1.The clinical efficacy of the different margins in nephron-sparing operation for small renal cell ;carcinoma

Objective To compare the clinical efficacy of different margins in nephron-sparing operation for patients with small renal cell carcinoma. Methods From September 2008 to April 2013, a total of 64 patients with local renal cell carcinoma (T1a period) and treated with nephron-sparing operation were selected, and the clinic data were analyzed. According to cutting edge size gotten from the surgery, the patients were divided in to A group (cutting edge 1-5 mm group, 30 cases) and B group (cutting edge 6-10 mm group, 34 cases). The operation condition and recurrence rate and survival rate of two groups were compared. Results The operative time in A group was significantly shorter than that in B group:(130.1 ± 24.0) min vs. (152.3 ± 28.0) min, P<0.05. The drainage time and the level of serum creatinine in two groups had no significant differences (P>0.05). The 3-year recurrence rate and 3-year survival rate in two groups had no significant differences (P>0.05). Conclusions The clinical efficacy of different margins in nephron-sparing operation for small renal cell carcinoma is similar. But 1- 5 mm cutting edge size nephron-sparing operation has less complications, and the recurrence rate and survival rate does not increase. It is worthy of spread .
2.Content Determination of Heavy Metals in Xiaohuoluo Pills by Atomic Absorption Spectrometry

Objective:To determine the content of lead, cadmium, arsenic, mercury and copper in Xiaohuoluo pills produced by 34 enterprises. Methods:Using microwave digestion, the contents of lead, cadmium and arsenic were determined by graphite furnace atomic absorption spectrometry;the content of mercury was determined by hydride generation atomic absorption spectrophotometry;the content of copper was determined by air-acetylene flame atomic absorption. Results: Lead, cadmium, arsenic and mercury in Xiao-huoluo pills from 34 different manufacturers were beyond the limits in various degrees. Conclusion: The method is simple and accu-rate, and can be used in the heavy metal determination of Xiaohuoluo pills.
3.Expression of IP-10 and Its Significance in Pathological Scar

Objective To explore the expression of 10 kDa interferon-gamma-induced protein(IP-10)in pathological scar and its role in the pathogenesis of pathological scar.Methods Imrnunohistochemistry was performed to detect the expression and distribution of IP-10 in 28 patients with keloid(group K),34 patients with hypertrophic scar(group HS),and 20 normal controls(group N).The data were collected and analyzed statistically.Results The expression of IP-10 was significantly higher in groups K and HS than in group N(P<0.01),but no significant difference in the expression of IP-10 was found between groups K and HS.Conclusion IP-10 may enhance the formation of pathological scar by attracting T lymphocytes and inducing immune/inflammatory response.
4.Analysis of nursing ethics and practice of physical constraints in the intensive care unit

Patients in intensive care unit were more critically ill, and often accompanied by dottiness and restless. In order to avoid the adverse events and self-injury, body constraints were often used on the patients. Therefore, this paper would discuss the ethics of body constraints and give the suggestions. According to the literature review and the analysis of the clinical nursing experience of relevant ethical principles, data was collected. The principle of informed consent, respect, and harmless was hard to decide on the issue of body constraints for patients in ICU. Nurses might pay attention to the application of ethical principles, and strengthen the humanistic care. On the issue of body constraints for ICU patients, nurses might pay special attention to the application of possible ethics principles in nursing process and reduce the application of the body constraints, and promote the physical and mental comfort of patients. It reflected the progress of the humanistic solicitude in the current clinical nursing and important value.

Objective To observe the ultrastructure of pancreas in the bustard( Otis tarda limaells) . Methods Three bustard's pancreas were observed with electron microscope. Results The exocrine secretory units of bustard's pancreas contain a lobulated tubulo acinar gland in which the lobulation is not so distinct as that of the mammalian gland due to the lack of interlobular connective tissue septa.The space between lobular pancreas is lined directly with pancreatic cells as the duct.Single long spindle shaped connective tissue cell could only be seen among three lobules and its cytoplasm becomes slender and stretches into the space between lobules as a distinct intermediate dense line.The exocrine secretory cells can be divided into two kinds of dark and light.In the light cell,particularly in swelled up rER with very little density are filled the greater part of the cytoplasm and made the cell appearance much lighter than other cells.\;The endocrine cells are divided into two types that are dispersed in pancreatic singly and clustered into pancreatic islets.The dispersed endocrine cells contain a lot of moderate dark electron dense granules in their process near basement membrane.There are two types of islets——A islet and B islet in the bustard pancreas.A islets comprise only one cell type with moderate electron density that contains dark endocrine granules.B islet contains two cell types:B and D cell.B cells are the most numerous cells with a long ovoid nucleus and contains two or three types of granules in the cytoplasm.D cells are relatively rare and can be seen in the periphery nearby basement membrane.Cells keep close to each other and desmosomes can be seen within the islet.The endocrine cells closely contact exocrine cells without basement membrane in the periphery partly.Conclusion\ These are some structural characteristics that adapt to fly and that are different from other animal in bustard's pancreas.
6.Changes of chemical components and anti-tumor activity of total ginsenosides from ginseng stems and leaves tranformed by submerged fermentation of ganoderma lucidum

Objective:To study the biotransformation rule of the chemical components of total ginsenosides from ginseng stems and leaves tranformed by submerged fermentation of ganoderma lucidum,and to lay the foundation for further development and research of the medicinal fungus of ginseng stems and leaves.Methods:The total ginsenosides from ginseng stems and leaves were fermented by ganoderma lucidum,the changes of chemical components before and after fermentation were measured by thin-layer chromatography;the levels of total ginsenosides from ginseng stems and leaves before and after fermentation were detected by ultraviolet spectrophotometry;the levels of ginsenoside Rb1,Rg3,Rd,CK,Re,Rg,and Rh1 before and after fermentation were measured by high performance liquid chromatography.The human hepatocellular carcinoma SMMC-7721 cells were divided into blank control group,low,medium and high doses of total ginsenosides from ginseng stems and leaves groups and medicinal fungus of ginseng stems and leaves groups.The dosages of total ginsenosides from ginseng stems and leaves and medicinal fungus of ginseng stems and leaves were 5,10 and 20 mg·L-1,respectively.The inhibitory rate(IR) of the cells was determined by MTT assay.Results:The results of thin-layer chromatography showed that the saponins before and after fermentation occured mutual transformation;the levels of total ginsenosides from ginseng stems and leaves before and after fermentation were 749.98 and 602.26 mg·g-1,respectively;the levels of panaxadiol saponins Rb1,Rg3,Rd and CK before fermentation were 24.54,2.21,87.22 and 0 mg·g-1,and the levels of panaxatriol saponins Re,Rg and Rh1 were 151.34,77.02,and 3.06 mg·g-1.After fermentation,the level of Rb1 was decreased by 61.45%,the levels of Rg3,Rd and CK were increased by 238.91%,34.43% and 268.00%.The levels of Re and Rg1 were decreased by 63.75% and 64.41%,and the level of Rh1 was increased by 408.88%;the IR of the SMMC-7721 cells in medium dose of medicinal fungus of ginseng stems and leaves group was higher than that in medium dose of total ginsenosides from ginseng stems and leaves group(P<0.05);the IR of the SMMC-7721 cells in high dose of medicinal fungus of ginseng stems and leaves group was higher than that in high dose of total ginsenosides from ginseng stems and leaves group(P<0.01).Conclusion:The total ginsenosides from ginseng stems and leaves transformed by the submerged fermentation of ganoderma lucidum can significantly change its chemical components,which have stronger anti-proliferation activity in the SMMC-7721 cells.
7.Clinical evaluation of 287 CAD/CAM zirconia ceramic restorations:A 3-year follow up

Objective:To evaluate the clinical performance of CAD/CAM zirconia all-ceramic restorations.Methods:287 all-ce-ramic Zirconia restorations in 206 patients were included in a 3-year prospectively survey with California Dental Association Standard (CDA)as a reference.The effects and the related factors such as restoration type,tooth region,fiber reinforced composite application on survival rate were analyzed.Kaplan-Meier survival analysis and Log-Rank test were used for data analysis.Results:Chipping frac-tures in 5 restorations,intense gingivitis at 4 restorations and periapical inflammation for 3 restorations were observed during the obser-vation period.The 3-year cumulative survival rate (CSR)of CAD/CAMzirconia restorations was 95.7%,The differences among the CSR of single crowns(96.3%),linked crowns (93.6%)and fixed partial dentures(95.7%)were not statistically significant(P >0.05).The difference was not statistically significant(P >0.05)between the CSR of the anterior region group(94.5%)and posterior region group(96.3%)as well as difference(P >0.05)between the CSR of fiber reinforced composite group (95.0%)and without fiber reinforced composite group(96.1%).Conclusion:The cumulative survival rate of CAD/CAM zirconia all-ceramic restorations is high.Chipping fracture is the main reason of failure.Restoration type,tooth region and fiber reinforced composite have no significant effect on the survival of zirconia restorations.
8.Dynamic contrast enhanced MRI and diffusion weighted imaging features of granulomatous mastitis: compared with breast cancer

Objective To investigate the radiological features of granulomatous mastitis (GM) in dynamic contrast enhanced MRI (DCE-MRI) and DWI and to differentiate it from the breast cancer in diagnose.Methods Forty five cases of GM and 64 cases of breast cancer confirmed by surgical histopathology or biopsy were retrospectively analyzed in the study.All of the patients were examined preoperatively by DCE-MRI and DWI.Including lesion type,location,enhancement pattern,nipple retraction,supplying arteries,skin thickening and axillary adenopathy in the two groups were evaluated and analyzed by using x2 test.One-way ANOVA was employed to compare the ADC values between the abscess area of GM and non-abscess area,and the difference among the breast cancer lesion area.Dynamic enhancement MR pharmacokinetic parameters were used to measure including early-phase enhancement rate (EER),peak enhancement ratio(Emax),and time to peak ehhancement(Tmax).The statistical differences of EER,Emax and Tmax between the two groups were calculated by using Wilcoxon test.Results In 45 cases of GM,DCE MR images showed nonmass-like lesions (43 patients) and mass-like lesions (2 patients); the nipple involved(16 patients) and segment involved (29 patients);rim-like with heterogeneous enhancement (40 patients) and heterogeneous enhancement (5 patients); nipple retraction (24 patients) supplying arteries dilatation (42 patients),skin thickening (29 patients),and axillary adenopathy (17 patients).Corresponding to the radiological features above,in the 64 breast cancer cases,it showed 54,10,5,59,30,34,16,51,12 and 20,respectively.There were statistical significance between GM and breast cancer in lesion type,location,enhancement pattern,and nipple retraction (x2=67.574,13.075,20.297,20.398 and 23.510,respectively,all P<0.01).But no differences were existed between 2 groups in supplying arteries and axillary adenopathy(x2=3.928 and 0.502,P>0.05).EER,Emax and Tmaxin GM were 146.58%,191.13%,195.00 s in GM and 118.13%,159.43%,183.33 s in breast cancer,respectively.Significant statistic differences between GM and breast cancer were found in EER and Emax(Z values were-2.271 and-2.948,P<0.01).But it did not show significant difference in Tmax (Z =-0.548,P>0.05).The ADC values of GM on abscess area,non-abscess area,and breast cancer lesion area were (8.0±2.6) × 10-3,(11.3± 1.7) × 10-3 and (8.2± 1.1) × 10-3mm2/s,respectively.There were significant differences in the groups (F=52.167,P<0.01).Conclusions The characteristic of radiological findings can be found in GM by using advanced MR imaging techniques.DCE-MRI combined with DWI is useful in the differential diagnosis between GM and breast cancer.
9.Study on the immunogenicity and the stability of the toxicity,antigenicity of Neisseria meningitidis serogroup A/C

Objective To study the stability of the toxicity,antigenicity and immunogenicity of Neisseria meningitidis serogroup A/C[CMCC(B)29201/29205],and to analyze the quality of the capsular polysaecharide extracted from Neisseria meningitidis.Methods The toxicity of the 3rd,5th,10th,15th,20th,25th and 30th passage of the Neisseria meningitidis was assayed in mice,and the antigenicity of each passage was measured by the tube agglutination test(TAT)and ELISA.The effect of individual 30 passages of Neisseria nveningitidis on the brain tissue and the immunogenicity of each passage were tested in mice,and the capsular polysaccharide was purified and analyzed.Resuits The LD50 of the strains CMCC(B)29201/29205 of each passage was low(≥109/ml),and all the 30 passages of the two strains had no effect on the brain tissue of the mice.The titer of each passage was 1∶320 in TAT and more than 1∶3752 in ELISA.After immunization with individual 30 passages of the Neisseria meningitidis the titers in serum bactericidal assay (SBA)were all more than 1∶32.The capsular polysaccharide purified from Neisseria meningitidis met the quality standard of the requirement.Conclusion The strains of Neisseria meningitidis serogroup A/C used in the manufacture of the meningococcal conjugate vaccine,are stable in the toxicity,antigenicity and immunogenicity.And the capsular polysaccharide has met the quality standard.
10.Immunogenicity and hereditary stability of Neisseria meningitidis serogroup W135/Y strains used in conjugate vaccine production

Objective To evaluate the immunogenic stability and hereditary stability of Neisseria meningitides serogroup W135/Y[CMCC(B)29037/CMCC(B)29028]within all the passages,which isolated from china.Methods The toxicity of the 3rd,5th,10th,15th,20th,25th and 30th passage of the Neisseria meningitidis was assayed in mice.Serological detection and biochemical detection were measured,and immunized mice subcutaneously.The antigeeicity of each passage of Neisseria meningitides serogroup W135/Y were measured by serum bactericidal test and the indirect ELISA.With the 30 passage of Neisseria meningitides serogroup W135/Y,the effect to the encephalic tissue was measured in mice.Fermented the Neisseria meningitides serogroup W135/Y with 30 passage and purified the capsular polysaccharide,then analyzed the quality respectively.Results The LD50 of the strains CMCC(B)29037/29028 of each passage was low(LD50 ≥ 109),and all the 30logical detection and all the 30 passage of the two strains were half in the tube agglutination.Glucose and maltose fermentation test were positive.Fructose,sucrose and lactose fermentation test were negative.The GMT of immunogenicity were 1114 and 2229 respectively and all the 30 passage were more than 640 and 1040 respectively.After Immunization with individual 30 passage of the Neisseria meningitides,the titer in serum bactericidal assay(SBA)and indirect ELISA were no difference.The capsular polysaccharide purified from Neisseria meningitides serogroup W135/Y met the quality standard.Conclusion Neisseria meningitides serogroup W135/Y,CMCC(B)29037/29028,used in the manufacture of the meningococcal conjugate vaccine,are stable in the toxicity,antigenicity,immunogenicity.Serological detection and biochemical detection are qulified,and the capsular polysaccharide has met the quality standard.
