1.Analysis of Drug-resistant Bacteria Monitoring and Early Warning and Clinical Application of Antibacterial Drugs in Our Hospital in 2014

OBJECTIVE:To provide reference for clinical rational drug use. METHODS:Combined with bacterial drug resis-tance monitoring data in our hospital in 2014,the varieties,the amount of antibacterial drug,DDDs and other index were analyzed retrospectively for the same period. RESULTS:The detection rate of Gram-negative bacteria (79.27%) was higher than that of Gram-positive bacteria (20.73%);the most common Gram-negative bacilli and Gram-positive cocci were Escherichia coli (38.62%) and Staphylococcus aureus (5.22%). Pseudomonas aeruginosa and Stenotrophomonas maltophilia were the most com-mon non-fermentation bacteria;Main Gram positive bacteria were most sensitive to vancomycin,tigecycline and furadantin,etc. Main Gram-negative bacteria were most sensitive to imipenem,ertapenem,etc. Top 3 antibacterial drug in the list of DDDs were Levofloxacin injection,Cefazolin injection,Cefoperazone/sodium sulbactam for injection. CONCLUSIONS:Third generation of cephalosporins are used frequently in our hospital,which is a major cause of ESBLs producing and drug resistance,bacterial drug resistance monitoring and early warning data has played a positive role on guiding reasonable application of antibacterial drug in our hospital.
2.Treatment of 60 Cases of Acne by Acupuncture and Herbal Medicine

60 cases of acne were treated by applying the collateral-pricking and cupping on Dazhui (GV 14), Feishu (BL 13), Ganshu (BL 18), Xinshu (BL 15) and Geshu (BL 17), in combination with herbal medicine. After two courses of the treatments, the results showed cure in 42 cases, effect in 18 cases, the curative rate in 70% and the total effective rate in 100%.
3.Effect of dezocine on the agitations after remifentanil-sevoflurane anaesthesia

Objective:To analyze the effect of dezocine on the agitations after remifentanil-sevoflurane anaesthesia.Methods: Choose patients received remifentanil sevoflurane anesthesia in our hospital as study object, randomly divided into observation group received Dezocine intervention and control group received saline, compared with agitation score, VAS and BCS score, CRP and inflammatory cytokines level.Results:1)observation group patients’ average score of agitation restlessness and probability after intervention were significantly lower than control group; 2)observation group patients’ BCS and VAS scores after intervention were significantly lower than control group patients; 3)CRP and inflammatory cytokines recovery period of two groups of patients have risen, but observation group were significantly better than control group patients(t=3.950,t=4.739,t=6.493;P<0.05).Conclusion: Dezocine help mitigate wake of remifentanil sevoflurane agitation in patients, reduce pain experience while optimizing factor levels in vivo, with a positive clinical significance.
4.Determination of the Content of Baicalin in QiLiKeGanKang Oral Liquid by RP-HPLC

OBJECTIVE:To establish a method for determination of baicalin in QiLiKeGanKang oral liquid.METHODS:The RP-HPLC method was employed taking Nova-pak C18 column as fixed phase and methanol-water-phosphoric acid(45∶ 55∶ 0.2)as mobile phase.Detection wavelength was 280 nm.RESULTS:The detection ranges were 0.179μ g~1.434μ g.The average recovery of baicalin was 101.35% (RSD=2.40%,n=5).CONCLUSION:This HPLC method is convenient,rapid,accurate and could be used for quality control in the production of QiLiKeGanKang oral liquid.
5.Determination of Geniposide in Longdanxiegan Oral Liquid by RP-HPLC

Objective: To establish a RP HPLC method for determination of geniposide in Longdanxiegan Oral Liquid. Methods: A Nova Pak C 18 column was used. The mobile phase was methanol water(20:80). The detection wavelength was 240nm. Results: The linear range of geniposide was 0.569~1.707?g, r=0.9998. The average recovery was 98.67%, RSD=1.54% (n=6). Conclusion: This method is convenient, rapid and accurate. It can be used for quality control in production of Longdanxiegan Oral Liquid.
6.Content Determination of Dexamethasone Sodium Phosphate in Compound Baifu Pellicles by RP-HPLC

OBJECTIVE:To establish a RP-HPLC method for content determination of Dexamethasone Sodium Phos-phate in Compound Baifu Pellicles.METHODS:A X-Terra RP 18 column was used,the mobile phase was Methanol-0.34%KH 2 PO 4 solution-triethylamine(52∶48∶0.2),detecting wavelength was242nm and the flow rate was0.7ml/min.RE-SULTS:Dexamethasone Sodium Phosphate was in the good linear range when the sample size was between0.0403?g~2.016?g(r=0.9999,n=3).The average recovery rate was99.85%(RSD=3.00%,n=6).CONCLUSIONS:The established method is convenient,accurate,replicable,which can be applied to quality control of Compound Baifu Pellicles.
7.Analysis on results difference of two methods for detecting 146 cases of anti-nuclear antibody

Objective To investigate the results difference of the indirect immunofluorescence (IIF) and immunoblotting (LIA) for detecting ant-inuclear antibodies (ANA) and clinical value .Methods One hundred and forty-six ANA detection specimens of IIF negative and LIA positive were collected and performed the detection of HBV antibodies and anti-HCV antibodies .Results (1) In 146 specimens of IIF (-)LIA (+ ) ,the positive specimen numbers of single anti-Ro52 antibody ,anti-SSA antibody and anti-AMA-M2 antibody were 69 cases ,42 cases and 15 cases respectively ,which of anti-RNP antibody ,anti-PCNA antibody and PM-Scl antibody were 3 cases ,5 cases and 2 cases respectively ,the combined 2-item positive was in 8 cases .(2)Among positive specimens of single anti-Ro52 antibody ,HBsAg(+ ) ,anti-HBe(+ ) ,anti-HBc(+ ) model and HBsAg(+ ) ,HBeAg(+ ) ,HBcAb(+ ) model of hepatitis B ,and hepatitis C were 51 cases ,4 cases and 7 cases respectively ,7 cases were non-hepatitis patients .(3) Among positive specimens of single ant-SSA antibody ,6 cases were hepatitis B patients and 36 cases were the patients with non-hepatitis B .Conclu-sion Anti-Ro52 antibody and anti-SSA antibody are easier to be missed by the IIF detection .Anti-Ro52 antibody positive has a cer-tain relation with small three positive of hepatitis B .
8.ARDs of Co-Salvia Miltiorrhiza Injection

OBJECTIVE:To analyze the relevant factors contributing to ARDs of Co-Salvia Miltiorrhiza injection (CSMI).METHODS:The ARDs of CSMI,reported in domestic literature in recent years,were collected and analyzed.RESULTS:About 7 kinds of clinical manifestation of ARDs were reported,of which,the allergic reaction was the commonest one.CONCLUSION:Clinical physicians should pay attention to the ARDs of CSMI and rationally use the preparation.
9.Determination of the Content of Ephedrine Hydrochloride in Compound Baifu Pellicle by RP-HPLC

OBJECTIVE:To establish a HPLC method for determination of the content of ephedrine hydrochloride in compound Baifu pellicle.METHODS:A XTerra RP 18 column was used.The mobile phase was methanol-1%ammonium acetate solution(40∶60).The detection wavelength was215nm.The column temperature was30℃.The flow rate was0.6ml/min.RESULTS:There was good linearity of ephedrine hydrochloride in the range of0.1034?g~4.136?g(r=0.9994,n=3). The average recovery was101.29%(RSD=2.34%,n=6).CONCLUSION:This method is simple,accurate and repeatable,and can be used for quality control of compound Baifu pellicle.
10.Therapeutic effects of local mild hypothermia for 52 patients with acute cerebral infarction

Objective To explore the therapeutic effects of local mild hypothermia for acute cerebral infarction(ACI). Methods 99 hospitalized patients with ACI form January 2006 to July 2009 were randomly divided into mild hypotherrmia group(therapy group,n =52) and control group(n =47). All the patients were treated with routine medication,while those in therapy group patients were aslo treated local mild hypothermia. Before treatment,the 10th and 30th day after treatment ,the neurological deficits and activity of daily living(ADL) were evaluated according to European Stroke Scale(ESS) and Barthel Index respectively,and were evaluated according to Barthel Index on the 90th day. Results On the 10th and 30th day the increase rate of ESS was evaluated. On the 30th day,there was significant difference between the therapy group (74. 4 ± 13. 8) and controll group (65.0 ± 17.6) (t = 3. 121,P < 0.01).On the 10th and 30th and 90th day the improvement of ADL was observed. The increase rate of ADL was(39.2 ±32. 2,59.3 ±29. 7,64. 0 ±31.5) in the therapy group and(23.9 ± 19. 8,49. 3 ±27. 3,56. 0 ± 30. 1) in the controll group (t = 2.611 ~ 2.73.6, P < 0.05 ~ 0.01). The effective rate of the therapy group was 78. 8%, and the effective rate of the controll group was 46. 7%. There was significant difference between the therapy group and the controll groups (x~2 = 10. 953, P < 0.01). Conclusion The local mild hypothermia is more effective than those in the controll group for ACI.
