1.Application of percutaneous puncture and catheterization sclerotherapy for the treatment of kidney cyst under ultrasonographic guidance

Objective To investigate the efficacy and feasibility of percutaneous puncture and catheterization sclerotherapy for the treatment of kidney cyst under ultrasonographic guidance. Methods Fifty-seven patients with simple renal cysts were divided into study group (30 cases) and control group (27 patients) according to the therapies. In study group, patients who had the cyst diameter ≤6.0 cm( 17 cases) received direct puncture sclerotherapy, while >6.0 cm (13 cases) received catheterization puncture sclerotherapy. In control group, patients who had performed laparoscopic surgery. Observed and compared the efficacy, hospitalization time, operative time and complications of two groups. Results Both groups had no case of recurrence after surgery. The effective rates in study group and control group were 93.3%(28/30 ) and 96.3% (26/27) respectively, and there was no statistical difference between two groups (P > 0.05). The hospitalization time and operative time in study group [( 2.3 ± 1.7 ) d, ( 13.5 ± 3.7 ) min] were significantly shorter than those in control group [(5.8 ±2.2) d, (57.7 ±8.9) min] (P<0.05); but the incidence of complications had no statistical difference between two groups (P > 0.05). Conclusions Percutaneous puncture and catheterization sclerotherapy for the treatment of kidney cyst under ultrasonographic guidance has some advantages as good efficacy, less traumatic, rapid recovery and fewer complications. It is especially suitable to be used in the primary hospital.
2.On Changes of Activity of Antioxidases in Hippocampus of Rats with Multi-Infarct Dementia and the Intervention Effects of Acupuncture

Objective: To explore the changes of activity of antioxidases in hippocampus of the rats with multi-infarct dementia and the intervention effects of acupuncture.Method: The multi-infarct dementia model was made by injected emboli into the internal carotid artery.The demented rats were allocated to 3 groups randomly: model,acup and sham-acup;meanwhile normal and sham-operated rats were used as control.The parameters of oxidative stress were assessed by measuring the malondialdehyde(MDA),superoxide dismutase(SOD),catalase(CAT) and glutathione peroxidase(GSH-Px) levels in hippocampus after acupuncture treatment.Result: Compared to normal group,MDA levels of hippocampus in model animals increased significantly,while SOD and GSH-Px decreased much more;No significant difference was observed in CAT among groups.Interestingly,the acupuncture treatment could significantly improve all those indexes.Conclusion: SOD/GSH-Px pathway may be playing a major role in combating oxidative stress in the brain of MID rats,the CAT activity not the key enzyme.Acupuncture treatment could increase the activity of antioxidase SOD/GSH-Px,alleviate the injury of free radical and maintain the oxidant-antioxidant balance to a great extent.This may be one of the mechanisms of its therapeutic effect.
3.miR-222 enhances HBx-HepG2 cell growth via regulation of BCL2L13 gene

AIM:To investigate the regulation of miR-222 on BCL2L13 gene and its effect on the growth and apoptosis of HBx-HepG2 cells, and to explore the underlying molecular mechanisms .METHODS:The expression level of miR-222 was detected by RT-qPCR.The HBx-HepG2 cell growth was examined by MTT and colony formation assays .The cell cycle and apoptosis were analyzed by flow cytometry .The recombination vector pmirGLO-BCL2L13 was constructed, and dual-luciferase reporter experiment was performed to validate the target of miR-222.RESULTS:The expression level of miR-222 in the HBx-HepG2 cells was significantly higher than that in the L 02 cells ( P<0.05 ) .Over-expression of miR-222 enhanced HBx-HepG2 cell growth, changed cell cycle, and inhibited apoptosis (P<0.05).Knockdown of miR-222 reduced HBx-HepG2 cell growth, changed cell cycle, and increased cell apoptotic rate (P<0.05).BCL2L13 was down-regulated in the HBx-HepG2 cells as compared with L02 cells (P<0.05), and knockdown of miR-222 in the HBx-HepG2 cells increased the expression level of BCL2L13 (P<0.05).The results of dual-luciferase reporter assay and re-store experiment showed that miR-222 negatively regulated the expression of BCL2L13 via targeting 3’ UTR of BCL2L13, resulting in the promotion of HBx-HepG2 cell growth .CONCLUSION: miR-222 enhances HBx-HepG2 cell growth via down-regulation of BCL2L13.
4.Effects of acupuncture on expression of OPG and BMP-2 in SAMP6 mouse with osteoporosis

Objective: To explore the mechanisms of acupuncture in improving osteoporosis. Methods: In this study, the expression of OPG and BMP-2 proteins of femur in SAMP6 control group, SAMP6 acupuncture group, SAMP6 sham-acupuncture group and SAMR1 control group were determined by Western Blot technique. Results: Compared with SAMR1 control group, the expression of OPG and BMP-2 proteins of the femur in SAMP6 control mice decreased obviously. Acupuncture could stimulate the expression of OPG and BMP-2 and no differences were detected between the SAMR1 control group and SAMP6 acupuncture group. Conclusion: Osteoporosis observed in SAMP6 was correlated with low expression of OPG and BMP-2. After acupuncture treatment, both of them showed increased expression, it suggested that the curative effects of acupuncture on osteoporosis were partly due to stimulating skeleton formation, inducing the production of some local factors of bone metabolism and decreasing the bone turnover rate.
5.Clinical analysis of testicular trauma: a report of 30 cases

Objective To explore the characteristics of diagnosis a nd treatment of testicular trauma. Methods A retrospective study was done on 30 cases of testicular trauma. The treatment effects of non -operation and operation on different types of injuries were observed and com pared. Results Follow-up time ranged from two months to tw o years. There were three cases with testicular atrophy, one with chronic pain l asting for one year and eight with non-operation. All of 22 cases treated with surgical operation were good except for one with sperm disorder due to bilateral testicular trauma. Conclusions Testicular trauma is a speci al type of trauma. Ultrasound is an important diagnostic method for testicular trauma. Surgical exploration should be performed as soon as possible so as to a void testicular atrophy and sperm disorder.
6.Analysis of volatile oil in Gardenia jasminoids roasted with ginger juice by GC-MS.

OBJECTIVETo compare the components and contents of volatile oil in Gardenia jasminoids, Zingiber officinale, G. jasminoids roasted with ginger juice and to provide scientific basis for intrinsic material of G. jasminoids roasted with ginger juice.
METHODThe volatile oil in the three herbal medicines was extracted by steam distillation, the components were analyzed by GC-MS and the contents of the components were calculated by area normalization method.
RESULTThe contents of volatile oil in G. jasminoids, Z. officinale, G. jasminoids roasted with ginger juice were 1.0, 2.0, 1.5 microL x g(-1), respectively. A total of 89 components were identified by GC-MS. Although 42, 75 and 77 peaks were separated, only 30, 58 and 67 components were identified accordingly, which accounted 94.1%, 90.52% and 94.38% of the whole volatile oil.
CONCLUSIONAfter being processed with ginger juice, the components and contents of volatile oil in G. jasminoids has been changed obviously.

7.Management of Non-medical Prescribing in Britain and Its Enlightenment to China

OBJECTIVE:To systematically investigate the management mode of non-medical prescribing in Britain and its enlightenment to the establishment and implementation of non-medical prescribing in China. METHODS:By retrieving domestic and foreign literatures,non-medical prescribing in Britain was introduced systematically in terms of development history,types, core elements,implementation status and effect;the suggestions were put forward for the development of non-medical prescribing in China. RESULTS & CONCLUSIONS:Britain implemented non-medical prescribing since 1994. After years of practice and a series of legislation and revision,independent prescribers (including nurses and pharmacists) have owned the statutory right to prescribe any medicine for patients by 2012. In Britain,non-medical prescribing is divided into independent prescribing which mainly exists in community pharmacies and is limited to specific diseases,supplementary prescribing which exists in hospitals or clinic and needs to be signed with doctors. The management institutions are mainly composed of one state administrative department (British Department of Health) and three professional associations (British Nursing and Midwifery Council,The Royal Pharmaceutical Society of Great Britain,The Pharmaceutical Society of Northern Ireland). There are detailed and strict stipulations on the qualification and corresponding responsibilities of the prescribing authority. The British government has provided legal protection for the development of the non-medical prescribing model,and the number of non-medical prescribers is increasing. The implementation of this model has increased the patients'access to medical services. Drawing lessons from the development of non-medical prescribing management in Britain,our country needs to improve legislation,provide legislative protection for the implementation of non-medical prescribing,expand the scope of practice of nurses and pharmacists,establish and improve the training program of non-medical prescribing professionals to promote the development of non-medical prescribing model in China.
8.How to deal with dual challenges of diagnosis related groups payment and drug zero plus in department of critical care medicine: 1 year review of medical reform in Liuzhou Worker's Hospital

Objective To explore the impact of diagnosis related groups (DRGs) payment and drug zero plus on the management of intensive care medicine department. Methods The clinical data of patients in one year from 2016 to 2017 admitted into the Department of ICU in Liuzhou Worker's Hospital concerning their numbers of discharged patients, transferred patients, bed utilization rate, number of bed turnover, average length of stay of discharged patients, cure and improvement rates, admission and discharge diagnostic coincidence rate, 3-day definite diagnosis rate, clinicopathological diagnosis coincidence rate, rescue success rate, total income, drug proportion, consumable proportion, DRGs payment and settlement data, etc were retrospectively analyzed to explore the dual challenges, DRGs payment and drug zero plus, facing the department and how to respond and deal with them. Results In 2016 and 2017, the total incomes of the department of critical care medicine in our hospital were 42.107 0 million yuan and 41.371 3 million yuan respectively, and the medical insurance incomes were 15.03 million yuan and 16.69 million yuan respectively;in 2016 and 2017, 2 693 patients and 2 922 patients were admitted and treated respectively; 595 patients and 577 patients were discharged respectively, with 2 071 patients and 2 334 patients transferred respectively; the balances of the department were 15.48 million yuan and 29.11 million yuan, respectively. From July to December 2017, the medical insurance DRGs payment data suggested that the proportion of loss of the department be 7.02%. Accelerating the Grade 6 electronic medical records and informationization construction, adopting the severe disease information solution program and fine quality control management in the department of critical care medicine can reduce the cost of manpower. Conclusion Our future development direction in the Department of Intensive Care Medicine includes the following aspects: Open source and reduce expenditure, strictly control the proportions of drugs and consumables, improve the balance of the department, and actively respond and deal with the medical insurance DRGs payment.
