were collected,frozen sections were prepared to monitor the distribution of NeuroD in mice intestinal tract under fluorescence microscope,and pathological sections were prepared for H&E staining to evaluate the therapeutic effect of NeuroD protein.Results The NeuroD-EGFP fusion protein was purified by Ni-NTA column and verified by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE).Visible green fluorescence gathered within the cells after NeuroD-EGFP fusion protein was added in the culture medium,suggesting that NeuroD-EGFP could penetrate the cell membrane into the cells.Five hours after intraperitoneal injection of NeuroD-EGFP,visible green fluorescence gathered within the intestinal epithelial cells in villi.At 3.5 d after irradiation,NeuroD-EGFP treated mice showed significantly higher villus (F =49.49,P < 0.01) and crypt depth (F =16.72,P < 0.01) and more crypts per circumference (F =10.32,P < 0.01) compared with PBS and EGFP groups.Conclusion NeuroD protein can accelerate the post-irradiation recovery of injured villi and crypt of intestinal tract and could be used to treat radiation-induced intestinal injuries.
6.Preliminary comparative study between elective nodal irradiation and involved field radiation therapy for clinical early-stage esophageal carcinoma

Objective: This study was performed to analyze the feasibility of elective nodal irradiation (ENI) for early-stage esophageal carcinoma (EEC), evaluate the curative effect of radiation therapy for EEC, and determine the causes of treatment failure. Methods:Data were collected from 123 patients with esophageal squamous cell carcinoma of clinical T1-2 N0-1 M0 stage. Patients were divided into two groups based on different types of radiation therapy. Among the 123 patients, 102 underwent involved field irradiation (IFI), whereas 21 received ENI. A comparative analysis of patients in the two groups was conducted. Results:In 123 patients, the one-, three-, and five-year overall survival rates were 87.8%, 47.2%, and 36.6%, respectively. By contrast, the one-, three-, and five-year local control rates were 89.4%, 67.5%, and 48.8%, respectively. After comparing the one-, three-, and five-year survival rates in the ENI patients (i.e., 90.5%, 47.6%, and 42.9%, respectively) with those in the IFI patients (i.e., 86.3%, 49.0%, and 35.2%, respectively), no significant difference was found (χ2=0.290, P=0.588 8). The results indicate that ENI possibly decreased nodal metastases (χ2=5.778, P=0.016). Conclusion:Three-dimensional conformal radiotherapy is one of the best therapeutic regimens of radiation for EEC. ENI is possibly effective for preventing regional nodal metastasis. Whether ENI leads to an improved overall survival needs further investigation.

Objective To evaluate 18F-FDG hPET/CT in the diagnosis of postoperative recurrence and metastasis of colorectal cancer. Methods GE HAWKEYE coincidence SPECT was carried out in 81 colorectal cancer patients with suspected recurrence or metastasis after intravenous injection of 259 ~ 298 MBq (7-8 mCi) 18F-FDG. The acquired data were reconstructed using iterative algorithm and attenuation-corrected X-ray. The results were compared with the final diagnosis established by histological examination of resected specimens、and clinical follow-up. Results The sensitivity, specificity, positive predictive value (PPV)and negative predictive value(NPV)was 93% (57/61)、80% (16/20)、93% (57/61)、80% (16/20) for 18F-FDG hPET/CT respectively. For conventional CT the sensitivity, specificity, PPV and NPV was 67% (37/55)、73% (19/26)、84% (37/44)、51% (19/37) respectively; 18F-FDG hPET/CT detected 91 recurrent or metastatic lesions whereas only 46 lesions were detected by conventional CT in 65 patients. Conclusions 18 F-FDG hPET/CT has unique value in the diagnosis of recurrence and metastasis in postoperative colorectal cancer patients which was superior to conventional CT. Combined 18 F-FDG coincidence imaging with localizing CT improves the detection and localization of postoperative recurrence and/or metastasis in colorectal cancer patients.
8.Study on CsA Therapeutic Window Concentration in Renal Transplant Recipients

OBJECTIVE:To establish an optimal therapeutic window concentration of CsA trough levels in renal transplant recipients on triple immunosuppressants regimen METHODS:A total 1 874 samples from 268 renal transplant recipients were measured by fluorescence polarization immunoassay(FPIA) According to the duration after operation and clinical diagnoses ,the whole blood CsA trough levels were compared among subgroups RESULTS:The optimal therapeutic window concentration of CsA was 300~400?g/L(within 1 month after operation),250~350?g/L(2nd~3rd month),150~250?g/L(4th~6th month),100~200?g/L(7th~12th month)and 100~150?g/L(more than 12 months) CONCLUSION:The above mentioned therapeutic window concentratin of CsA trough levels was ideal for renal transplant recipients with no marked acute toxic effects and rejection reaction
9.The evaluation of health economics on the clinical pathway to infantile rotavirus enteritis

Objective To explore the influence of the clinical pathway to control on hospital stay and medical expenses of pediatric rotavirus enteritis.Methods 245 infants with IRE were randomly assigned into two groups,109 in research group,136 in control group.All were treated with conventional therapy,for research group with clinical pathway.136 patients with previous medical routine therapy patients in the control group were compared.Compared two sets of average day in hospital,cure rate and cost,expenses for medicine and checking expenses proportion.Results The results shwed that the observation group of children's average hospital stay was lower than the control group,the cure rate higher than that in the control group,each time of total cost,expenses for medicine,each day medicine reducing( P < 0.05 ).Checking expenses proportion was increased ( P < 0.05),as to therapeutic effect,there was significant difference between two groups( P < 0.05 ).Both expenses of drug ratio and ratio of the average daily expenses of drug are reduced.Conclusion Clinical pathway is applied in pediatric rotavirus enteritis can improve recovery rate,reduce hospitalization time and lower hospitalization fees and expenses,charges,medicine and inspection charge proportion.And can improve health education effect and patient satisfaction.
10.Epigenetic Modifications: Novel Therapeutic Approach for Thyroid Cancer.

The incidence of thyroid cancer is growing the fastest among all cancers in the United States, especially in women. The number of patients with thyroid neoplasm is part of an even larger number of patients who often need to undergo an operation to exclude a cancer diagnosis. While differentiated thyroid cancer (papillary thyroid cancer and follicular thyroid cancer) accounts for most cases of thyroid cancer and has a relatively good prognosis, effective treatments for patients with de-differentiated and anaplastic thyroid cancer are still gravely needed. Despite progress in the identification of genetic changes in thyroid cancer, the impact of aberrant epigenetic alterations on thyroid cancer remains to be fully elucidated. Understanding of the roles of epigenetic changes in thyroid cancer could open new opportunities for the identification of innovative molecular targets for novel treatment modalities, especially for anaplastic thyroid cancer for which treatment is very limited. This article briefly reviews the studies that exemplify the potential for and promise of using epigenetic regulators in the treatment of thyroid cancer.
