1.Clinical Study of Xiyanping Injection for Treatment of Children with Acute Suppurative Tonsillitis

Objective To investigate the effectiveness and safety of Xiyanping Injection for the treatment of children with acute suppurative tonsillitis, so as to provide evidence for the prevention and treatment of pediatric acute suppurative tonsillitis with traditional Chinese medicine. Methods Two hundred children with acute suppurative tonsillitis were randomly divided into treatment group and control group, 100 cases in each group. The control group was given routine western medicine treatment, and the treatment group was given Xiyanping Injection based on the treatment for the control group. The treatment lasted for 5 days. The clinical outcomes covered the course of disease, the overall efficacy, adverse reaction, and time for the subsiding of fever, sore throat, swollen tonsils and purulent secretion, as well as the changes of white blood cell count, hyper-sensitivity C reactive protein, alanine aminotransferase, creatinine and creatine kinase levels before and after treatment. Results (1) After treatment, the total effective rate of the treatment group was 97.0% and that of the control group was 87.0%, the difference being significant (P < 0.01). (2) After treatment, time for the subsiding of fever, sore throat, swollen tonsils and purulent secretion as well as hospitalization time of the treatment group was much shorter than that of the control group (P < 0.05 or P < 0.01). (3) After treatment, white blood cell count and hyper-sensitivity C reactive protein level of the two groups were obviously decreased compared with those beforetreatment (P < 0.01) , and the decrease in the treatment group was superior to that in the control group (P <0.01). (4) The inter-group and intra-group comparison of alanine aminotransferase, creatinine and creatine kinase levels before and after treatment showed that the differences were insignificant between the two groups (P > 0.05). No adverse reactions of rash, nausea, vomiting, diarrhea, chest distress, shortness of breath, or drug fever were found in the two groups. Conclusion Xiyanping Injection is effective and safe for the treatment of children with acute suppurative tonsillitis by increasing the total efficiency, reducing the time for the subsiding of fever, sore throat, swollen tonsils and purulent secretion, shortening hospitalization time, and decreasing white blood cell count and hyper-sensitivity C reactive protein level. No obvious hepato-renal function damage occurred during the medication.
2.A risk analysis of acute kidney injury after orthotropic liver transplantation for benign end-stage liver disease in adults

Objective To analyze the risk factors of acute kidney injury after orthotropic liver transplantation for adult benign end-stage liver disease.Methods A retrospective analysis was conducted in 30 recipients (18 males,12 females,aged 23-68 years,ASA grade Ⅲ or Ⅳ) who underwent orthotropic liver transplantation for benign end-stage liver disease at Tongji Hospital from May,2014 to December,2014.Both demographic characteristics and perioperative parameters were collected,including general condition,surgery and anesthesia factors and intraoperative salvage autotransfusion or not.Perioperative laboratory findings related to renal function including urine volume,serum creatinine (Scr) and urea nitrogen (BUN) were collected,too.All variables tested in the univariate analysis with a P<0.10 were included in a multiple logistic regression analysis.Results There were less intraoperative salvage autotransfusion,more platelet transfusion and a higher using rate of vasopressors in the AKI group after surgery than those did not.Patients who received intraoperative salvage autotransfusion had 0.058 time odds (95%CI 0.005-0.649) of AKI than those did not;patients who required platelet transfusion had 10.706 times higher odds (95%CI 1.212-94.963) of AKI than those did not.Conclusion It is likely that intraoperative salvage autotransfusion was able to decrease the morbidity of AKI,while platelet transfusion and vasopressor administration to maintain blood pressure could increase the possibility of AKI.
3.Influencing factors for bone cement dispersion within the vertebral body after vertebroplasty

BACKGROUND:Percutaneous vertebroplasty and kyphoplasty are both effective in the treatment of osteoporotic vertebral compression fractures, but different in the distribution and dispersion of bone cement. <br> OBJECTIVE:To analyze the factors affecting the bone cement dispersion within the vertebral body in treatment of osteoporotic vertebral compression fracture with vertebroplasty or kyphoplasty. <br> METHODS:A total of 41 patients with osteoporotic vertebral compression fractures were included, and divided into three groups:group A (22 cases receiving high viscosity bone cement vertebroplasty treatment), group B (5 cases receiving high viscosity bone cement kyphoplasty), group C (14 cases receiving low viscosity bone cement vertebroplasty treatment). The groups A and C were divided into subgroups according to bone cement injection volume, time from fracture to operation, preoperative degree of vertebral compression. The distribution and dispersion of bone cement in the vertebra were reconstructed by the CT three-dimensional imaging and volume rendering analysis. <br> RESULTS AND CONCLUSION:In the three groups, after operation, relative dispersion area and relative dispersion volume in the vertebrae had no obvious difference, and the bone cement could al diffuse to upper and lower lamina cross the vertebral midline. There was no significant difference in bone cement diffusion coefficient among the three groups. In a certain range, the bone cement injection volume and bone cement dispersion volume was positively correlated. In groups A and C, bone cement diffusion coefficient decreased with the increasing of bone cement injection volume, time from fracture to operation, and the compression degree of the fractured vertebrae, but showed no significant correlation with bone cement viscosity.
4.Identification and Study on Processing End-point of Raw and Processed Products of Morus alba Based on UPLC Fingerprint and Chromaticity

OBJECTIVE:To provide reference for the identification and proces sing end-point determination of raw Morus alba and its processed products (honey-processed M. alba ). METHODS :UPLC method was adopted. The determination was performed on Waters BEH Shield RP C 18 column with mobile phase consisted of acetonitrile- 0.1% phosphoric acid solution (gradient elution ) at the flow rate of 0.30 mL/min. The column temperature was set at 30 ℃. The program wavelengths were set at 280 nm(0-4 min) and 320 nm(4-35 min). Similarity Evaluation System for Chromatogram Fingerprint of TCM (2012 edition)was used to establish UPLC fingerprint and carry out similarity evaluation of 13 batches of M. alba and honey-processed M. alba . The chromatographic peaks were identified with reference substance fingerprint. The colorimetric value (L,a,b) of 13 batches of M. alba and honey-processed M. alba powder were determined ,and average total colorimetric value (E)was calculated. OPLS-DA and cluster analysis were adopted to analyze the differences in fingerprints and colorimetric values of M. alba before and after processing. At the same time ,the dynamic change rule of fingerprint and colorimetric value of honey-processed M. alba at different processing time points were analyzed to determine the processing end-point. RESULTS :There were obvious differences in fingerprints before and after processing ,and the similarity of 13 batches of M. alba and honey-processed M. alba were all higher than 0.9. Totally 21 common peaks were calibrated for M. alba ,and 23 common peaks for honey-processed M. alba ;peak 1 and peak 2 were newly produced compounds of honey-processed M. alba . Peak 2,peak 7,peak 14 and peak 19 were identified as 5-hydroxymethylfurfural, mulberry glucoside A ,oxidized resveratrol ,mulberry flavonoids G. Results of OPLS-DA showed that the peak area-sample quantity ratio of peak 1,peak 2,peak 18,peak 20 and the chromaticity values (L,a,b)were the most important factors affecting the difference of raw and processed products of M. alba . When the E ranged 75.84-80.88 as the processing end-point of honey-processed M. alba ,the processing time was determined as 22-34 min. CONCLUSIONS : The established UPLC fingerprint and colorimetric value determination method can be used to identify the raw and processed products of M. alba as well as determine the processing end-point of honey-processed M. alba .
5.Analysis of urinary calculi composition in Longnan, Gansu province

【Objective】 To analyze the composition of urinary calculi in Longnan, Gansu province, and the relationship between the composition and clinical characteristics of patients, so as to provide reference for the prevention and treatment of this disease. 【Methods】 The composition of 500 cases of urinary calculi hospitalized in our department during Apr. 2021 and Feb.2023 were analyzed using the infrared spectrum stone composition analyzer. The clinical characteristics of the patients were evaluated and analyzed. 【Results】 The male-to-female ration of patients was 2.70∶1. Most patients aged 21 to 60 years (437, 87.4%). Most cases were ureteral calculi (72.8%), followed by renal calculi (22.2%), and bladder calculi (5.0%). There were 166 cases of calcium oxalate calculi, 293 cases of calcium oxalate + carbonated apatite calculi, 24 cases of calcium oxalate + carbonated apatite + hydroxyl apatite calculi, 4 cases of calcium oxalate + calcium bicarbonate calculi, 7 cases of urate calculi, 6 cases of carbonated apatite + struvite calculi. Oxalate calculi were the most common in all age groups, and urate calculi were the most common in the 21 to 40 age group. Calcium oxalate calculi were most common in the ureter (127, 76.5%), significantly higher than in other sites (χ2=3.222, P=0.020). Calcium oxalate + calcium hydrogen phosphate calculi was the least common in the bladder, significantly different from the other parts (χ2=2.092, P=0.037). Magnesium ammonium phosphate hexahydrate and/or calcium carbonate or calcium oxalate calculi were the most common in the kidney (50.0%), significantly different from the other parts (χ2=9.448, P=0.007). 【Conclusion】 In Longnan area, the incidence of urinary calculi is significantly higher in male than in female. Ureteral calculi are mainly composed of calcium oxalate + carbonated apatite and calcium oxalate. According to different risk factors, individual prevention programs should be developed.
