1.AN EXPERIMENTAL STUDY OF THE PATHOGENIC EFFECTS OF MULTIELEMENT CONTENTS AND ITS PATTERNS IN FEEDS FROM KESHAN DISEASE AREA IN RATS

The pathogenic effects of crops from Keshan disease area were studied by rat-feeding. As double controls, another two groups were fed with crops from non-endemic area and synthetic diet. Those from disease area caused necrosis of heart muscle vascular changes of myocardial mitochondria and fatty degeneration of liver. Compared with the controls, the free radical and lipid peroxide content in heart and liver were significantly higher, while the ATP and AN contents in these organs were apparently lower. The multielement contents and its constituent patterns in the feeds, whole blood, heart, liver, kidney and brown adipose tissue of rats in experimetal group were apparently different from those in the control groups. The statistic analysis of multiple factors showed that eight elements Se, Mo, Cu, Mn, Cr, Fe, Zn and Pr might be the perilous factors, that is, under the condition of low selenium, the constituent pattern of selenium and the eight elements might be the complicated factors of endemic crops.

BACKGROUND:Foreign injectable sulphate calcium has good biocompatibility, injectability and in situimmobilization, moulding based on adaptation to the shape of bone defects, but the price is expensive. OBJECTIVE:To explore the optimal fabricating parameters of bone repair materials with α-calciumsulfate hemihydrates as the main component, and to study the performance and characterization METHODS:α-Calciumsulfate hemihydrates powder was mixed with sodium hyaluronate at liquid-solid-ratios of 0.2, 0.25, 0.3, 0.35, 0.4 mL/g using vapor-heat method to prepare injectable bone materials. Performance, setting time and compressive strength of the injectable bone was detected. The best liquid-solid-ratio was 0.3 mL/g.α-Calcium sulfate hemihydrates powder was mixed with calcium sulfate dihydrate powder (1%, 2%, 3% mass fractionas) to fabricate injectable bone materials. Performance, setting time and compressive strength of the injectable bone was also detected; meanwhile, the biosafety of the injectable bone was determined. Theinjectable bone material that was made at the liquid-solid-ratio of 0.3 mL/g and by 2% calcium sulfate dihydrate was implanted into Ba-ma swine models of thoracic bone defects. At the time points of 8, 16 and 24 weeks after implantation, histological observation was done. RESULTS AND CONCLUSION: The injectable bone material was made at the liquid-solid-ratio of 0.3 mL/g and by 2% calcium sulfate dihydrate. The initial and final setting time was 4.0-5.0 minutes and 8.0-9.0 minutes, respectively. The compressive strength of the injectable bone reached (8.93±0.23) MPa. These findings indicate that the injectable boen material has good performance, initial setting time and compressive strength meeting the requirements of clinical application and good biosafety. Animal experiments show that the injectable bone can provide space for new bone in creeping substitution way by auto-degradation, with osteogenic activity.
4.Initial clinical application of domestic endoscopic surgical robot system for partial nephrectomy

Objective:To evaluate the safety and effectiveness of Kangduo endoscopic surgical robot system for partial nephrectomy.Methods:Consecutive patients with stage T 1 renal tumor meeting the inclusion criteria from the Department of Urology, Peking University First Hospital from December 2020 to February 2021 were prospectively enrolled. All patients underwent partial nephrectomy with the Kangduo endoscopic surgical robot system after signing the informed consent. Clinical data including preoperative, perioperative and postoperative pathology and follow-up were collected. Results:Among the 26 patients, there were 16 males and 10 females, with a median age of 53(33-74) years, and a median body mass index of 25.99(20.90-32.91) kg/m 2. There were 12 cases of left kidney tumor and 14 cases of right kidney tumor. The median tumor diameter was 2.2(1.0-3.5) cm. The median time of warm ischemia was 17.7(7.1-29.2) minutes, and all of them were less than 30 minutes. The median docking time was 4.7(2.3-9.9) minutes, and the median time of robotic arm operation was 65.0 (37.0-155.0) minutes. The median National Aeronautics and Space Administration Task Load Index (NASA-TLX) score was 5.3 (2.0-28.0), and no instrument-related adverse events occurred intraoperatively. The median postoperative hospital stay was 4 (4-5) days. All tumor margins were negative on pathologic reports. No Clavien Ⅱ stage operative complications occurred in all patients during perioperative period and 1 month after the surgery. Conclusions:The partial nephrectomy using the kangduo endoscopic surgical robot system were completed successfully, and no instrument-related adverse events and complications occurred, showing that this surgical system used for partial nephrectomy is safe and effective.

Ureteropelvic junction obstruction (UPJO) is a condition characterized by the blockage of urine flow from the kidney to the ureter.With advancements in endoscopic technology, robotic-assisted laparoscopic dismembered pyeloplasty has become the dominant approach.However, approximately 10% of patients who undergo pyeloplasty still experience treatment failure, often due to long-segment proximal ureteral strictures (length greater than 2 cm).This increases the difficulty and risk of subsequent surgeries.In the past, ileal ureter replacement or kidney autotransplantation procedures were used as alternatives to shorten or replace the ureter.However, these procedures are associated with metabolic and vascular complications and are not always preferred.A pelvic flap, which utilizes the enlarged wall of the renal pelvis, is a good solution for bridging longer segments of a diseased ureter.This article reviews the specific applications of pelvic flap pyeloplasty in the existing literature and summarizes the technical improvements and research progress of renal pelvic flap pyeloplasty in our center to provide a reference for clinical application.
6.The design and assessment of a novel simulated training system for cardiac surgery.

Simulations can mimic the environment that refers to the surgery operation to improve the technical skills of the trainees. In this paper, we designed a new cardiac surgery simulative training system. The isolated pig heart was selected as the heart model. A mechanical device was designed to achieve the beating of heart model. At the same time, adjusting frequencies of mechanical movement could change the rating of heartbeat. In order to validate the rationality of the system, 12 non-medical specialty students and 12 medical specialty students were divided into two groups, which consecutively accepted seven-days of training for off-pump coronary artery bypass grafting using the cardiac surgery simulative training system. The time for completing bypass grafting before and after training were recorded. And the bridging outcomes of each trainee were assessed by 3 surgery cardiac surgeons using the object structured assessments of technical skill (OSATS) criteria. After training, each trainee could finish the bypass suturing in a shorter time than before training, and the scores of each trainee assessed by OSATS criteria were also improved. The results showed that the cardiac surgery simulative training system had better training effect in improving the surgical techniques, operation skills and proficiency of surgical instruments of trainees.
7.Effect of Reduning Injection, Shenfu Injection Combined with Gefitinib on Proliferation, Stemness Characteristics and Metabolism of Non-small Cell Lung Cancer Cells

ObjectiveTo compare and observe the effect of Reduning injection (mainly clearing heat), Shenfu injection (mainly warming Yang) combined with gefitinib on the proliferation, apoptosis, stemness characteristics and metabolism of lung cancer cells. MethodDifferent non-small cell lung cancer (NSCLC) cell lines were selected and intervened with gefitinib (5, 10, 20 μmol·L-1), Reduning injection (0.6%, 0.9%), Shenfu injection (0.6%, 0.9%), gefitinib combined with Reduning injection, and gefitinib combined with Shenfu injection. Cell proliferation in each group was detected by cell counting kit-8 (CCK-8) assay, and cell apoptosis was detected by flow cytometry. The mRNA and protein expressions of lung cancer stem cell markers sex determining region Y-box 2 (Sox2) and aldehyde dehydrogenase family 1 member A1 (ALDH1A1) were determind by real-time quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. The redox ratio of lung cancer cells was observed by femtosecond label-free imaging (FLI) and energy metabolism instrument was used to determine the glycolysis level in cells. ResultCompared with the blank group, Reduning injection reduced the survival rate of lung cancer cells (P<0.05), increased the apoptosis rate (P<0.05), down-regulated the mRNA and protein expressions of Sox2 and ALDH1A1 (P<0.05), and up-regulated the redox ratio of cells (P<0.05), while Shenfu injection exerted no remarkable effect on the above indexes. In addition, compared with gefitinib alone, Reduning injection combined with gefitinib inhibited the survival rate of lung cancer cells (P<0.05), promoted the cell apoptosis (P<0.05), down-regulated the mRNA and protein expressions of Sox2 and ALDH1A1 (P<0.05), up-regulated the redox ratio of cells (P<0.05), and lowered the proton efflux rate of glycolysis (P<0.05), while Shenfu injection combined with gefitinib failed to affect these indexes of lung cancer cells significantly. ConclusionReduning injection may inhibit stemness characteristics of tumor cells by regulating their metabolism to enhance the proliferation-inhibiting and pro-apoptotic effects of gefitinib on lung cancer cells, while Shenfu injection had no significant enhancing effect on gefitinib. This indicates that epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) should be used in combination with heat-clearing Chinese medicines.
