2.Chemical constituents in heartwood of Taxus yunnanensis

Objective To intensively investigate the chemical constituents in the heartwood of Taxus yunnanensis Cheng et L.K.Fu.Methods The heartwood was extracted with ethanol,the ethanol extract was subjected to the extraction with methylene trichloride,which was submitted to chromatography on sili-ca gel and Sephadex LH-20 column to isolate some compounds.And their structures were elucidated on the basis of spectral analysis(UV,IR,ESI-MS,1H-NMR,13C-NMR).Results The compounds were identified as 2?,5?,7?,9?,10?,13?-hexaacetoxy-4(20),11-taxadiene(Ⅰ),taxusin(Ⅱ),taxa-4(20),11-diene-2?,5?,10?-triacetoxy-14?,2-methybutyrate(Ⅲ),10?-hydroxy-2?,5?,14?-triacetoxy-4(20),11-taxadiene(Ⅳ),1-dehydroxybaccatin Ⅳ(Ⅴ),baccatin Ⅳ(Ⅵ),baccatin Ⅵ(Ⅶ),7,9-deacetylbaccatin Ⅵ(Ⅷ),10-deacetyltaxuyannine(Ⅸ),1?-accetoxy-5-deacetyl-baccatin Ⅰ(Ⅹ),baccatin Ⅰ(Ⅺ),taxuchin A(Ⅹ Ⅱ),secoisolariciresinol(Ⅹ Ⅲ),?-conidendrin(Ⅹ Ⅳ),isotaxiresinol(Ⅹ Ⅴ),lariciresinol(Ⅹ Ⅵ),sequoitol(Ⅹ Ⅶ),?-sitosterol(Ⅹ Ⅷ).Among them compounds Ⅰ,Ⅴ,Ⅵ,Ⅺ,Ⅹ Ⅱ,Ⅹ Ⅵ were obtained from the heartwood of T.yunnanensis for the first time.Conclusion The chemical constituents in the heartwood differ from the other parts of T.yunnanensis,but there is little difference within the species of Taxus L.
3.Studies on chemical constituents of planted Taxus mairei (III)

From the chloroform-soluble fraction of the ethanol extracts of the whole plant of Taxus chinensis var. mairei (Lemee et Levl), four compounds were isolated by using repeated column chromatography on silica gel and Sephadex LH-20. Based on spectroscopic data (UV, IR, ESI-MS, 1H NMR and 13C NMR), the compounds were identified as taxamairin K (1), 2α, 4α-dideacetoxy-7β-benzoyloxy-5β,20-epoxy-9α,10β,13α,15-tetrahydroxy-11(15→1)abeotaxa-11-ene (2), 7β-xylosyl-taxol (3), 10-deacetoxy-7-xylosyl-taxol (4). Among them, taxamairin K is a new compound.
5.Prevention Measures after Occupational Exposure to HIV

OBJECTIVE To explore the effective methods for medical personnel preventing and controlling the infection of HIV after occupational exposure. METHODS The 7 cases contacted the AIDS patients had been analyzed and a set of institutions and monitoring methods were established. RESULTS Because of the emergency measures,the 7 medical personnels hadn′t infected HIV for the 2 follow-up years. CONCLUSIONS To think highly of the safety education of occupational exposure and enhance the monitoring and management of HIV infection can decrease the occupational exposure rate and ensure the occupational safety of medical personnels.
6.Antibacterial Use in Department of Orthopedics: An Investigation and Analysis

OBJECTIVE To investigate the reasonable use of antibacterial in department of orthopedics.METHODS The patients in our department were registered according to the inventory of antibacterial use made by the hospital infection monitoring network of Medical Administration Bureau,Ministry of Health of the People′s Republic of China,the(operative) incision was classified according to Clinical Surgery.The data of reasonable use of antibacterial were(obtained) after evaluation and(analysis).RESULTS Of the 257 patients in our study,79.37% of them were used antibacterial,20.62% were not used antibacterial,24.50% were used antibacterial for therapy,75.49% were used antibacterial for infection(prevention);95.09% were used systematically;74.50% were reasonable(usage,) 25.49% were unreasonable usage;of the 204 patients who were used antibacterial,only 34 patients were tested by drug sensitivity trial,and the others were used by experience.CONCLUSIONS In order to use antibacterial reasonably,we suggest two points,one is that the medicine management department should workout a feasible antibacterial use institution and mass(evaluation) criterion,take mass examination regurally,and connect the bonus to the examination result;the other is that we should enhance the management of antibacterial market,avoid the improper competition,rise the morality view of the doctor,and suggest them to use antibacterial reasonably(according) to the scientific theory.
7.Lower Respiratory Tract Infection in Patients with Cerebral Hemorrhage and Hypertension

OBJECTIVE To study the characteristic of the lower respiratory tract infection in patients with the cerebral hemorrhage and hypertension,and find out the measures of prevention and control. METHODS The discharged patients with the cerebral hemorrhage and hypertension from Jan 2002 to Dec 2004 underwent retrospective investigation and analysis for the sites and reasons and the results of bacterial culture. RESULTS The incidence of the lower respiratory tract infection in patients with the cerebral hemorrhage and hypertension was 55.55%.Major infection factors correlated with neurogenic pulmonary edema, irritable hyperglycemia,bulbar paralysis,etc.The Gram-nagative bacteria were predominant among the pathogens. CONCLUSIONS The infections are interrelated with many factors.Abiding by the principle of using antibiotic and the strict aseptic manipulation may reduce the occurrence of the infection,but treating the primary affection and the comprehensive therapy are important measures for reducing infection.
8.Hospital Infections among 246 Elderly Patients:Analysis and Countermeasures

90 years old patients was 13.04%.The longer the hospitalization time the incidence among inpatients longer than 60d was 43.51%,the higher was the hospital infection rate.CONCLUSIONS There are many risk factors inducing hospital infections among elderly,there fore the monitoring must be strengthened,and the disinfection isolation and asepsis must be strictly conducted,with limited invasive manipulations,reasonable use of antibiotics,and active treatment of the primary affection,all above can effectively reduce the occurrence of hospital infection among ellerly patients.
9.The correlation of serum creatine kinaseisoenzyme-MB and the prognosis of patients with acute coronary syndrome

Objective To investigate the correlation of serum ereatine kinase iseenzyme-MB (CK-MB) and recent cardiac events(30 days, 180 days) and late cardiac events (360 days) in patients with acute coronary syndrome (ACS). Methods Serum creatine kinase (CK) and CK-MB in 168 patients with ACS were measured, and these patients were divided into four groups according to the levels of CK and CK-MB. The occurrence of cardiac events analysis of CK and CK-MB was followed up for 30, 180 and 360 days and the relationship between the occurrence of cardiac events and the levels of CK and CK-MB were studied. Results There was statistical correlation between increased CK-MB and abnormal ECG Q wave, ST segment change (P< 0.05), 168 patients had followed up 30, 180 and 360 days, in which cardiac events happened in 57 common patients (64 examples), 27 examples (16.1%) happened in the recent cardiac event, and 37 examples(22.0%) happened in the late cardiac event. The cardiac event and CK-MB elevation had statistical correlation (P<0.05). Conclusion ACS patients with increased CK-MB levels prompt a poor prognosis, and could be an independent indicator of recent and late cardiac events in patients with ACS.
10.Metastatic tumors in the colorectum:a clinicopathologic analysis of 53 cases

Purpose To investigate the clinicopathologic characteristics of colorectal metastatic tumors. Methods Cases which were clinically diagnosed as metastatic tumors to the colorectum were retrieved from the data base of the Department of Pathology, Peking U-niversity Third Hospital. The clinicopathologic features were studied and immunohistochemical stains were performed on some of the ca-ses for differential diagnosis. Results Fifty three cases which showed intestinal wall involvement were studied. In 43. 4% (23/53) of them, only serosa/subserosa was involved and mucosal involvement was observed in 37. 7% (20/53) of the cases. Sigmoid colon and rectum were the most frequently involved intestinal segments (24/53, 45. 3%). Female reproductive system was the most common ori-gin of the metastatic tumors (33/53, 62. 3%), followed by the digestive system (15/53, 28. 3%). The most common histological type was ovarian serous carcinoma (22/53, 41. 5%). 96. 2% (51/53) of the metastatic tumors were centrally located at the serosa/subserosa or the muscular propria and prominent lymphovascular invasion was observed in 26. 4% (14/53) of the cases. In cases with mucosal involvement, 35% ( 7/20 ) had foci that were lack of desmoplastic reaction, 20% ( 4/20 ) exhibited in-situ growth and/or paradoxical maturation of tumor cells which led to misdiagnosis in 2 of the 17 pre-operative biopsies. Conclusions Metastatic carcino-ma in the colorectum can be highly informed by predominantly involving the serosa/subserosa or muscular propria and prominent lym-phovascular invasion. In-situ growth and paradoxical maturation of tumor cells in the mucosa are diagnostic pitfalls in pre-operative co-lonic biopsy and should be aware in practice.
