1.The Effect of Valsartan on the Plasma Endothelin of Patients With Chronic Renal Failure Complicating Hypertension Patients and Its Clinical Significance

OBJECTIVE:To observe the influence of Valsartan on the serum ET,CGRP levels,urinary protein and Q - Td of patients with CRF complicating hypertension.METHODS: CRF research group including 24 patients with CRF complicating hypertension receiving Valsartan 80mg/ d for 4 weeks; CRF control groups including 20 CRF patients with normal blood pressure; health group including 9 health people.The serum ET and CGRP levels, urinary protein, and Q - Td were measured.RESULTS: (l)The serum ET level of CRF patients was higher than that of health people;The serum ET level of CRF research group(before treatment) was higher than that of CRF control group and decreased after treatment of Valsartan.(2) The serum CGRP level of CRF patients was higher than that of health people .The serum of CGRP level of CRF research group (before treatment) was higher than that of CRF control group and continued to increase after treatment of Valsartan. (3) The content of urinary protein of CRF patients was significantly higher than that of health people.The content of urinary protien of CRF research group before treatment was higher than that of CRF control group and decreased after treatment of Valsartan. (4) The Q - Td of CRF research group was longer than that of health group and CRF control group.But the Q - Td of CRF research group did not change after treatment of Valsartan.CONCLUSION: Valsartan could effectively control blood pressure and decrease content of urinary protein, and delay the deterioration of renal function, but could not reverse the cardiovascular remoulding.
2.Experimental Study on Combination of Intratumor Scutellaria Barbata Injection and Thalidomide on Liver Cancer

Objective BALB/C mice were inoculated subcutaneously with hepatoma 22 (H22) cells, and the treatment effect of thalidomide and intratumor injection of ethanol extract of Scutellaria Barbata (SB) on tumor growth were evaluated. Methods In vitro, mouse H22 liver cells were cultivated, and H22 cells in the logarithmic growth phase were tested. SB was extracted in the ethanol recirculation. A total of 28 BALB/C male mice inoculated subcutaneously with H22 cells were randomly divided into 4 groups, 7 mice per group. Group A:intragastric administration with thalidomide. Group B:SB intratumor injection. Group C:combination of thalidomide and SB with the same administration as Group A and Group B. Group D (control group):1% Ethanol 0.2~0.5 mL intratumor injection. Tumor volumes of four groups were observed and the tumors were weighed. The microvessel densities (MVD) of all transplantation tumors were measured by immunohistochemical staining with anti-CD31 monoclonal antibody. Objective:To study the change of Neuropeptide Y (NPY) plasma and its clinical si gnificance and the effects of Astragalus on NPY in plasma in chronic renal fail ure patients.Methods:Radioimmunoassay was used to measure plasma concentration of NPY.Results:①The plasma concentration of NPY in CRF patients was significantly high er than tha t in healthy group,and the plasma concentration of NPY in uremia patients was si gnificantly higher than that in nitremia patients.②The plasma concentration of NPY was positively correlated with the serum value of CRE and the level of MAP in CRF patients.③After Astraglus treatment for 3 weeks,the level of NPY in plas ma was significantly decreased in experiment group,but in control group,there wa s no significant change.Conclusion:The disturbance in synthesis,secretion or metabolism of NPY occurred in CRF patients,which significantly correlates with the occurrence and development of CRF.Astraglus can influence the NPY in plasma in CRF patients,so as to improve the renal function.
4.Efficacy of different doses of atorvastatin in the treatment of coronary heart disease with chronic heart failure

Objective To observe the effect of different doses of atorvastatin in the treatment of chronic heart failure of coronary heart disease (CHD), and to provide scientific reference for the choice of clinical medication. Methods 54 cases from June 2015 ~2017 year in April in our hospital with 20mg/d atorvastatin therapy regimen in the treatment of chronic heart failure of coronary heart disease patients as the study group, 54 cases of chronic heart failure of coronary heart disease patients with another force selected by 40mg/d atorvastatin therapy regimen as study group. LDL-C, LVESD, LVEDD, LVEF, hs-CRP, 6MWT, NT-proBNP and other indicators as evaluation basis, through the observation of the two groups before and after treatment the indexes to evaluate clinical efficacy, adverse events were observed after treatment in two groups. Results There was no significant difference between the groups before and after treatment, and there was no statistical significance(P<0.05). The indexes of clinical curative effect were improved in different degree after treatment, and the improvement effect in observation group was better than that in control group (P<0.05). Conclusion Atorvastatin 40mg daily oral drug treatment of chronic heart failure in patients with coronary heart disease is the best choice of atorvastatin dose, the dose range of atorvastatin treatment effectiveness and safety protection, improve clinical symptoms, promote the improvement of the quality of life, worthy of clinical application.
5.The research and practice of innovation and development in nursing management in military hospital

Objective: To explore the way of innovation and development of the pattern and means in nursing management in military hospital.Methods:Through various innovative practice from the following aspects,such as nursing service philosophy and measures for soldiers,the methods of nursing management and the elite-cultivating,nursing security management,which demonstrates the availability of innovation and development of the pattern and means in nursing management in military hospital.Results:The satisfactory rate of nursing service reached up to 99% in the examination of serving for soldiers of the whole army.The instruments which are manufactured for battle-scarred wounds and injuries use the flat of the Wound Nursing Centre,have reached the stage of clinical trial,we have completed five items of serial nursing research.The control rate of nursing quality of the whole hospital increased from 93.12% in 2003 to 98.9% in 2006,the satisfactory ratio of serious in-patient increased from 90.2% in 2003 to 96.7% in 2005.The ratio of health education achieves 100%.The satisfactory ratio of in-patient was 98.4% in September 2005,compared with 89.3% in February 2004.Conclusion:Only innovation can make nursing management develop and improve in military hospital.
6.THE PRODUCTION OF ANTIBACTERIAL PROTEIN IN TEA FUNGUS

The Acetobacter xylinum and Schizosacchromyces sp. were isolated from tea fungus culture. Each strain 's function on the production of antibacterial protein was explored by single-strain culture and double-strain culture under various condition. The preliminary conclusion is that the protein is synthesized by the yeast cells and its antibacterial activity is significantly improved through modification by bacteria enzymes. The results of co-cultures of several yeasts with the Acetobacter xylinum show that Schizosacchromyces is superior to other yeasts.

8.Open reduction and internal fixation with bone grafting to treat calcaneal displaced intra articular fractures

Objective To investigate the classification and treatment of displaced intra articular fractures of the calcaneus. Methods Twelve displaced intra articular fractures of the calcaneus in 8 patients were included in this study. Classification of calcaneus fractures was done according to Sanders' computed tomographic classification system. There were 2 cases of type Ⅱfractures, 4 type Ⅲfractures and 6 type Ⅳfractures. The open reduction and internal fixation (ORIF) and bone grafting were performed using a standard extended lateral approach, and the fractures were fixed with small fragment AO bridge plates with autogenous bone grafting. Average follow up was 28.6 months (range 24 to 33 months). The Creighton Nebraska Health Foundation Assessment Score System for fractures of the calcaneus was used for evaluation. Results The average score was 89.7 for type Ⅱ, 86.5 for type Ⅲ, and 73.5 for type Ⅳfractures. 9.First aid protocols for severe pelvic fractures

Objective To discuss diagnostic classification and emergency treatment protocols for complex pelvic fractures. Methods The first aid management of 23 cases of major pelvic fractures were analyzed retrospectively. Results 22 of the patients survived, but one died of hemorrhagic shock.Conclusions Correct classification of severe pelvic fractures is of guidance value to the first aid management. Cooperation among multiple departments, comprehensive application of various surgical methods, early stabilization of hemodynamics, rapid repair of injured organs, stabilization of pelvic ring with external fixator are effective techniques to enhance the survival rate of patients with severe pelvic fractures.
10.Application of hyperextension reduction with percutaneons kyphoplasty in treatment for osteoporotic vertebral compression fracture

ObjectiveTo discuss the clinical efficacy of hyperextension reduction with percutaneous kyphoplasty in treatment for osteoporotic vertebral compression fracture.MethodsTwenty-three patients with 23 vertebral compression fractures underwent hyperextension reduction with percutaneous kyphoplasty from September 2010 to October 2011.The vasual analogue scale(VAS) score and complications were recorded during followed up.Results All cases were successfully experienced the procedure using unilateral or bilateral percutaneous pedicle underwent hyperextension reduction.The operation time was (75 ± 45) minutes,the mean volume of cement injected into each vertebral body was(4.9 ± 1.9) ml.All patients had successful intervention without spinal cord injury,pulmonary embolism.Followed up(7.5 ± 6.5 ) months,the symptoms of the patients were improved,preoperative VAS score was (7.5 ± 0.7 ) scores,reduced to (3.4 ±0.4) scores at day 1 postoperatively,(2.7 ±0.5) scores at day 7 postoperatively and (2.3 ±0.5) scores at month 1 postoperatively,there was significant difference between preoperative and postoperative (P< 0.05).ConclusionsApplication of hyperextension reduction with percutaneous kyphoplasty can relieve pain and restore the vertebral body height,it may be an effective treatment method for patients with osteoporotic vertebral compression fracture.
