AIM: To explore the relationship between interleukin-1receptor antagonist (IL-1ra) and airway hyperresponsiveness (AHR) of asthma. METHODS: The airway responsiveness to inhaled histamine as PC 20 was evaluated among the normal guinea pigs, and sensitized with ovalbumine (OA) and challenged 14 days later (athma group), and injected IL-lra 1 mg?kg -1 intravenously 30 minutes before challenged (asthma plus IL-lra group). 4.Change of V-ATPase B subunit in transforming growth factor β1-induced epithelial-mesenchymal transition of rat renal proximal tubular cells

Objecfive To investigate the change of V-ATPase B subunits on epithelial to mesenchymal transition (EMT)in rat renal tubular epithelial cells (NRK52E) stimulated by transforming growth factor β1 (TGF-β1). Methods NRK52E cells were stimulated by TGF-β1 (10 μg/L)for O h(control),12 h,24 h,48 h,72 h after sefrum-free culture for 24 h.The mRNA and protein expression of E-cadherin,α-SMA,B2 and B1 subunits of V-ATPase were detected by real-time PCR,Western blotting and immunofluorescence. Results After stimulated by TGF-β1 (10 μg/L)for 48 h,the expression of α-SMA was markedly increased(P<0.05),but the expression of E-cadherin was dramatically decreased(P<0.05).Meanwhile,the expressions of V-ATPase subunit B2 was significantly increased (P<0.05).However,the B1 subunit distributed rarely in NRK 52E cells,and did not increase after TGF-β1 stimulation.Double-label immunofluoerscence staining also showed that the V-ATPase B2 subunit was increased in the cytosol.tending to accumulate to the cell membrane after TGF-β1 stimulation. Conclusions The main isoform of V-ATPase distributed in NRK52E cells is B2 subunit.B2 subunit is increased alone with TGF-β1-induced EMT.It may suggest that V-ATPase B2 subunit may play a potential role in TGF-β1-induced tubular EMT and renal fibrosis.
5.Study on elderly mental sub-health status and influencing factors of the three gorges reservoir

Objective To investigate the status of psychosomatic health on the aged in reservoir area of three gorges .Study men-tal sub-health population distribution and the related influencing factors of the aged .Methods 604 elderly of reservoir area of three gorges were investigated by using self-made general questionnaire ,Cornell Medical Index(CMI) ,Eysenck Personality Questionnaire-R Short Scale(EPQ-RSC) ,Social Support Rating Scale(SSRS) ,Activities of daily living (ADL) ,Memorial University of Newfound-land Scale of Happiness(MUNSH) .Results 14 .7% aged′s mental health was in the sub-healthy status .Older age ,low education level ,no spouse ,urban elderly people were higher in mental sub-healthy .There was significant difference between mental sub-healthy group and mental healthy group in the aspect of personality 、social support、subjective well-being、activities of daily living (P<0 .05) .Conclusion The elderly psychosomatic health status of the Three Gorges Reservoir area is good ,with low incidence of mental sub-health ,their psychosomatic health more affected by the state of mind .
6.Mechanism of adult offspring rats'anxiety-like behavior induced by morphine addiction and withdrawal in parents before mating

ObjectiveTo explore the possible mechanism of adult offspring rats'anxiety-like behavior induced by parents experienced morphine addiction and withdrawal.MethodsEstablishing the model of Sprague-Dawley rats morphine addiction,Male and female rats were mated after morphine withdrawal 21 days.Meaning-while,saline control group was established in the same method.5 female and 5 male offspring's brains were obtained to observe the neuronal morphology of hippocampal CA1 through Golgi staining when they were 8 weeks old,the same number of female and male's hippocampus were derived after deeply anesthetized to perform the whole genome expression profiles analysis.ResultsThe total length and the number of basal dendrites branches on hippocampal CA1 neurons in offspring of morphine groups were significantly decreased compared to the offspring of saline group.Comparison with the offspring of saline group,663 up-regulated genes (ratios ≥2.0) and 499 down-regulated genes ( ratios ≤0.5 ) were detected in the male offspring of morphine groups,and 350 up-regulated genes (ratios ≥2.0) and 188 down-regulated genes (ratios ≤0.5) were done in the female.Furthermore,they included many genes associated with regulation of emotional behavior,such as 5-HT2c receptor up-regulation 7-fold,Igf-2 up-regulation 7.1-fold and reelin down-regulation 3.3-fold were observed.ConclusionExperienced morphine addiction and withdrawal in parents prior to mating leads to dysplasia of dendritic morphology in hippocampal CA1 neurons of adult offspring rats,and 5-HT2c,Igf-2,reelin expressing abnormally,which may be the possible mechanism of anxiety-like behavior in adult offspring rats.
7.The role of heparin-binding protein, procalcitonin, C-reactive protein and white blood cell in respiratory tract bacterial infection

Objective To evaluate the diagnostic value of the heparin-binding protein (HBP),procalcitonin (PCT),C-reactive protein (CRP),white blood cell (WBC) in respiratory tract bacterial infection.Methods 66 respiratory tract bacterial infection patients,37 respiratory tract non-bacterial infection patients and 39 control group in the Third Xiangya Hospital from October 2015 to March 2017 was selected as objects in this prospective study.The levels of HBP,PCT and CRP in blood of the objects were tested with ELESA,immunofluorescence assay,immunoturbidimetry respectively;WBC counts were taken by Sysmex XE-5000 blood analyzer.The difference among the three groups was analyzed by Student's t test,one-way ANOVA or Wilcoxon test.Receiver operating characteristic curve was utilized to analyze the diagnostic value of HBP,PCT,CRP and WBC in respiratory tract bacterial infection.Results The plasma level of HBP were 36.30 (7.78-89.36) ng/ml,5.57 (4.37-8.23) ng/ml,2.84 (1.53-6.51) ng/ml in respiratory tract bacterial infection group,respiratory tract non-bacterial infection group and control group respectively.The socre of PCT were 0.08 (0.04-0.83) ng/ml,0.09 (0.04-0.30) ng/ml,0.04 (0.03-0.08) ng/ml.The socre of CRP were 56.20 (19.33-76.23) mg/L,34.40 (2.15-83.95) mg/L,(2.20 ± 0.99) mg/L.The socre of WBC count were (10.59 ±4.58) × 109/L,8.40 (5.80-11.88) × 109/L,(6.14± 1.31) × 109/L.There were statistically significant differences in HBP scores between respiratory tract bacterial infection group and respiratory tract non-bacterial infection group or control group (Z =-4.828,P <0.001;Z =-5.685,P < 0.001).There were no statistically significant differences in PCT,CRP and WBC scores between respiratory tract bacterial infection group and non-bacterial infection group (F =0.045,P > 0.05;F =0.100,P > 0.05;F =2.417,P > 0.05),but significant differences between respiratory tract bacterial infection group and control group (Z =-2.881,P < 0.05;Z =-6.595,P < 0.001;t =6.499,P < 0.001).The area under curve (AUC) of HBP,PCT,CRP and WBC diagnosing respiratory tract bacterial infection was 0.89,0.69,0.95 and 0.85 respectively.The AUC of HBP differential diagnosising was 0.80.Conclusion HBP can be used as an efficient supplementary indicator for respiratory tract bacterial infection,the differential diagnostic value is superior to PCT,CRP and WBC.
8.Exploratory study on the application of nasal high-flow oxygen therapy during breaks off noninvasive ventilation for acute exacerbation of chronic obstructive pulmonary disease

Objective:To compare the therapeutic effects of nasal high-flow oxygen therapy (HFNC) and nasal canal oxygenation (NCO) during breaks off non-invasive ventilation (NIV) for acute exacerbation of chronic obstructive pulmonary disease (AECOPD), and to explore the feasibility of NIV combined with HFNC in the treatment of AECOPD.Methods:From August 2017 to July 2019, AECOPD patients with type Ⅱrespiratory failure (arterial blood gas pH <7.35, PaCO 2 > 50 mmHg) who were treated with NIV were randomly (random number) assigned to the HFNC group and NCO group at 1:1. The HFNC group received HFNC treatment during breaks from NIV and the NCO group received low-flow NCO during the NIV interval. The primary endpoint was the total respiratory support time. The secondary endpoints were endotracheal intubation, duration of NIV treatment and breaks from NIV, length of ICU stay, total length of hospital stay and so on. Results:Eighty-two patients were randomly assigned to the HFNC group and the NCO group. After secondary exclusion, 36 patients in the HFNC group and 37 patients in the NCO group were included in the analysis. The total respiratory support time in the HFNC group was significantly shorter than that in the NCO group [(74 ± 18) h vs. (93 ± 20) h, P = 0.042]. The total duration of NIV treatment in the HFNC group was significantly shorter than that in the NCO group [(36 ± 11) h vs. (51 ± 13) h, P=0.014]. There was no significant difference of the mean duration of single break from NIV between the two groups, but durations of break from NIV in the HFNC group were significantly longer than those in the NCO group since the third break from NIV ( P < 0.05). The intubation rates of the HFNC and NCO groups were 13.9% and 18.9%, respectively, with no significant difference ( P=0.562). The length of ICU stay in the HFNC group was (4.3 ± 1.7) days, which was shorter than that in the NCO group [(5.8 ± 2.1) days, P=0.045], but there was no significant difference in the total length of hospital stay between the two groups. Heart rate, respiratory rate, percutaneous carbon dioxide partial pressure and dyspnea score during the breaks from NIV in the NCO group were significantly higher than those in the HFNC group, and the comfort score was lower than that in the HFNC group ( P<0.05). Conclusion:For AECOPD patients receiving NIV, compared with NCO, HFNC during breaks from NIV can shorten respiratory support time and length of ICU stay, and improve carbon dioxide retention and dyspnea. HFNC is an ideal complement to NIV therapy in AECOPD patients.
9.Effects of mirror therapy combined with imagination therapy on upper limb function after stroke

Objective To observe the effect of mirror therapy (MT) combined with movement imagination therapy (MI) on the upper limb function of hemiplegic stroke patients.Methods Ninety stroke survivors with hemiplegia were randomly divided into three groups:A,B and C.Their routine rehabilitation was supplemented with MI in group A,with MT in group B and in group C with both for 8 weeks.The evaluations used the Fugl-Meyer motor function assessment (FMA),Carroll's upper extremity function evaluation (UEFT) and the modified Barthel index (MBI).Results After 4 and 8 weeks of treatment,significant improvement was observed in the average FMA,UEFT and MBI scores of all three groups compared with before the treatment.After 4 and 8 weeks all of the average scores in group B were significantly higher than those of group A at the same time point,For group C,their average scores were significantly better than those of both of the other groups.Conclusion Mirror therapy is superior to exercise imagination therapy,but their combination is more effective in promoting upper limb function and improving ability in daily activities than either alone.Longer treatment has better effects.
