Chinese Journal of Digestive Surgery 2017;16(4):359-362
Hepatolithiasis is still a common biliary disease in China with unknown pathogenesis and poor long-term outcomes.Surgery is probably the only curative treatment to hepatolithiasis. However, surgeons and patients cannot keep trouble from high recurrence and reoperation rates after surgery.Hepatolithiasis is a serious disease, which can cause cholangitis, liver abscess, liver cirrhosis, and even cholangiocarcinoma.Sphincter of Oddi, which controls the unidirectional outflow of bile and separates the bile duct from the bacteria-filled intestinal tract,is thought to be a gatekeeper of the almost-sterile biliary tract.Dysfunction of sphincter of Oddi, including stenosis, spasm and laxity,is closely associated with biliary disease,A lax sphincter of Oddi will aggravate bilioenteric reflux, leading to biliary infection and calculi development eventually. This issue has gradually gained enough attention. How to evaluate sphincter of Oddi function reliably and restore or replace its function, which is key to treat hepatolithiasis and prevent its recurrence, remain unclear.

BACKGROUND:In recent years, the researchers have studied the adjacent segment degeneration after cervical fusion. Several studies concerned the adjacent segment degeneration after anterior cervical corpectomy and fusion.
OBJECTIVE:To evaluate the correlative factors of adjacent segment degeneration after anterior cervical corpectomy and fusion.
METHODS:122 patients were included in the study from 126 patients who underwent anterior cervical corpectomy and fusion that operated by the same treatment team and involved a single vertebral body and the two plates between February 2009 and February 2012. The final fol ow-up period was 3 years. According to the degeneration progress of adjacent segment degeneration, these patients were divided into non-progression group and progression group. The gender, age, preoperative adjacent segment degeneration, cervical range of motion, upper and lower vertebral range of motion, surgical segment, titanium mesh subsidence, plate-to-disc distance, and cervical curvature index were compared between two groups. Logistic regression analysis was applied to analyze the correlative factors of adjacent segment degeneration.
RESULTS AND CONCLUSION:(1) There were significant differences in preoperative adjacent segment degeneration, postoperative cervical range of motion, postoperative upper and lower vertebral range of motion, and postoperative cervical curvature index (P<0.05), which were included in logistic regression analysis. (2) Logistic regression analysis regression model:adjacent segment degeneration=0.317× postoperative cervical range of motion+3.374×preoperative adjacent segment degeneration+1.862 × postoperative lower vertebral range of motion-0.795×postoperative cervical curvature index-16.481. Logistic regression analysis demonstrated that preoperative range of motion, postoperative cervical range of motion, postoperative lower vertebral range of motion and postoperative cervical curvature index were correlative factors of range of motion after adjacent segment degeneration (P<0.05). (3) These results indicated that preoperative adjacent segment degeneration, postoperative cervical range of motion, postoperative lower vertebral range of motion and postoperative cervical curvature index were correlative factors of adjacent segment degeneration. We should actively carry out effective measures to prevent the progress of the adjacent segment degeneration before, during and after anterior cervical corpectomy and fusion.

Most of the S. pneumoniae isolates were from non-meningitis patients, showing high resistance rate to macrolides and tetracycline, but very low resistance rate to quinolones. Enterococcus isolates were mainly E. faecium and E. faecalis. More E. faecium were resistant to high-level gentamicin and high-level streptomycin than E. faecalis. E. faecium isolates were generally more resistant than E. faecalis to most of the antimicrobial agents tested except clindamycin and tetracyclines. But no gram-positive cocci were found resistant to vancomycin, linezolid or tigecycline. ESBLs-producing strains accounted for 53.1% of the E. coli strains and 28.5% of K. pneumoniae isolates. Enterobacteriaceae isolates were still very susceptible to carbapenems. E. coli isolates were more resistant to most of the antimicrobial agents tested than other Enterobacteriaceae except to piperacillin-tazobactam and carbapenems. Enterobacteriaceae showed higher resistance to trimethoprim-sulfamethoxazole than the other antibiotics tested. Majority of P. aeruginosa isolates were susceptible to all the antibiotics tested (<10% resistant). Objective To investigate the clinical significance of changes of serum amylase, CRP and SAA in the diagnosis of acute pancreatitis. Methods The levels of serum and urine amylase, CRP and SAA in patients of mild acute pancreatitis (MAP) and severe acute pancreatitis (SAP) at 24 h, 48 h, 72 h and the seventh day after the onset of pancreatitis were measured. Results The levels of serum, urine amylase, CRP and SAA in SAP patients at 24h were (904.5±402.2)U/L, (2280.3±1207.3)U/L, (155.6±36.2) mg/L, (521.9±109.4)mg/L, respectively, and significantly higher than those of MAP patients (P<0.05 or P<0.001). The peak value of serum amylase appeared at 24h, however, the peak value of urine amylase, CRP and SAA appeared at 48 h, and the corresponding values were (2173.5±1110.6) U/L, (185.3±41.4) mg/L and (717.5±144.2)mg/L, respectively. The levels of serum and urine amylase significantly decreases in MAP and SAP patients at the seventh day (P<0.05). The levels of serum CRP and SAA significantly decreased in MAP patients at the seventh day (P<0.05), however, the levels of serum CRP and SAA did not significantly decrease in SAP patients at the seventh day (P>0.05). Serum levels of CRP and SAA were related to the severity of acute pancreatitis. Meanwhile CRP showed a positive correlation with SAA (r = 0.761, P<0.05). Conclusions The change of serum levels of amylase, CRP and SAA can help early diagnose acute pancreatitis; CRP and SAA may predict the development of SAP at early stage.
