1.Clinical analysis on complications of initial phacoemulsification in a grass-roots hospital

Objective To review the complications of initial phacoemulsification in a grass-roots hospital and to investigate preventive measures from complications. Design Retrospective cases series. Participants 184 cataract patients of initial phacoemulsification in the Fangshan First Hospital. Methods 184 patients (188 eyes) were performed phacoemulsification and intraocular lens (IOL) implantation. The complications were observed and recorded. All patients were followed up 3-6 months. Main Outcome Measures The rate of main complications. Results Major complications during and after phacoemulsification cataract surgery included posterior capsule rupture in 8 eyes (4.3%), iris injury in 2 eyes (1.1%), corneal edema in 23 eyes (12.2%), uveitis in 16 eyes (8.5%), ocular hypertension in 4 eyes (2.1%), detachment of retina in 2 eyes (1.1%), after-cataract in 21 eyes (11.2%). At the last visit, the post-surgery visiual acuities were 0.5 or better in 149 eyes (79.3%). Conclusion There may be more complications happened in the initial phacoemulsification in a grass-roots hospital. It should be gradually developed under the direction of skilled surgeons.
2.The Overview of Ontology Alignment techniques and Their Applications to Traditinoal Chinese Medicine (TCM)

Ontology alignment technology is a knowledge engineering method to realize the concept and relationship integration of different ontologies.In view of the same scope of ontology,such as disease ontology,developed by researchers from a number of different areas or institutions of independent research and development,there is a big difference between the term expression and the concept of the relationship.Thus,how to achieve the integration of multi-source ontology through alignment processing has been recognized as a significant methodological problem.In this paper,the concept,technology,method and corresponding tool of ontology alignment were expounded at full length.The technique of ontology alignment based on linguistic features and structural features was emphasized.Combined with the two international disease ontology:Disease Ontology and Orphanet,the experiment and analysis of the technique of ontology alignment were carried out and detailed the application of alignment technology.Furthermore,for the existing multi-source problems on TCM ontology database,the necessity and application of ontology alignment were discussed.

Objective Approaching characteristic of syndrome of type 2 diabetes mellitus affiliated metablic syndrome, usage of herbs based on the theories of relationship between formula (herbs) and syndrome. Methods Gathering 265 cases coming from multicentric clinic cases, feature of syndrome and classify would be approached by means of free-scale networks. Result Herbs of supplementing Qi and nourishing Yin is its foundation, effectiveness of herbs showed tendency in eliminating heat nourishing yin, then supplementing Qi and nourishing yin, eliminating heat, and invigorate vital energy promoting blood flow, warming yang, dissipating hygrosis for diuresis. Honey-fried radix astragal, pseudostellariae are important herbs, rehmannia dried rhizome, fructus schizandrae, chuanxiong, angelicae sinensis paeoniae rubra ophiopogonis tuber, anemarrhenae are correlation with them. Conclusion Deficiency of vital energy, Yin asthenia are fundamental syndrome of type 2 diabetes mellitus affiliated metablic syndrome. Early, Yin asthenia and excessive heat are cardinal. Intermediate stage, deficiency of vital energy Yin asthenia are fundamental. Meanwhile excessive heat, phlegmatic hygrosis and stagnated blood existed together. Advanced stage, deficiency of vital energy and stagnation of blood are main syndrome. Yang asthenia, phlegmatic hygrosis, retention of fluid in the body are affiliated syndrome frequently. Drug for invigorating blood circulation and eliminating stasis penetrated through the courses all the while.
4.Treatment of exudative lesions in the mammary duct with fiberoptic ductoscopy

Objective To discuss the efficacy of fiberoptic ductoscopy(FDS)in the treatment of mammary duct diseases with nipple discharge.Methods Fiberoptic ductoscopy was utilized for diagnosing and treating exudative lesions in the mammary duct in 72 cases(80 sides).After the lesion was detected under ductoscope,repeated irrigation using normal saline was performed until the flocculent discharge disappeared.Intraductal drug administration(dexamethasone 5 mg and gentamycin 40 000 U)was given during the retreat of ductoscope.Results There were 57 sides of galactophoritis and 23 sides of mammary duct ectasia.A complete recovery was achieved in 65 sides(nipple discharge disappeared completely and no relapse occurred in 6 months),in which a follow-up for 1~2 years showed no recurrence.A remarkable improvement was obtained in 15 sides(nipple discharge significantly reduced,or progressively decreased within 6 months,or underwear wetted occasionally).Conclusions Fiberoptic ductoscopy in the treatment of mammary duct lesions with abnormal nipple discharge is effective.
5.Effects of nitric oxide synthases inhibitor on severe hemorrhagic shock

Objective To evaluate the effects of aminoguandine(AG) used as a selective inhibitor of the inducible isoform of nitric oxide synthase(iNOS) and L NAME used as a non selective inhibitor of nitric oxide synthase(NOS) on severe hemorrhagic shock Methods Forty rabbits were bled to mean arterial pressure of 4 4 66 kPa via femoral artery Hypotention was maintained for 120 min, the shed blood was then returned, followed by an infusion with Ringer′s solutions Animals were randomly divided into three groups: hemorrhagic shock group (n=14), AG group (AG 20mg/kg was infused intravenously ,n=14), L NAME group (L NAME 30mg/kg was infused intravenously with ,n=12) Plasma levels of endotoxin, tumor necrosis factor(TNF?) and nitric oxide (NO) were determined before and after shock, immediately after resuscitation and 0 5h, 2h, 4h after resuscitation The 24h and 48h survival rates were recorded Lung, intestine, liver and kidney tissues were obtained 48 h after shock for microscopic examination Results The plasma endotoxin, TNF? and NO levels markedly increased and were kept at high levels after shock Lower plasma levels of endotoxin TNF? and NO, less tissue damages and high survival rates were observed in AG group as compared with those in L NAME group and shock group Conclusions The endotoxin,TNF? and NO play an important role in the development of irreversible shock AG is beneficial in the treatment of hemorrhagic shock, while L NAME does not improve the outcome of shock
6.Control of the maxillary occlusal plane by micro-implant in the process of en-mass retraction:A finite element analysis

Objective:To investigate the changes of occlusal plane in the process of retracting maxillary anterior teeth with different vertical traction force.Methods:The three dimensional finite element model of maxillary anterior teeth en-masse retraction by micro-implant was constructed with cone beam CT scanning and MIMICS and ANSYS software.Then the movement tendency of tooth and the changes of the maxillary occlusal plane was calculated when the vertical traction force was 0,0.5,0.75 and 1 N in the process of en-mass retraction,respectively.Results:Lingual tipping of anterior teeth,distal tipping of the first molar and the maxillary occlusal plane clockwise rotation by the retraction forces were observed.With the vertical traction force increasing,lingual tipping of anterior teeth and distal tipping of the first molar were decreased,and the occlusal plane clockwise rotation was prevented.Conclusion:The upper dentition movement and occlusal plane rotation can be changed effectively by the vertical traction with different force.When retract anterior teeth,0.5-0.75 N vertical traction force is beneficial to prevent clockwise rotation of the maxillary occlusal plane.
7.Effect of gardenoside on the damage of dopaminergic neurons induced by inflammation and its mechanism

Objective To explore the protective effect and mechanism of gardenoside on the damage of dopaminergic neurons induced by lipopolysaccharide (LPS). Methods Both neuron-enriched cultures and neuron-astrocyte cultures were pretreated with vehicle or gardenesides ( 10, 20 and 40 mg/L) for 30 min at 37℃. The culture media were subsequently renewed in order to remove gardenesides. LPS was then added into all culture media at a final concentration of 10 mg/L Twenty-four hours later, the culture media was collected to measure TNF-α, NO, IL-6, GDNF and MMP-9; the cells were collected to count the number of cells labeled with an antibody against tyrosine hydroxylase (TH) and to assess the expression of TH mRNA using reverse transcription-polymerase chain reaction. Results Gardeneside didn't promote the survival of dopaminergic neurons in neuron-enriched culture, but significantly increased the survival of dopaminergic neurons in neuron-astrocyte culture, compared with the vehicle group, the survival of dopaminergic neurons increased from 203.0%±17.4% to 256.7%±15.2% ( F = 17.22, P = 0.001 ) in 40 mg/L gardenaside group. The amount of TNF-α, NO and GDNF released from the neuron-astrocyte cultures after 24 h of addition of LPS was not changed significantly, while the expression of IL-6 and MMP-9 was increased significantly. In this study, the gardenoside concentration-dependently attenuated the LPSinduced increase of the expression of IL-6 and MMP-9, compared with the vehicle group, the expression of IL-6 and MMP-9 decreased to 67.2%±6.4% (F= 12.89,P =0.001 ), 77.3%±9.8% (F =8.27,P = 0.001 ) respectively in 40 mg/L gardenoside group. Conclusions Astrocytes play a neuroprotective role on dopaminergic neurons, which is decreased by LPS via inducing the secretion of pro-inflammatory factors. Gardeneside may protect dopaminergic neurons from LPS-induced injury in an astrocyte-dependent manner and it inhibits the production of proinflammatory factors instead of promoting the secretion of GDNF. From the point of view that a very low toxicity of gardenesides has been well documented, this report may reveal a new way of developing therapeutic interventions for inflammation-related diseases such as Parkinson's disease.
8.Effect of depressive disorder on ventricular remodeling and its mechanism in acute myocardial infarction rats

Objective To observe the effect of depressive disorder on ventricular remodeling and its mechanism in acute myocardial infarction (AMI) rats. Methods Forty-six AMI Wistar rats were randomly divided into sham-operation (n=10), AMI (n=12), depression (n=12), neurostan by open field test, and the detection of angiotensin Ⅱ (Ag Ⅱ ), aldosterone (ALD), malondialdehyde (MDA), superoxide dismutase (SOD) were performed, the pathological sections were observed under light and electron microscopes. Results Compared with sham-operation group, depression group had decreased values of squares crossing times, rearing times and grooming time, increased time of staying in the central square and defecation. Compared with depression group, AMI and neurostan treatment groups had increased values of squares crossing times, rearing times and grooming time,decreased time of staying in the central square and defecation (F=16. 9, 44.56, 71.79, 34.86,29. 18, P<0.01). At the 4 week of test, the left and right ventricular relative weights (LVRW,RVRW) and thickness of interventricular septum were (1.63±0.15) mg/g, (0. 48±0. 10) mg/g and (1.75 ± 0. 38) mm in sham-operation group, the corresponding data were (2.06±0.21) mg/g,(0.62±0.10) mg/g and (2.25±0.30) mm in AMI group, (2.90±0.47) mg/g, (1.00±0.28) mg/gand (2.58±0.34) mm in depression group, (2.20±0.34) mg/g, (0.67±0.15) mg/g and (2. 25±0.23) mm in neurostan treatment group. Compared with sham-operation group, AMI, depression and neurostan groups had obviously increased values of LVRW, RVRW and thickness of interventricular septum. Compared with depression group, AMI and neurostan groups had decreased LVRW, RVRW and thickness of interventricular septum (F=6.31, 21.9, 115.7, 9.91, P<0.05). And the depression also could aggravate edema and injury of ultrastructure in myocardial tissue. The values of AgⅡ , ALD, MDA and SOD were (1957.5±662.6) ng/L, (0.453±0.111) ng/L, (16. 00±3.03)nmol/L and (80.57 ± 7.00) U/ml in depression group, the corresponding data were (1143.8± 98.0)ng/L, (0.198±0.087) ng/L, (8.03 ± 0.44 ) nmol/L and (95.20 ± 4.87) U/ml in sham-operated group, (1407.5±255.8) ng/L, (0.295±0.027) ng/L, (11.18±4.30) nmol/L and (87.33±3.51)U/ml in AMI group, (1400.0±239.0) ng/L, (0.326±0.073) ng/L, (11.88±3.36) nmol/L and (89.13 ±0.17) U/ml in neurostan group. After 4 weeks, the values of Ag Ⅱ , ALD and MDA increased in depression group while the level of SOD reduced (F=6.58, 11.9, 11.39, 8. 82, P<0.05). Conclusions Depressive disorder after AMI in rats can aggravate ventricular remodeling and lower the ability of antioxygen.
9.A Study on the Named Entity Recognition Method on Symptom Names in the History of Present Illness in Traditional Chinese Medical (TCM) Clinic

Clinical cases of TCM are used as important clinical data to record the whole process of the interaction between doctors and patients in the form of text.However,in the context of big data,there is a lack of research on the use of information covered in clinical cases.Therefore,we studied the method of extracting the symptom term from the history of present illness in TCM clinic in this paper,in order to lay the foundation for the further use of clinical cases.First,twelve thousand,three hundred and sixty-seven history data of present illness were obtained by random selection and expert review.According to the different disease types,they were divided into the two groups of the experiments:4,838 data in the diabetes group,7,529 data in the spleen and stomach disease group and 12,367 data in the mixed or combined group.A glossary of symptom terms covering 22,996 words were compiled.Then,five feature templates,such as sliding window feature,prefix and suffix character and lexical features,were selected.CRFs model was adopted to carry out named entity extraction experiment.As a result,in the open test,the performance of diabetes,spleen and stomach disease and mixed group were (0.83,0.8,0.82),(0.9,0.9,0.89) and (0.88,0.87,0.87),respectively,while the results were (0.83,0.82,0.83),(0.95,0.95,0.95) and (0.93,0.92,0.92) in the ten-fold cross validation.In conclusion,the results showed that the CRFs algorithm was an excellent sequence labeling algorithm and applied to the named entity extraction task of symptom history.
10.Protective effect of trimetazidine on myocardial ischemia/reperfusion injury in rats

Objeetive To investigate protective effect of trimetazidine on myocardial ischemia/reperfusion injury (MIRI) in rats.Methods Forty Wistar rats were randomly divided into MIRI group (n =10 rats),trimetazidine high-dosage group (n =10 rats; 20 mg/kg),trimetazidine low-dosage group (n=10 rats; 10 mg/kg),and the normal control group (n =10 rats).After MIRI,hemodynamic changes were observed,the concentration of IL-6 and TNF-α was determined,and the cardiac muscle histology under the microscope was observed.Results Hemodynamic studies:Compared to the indices LVSP(198 ±35.5) mmHg,LVEDP (17 ±9.18) mmHg,+ dp/dt max (11050 ± 1517.4) mmHg/s,and-dp/dtmax (9175± 1900) mmHg/s] in the sham-operated group,the indices [LVSP (143 ± 24.5) mmHg,LVEDP (37.5 ±7.16)mmHg,+ dp/dtmax (7450 ± 1755.1) mmHg/s,and-dp/dtmax (6075 ± 1641) Hg/S] in the MIRI group,the indices [LVSP (154.5 ± 31.1) mmHg,LVEDP (31.3 ± 12.6) mmHg,± dp/dtmax (8527.7 ±2251.5) mmHg/s,and-dp/dtmax (6694 ± 2242.2) mmHg/s] in the trimetazidine low-dosage (10 mg/kg)group,the indices[LVSP (168.3 ± 17.6) mmHg,LVEDP (28 ± 10.05) mmHg,+ dp/dtmax (9213.6 ±1747) mmHg/s,and-dp/dtmax (7568 ± 1462.4) mmHg/s] in the trimetazidine high-dosage (20 mg/kg)group,left ventricular remodeling end diastolic pressure (LVEDP),left ventricular systolic pressure (LVSP),and left ventricular pressure maxial rate of rise and fall (± dp/dtmax) were significantly decreased.Compared to the MIRI group,LVSP and ± dp/dtmax in the trimetazidine high-dosage (20 mg/kg)group were significantly increased (P < 0.01),and myocardial damage of MIRI group was more severe in microscope.Compared to the sham-operated group [IL-6 (2556.5 ± 662.9) ng/ml,and TNF-α (134 ± 73.7)ng/ml],the corresponding indices [IL-6 (3664.0 ± 995.7) ng/ml,and TNF-α (443 ± 22.1) ng/ml] in the MIRI group,[IL-6 (3692.8 1545.2) ng/ml,and TNF-α (295 ± 24.2) ng/ml] in the trimetazidiue low-dosage (10 mg/kg) group,and[IL-6(2654.8 ±681.7) ng/ml,and TNF-α(230 ±7.8) ng/ml]in the trimetazidine high-dosage (20 mg/kg) group,the levels of IL-6 and TNF-α were significantly increased.Compared to the MIRI group,the levels of IL-6 and TNF-α were significantly decreased in the trimetazidine high-dosage (20 mg/kg) group (P < 0.01).Conclusions The high-dosage (20 mg/kg) of trimetazidine had a protective effect on myocardial ischemia-reperfusion injury.
