1.Advanle in the diagnosis and treatment of posttransplantation lymphoproliferative disorder

Recenly,the incidence of posttransplatation lymphoproliferative disorder(PTLD) is rising,Immunodeficiency and EBV-infection are risk factors of pathogenesis related. The majority of PTLD originates from malignant proliferation of B-cell.Typical pathologic findings of PTLD are large number of plasmacytoid B cells in lymphotic tissue and often with necrosis in regional areas. Molecular biology findings are ras or p53 gene mutation and c-myc?bcl-6 gene rearrangement. Highly immunosuppressed patients may present with diffuse infiltration and functional failure of multiple organs,the prognosis is poor. Except for the decrease or discontinuation of the immunosuppressive therapy,antiviral agents and interferon- ?,anti B-cell monoclonal antibodies,autologous EBV-specific cytotoxic T cell and combination chemotherapy may achieve better responses.

In the 46th annual meeting of the American Society of Hematology, the advance in the treatment of maligmant hematological disease concentrats on targeted therapies, including oblimersen,combination therapy rituximab with the chemotherapeutic agent, proteasome inhibitor bortezomid, flavorpiridol and radioimmunotherapy. Hematopoietic cell transplantation (HCT) is an effective therapy of maligmant hematological disease. The foremost advantage of the nonmyeloablative or reduced-intensity HCT is the reduction of treatment-related mortality .

Objective To investigate the association of the polymorphism of serotonin transporter gene linked polymorphic(5-HTTLPR) region with depression and its clinical symptoms in the Han Chinese.Methods PCR-based technique was used to genotype 5-HTTLPR in 64 patients with major depression and 67 unrelated controls,and the association between the polymorphism and clinical symptoms of depression was explored according to HAMD score.Results Fewer patients tended to be of L/L genotype(9.4%) than controls(19.4%),but there was no significant difference.Among women,fewer patients tended to be of L/L genotype(2.9%) than controls(13.9%),but there was no significant difference.The symptom analysis showed that depressive symptoms were significantly associated with 5-HTTLPR genotypes or alleles in the patients of L/L genotype,and that L allele had significantly lower depressive scores compared with those of other genotypes or alleles(P
4.Treatment of developmental mandibular asymmetry deformity by using combination of orthognathic surgery with orthodontics

Objective Developmental mandibular asymmetry is a common deformity in the human craniofacial skeleton and the treatment is not easy to get satisfactory results. A retrospective study was performed to investigate the clinical results and stability after treatment of developmental mandibular asymmetry by using combination of orthognathic surgery with orthodontics. Methods 58 cases of developmental mandibular asymmetry were treated by preoperative orthodontics-orthognathic surgery -postoperative orthodontics procedure. 82 times of operation, such as mandibular osteotomy, bimaxillary osteotomy, jaw plastic surgery, genioplasty, and distraction osteogenesis were performed according to different type of asymmetry. The facial appearance and occlusion relationship were evaluated with 1~4 years of follow-up. Results Satisfactory results were achieved in 53 cases (91.4 %). Primary satisfactory results were obtained in 4 cases (6.9 %). 1 case (1.7 %) with unsatisfactory result obtained aesthetic effect and ideal occlusion after operation again. Conclusions It is essential to use both of orthodontics and orthognathic surgery in order to ameliorate facial appearance, resume occlusal function, achieve good corrective result and prevent the recurrence of the developmental mandibular asymmetry.
5.Study on gene mutation characteristics of a spinocerebellar ataxias family in Ningxia region

Objective To study the gene mutation characteristics of patients with spinocerebellar ataxias(SCA) in Ningxia region.Methods The SCA3/Machado-Joseph disease(MJD)trinucleotide CAG repeat number was detected by polymerase chain reaction(PCR),capillary gel electrophoresis(CGE) and DNA sequencing in 6 patients with SCAs and 4 normal kindreds from a family in Ningxia region.Results All patients and 1 kindred were positive for SCA3/MJD;other 3 kindreds were negative for SCA3/MJD.The CAG repeat number was expanded to 66~81 in the patients with SCA3/MJD and a kindred,and 22~33 in other 3 kindreds.Conclusion There is SCA3/MJD in the patients with SCAs of Ningxia region.Gene examining has the great significance for definite diagnosis in the patients with SCAs.
6.Clinical efficacy of EH composite material combined with CAD/CAM technique in plastic treatment of unilateral micrognathia

Objective To evaluate the clinical efficacy in the treatment of unilateral micrognath ia by using EAM medical resin and hydroxyapatite (EH) composite material combined with the CAD/ CAM technique.Methods From July 2011 to October 2015,12 cases of unilateral micrognathia caused by different reasons were treated,based on the representative traits and requirements,refering to the unsymmetric counter part by right of CAD/CAM technique.By the 3-dimensional design and reconstruction,we reformed the EH composite material into purposed shape,and insert it in the operative area.Results All the materials were closely suitable to the mandible surfaces.1 case failed because the wound was torn apart;the other 11 patients recovered more than 6 months and were satisfied with the external appearance.Conclusions The EH composite materials combined with CAD/ CAM techniques could be a potential characterized remedy for the unilateral micrognathia.
7.Transurethral electroscission fenestration for the treatment of ureterocele in children (report of 88 cases)

Ⅳ?were cured by anti-reflux ureteral reimplantation after 1 year;in the other 9 cases, VUR disappeared within 4 years. In the 18 cases ac ;ompanied with hydronephrosis/partial hydronephrosis, hydrops disappeared in 3 years in 15 cases,and it cisappeared after 10 years in 3 cases. All the ureteroceles in 88 children disappeared 1 year after the procedue. Conclusions Transurethral electroscission fenestration of ureterocele through cystoscope is a simple,effective and minimal invasive procedure for children with ureterocele.
8.Medial pancreatectomy

Medial pancreatectomy has been gradually used in the treatment of benign tumors in neck and body of pancreas since less removal of the pancreas and the retainment of the duodenum,spleen and functional pancreas,it is less harmful to the endocrine and exocrine function of the patients.Seventeen patients received medial pancreatectomy for benign tumors in neck and body of the pancreas in the China-Japan Union Hospital of Jilin University from November 2005 to December 2010.Fifteen patients received the closure of broken ends of pancreatic head,pancreaticojejunostomy,and the other 2 received pancreaticojejunostomy.No perioperative death was found.All the patients were followed up for 2 to 45 months,and no death was recorded.No patient got new-onset diabetes and pancreatic pseudocyst,and their tumors were not relapsed.Now the retrospective analysis was carried out to this group of patients and to further regulate the surgical operation of the medial pancreatectomy.

Objective To deal with measurements of the relative convexity, to observe the morphological characteristics of zygoma and zygomatic arch, and to provide references for anthropology and medical science. Methods 108 dry skulls (62 male and 46 female) from Wuhan University Medical School were used, the zygomatic relative convexity of both sides were measured, and the morphological characteristics were observed in this study. Results Zygomatic process point lied in the middle of zygoma in horizontal direction, and situated 15-20 mm below the external middle 1/3 of infra-orbital margin. 10.Protective effect of miconazole on white matter damage induced by anoxia and ischemia in rats

Objective To explore the protective effect of miconazole on white matter damage (WMD) in neonatal rats. Methods Three-day-old neonatal SD rats were randomly divided into sham group, WMD model group, 10 mg/(kg·d) miconazole group and 40 mg/(kg·d) miconazole group with 15 rats each. Rats in WMD model group were subjected to the ligation of right carotid artery, and then kept in a chamber with 6% oxygen and 94% nitrogen for 80 min to establish the white matter damage model. The rats in miconazole group were intraperitoneally injected with different doses (10 and 40mg/kg) of miconazole, dissolved in dimethyl sulfoxide (DMSO), for five consecutive days, and rats in WMD model group were injected with the same volume of DMSO. Myelin basic protein (MBP) of white matter was detected by immunofluorescence staining and western blot. Myelin sheaths of corpus callosum were observed by transmission electron microscopy. Weight changes of rats were compared among groups. Results Immunofluorescence staining and western blot showed that, after treatment with miconazole, the MBP expression level of corpus callosum was higher than in WMD model group (P<0.05). In WMD model group, the myelin sheath of corpus callosum had loose structure and a large number of small vacuoles with decreased thickness of myelin sheath. After treatment with miconazole, myelinolysis induced by anoxia and ischemia could be improved significantly. The increase in weight of rats in WMD model group was significantly less than that in sham group. And after miconazole treatment, the rate of weight gain of rats was increased. Conclusion Miconazole can significantly reduce the brain white matter damage induced by anoxia and ischemia through promoting myelination, and then improves the growth and development in rats.
