1.Clinical effect of Wenxin granule combined with metoprolol on arrhythmia in elderly patients with coronary heart disease

Objective To explore the clinical effect and safety of Wenxin granule combined with metoprolol on arrhythmia in elderly patients with coronary heart disease (CHD).Methods 186 elderly CHD patients with arrhythmia were divided into 3 groups according to therapy:group A (n=62,treated with Wenxin granule),group B (n=60,treated with metoprolol) and group C (n=64,treated with Wenxin granule plus metoprolol).Clinical effect and adverse effect were retrospectively analyzed.Results The excellence rate and total efficiency rate were higher in group C than group A andB [53.1% (34 cases) vs.32.3% (20 cases),30.0% (18 cases); 93.8% (60 cases) vs.40.3% (25 cases),40.0% (24 cases) ; respectively,all P<0.05].There were no significant differences in the incidence of adverse effect in the 3 groups [4.8% (3/62),8.3% (5/60) vs.8.3% (5/64),all P >0.05].Conclusions Wenxin granule combined with metoprolol improves clinical effect,decreases premature beat and has a better safety for elderly CHD patients with arrhythmia,which is worthy of clinical application.

It can measure and store the patient's body temperature during the desired time span, and can transmit the data to the computer terminal or the cell phone terminal through wireless transmission system.
3.Application of perforator flaps to repair chronic osteomyelitis with soft tissue defect in the distal lower extremity

Objective To evaluate free or pedicled perforator flaps for repairing chronic osteemylitis with soft-tissue defect in the distal lower extremity. Methods From May of 2006 to October of 2007, 28 consecutive patients of chronic osteomylitis with soft-tissue defect in the distal lower extremity underwent surgical debridement and reconstruction with free or pedicled perforator flaps. There were 13 free flaps. The free anterolateral thigh flaps were used in 2 cases to repair the soft defects in the front of leg, 3 cases in the front of the malleolus, 2 cases in the dorsum of foot, 2 cases in the heel. The free lateral crural flaps nourished by perone al artery were used in 4 cases to repair the soft defects in the dorsum of foot. There were 15 pedicled flaps. Posterior tibial artery perforator flaps were used in 4 cases to repair the soft defects in the front of leg, and 2 cases in the medial malleolus. Lateral retromalleolar perforator flaps nourished by peroneal artery were used in 6 cases to repair the soft-defects in the heel, 1 case in the lateral malleolus and 1 case in the dorsum of foot, the first dorsal metatarsal artery perforator flap was used to repair the proximal dorsum of hallux. The wound was closed with irrigation-suction in 7 cases and with vancomycin-impregnated gelatin in 8 cases. Results All 27 flaps were successfully survived except insuffcient vein refluence in 1 posterior tibial artery perforator flap, which resulted in a superficial necrosis and healed spontaneously. The follow-up period from 6 months to 2 years revealed that recurrence developed in two diffuse type patients and both were treated once and twice with success, respectively. The others healed without any signs of recurrences. No debulking procedure was necessary in any case. Secondary bone graft was performed in 3 cases. All patients were ambulatory and fully weight-bearing with normal clinical parameters at the time of last review. According to the evaluating criteria for the treatment of foot disease, the mean score was 84.5. Conclusion Free or pedicled perforator flap has been shown to be well vascularised, and it is feasible for the treatment of chronic osteomyelitis with soft-tissue defect in the distal lower extremity.
4.Role of PPARα/PGC-1αin doxorubicin induced mouse dilated cardio-myopathy

[ ABSTRACT] AIM:To investigate the changes of peroxisome proliferator-activated receptors ( PPAR)α/peroxi-some proliferator activated receptor coactivator 1 alpha ( PGC-1α) in doxorubicin ( DOX) induced dilated cardiomyopathy ( DCM) and its effect on the energy metabolism and myocardial function in mice .METHODS:Forty mice were randomly divided into 4 groups:control group, DOX group, PPARαinhibitor group and PPARαagonist group.The DCM model was established by injection of DOX.The protein levels of PPARα/PGC-1αwere detected.The PPARαinhibitor and PPARαagonist were used 2 weeks beforeinjection of DOX.The contents of adenine acid and phosphocreatine ( Pcr) in the mito-chondria were measured by high-performance liquid chromatography ( HPLC) .The ANT activity was analyzed by the atrac-tyloside-inhibitor stop technique.The changes of the echocardiography and hemodynamics were also observed.RESULTS:DOX induced DCM model was successfully established.The protein levels of PPARαand PGC-1αin control group were significantly higher than those in DOX group (P<0.05).Both of the high-energy phosphate contents and the transport ac-tivity of ANT were decreased in DOX group (P<0.05), and the hemodynamic parameters were disordered (P<0.01). Compared with DOX group, PPARαinhibitor pre-treatment significantly reduced the PPARα/PGC-1αexpression.Mean-while, high-energy phosphate contents in the mitochondria and the ANT transport activity of the mitochondria decreased, as well as the left ventricular function ( P<0.05) .On the other hand, PPARαagonist significantly increased the expression of PPARαand PGC-1α, and improved the transport activity of ANT.In addition, the hemodynamic parameters were amel-iorated, but the high-energy phosphate contents of the mitochondria did not significantly change.CONCLUSION:PPARα/PGC-1αplays an important role in the regulation of ANT transport activity in dilated cardiomyopathy induced by DOX, and the activation of PPARα/PGC-1αhas protective effects on the DCM induced by DOX.
5.Effect of Fujian Tablets on Expression of Microtubule-associated Protein 2 in Cerebral Infarction Rats at Different Time Points

Objective To observe the effect of Fujian Tablets(FT),a prescription with the actions of nourishing liver and kidney,on the expression of microtubule-associated protein 2(MAP-2) in cerebral infarction rats at different time points,and to explore its mechanism on promoting nerve cell differentiation.Methods A total of 240 male Sprague Dawley rats were divided into normal control group,sham-operation group,model group and FT group.Focal cerebral ischemia models were established by occlusion of the middle cerebral artery according to the reformed Longa method in rats,and were subdivided into 3-,7-,14-,28-,and 42-day cerebral infarction subgroups.FT group was given FT in the dose of 9 g?kg-1?d-1,and the other groups were given the same dose of distilled water.Results In the model group,MAP-2 expression in the hippocampus increased and reached a peak on day 3,which were notably different from that in the normal control group(P0.05).In FT group,MAP-2 expression increased on day 3,reached a peak on day 7,remained high level till day 14,and then decreased,the difference being significant compared with the model group(P
6.Effect of Group Education Intervention in Self-control on Schizophrenics

Objective To explore the rehabilitation effect of group education intervention in self-control on schizophrenic patients. Methods 112 schizophrenic patients were divided into study group (56 cases) and control group (56 cases). Two groups were treated with antipsychotics. The control group was subjected to general healthy education, while the study group received education intervention of self-control. All patients of two groups were assessed by Self-care Behavior Questionnaire(SCBQ), Brief Psychiatric Rating Scale(BPRS) and Social Disability Screening Schedule(SDSS) before and affter intervention, and drug-compliance was also assessed.Results After one year, the total score of SCBQ was significantly higher in the study group than in the control group, while the total scores of BPRS and SDSS were significantly lower in the study group than in the control group (P<0.01); Drug-compliance was significantly more in the study group than in the control group(P<0.01).Conclusion Group education intervention of self-control can improve distinctly the ability of self-control disease, have positive effect on remission psychical symptom, and improve the social function and retention psychosomatic healthy.
7.Identification of Pig-Hide Gelatin from Tortoise-Shell Glue by UPLC-ESI-QTOF-MS/MS

This study aimed at identifying pig-hide gelatin from tortoise shell glue using ultra-performance liquid chromatography-electrospray-time of flight mass spectrometry(UPLC-ESI-QTOF-MS/MS).The samples of tortoise shell glue were digested by trypsin before the analysis on UPLC coupled with QTOF mass spectrometry.The ions at m/z 925.4 were selected as the marker peptides of pig-hide gelatin.It was found that the test results of the ions at m/z 925.4 were positive in the samples with low contents of the marker peptides of tortoise shell glue,indicating the existence of pig-hide gelatin.However,the result of bovine-hide gelatinordonkey-hide gelatin was negative,while negative results were detected in tortoiseglue standard.The finding was in conformity with previous references over the fragmentation regularity of MS/MS.In conclusion,the method can be adopted to identifying the pig-hide gelatin from tortoise shell glue and applied to the quality control of tortoise shell glue with high specificity and sensibility.
8.Identification of Bovine Hide Gelatine from the Shell Glue of the Red-Eared Slider (Trachemys scripta elegans) Using UPLC-ESI-QTOF-MS

This study aimed at developing an ultra-performance liquid chromatography electrospray time of flight-mass spectrometry (UPLC-ESI-QTOF-MS) method for the identification of bovine hide gelatine from the shell glue of the red-eared slider.Tortoise shell glue was made from the shell of red-eared slider or Reeves' turtle (Chinemys reevesii).The samples were analyzed on UPLC-ESI-QTOF-MS after being digested by trypsin,and the ions at m/z 604.8,m/z 641.3,m/z 790.8 of the marker peptides of bovine hide gelatin were detected.As a result,it was found that the test results of the ions made from the shell of red-eared slider at m/z 641.3 were positive,taking the same fragmentation regularity of MS/MS as the standard of bovine-hide gelatine,while negative results were detected from the ions made from the shell of Reeves' turtle.In conclusion,the method was simple and accurate provided a basis for distinguishing the tortoise shell glue from bovine hide,which could be also used for the quality control of tortoise shell glue.
9.Complications caused by EN through nasogastric tube after stomach cancer surgery

Objective To evaluate the causation and management of complications caused by enteral nutrition (EN)after stomach cancer surgery.Methods The clinical data of 195 gastric cancer patients which used EN after surgery were analyzed retrospectively from September 2002 to April 2008 in our hospital.Results Among the 195 patients,29(14.87%)developed abdominal distention,17(8.71%)diarrhoea,Six(3.07%)metabolic complica tions,three anastomotie leakage,1 patient with colon perforation.Conclusion These complications caused by EN after stomach cancer surgery associated with the stress of surgery,the speed and concentration of nutrition infusion, patients'metabolic conditions and other related factors.
10.Applied anatomical and clinical application of reconstruction of the medial malleolus with bone-severed vascularized fibular head epiphysis

Objective To provide an anatomical basis for repairing the medial malleolus with bone-severed vascularized fibular head epiphysis, and to explore the effect of clinical application with this method. Methods Figures of fibular head and medial malleolus were measured on 20 fresh lower limbs specimens of child age from 2 to 12 years old, then bone-severed formula was deduced. The bone-severed composite fibular head epiphysis to repair the defect of medial malleolus were carried for 6 child patients of emergency or post-poned cases on one stage. Obersved the clinical effect by following-up. Results The angle between fibular head and stem (M) was(170±8)°, angle of fibular head sadacc(N) was (145 ±6)°, length(1.5±0.2)cm and width (1.4±0.2)cm; angle between medial malleolus and stem(1) was(152±8)°, length of the articular surface of medial malholus was(1.25 ± 0.2)cm and width (1.25 ± 0.2)cm. Angle between defect surface and tibia was(Q). Formula: angle of bone-severing X = L-N-Q, and apex at the upper 1/6 of the reversed articular surface of fibular. 6 cases with this method was completed, all healed at stage one, following-up 1 to 3 years, medial malleolos developed well and no epiphysis ossification anticipation, and the ankle joint has no inversion with its loadind and walking function good. Conclusion The fibular head epiphysis and the medial malleolus differ in shape to some extent, but good donor can be got by bone-severing, can repair the epiphysis and soft tissue defect of the medial malleolus at one stage with additional flap, developing with the child at the same time, it is a perfect method to reconstruct the traumatic defect of child medial malleohs.
