1.Clinical significance of combined Tests with malignant tumor in digestive system

Objective To investigate the clinical value of combined tests,including A-fetus protein(AFP)、cancer embryo agent(CEA),carbonhydrate agent 19-9(CA19-9)for the diagnosis of malignant tumors in digestive system.Methods Chemicalill-umination tests were used to detect serum levels of AFP,CEA and CA 19-9 in 821 patients with diseases of digestive system.Results The levels of AFP,CEA,CA19-9 in malignant group and the benign group were significantly higher than the control group(t=2.345,2.224,all P<0.05;t=2.785,2.749,2.897,2.865,all P<0.01=;The levels of AFP、CEA、CA19-9 in malignant group were significantly higher than benign group (t=2.445,P<0.05;t=2.745,2.819,all P<0.01);The level of AFP in liver cancer group was significantly hisher than other groups(t=2.715,2.812,2.877,2.845,2.799,all P<0.01);The level of CEA in hver cancer group was significantly lower in patients with liver colorectal cancer and pancreatic cancer(t=2.775,2.801,all P<0.01);The level of CA19-9 in bile duct cancer group was significantly hisher than that of other cancers (t=2.733,2.851,2.788,all P<0.01);The positive rates of liver、stomach、colorectal cancer、pancreatic cancer、bile duct cancer combined detection in three item were 91.0%,90.0,88.0%,90.0%,70.2%which were higher than the single check-positive rate of 85.0%,72.0%,67.5%,79.4%、55.3%(χ2=3.781,3.841,3.798,3.814,3.994,all P>0.05).Conclusion The three tumor markers could be used as auxiliary diagnostic and ioint detection which could improve the detection rate in patients with malignant tumors of digestive system.
2.Establishment of 3-dimensional finite element model of post-inlay restoration of the first mandibular residual molar crown

BACKGROUND:During the restoration of residual molar crown, a little part of tooth is still remained commonly. After the restoration, with various forces, stress distribution affects directly the results after restoration. Finite element method is gradually applied in stress analysis on artificial tooth.OBJECTIVE: To establish the three-dimensional (3-D) finite element model of post-inlay restoration of the first residual mandibular molar crown so as to provide experimental data for improving model establishment of complicated teeth and analysis on the property of stress distribution of restoring methods.DESIGN: Repeated observation and measurement were given.SETTING: Department of Stomatology and Department of Radiology of First Hospital affiliated to Sun Yat-sen University;Department of Solid Mechanics,College of Traffics and Communications, South China University of Technology; Department of Restoration of Guanghua College of Stomatology.MATERIALS: The experiment was performed in Department of Solid Mechanics, College of Traffics and Communications of South China University of Technology from November 2003 to December 2004. Six first mandibular molars on the right side with normal morphology in vitro were collected, and Toshiba Xpress/SX spiral CT machine, image photo synthesis software and finite element analysis software ANSYS were applied in the experiment.METHODS: 1 of the 6 first mandibular molars on the right side with normal morphology in vitro was selected for pulpectomy, which was the best in density and near to clinical requirement in morphology. With pulpectomy, the prosthesis of braking-lock post-inlay restoration was prepared. Spiral CT-cross scanning was performed in premolar crown before the restoration, the residual crown with post-inlay in main root canal after restoration and the residual crown with braking-lock second post-inlay restoration. With image photosynthesis software, 3-D digital model of residual tooth and metal part was established and the entire tooth model was prepared after adhesion of two parts. In order to provide better boundary conditions of simulated natural tooth in practice, alveolar bone was considered. Under Mesh order in ANSYS software, automatic mesh generation was performed in the model directly.MAIN OUTCOME MEASURES: Establishment of 3-D finite element models of residual tooth before restoration, post inlay, and alveolar bone and tooth after restoration and the results of mesh generation.RESULTS: By establishing 3-D finite element models of residual tooth before restoration, post inlay, alveolar bone and tooth after restoration and automatic mesh generation, there were altogether 117720 units and 20988nodes. Good geometric similarity presents between the construction model of 3-D finite element model and solid tissue.CONCLUSION: Combination of 3-D finite-element model with spiral Ctcross technology establishes complex dental models, simulates practical conditions authentically and is good in operation.
3.Correlation analysis of serum glycated hemoglobin A1c level and early neurological deterioration of acute cerebral infarction

Objective To investigate the predictive value of serum glycated hemoglobin A1c (HbA1c)level and early neurological deterioration (END)in patients with acute cerebral infarction. Methods From June 2014 to January 2016,the consecutive patients with acute ischemic stroke (interval time from onset to admission < 3 days)admitted to the Department of Neurology,Suqian People′s Hospital were enrolled retrospectively. The neurological deficits of all patients were evaluated on the day of admission with the National Institutes of Health Stroke Scale (NIHSS). The symptoms of neurological deficits in patients were evaluated repeatedly for all patients within 7 days after admission. Any score increased ≥2 compared with before admission was defined as END. The 226 patients met the inclusion and exclusion criteria were enrolled,and they were divided into either an END group (n = 50,22. 1%)or a non-END group (n = 176,77. 9%)according to whether END occurred after acute cerebral infarction. Univariate analysis was used to analyze the differences between the 2 groups. Multivariable Logistic regression analysis was used to analyze the correlation between the HbA1c level and END. Results Compared with the non-END group,there were significant differences in the age,prevalence of diabetes,NIHSS score,hypersensitive C-reactive protein level,rate of ASPECT score 0-7,and serum HbAlc level in the patients of the END group (all P < 0. 05). The results of logistic regression analysis showed that the elevated hypersensitive C-reaction protein and serum HbA1c levels were the independent risk factors for END of acute cerebral infarction (OR,1. 048 and 1. 809 respectively,95% CI 1. 008 -1. 089 and 1. 429 -2. 292 respectively;P = 0. 018 and 0. 002 respectively). Conclusion The increased serum HbA1c level is an independent risk factor for END of acute cerebral infarction. It has certain predictive value for END.
4.Survey on cognition status about AIDS prevention knowledge of urban and rural residents in Xinjiang Yili state

Objective To understand cognition status about AIDS prevention knowledge of urban and rural residents in Xinjiang Yili state ,and to provide a scientific basis for formulating measures of AIDS publicity ,education ,prevention and control for the fu-ture .Methods For simple random sampling with Database of country infectious diseases special projectsthat Yili state has been successfully uploaded ,obtain the required sample ,then conducted questionnaire survey .Results In 1 101 respondents ,cognition rate about AIDS prevention knowledge of urban and rural residents was 79 .02% ,the higher level regions of AIDS epidemic classifi-cation ,respondent in the higher cognition rate ;the cognition status about blood transmission ,mother-to-child transmission was bet-ter ,the sexual transmission was poorer .Male ,16 to 30 years old ,Uygur ,the higher education level of respondents ,AIDS prevention knowledge in relatively good condition status .Conclusion The cognition status about AIDS prevention knowledge in Xinjiang Yili state basic meet the national standards ,but remains to be improved .We should enhance AIDS prevention knowledge education cope with high age group ,Kazakhs and other ethnic minorities ,low education level of the respondents ,it should also focus on strengthe-ning the publicity education and proper guidance with non-transmission .According to different popular level classification ,publicity and education should be carryed out combined with local actual formulation is more advantageous to measure .
5.The diagnostic value of dual-energy dual-phase CT in small clear cell renal cell carcinoma

Objective To detect the best phase and best tube voltage for the diagnosis of small (diameter ≤ 3 cm) clear cell renal cell carcinoma with dual-energy dual-phase CT.Methods Image manifestations of 27 patients with small (diameter ≤ 3 cm) ccRCCs confirmed by pathology were retrospectively analyzed.All subjects underwent dual-energy biphase (early corticomedullary and delayed phase) scan preoperatively.Two senior radiologists analyzed the images in consensus.The definition of images in different phases and with different tube voltage was classified into 4 levels and was compared by Wilcoxon and Friedman test.The attenuation of the lesions and the adjacent renal parenchyma,the SD value of the anterior abdomen fat were measured.The contrast noise ratio (CNR),lesion kidney ratio (LKR) and the early corticomedullary phase and delayed phase value were calculated.They were all compared with oneway ANOVA.Results The score of definition of lesions in early corticomedullary phase at 80 kV,140 kV and average-weighted 120 kV were 3.30 ± 0.87,2.81 ± 0.92 and 3.11 ± 0.85,respectively,which in delayed phase were 3.70 ±0.54,3.30 ±0.82 and 3.52 ±0.64,respectively.Definition of lesions was better in delayed phase than that in early corticomedullary phase (Z =-2.296,-2.446 and-2.392,respectively; P < 0.05).Either in early corticomedullary phase or in delayed phase,CT value,noise value and CNR were the highest on 80 kV images,which were(302 ± 80)HU,(16.2 ± 2.2) and (4.1 ± 3.4) in corticomedullary phase and (152 ± 31) HU,(16.4 ± 2.7) HU,and (4.7 ± 1.7) in delayed phase.The change of lesion attenuation between early corticomedullary phase and delayed phase on 80 kV,140 kV and averageweighted 120 kV images were (150 ± 76),(72 ± 33) and (96 ± 46) HU,respectively.There was significant difference among the three groups (F =4.541,P < 0.01).Conclusions Delayed phase scan is in favor of small clear cell renal cell carcinoma display.80 kV images are the best for detecting and qualitation of small clear cell renal cell carcinoma when compared with 140 kV and the average-weighted 120 kV images.
6.Construction of bait plasmid containing SIGIRR cytoplasmic tail and detection of self-activation in yeast two hybrid system

Objective To construct the bait plasmid of pSos-single immunoglobin IL-1 receptor related protein (SIGIRR) in Cy-toTrap yeast two hybrid system ,and to test its self-activation .Methods The cDNA fragments of SIGIRR(480 -1 230 bp) were amplified from pReceiver-LV19-SIGIRR and ligated into the bait plasmid pSos to generate the plasmid pSos-SIGIRR .The pSos-SI-GIRR was identified by DNA sequencing and dual-site endonuclease digestion .Then the recombinant plasmid and control plasmid were introduced into the yeast cell cdc25H .The transformants were inoculated on plates of 25 ℃ /SD/Glucose(-UL) ,25 ℃/SD/Ga-lactose(-UL) ,37 ℃ /SD/Glucose(-UL) and 37 ℃ /SD/Galactose ,respectively and the proliferation ability of transformant was ob-served for 6 d .The Western blot was adopted to detect the expression of target protein .Results The pSos-SIGIRR vector was cor-rectly constructed and proved of no self-activation and toxic action .The Western blot showed that the target protein was expressed in a form of fusion protein of 170KD .Conclusion The bait plasmid containing SIGIRR cytoplasmic tail can be applied to the yeast two-hybrid system and lays the important foundation for seeking the interacting protein with SIGRR from the human lung cDNA li-brary in .
7.Lipopolysaccharide-binding protein inhibitory peptide inhibits the binding of LPS to U937 cells

AIM:To investigate the inhibitory effect of P12,a kind of lipopolysaccharide(LPS)-binding protein(LBP) inhibitory peptide,on the binding of LPS to macrophage in vitro.METHODS:Human monocyte-like cell line(U937 cells) was grown in RPMI-1640 and stimulated with PMA in order to induce their differentiation to macrophage stage.The relative affinity of P12 to LPS was determined by enzyme-linked immunosorbent assay(ELISA).The effects of P12 on the binding of LPS to U937 cells were determined by flow cytometry analysis.The production of tumor necrosis factor-alpha(TNF-?) was measured by ELISA.RESULTS:The relative binding activity of P12 to LPS was higher than that of LBP in the same mass concentration.P12 inhibited the binding of FITC-conjugated LPS(FITC-LPS) to U937 cells.The productions of TNF-? was also significantly suppressed by P12.CONCLUSION:The results suggest that blockage of LBP at the inflammatory sites might attenuate LPS-induced circulatory shock.

Objective To compare the early result of endovascular repair and conventional open repair for abdominal aortic aneurysm. Methods We analyzed the clinical data of 34 patients with infrarenal AAA. Among them, 15 patients were treated with endovascular repair ( EVR group), 19 with conventional open repair ( COR group). Pre-operative status, intra-operative variables, post-operative outcomes and laboratory findings of the two groups were compared. Objective:To observe the effect of protoscolex on Th subsets and correlative cytokine in mice spleen cells in vitro.Methods:Co-culture spleen cells from BALB/c mice with protoscolices,then IL-4,IFN-γand TGF-βproduction in cell culture supernatants were analyzed by ELISA.The percentage of Th subsets were detected by Flow Cytometry analysis.Results:Secretion levels of IL-4 and TGF-βwere significantly increased in spleen cells at different time point in co-culture system with protoscolices.Ratios of Th2 and Treg cells were also significantly increased in co-culture system at different time points than the control groups.However,there was no statistical significance for ratio of Th1 cells at different time points.Conclusion:The protoscolex can increase the ratios of Th2 cells and Treg cells from spleen cells.Secretion levels of IL-4 and TGF-βwere also increased in spleen cells co-cultured with protosco-lices.The results suggest that these Th cell subsets play a role in the immune escape of the hydatid disease.

Objective:To investigate the expression levels of PPARα/γin RAW264. 7 cells in the early stages of co-cultivation with Echinococcus granulosus in vitro. Methods:RAW264. 7 cells were co-cultured with E. granulosus and collected at 12,24,36,48, 72 h. The mRNA levels of PPAR-γ,PPAR-α,M1 macrophages-associated cytokines including TNF-α,MCP-1 and IL-1β,and M2 mac-rophages-associated cytokines including Arg-1,TGF-β and Fizz-1 were detected by qRT-PCR. The protein levels of Arg-1 and MR were analyzed by ELISA. Results:The expression levels of PPAR-γ, PPAR-αand the M2 macrophages-associated cytokines including Arg-1,TGF-β,Fizz-1 and MR were significantly increased,especially at 72 h (P<0. 05). M1 macrophages-associated cytokines including TNF-α,MCP-1 and IL-1β were decreased at 72h although increased at first. Conclusion:During the early stages of co-cultivation with Echinococcus granulosus in vitro, the levels of PPAR-γ/α are up-regulated in RAW264. 7 cells, which may drive macrophage polarization and play a role in the immune escape.
