1.Phantom test for daily quality assurance of on board image system

Objective To study the method of using phantom test for daily quality assurance of on board image(OBI) system. Methods The routine procedures of radiotherapy, including CT simulation, planning,setup,cone beam CT(CBCT) scan and kilovolt X-ray orthogonal film were carried out on a head phantom. The procedures repeated once a day in the following 10 days. The geometric errors of the phantom were recorded. Results The geometric errors of the phantom by CBCT were [0.06±0.11] cm, [0.03±0.05] cm, [0.07±0.07] cm and [0.03±0.10] cm in longitudinal, vertical, lateral and rotation directions, respectively. The geometric errors of the phantom by kilovolt X-ray orthogonal film were [ 0.04±0.10] cm, [0.03±0.05] cm, [0.08±0.06] cm and [0.05±0.05] cm, respectively. The differences of geometric errors of the phantom by CBCT and kilovolt X-ray orthogonal film were not significant(t = 0.44,P=0.667 in longitudinal direction ; t=0.00, P=1.060 in vertical direction ; t=0.34, P=0.735 in lateral direction; t=0.58,P=0.568 in rotation direction). Conclusions The OBI system of our accelerator is reliable and at excellent performance status. The method by using phantom test for the daily quality assurance of OBI system is easy and reliable.
2.Application of balance technique and measurement osteotomy technique under Offset Repo-Tensor in total knee arthroplasty

BACKGROUND:At present,measurement osteotomy technique and gap balance technique are the two main surgical methods in total knee arthroplasty.Both methods have their advantages and disadvantages.By adjusting the osteotomy angle,the gap balance technique can reduce the release of soft tissue and obtain a more balanced flexion and extension space.The clinical efficacy of gap balance technique is superior,but this surgical method lacks tools and is easily influenced by the surgeon's surgical experience and subjective judgment,with more errors.Measurement osteotomy technique has a short learning curve,but it relies on anatomic markers and is prone to many complications due to inaccurate positioning.In recent years,many instrument companies have successively developed and launched tools that can improve the quality of surgery.However,there are still few reports about the tool. OBJECTIVE:To compare the clinical efficacy of gap balance technique and measurement osteotomy technique under Offset Repo-Tensor in total knee arthroplasty,and explore the value of Offset Repo-Tensor in total knee arthroplasty. METHODS:The medical records of 85 patients with total knee arthroplasty were collected and divided into two groups according to the operation method.Group A consisted of 44 patients who underwent total knee arthroplasty using a gap balance technique combined with Offset Repo-Tensor.Group B consisted of 41 patients who underwent total knee arthroplasty using measurement osteotomy technique.Surgical time,American Knee Society Score,knee range of motion,changes in lower limb alignment,and postoperative complications were compared between the two groups to evaluate the clinical efficacy of gap balance technique combined with Offset Repo-Tensors in total knee arthroplasty. RESULTS AND CONCLUSION:(1)All 85 patients were followed up.(2)Postoperative knee range of motion in both groups was higher than that before surgery(P<0.05),and the improvement in group A was more significant than that in group B(P<0.05).(3)American Knee Society Score in both groups after surgery was higher than that before surgery(P<0.05),and the American Knee Society Score in group A was higher than that in group B at 2 weeks,1,3 months,and the last follow-up(P<0.05).(4)The operation time of group A was slightly shorter than that of group B,but the difference was not significant(P>0.05).(5)The lower limb alignment in both groups was improved after operation,but there was no significant difference between the two groups(P>0.05).(6)The complication rate of group A(2%)was lower than that of group B(7%),but there was no significant difference between the two groups(P>0.05).(7)It is indicated that compared with the measurement osteotomy technique,the combination of Offset Repo-Tensor and gap balance technique can elevate the clinical effect and improve the function and motion range of the knee joint.
3.Application of online and offline blended learning based on network platform in general theory of surgery

Objective:To explore application of blended learning mode in the teaching of the general theory of surgery.Methods:Clinical undergraduates from Batch 2015 and Batch 2016 of Medical College of Hunan University of Medicine were taken as participants. One teaching class of Batch 2015 was selected as control group ( n=117), and the traditional teaching method was adopted; one teaching class from Batch 2016 was selected as experimental group ( n=115), and the blended learning model was adopted. The results of two groups' process assessment and final assessment were compared. A questionnaire was conducted among the clinical undergraduates of Batch 2016 who practiced blended learning to evaluate their satisfaction with this learning mode. SPSS 22.0 was used for t test. Results:The experimental group was significantly better than the control group in process assessment and final assessment results [(93.65±3.71) vs. (91.46±5.63); (68.36±8.14) vs. (64.94±8.98)]. The analysis of the questionnaire survey showed that students had a high satisfaction with the blended learning mode, and their approval rate reached over 90%.Conclusion:In the teaching process of clinical course of general theory of surgery, blended learning mode is highly recognized by students, which can significantly improve students' independent learning ability and interest, simultaneously increase students' classroom participation and teacher-student interaction, and finally improve students' professional overall qualities and critical thinking.
4.The therapeutic effect of Kuijie enema combined with Western medicine on patients with ulcerative colitis

Objective:To explore the therapeutic effect of traditional Chinese medicine formula Kuijie enema combined with Western medicine on patients with ulcerative colitis.Methods:A clinical prospective randomized trial was conducted, and 107 patients with ulcerative colitis admitted to the Shiyan People′s Hospital from January 2018 to December 2022 were included in the study. They were randomly divided into a combination group of 54 cases and a control group of 53 cases using a random number table method. Both groups of patients received the same basic treatment plan, and the combination group was treated with traditional Chinese medicine formula for ulcerative colitis enema on this basis. Both groups of patients were treated continuously for 4 weeks, We observed the differences in serum inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-10 (IL-10), monocyte chemokine 1 (MCP1)], Mayo score, endoscopic mucosal score, traditional Chinese medicine symptom score, and clinical treatment efficacy between two groups of patients before and after treatment.Results:After treatment, the serum levels of TNF-α, IL-6, and MCP1 in the two groups were lower than those before treatment, and IL-10 was higher (all P<0.05), and the changes of the above inflammatory factors were more obvious in the combined group, with statistically significant differences (all P<0.05). After treatment, the Mayo score and endoscopic mucosal score of both groups of patients decreased compared to before treatment (all P<0.05), and the Mayo score and endoscopic mucosal score of patients in the combination group were lower than those in the control group (all P<0.05). After treatment, the traditional Chinese medicine syndrome scores of both groups decreased compared to before treatment (all P<0.05), and the combined group patients had lower scores for diarrhea, abdominal pain, pus and bloody stools, urgency and weight, and anal burning than the control group, with statistical significance (all P<0.05). Both groups of patients were treated continuously for 4 weeks, and the overall efficacy distribution of the combination group was better than that of the control group ( Z=-2.274, P=0.023). Conclusions:The combination of traditional Chinese medicine formula Kuijie enema liquid and Western medicine can more effectively reduce the degree of inflammation in patients with ulcerative colitis, alleviate clinical symptoms, and improve clinical treatment effectiveness.
5.Analysis on factors associated with taking subsequent confirmation test among men who have sex with men after being tested positive in oral fluid HIV antibody test in Beijing.

OBJECTIVETo analyze the proportion and associated factors of taking subsequent confirmation test among men who have sex with men (MSM) after being tested positive in oral fluid HIV antibody test.
METHODSBy using successive sampling, 1 003 MSM, who were tested positive in oral fluid HIV antibody test in China-Bill & Melinda Gates Foundation AIDS prevention Program (Extension program) in Beijing during May 1 to December 31, 2013, were recruited. The inclusion criteria included: the objects were men who reported having sex with men; the objects aged more than 18 years old; the objects were tested positive in oral fluid HIV antibody test; the objects had not been reported as HIV positives in China Information System for Disease Control and Prevention previously. According to the program strategy, MSM grassroots organizations transferred the respondents to seek subsequent confirmation tests in specific Center for Disease Control and Prevention (CDCs) or hospitals. The subsequent confirmation tests included: fingertip blood HIV antibody rapid test, venous blood Enzyme Linked Immunosorbent Assay (ELISA) HIV antibody test and venous blood Western Blot (WB) HIV antibody test. Chi-square test was adopted to compare the proportion of taking subsequent confirmation tests in different groups. Nonconditional multivaritae binarylogistic regression analysis was taken to identify the associated factors with whether taking subsequent confirmation tests and to calculate the OR (95% CI) values.
RESULTSThe 1 003 respondents were (30.9 ± 9.1) years old. Among all objects, 87.8% (881/1 003) of them took fingertip blood HIV antibody rapid tests and the positive rate was 85.4% (752/881). 98.0% (737/752) of those who were identified as positive in fingertip blood HIV rapid tests took ELISA and WB tests, and the positive rate was 94.4% (696/737). Comparing with those who were expected to seek subsequent confirmation tests in CDCs, the OR (95% CI) value of those who were expected to seek tests in hospitals was 5.10 (1.69-15.36). The OR (95% CI) values of those who used condom sometimes and those who never used condom in anal sex were 5.81 (2.14-15.77) and 3.45 (2.00-5.97) respectively, in comparison with those who reported not having anal sex or using condom consistently in anal sex during the past 6 months. Comparing with the respondents recruited from the internet, the OR (95% CI) values of those recruited in bathrooms, parks/toilets and bars were 0.17 (0.05-0.53), 0.10 (0.04-0.29) and 0.22 (0.06-0.79) respectively. The likelihood of taking subsequent confirmation test decreased with the increase of number of male sexual partners in the past 3 months, and the OR (95% CI) value was 0.92 (0.86-0.99).
CONCLUSIONThe potential HIV positive MSM in the bathroom, park/toilet and bars are less likely to take subsequent confirmation test. Those who do not use condom consistently during anal sex are more likely to seek subsequent confirmation test. Medical organization conducting subsequent confirmation tests is more likely to increase the confirmation test rate of potential HIV positive MSM. The number of male sexual partners has negative correlation with whether to accept the subsequent confirmation test.
