1.The effect of hyperuricemia on vascular complications of type 2 diabetes and the related-factors analysis

Objective To evaluate the effect of hyperuricemia on vascular complications of type 2 diabetes and analysis of the possible mechanism. Methods 586 cases with T2DM were divided into two groups: normal serum uric acid (n=397) and hyperuricemia (n=189).The morbidity of macrovascular and microvascular complications and the serum levels of CRP,TNF,IL-1,IL-2,IL-6,IL-8 and PAI-1 were detected. Results (1)Hyperuricemia group showed higher BMI, blood pressure, TG,TC,LDL,WHR ratio and lower serum HDL-C than the group with normal uric acid (all P<0.05).(2)Group with hyperuricemia showed higher morbidities of macrovascular and microvascular complications than the group with normal uric acid (all P<0.05).(3) Group with hyperuricemia had higher levels of CRP,TNF,IL-6,IL-8,PAI-1 (all P<0.05). Conclusions (1) Hyperuricemia is associated with obesity and dyslipidemia.(2) Hyperuricemia may cause atherosclerosis by inflammation of vascular endothelium, resulting macrovascular and microvascular complications.
2.Progress in the studies on the induction of pluripotent stem cells

The main objective of human tissue engineering is to grow cells in vivo to promote proliferation and differentiation and replace, repair, maintain, or enhance the functions of impaired tissues and organs. Embryonic stem cells are the first selected seed cells in tissue engineering due to their unlimited proliferation capability and the potential to differentiate into different kinds of cells. In this review will give a sketch of recent advances in the preparation of induced pluripotent stem cells, which can differentiate into many other kinds of cells, by applying different transcript factors including Oct3/4, Sox2, c-myc, Klf4, Nanog and LIN28 to somatic ceils via retroviral transduction.
3.Application of Integrative Chinese and Western Medicine Fast-track Rehabilitation in Surgery for Patients During Peri-operation Period of Gynecological Laparoscopy

Objective To observe the effect of integrative Chinese and western medicine fast-track rehabilitation in surgery (FTS) for patients during peri-operation period of gynecological laparoscopy. Methods A total of 226 patients receiving gynecological laparoscopic surgery were randomized into observation group and control group, 113 cases in each group. The observation group received integrative Chinese and western FTS treatment, and the control group received routine non-surgery treatment. After operation, time for the first post-operation anal gas evacuation, time for out-of-bed activity, hospitalization time, and incidence of post-operation complications were compared between the two groups. Results In the observation group, time for the first post-operation anal gas evacuation and time for out-of-bed activity were earlier, and hospitalization time was shorter than those in the control group (P<0.05). The incidence of post-operation complications was 0.9%, lower than 4.4% in the control group ( P<0.05). Conclusion Integrative Chinese and western medicine fast-track rehabilitation in surgery shows certain effect on promoting enterokinesia, shortening time for the anal gas evacuation, reducing post-operation complica-tions, promoting post-operation recovery of gastrointestinal function of patients receiving gynecological la-paroscopy .
4.Clinical Study of Acute Hypervolemic Hemodilution on Meningioma Operation

Objective To observe the clinical application of acute hypervolemic hemodilution (AHH) on patients undergoing surgical resection meningioma and the effects on the reduction of heterologous blood transfusion. Methods 40 scheduled mengingioma surgical patients were randomly divided into group I (AHH, n =20) and group II (control, n =20). All patients had same general anesthetic techniques. Amounts of heterologous blood transfusion and changes of postoperative hemoglobin in two group's were recorded and compared. Results BZ In AHH group 15 patients had no heterologous blood transfusion, 5 patients had 200~400 ml heterologous blood transfused. In control group all patients had 400~800ml heterologous blood transfused. Conclusion Acute hypervolemic hemodilution can significantly reduce the volumes of heterologous blood transfusion in mengingioma surgical patients.
5.Effect of ischemic postconditioning on IL-6 and ATP level during renal ischemia-reperfusion injury in obstructive jaundice rats

Objective To investigate the apoptosis effect of ischemic postconditioning on levels of IL-6 and ATP during renal ischemia/reperfusion injury in obstructive jaundice rats. Methods One hundred SD rats were randomly divided into 5 groups: the Sham control group , I/R group , OJ-Sham group , OJ-I/R group , and OJ- I/R+IPO group, with 20 cases in each group. According to the time after ischemia/reperfusion, each group was divided into four subgroups (n = 5), with reperfusion time of 0 h, 1 h, 3 h, 6 h, respectively. After 1 week of biliary obstruction , rats were sacrificed at 0 h , 1 h , 3 h , 6 h post-I/R , and the left kidneys were taken, renal tissue was used for determination of the levels of interleukin-6 (IL-6) and adenosinetriphosphate (ATP). Results Compared with the OJ-I/R group, the serum level of BUN,Cr decreased significantly in the OJ-I/R-IPO group (P < 0.05). Conclusion Ischemic postconditioning can reduce the injury of kidney ischemia-reperfusion in OJ rais , which may be related to the reduced inflammation reaction and the energy metabolism in the kidney.
6.Application of Plato analysis method in identifying the security risks of the clinical cases

Objective To apply Plato analysis method to clinical cases which were reported from clinical nursing, finding out the key link of the risks which were related to the cases:the key age, the key period, the key spectrum of disease and the key projects, etc. To carry out the feedforward control of the quality of care, and ensure the quality and safety of nursing. Methods A database were set up including clinical special cases, high-risk cases, security hidden danger cases and emergency cases. Thetwo eightprinciple of Plato analysis method was used to find out the main factors, the principal contradiction, the key hazard and the key problems of the cases, etc. The aim was to provide guidelines of risk analysis, risk prevention and risk aversion for nurses, and improve their consciousness of security hidden danger, and reduce security hidden danger cases. Results The results of 207 cases collected from 2014 to 2015 showed that the main risks involved the age groups were 41-80 years old and 21-30 years old. The major risks involved the spectrum of disease were the nervous system, cardiovascular disease, surgical anesthesia related problems, kidney disease, cancer and respiratory system. The major risks involved the time were the midnight, morning and afternoon. The major risks involved the locations were sickroom, emergency room, auxiliary inspection departments and outpatient. The major risks involved items were disease mutation, psychological problems, medication safety and instrument safety, among the disease mutation, timely discovered, rescued 13 patients with heart and breathing pause, stop 13 patients who attempted suicide. Since the Plato analysis method was applied to identify the security risks of the clinical cases, the number of falling, all kinds of pipe shed, skin problems and suicides were decreased from 6, 6, 7, 5 in 2014 to 2, 2, 2, 1 in 2015, declining trend year by year. Conclusions Applying the Plato analysis method to identify the security risks of the clinical cases which can find out the main problems of cases and improve nursing quality and management efficiency.
7.Expression and clinical value of serum miR-148a-3p and miR-551b-5p in acute pancreatitis

Objective:To investigate the expression level and clinical value of serum miR-148a-3p and miR-551b-5p in patients with acute pancreatitis (AP).Methods:The clinical data of 152 patients with AP admitted to Danzhou people's Hospital from January 2017 to September 2020 were selected. According to the severity of their illness, they were divided into MAP group ( n=70), MSAP group ( n=40), and SAP group ( n=42). The SAP group was divided into survival group ( n=25) and death group ( n=17) according to the prognosis of the patients. Another 50 healthy people were selected as the control group. Fastening venous blood was collected on the day of onset, and the expression levels of serum miR-148a-3p and miR-551b-5p in each group were detected by real-time quantitative PCR. The receiver operating characteristic curve (ROC) was drawn and the area under the curve (AUC) was calculated. The expression levels of serum miR-148a-3p and miR-551b-5p were analyzed to evaluate the prognosis of SAP. The correlation between serum miR-148a-3p and miR-551b-5p expression levels in SAP patients was analyzed by Pearson method. Results:The expression levels of serum miR-148a-3p (3.18±1.27 vs 0.96±0.28) and miR-551b-5p (1.94±0.85 vs 0.51±0.12) in AP group were significantly higher than those in control group ( P<0.001). The expression levels of serum miR-148a-3p (4.36±1.70 vs 2.84±1.10, 2.50±0.92) and miR-551b-5p (2.80±1.04 vs 1.68±0.53, 1.42±0.45) in SAP group were significantly higher than those in MSAP group and MAP group ( P<0.001). The expression levels of serum miR-148a-3p (5.30±1.95 vs 3.47±1.40) and miR-551b-5p (3.62±1.37 vs 2.08±0.91) in death group were significantly higher than those in survival group ( P<0.001). ROC curve analysis showed that the AUC of the combination of miR-148a-3p and miR-551b-5 in judging the prognosis of SAP was significantly higher than that of the single index of miR-148a-3p and miR-551b-5[0.943(95% CI0.886-0.997) vs 0.860(95% CI0.797-0.924), 0.822(95% CI0.795-0.880)], with a sensitivity of 98.3% and specificity of 84.6%. Correlation analysis showed that there were positive correlation between the expression level of serum miR-148a-3p and miR-551b-5p in SAP patients ( r=0.835, P<0.001). Conclusions:The expression levels of miR-148a-3p and miR-551b-5p in serum of AP patients were significantly increased, and the combined detection of miR-148a-3p and miR-551b-5p had a good value in judging the prognosis of SAP.
8.Coumarins of Anemone raddeana Regel and their biological activity.

To study the coumarins of Anemone raddeana Regel, the compounds were separated by silica gel column chromatography and HPLC. Their structures were identified by their physicochemical property and spectral analysis. Two new compounds were isolated and identified as 4, 7-dimethoxyl-5-methyl-6-hydroxy coumarin (1) and 4, 7-dimethoxyl-5-formyl-6-hydroxycoumarin (2). The bioassays indicated that compounds 1 and 2 could significantly inhibit the proliferation of cancer cell, and showed the agonist effect on the transactivity of retinoic acid receptor-alpha (RARalpha). In addition, the two compounds had inhibitory effect against human leukocyte elastase (HLE).
9.Effect of anisodamine on sepsis-induced multiorgan dysfunction syndrome

Objective To investigate the effect of anisodamine on oxygen metabolism and organ function in dogs with sepsis.Methods Twenty beagles were divided into sepsis group (n =8) and anisodamine-treated group (n =12) according to the random number table.The sepsis model was established by cecal ligation and puncture.Changes in thrombomodulin (TM),arterial oxygen partial pressure (PaO2),oxgen uptake rate (ERO2) and lactate were mearsured before and 1,3,8 and 24 h after operation.Wet/dry ratio of lung tissues and incidence of multiorgan dysfunction syndrome (MODS) were detected 24 h after operation.Results Level of TM was increased progressively after operation,which was significantly higher in sepsis group than in anisodamine-treated group at 3,8 and 24 h [(28.7±15.9) vs (19.5 ±10.6)ng/ml,(36.5±20.8) vs (21.9±12.8)ng/ml,(52.6±32.7)vs (31.3 ± 18.4)ng/ml respectively] (P < 0.05).The PaO2 decreased slowly after operation,with the decrease more significant in sepsis group than in anisodamine-treated group at 24 h [(65.6 ± 14.5) vs (83.3 ± 16.3)mmHg] (P <0.05).The ERO2 had a rise-and-fall trend after operation,with the level lower in sepsis group than in anisodamine-treated group at 24 h [(16.7 ± 3.1) % vs (21.7 ± 2.9) %] (P < 0.05).Level of blood lactate increased gradually,with the level higher in sepsis group than in anisodamine-treated group at 24 h [(4.4 ± 1.2) vs (3.1 ± 0.8) mmol/L] (P < 0.05).Wet/dry ratio of lung tissues in sepsis group was (510 ± 80) % versus (370 ± 50) % in anisodamine-treated group at 24 h (P < 0.05).Incidence of MODS in sepsis group was 38% versus 17% in anisodamine-treated group at 24 h (P < 0.05).Conclusion Anisodamine may ameliorate the injury to vascular endothelial cells and oxygen metabolism disorder after sepsis and hence contributes to the reduction of sepsis-induced MODS.
10.Expression of cysteine rich 61 and vascular endothelial growth factor in patients with multiple myeloma and their relationship with staging and therapeutic response

Objective To investigate the expression of cystein rich 61 (Cyr61) and vascular endothelial growth factor (VEGF) in different stages of multiple myeloma (MM) patients,evaluate their relationship with angiogenesis and prognosis,and to determine the relationship between Cyr61 and VEGF.Methods Expression of Cyr61 and VEGF in BMMNC from 31 patients with different stages of MM and 10 cases of control were detected by RT-PCR.Results Expression of Cyr61 and VEGF in patients with MM (0.3652±1.5423,0.4815±1.3423) were significantly elevated in comparison to control (0.1862±0.7542,0.2012±1.2331) (P < 0.05,P < 0.01).Expression of Cyr61 and VEGF in stage Ⅲ (0.4632±0.1634,0.5356± 0.2342) was significantly higher than those in stage Ⅰ and stage Ⅱ (t =2.423,2.524,P < 0.05),but there was no difference between stage Ⅰ and stage Ⅱ (0.2513±0.1365,0.3064±0.2124; 0.3084±0.2254,0.3653±0.1265) (t =1.782,1.824,P > 0.05).The levels of Cyr61 and VEGF were significantly decreased after chemotherapy compared to before chemotherapy (P < 0.01).Expression of Cyr61 and VEGF were positively correlated (r =0.8941,P < 0.01).Conclusion Cyr61 and VEGF may play roles in the angiogenesis and pathophysiology of MM.It can be used to guide treatment and estimate prognosis by monitoring Cyr61 and VEGF.
