1.Progress of clinical indicators in the diagnosis of neonatal sepsis

Neonatal sepsis progresses very fast,and its clinical signs are non-specific,and it can cause severe complications,such as shock,MODS.Therefore,early diagnosis is key to reduce mortality and improve prognosis.The clinical indicators in diagnosis of neonatal sepsis includ bacterial culture,peripheral hemogram,acute phase reaction proteins,cytokines,cell surface antigen and bacterial gene detection.This article reviews the clinical significance of these indicators in the early diagnosis,condition assessment,treatment monitoring of neonatal sepsis.
2.Combined detection of procaicltonin and C-reactive protein in the early diagnosis of neonatal nosocomi-al infection

Objective The early stage of neonatal sepsis is short of specific clinical manifestations that easy to be misdiagnosed.This study aimed to demonstrate the clinical value of combined markers[proca-icltonin(PCT)and C-reactive protein(CRP)]in the early diagnosis of neonatal hospital-acquired infections by dynamic monitoring of PCT and CRP.Methods The study included 111neonates in the 1st Neonatal Ward of Shengjing Hospital from June 2013to August 2014which were divided into three groups and retro-spectively reviewed,including 37cases of diagnosed sepsis group,42cases of clinical sepsis group,and 32ca-ses of control group(non-sepsis neonates).We measured the serum levels of PCT and CRP in two sepsis group before antibiotic administration,12h and 24h after infection,3d and 7d after infection controlled,and in the control group before antibiotic administration.Results Before antibiotic administration,serum levels of PCT and CRP were significantly higher in two sepsis groups than in the control group(P﹤0.01).In two sepsis groups,PCT reached peak at 12h after infection[(15.00±15.51)ng/ml and(17.93±13.44)ng/ml] and decreased to normal at 3d after infection controlled[(0.49±0.47)ng/ml and(0.42±0.34)ng/ml];CRP reached peak at 24h after infection[(37.53±30.29)mg/L and(32.41±29.33)mg/L]and decreased to normal at 7d after infection controlled[(5.72±2.98)mg/L and(5.06±3.07)mg/L].The optimal cut-off values were PCT﹥2ng/ml and CRP﹥10mg/L(Youden index 76.11%,59.45%),the sensitivity were 88.61%and 75.70% ;specificity were 87.5% and 83.75% ;positive predictive value were 94.59% and 95.65% ;negative predictive value were 75.68%and 46.15%.Receiver operating characteristic area under the curve were 0.964,0.887.Conclusion In early stage of sepsis,both PCT and CRP increase.The optimal cut-off values are CRP﹥10mg/L and PCT﹥2ng/ml.CRP reaches peak at 24h after infection,decrease to normal at 7d after infection controlled,while PCT reaches peak at 12h after infection,decrease to normal at 3d after infection controlled.Combined detection of PCT and CRP can improve the sensitivity and specificity of the early diagnosis of neonatal hospital-acquired infections.
3.Diagnosis and treatment strategy of neonatal sepsis and septic shock

Sepsis is defined as that a variety of pathogenic microorganisms(including bacteria,fungi,viruses and protozoa) invade the blood circulation,produce toxins and cause systemic infection.The early symptoms of neonatal sepsis are atypical.Neonatal sepsis is urgent and progresses quickly,especially in preterm infants,and it is easy to prone to septic shock which is life-threatening.Therefore,early identification,accurate diagnosis,and active intervention for neonatal sepsis and septic shock is essential to reduce mortality and improve prognosis.
4.Impact of the effective parts of Acanthopanax senticosus on GABA,TH and GFAP in C_(57) BL/6 mouse model of parkinson's disease

AIM: To study the effect of effective parts of Acanthopanax senticosus on Parkinson's disease mouse model(from C_(57) BL/6 strain) concerning Gammaamino butyric acid(GABA),nigra-striatum tyrosine hydrozylase(TH) and hippocampal glial fibrillary acidic protein(GFAP). 5.Co-expression network and function analysis of TP53 and NOTCH1 in head and neck squamous cell carcinoma

Objective To find out the expression relation between TP53 and NOTCH1,and to explore their effects in head and neck squamous cell carcinoma.Methods Obtained the differentially expressed genes data of head and neck squamous cell carcinoma from 279 samples in TCGA database.Analyzed the co-expression relation between TP53 and NOTCH1 through Pearson and Spearman method.Cbioportal was used to analyze their co-expressed genes.Establish the co-expression network of TP53 and NOTCH1 with String database.The pathway and function of co-expression network was identified through KEGG and DAVID database respectively.Results Among the 279 samples,TP53 and NOTCH1 was co-expressed in head and neck squamous cell carcinoma.(Pearson score =0.45;Spearman score =0.41) There were 182 interaction pairs of TP53 and NOTCH1 related co-expressed gene according to the String database.(Pearson and Spearman score > 0.3)These genes were enriched in some pathways such as T cell receptor signaling pathway,cell cycle,cell adhesion molecules and so on.These genes were enriched in some tumor related function including immune response,regulation of transposition,regulation of apoptotic process,cell cycle,regulation of GTPase activity and so on.Conclusion TP53 and NOTCH1 was co-expressed.Through establishing co-expressed network of TP53 and NOTCH1 and bioinformatics analysis,their function and signaling pathway were explored.The data generated from this study could provide a new reference in mechanism research of head and neck squamous cell carcinoma.
6.Risk factors of liver metastasis from gastric cancer

Objective To analyze the risk factors of liver metastasis from gastric cancer. Methods The clinical data of 225 patients with liver metastasis from gastric cancer who had been admitted to our hospital from January 1996 to December 2001 were retrospectively analyzed. Synchronous liver metastasis was observed in 123 patients and metachronous liver metastasis in 102 patients. The risk factors of liver metastasis from gastric cancer were evaluated. Results The gender of patients, location and size of gastric cancer foci, differentiation and invasion depth of gastric cancer, Lanren classification, lymph node metastasis and lymph node metastasis in region Ⅷ, vascular invasion, extrahepatic metastasis, ascites and peritoneal metastasis were the significant factors associated with liver metastasis from gastric cancer (X2 = 43.560-263. 907, P<0.05). All the factors except the size of gastric cancer foci, extrahepatic metastasis and ascites were found to be the significant factors associated with different types of liver metastasis from gastric cancer (X2 = 6.673-26. 555, P < 0.05 ). Logistic regression analysis demonstrated that the gender of patients, lymph node metastasis and peritoneal metastasis were the factors that determined the types of liver metastasis from gastric cancer. Conclusion The gender of patients, lymph node metastasis and peritoneal metastasis are the important factors to evaluate the occurrence of different types of liver metastasis from gastric cancer.
7.The value of negative lymph node count in T3 gastric cancer

Objective To evaluate negative lymph node count (NLNC) in prediction of prognosis of T3 gastric cancer after radical resection.Method 214 T3 patients of radical gastrectomy with complete clinical and follow-up data between Jan 2003 to Dec 2007 were enrolled.Survival was determined by the Kaplan-Metier method and univariate analysis was done by Log-rank test,Multivariate analysis was performed using the COX proportional hazard regression model.-2loglikelihood value and the hazard ratio (HR) value were used to compared the value of number of lymph node-negative (NLNC) staging and pN staging and lymph node metastasis rate (MLR) in gastric cancer prognosis evaluation.Results Univariate analysis showed that,pN stage (x2 =31.664),MLR stage (x2 =34.123),tumor size (x2 =5.025),type of differentiation (x2 =5.993),Borrmann classification (x2 =5.401),NLNC stage (x2 =37.256) were related to survival (P < 0.05).COX multivariate analysis showed that-2loglikelihood of pN staging is 1 336.761,HR value is 1.464,-2loglikelihood value of MRL staging is 1 335.821,HR value is 1.441.-2loglikelihood value of NLNC staging is 1 326.902,HR value is 1.725.The N0 and N1 staging prognosis in different NLNC staging was significant (P =0.008,P =0.014).Conclusions Sufficient number of negative lymph node prolongs survival and reduces the risk of early recurrence in advanced gastric cancer.
8.Screening of functional gene modules and the potential anti-cancer drugs in hypopharyngeal cancer

Objective Screening the functional gene modules that can play important roles in hypopharyngeal cancer and the potential anti-cancer drugs.Methods GEO database and MeV software were employed to screen the differentially expressed genes in hypopharyngeal cancer.Using STRING database,the protein-protein interaction network was identified.MCODE plugin of Cytoscape was used to identify the functional gene modules in the network.Based on DAVID database,the functions of modules were identified.DrugBank was used to screen the potential drugs that regulate the target genes of modules.Results 1 222 differentially expressed genes including 219 interaction pairs were i-dentified in whole genome profiling(P <0.05 ).Seven functional modules were identified in the network.The results of function analysis showed the module genes were enriched in cancer development related-function ‘regulation of angiogenesis’,‘cell adhesion’,‘DNA meta-bolic process’.A total of 50 potential drugs that regulating the 5 modules were screened.Conclusion Five functional modules that regulate the progress of hypopharyngeal cancer were identified,and maybe they can promote hypopharyngeal cancer through some functions such as regulation of angiogenesis 18 up-regulated protein kinases were identified.Their kinase inhibitors may potential have a role in anti-cancer, which provides a new target point for molecular therapy of nasopharyngeal cancer.
9.Site assessment and analysis of military operation ability of plateau training troops

Objective To investigate the effect of plateau training on military operation ability ,evaluate the real situa-tion of combat forces on the plateau , and to provide data for effectively improving the combat effectiveness of the army stationed on the plateau .Methods According to the standards , the levels of high altitude acclimatization , physical work capacity (VO2max, PWC170) and mental work capacity (digit span, digit symbol, pursuit aiming and visual reaction time) were measured to assess the military operation ability of plateau training troops , including troops that entered the plateau from the plain(plateau-entering troops), troops stationed on the plateau (plateau troops) and troops stationed in the plain ( plain troops ) .Results The troops on the plateau all reached basic acclimatization .The aerobic capacities of these three types of troops were of the standard medium level , and the maximum oxygen uptake ( VO2max ) of the plain troops was greater than that of that of plateau-entering troops(P<0.01), and the VO2max of the plateau troops was greater than that of plateau-entering troops(P<0.01).Compared with the plain troops, the VO2 max and PWC170 of the plateau-entering troops decreased by 25.7%and 27.7%respectively.There was no significant difference in the digit span , digit symbol, pursuit aiming between these 3 kinds of troops , but the visual response of the plateau-entering troops was prolonged ( P<0.01), while the pursuit aiming of the plateau troops was lower than that of the plateau-entering troops(P <0.05). Conclusion Acclimatization-promoting measures and plateau training can contribute to high altitude acclimatization formation, but cannot overcome the objective adverse effects of the plateau on physical fitness .Two months of plateau training fails to improve physical fitness.Therefore, special attention should be paid to the research on scientific and effective measures to improve physical fitness on the plateau .
10.Surgical treatment of multiple primary colon cancer in aged patients

Objective Summarize the experience in the surgical treatment of multiple original colon cancer in aged patients. Methods The retrospective analysis was performed in the 46 cases of multiple original colon cancer in aged patients in our hospital from August 1955 to May 2000. Results The 46 cases of multiple original colon cancer in aged patients account for 7.6% (46/608) of the total cases of colon cancer in patients in the same period. There were 26 cases of the male and 20 cases of the female. 30 cases were colon cancer with tumors from other organs. "The different time cancer" could be found 31 years later. The follow-up rate was 100%. The survival rates for 3,5,10,15,20 years were 71.1%(27/38), 63.6%(21/33), 43.3%(13/30), 28.6%(8/28) and 16.0%(4/25) respectively. In this study, we found that the survival rate of the group was higher than that of the aged patients suffering colon cancer in the same period of time. Conclusions The number of multiple original colon cancer in aged patients has been increased in the decade. The incidence was 43.5% (20/46) in the study. With the development of society, both doctors and patients have improved their knowledge about multiple original carcinomas in aged patients. The positive rate of early diagnosis was promoted to a high level, especially in the monitor and treatment of per operation. Choosing the energetic operation, we will acquire the better therapeutic efficacy for treatment of multiple original cancer in aged patients.
