1.Discussion Ideas of Syndrome Differentiation and Treatment of Eczema from the Spleen

[Objective]This paper is to detail the system thinking of syndrome differentiation and treatment of eczema from the spleen and provide ideas for clinical diagnosis and treatment of eczema.[Methods]Combined with the understanding of eczema in Chinese and western medicine, detailing the ideas of syndrome differentiation and treatment of eczema from the spleen,which includes the etiology and pathogenesis, the basic rule and its application ideas, to reconcile the five internal organs, the syndrome differentiation of physique and caring spleen and stomach.[Results]The basic pathogenesis of eczema is the weak spleen loss the transport function.In clinical treatment,the main method to complement the spleen is preserving its transport function.On the basis of complementing the spleen,different times have different rules.It should clarify the rule of complementing the spleen to get rid of the wet evil.Types of hot evil or hot and wet evil,the rule is to get rid of the hot and wet evil.After symptom relief,herbs to complement the spleen should be added.Types of wet evil or blood deficiency resulting in wind evil,the rule is to complement the spleen to get rid of the wet evil.Attention to regulating the function of organs, not specifically treating the spleen,but make the spleen as the center to reconcile the five internal organs. The weak spleen is the physical tendencies of eczema patients. Individual differences have targeted treatment to correct the physique bias.Medication and daily life always pay attention to caring spleen and stomach.Diagnosis and treatment of eczema from the spleen can have guiding signficance. [Conclusion]The thinking of syndrome differentiation and treatment of eczema from the spleen has a guide to clinical significance,is worthy of further study and popularization.
2.Experience about Filter Maintenance and Replacing in Purifying Operating Room

The purifying operating room with tidiness, antisepsis and easy operation is widely used in more hospitals. The same time, depurative flow, manufacturing skill and technical standards of it are still developing. But filter maintenance and replacing as key component has major difference in different region and season. Through four-year exploration and practice, the methods of filter maintenance and replacing were explained, which carry out the questions of consumption and energy efficiency.
3.Diagnosis and treatment of occult breast cancer

Occult breast cancer is a rare disease with the primary symptom of enlarged ipsilateral axillary lymph node. At present,it is still difficult to diagnose occult breast cancer. The diagnosis is based on the core needle biopsy and following immunohistochemical staining for the enlarged lymph nodes. If the results support,the diagnosis can be confirmed after the exclusion of other primary tumors. The standard treatment of occult breast cancer is still the mastectomy and axillary lymph node dissection. Neoadjuvant chemotherapy is an independent factor affecting the prognosis,which is also recommended before the oper-ation. Chemotherapy,radiotherapy and endocrine therapy could be chosen based on immunohistochemical results after surgical treatment. The prognosis of occult breast cancer is similar to the other types of breast cancer at the same staging.
4.Effects of oxidative stress and its secondary protein impairment induced by high glucose content in human renal tubular epithelial cell

Objective To explore the effects of oxidative stress and its secondary protein impairment induced by high glucose content in human kidney proximal tubular epithelial cell line (HKC). Methods Cultured HKC were used to detect the reactive oxygen species (ROS) and protein carbonyl content. HKC were divided into three different groups: control group (normal glucose), short-term treatment with high glucose group and long-term treatment with high glucose group. The glucose concentrations in control group and the high glucose treatment groups were 5.5mmol/L and 30mmol/L, respectively. Six time-points were set in short-term treatment with high glucose group namely 30min, 1h, 6h, 12h, 24h and 48h; glucose treatment was instituted for 2 months in long-term treatment of high glucose group. 5-(and-6)-chloromethyl-2',7'-dichlorodihydrofluorescein diacetate, acetyl ester (CM-H2DCFDA) was used as ROS trapping agent, and flow cytometry and laser confocal microscopy were used to detect the concentration of ROS in HKC after treatment with high glucose for different periods of time. Flourescent probe JC-1 was used to test the change in mitochondrion membrane potential (MMP). The protein carbonyl content was determined by 2,4-dinitrobenzene hydralazine (DNPH) chromatometry. Objective:To study the course and mechanism of the immune response to SARS virus. Methods:The recombinant SARS virus S1 subunit was expressed in E. Coli according to the results of bioinformatics analysis. After purification, the recombinant S1 protein was identified by 6 serum samples of recovered SARS patients and 6 serum samples of health donors, which were collected before out-break of SARS. Results:Sequencing analysis confirmed that the recombinant protein has the same sequence of natural SARS virus S1 subunit. The recombinant S1 protein could react with all the samples from recovered SARS patients but not the control samples from healthy donors according to the results of Western blot. Conclusion:The recombinant SARS virus S1 subunit may provide a good tool for the research of immune response to SARS virus and the producing of recombinant vaccine to prevent people from SARS.
6.Clinical analysis of COs-laser assisted suspension laryngoscopic surgery for vocal fold cyst

Objective To compare the clinical effect between CO2-1aser assisted and cold instrument assisted suspension laryngoscopic surgery for vocal fold cyst.Methods From January, 2011 to December, 2014, 72 patients with vocal fold cyst, which diagnosed by strobolaryngoscopy, were randomly divided into CO2-1aser assisted group and cold instrument group.Strobolaryngoscopy, acoustic analysis and perceptual voice analyses were performed on each patient before surgery, 1 month, and 3 months after surgery, respectively.Results All operations were successfully completed.The complete vocal fold cyst resection rate of CO2-1aser assisted group was significantly higher than cold instrument group (29/36, 80.5% vs 21/36, 58.3%, P < 0.05), especially the left vocal fold cyst (13/16, 81.3% vs 9/19, 47.4%, P < 0.05).The complete right vocal fold epidermoid cyst resection rate was significantly higher than retention cyst (17/19, 89.4% vs 11/18, 61.1%, P < 0.05).Two recurrent cases were found in cold instrument group but no recurrent cases in CO2-laser assisted group (0/36, 0% vs 2/36, 5.6%, P > 0.05).Correlation analysis showed that vocal fold cyst recurrence was related to complete resection rate and has no relation with surgical methods, histopathological types and position.Subjective and objective assessment of voice quality in preoperative, 1-month postoperative and 3-month postoperative were similer between CO2-1aser assisted group and cold instrument group (P > 0.05).Conclusion The CO2-laser assisted suspension laryngoscopic surgery for vocal fold cyst, can increase the surgical precision, reduce the left hand impact, improve the complete resection rate and reduce the recurrence rate.
7.Bicyclol versus diammonium glycyrrhizinate for the treatment of drug-induced hepatic injury in patients with severe psoriasis: a randomized controlled trial

Objective To investigate the protective effect of bicyclol tablets on drug-induced hepatic injury in patients with severe psoriasis.Methods One hundred and six patients with severe psoriasis and drug-induced hepatic injury were enrolled in this study,and randomly divided into 2 groups:bicyclol group (53 cases) treated with oral bicyclol tablets (25 mg thrice a day) for 8 weeks,diammonium glycyrrhizinate group (53 cases) treated with diammonium glycyrrhizinate capsules (150 mg thrice a day) for 8 weeks.Serum biochemical indices of hepatic function were measured before and after treatment,therapeutic efficacy was evaluated,and adverse reactions were observed and recorded after treatment.Intergroup and intragroup differences in these parameters were assessed.Results During the treatment,3 patients were lost to follow up.After treatment,serum biochemical indices of hepatic function markedly improved in both the bicyclol group (52 cases) and diammonium glycyrrhizinate group (51 cases) (both P < 0.01),and the bicyclol group showed a better treatment response than the diammonium glycyrrhizinate group (P < 0.01).The marked response rate was 71.15% and 47.06% (P < 0.05),and the response rate was 84.62% and 66.67% (P < 0.05),respectively,in the bicyclol group and diammonium glycyrrhizinate group.No adverse reactions related to tested drugs were observed in either of the two groups.Conclusion Bicyclol can attenuate antipsoriatic drug-induced hepatic injury with good safety.
8.Temperature Control and Surface Cooler Reform in Operating Room Air Decontamination System

The principle of temperature control in operating room air decontamination system and the main reasons with difficulties of regulating temperature in transformation period of spring and autumn are analyzed. The reconstructed approaches of circulating water line on surface cooler are adopted appropriately, not only to solve the question of long temperature decreasing response time in operating room but also to save substantially a lot of expenditures and reduce the running loss.
