1.Clinical study of Radix Astragali, Radix Rehmanniae combined with glucocorticoid in treating systemic lupus erythematosus

Objective To observe the therapeutic effect of Radix Astragali,Radix Rehmanniae combined with glucocorticoid(GC) in treating patients with systemic lupus erythematosus(SLE) and its influence on some experimental indexes.Methods 72 cases of SLE were randomly recruited into the integrated traditional Chinese and western medicine treated group (treated group) and the western medicine treated group (control group).The control group was treated with prednisone and cyclophosphamide,while the treated group was treated with Radix Astragali and Radix Rehmanniae on the basis of the control group.Both groups had been treated for 6 months as a therapy course.The use of prednisone dosage were recorded and its influence on indexes of clinical and laboratory value were observed.Results 27 SLE patients in the treated group and 34 in the control group were completely observed.The dosages of prednisone in both groups were decreased gradually after the state of disease had been relieved.The predrisone dosage in the treated group was lower than that in the control group in the therapy course(15.41 ±7.65)mg vs (17.25±8.83)mg,but without significant difference (t=0.85,P>0.05).The total decreased prednisone dosage in the treated group (50.12±12.53 mg) was slightly higher than that in control group(48.96± 13.71) mg (t=0.34,P>0.05).The ratio of cases who had to add prednisone for aggravating disease in treated group(15.92%) was less than that in the control group (38.24%),the difference was significant (x2=3.24,P<0.05).The incidence rate of cushing syndrome,infection,diseases of digestive,cardiovascular anomalies in the treated group was slightly less than the control group (x2=0.42,0.98,0.01,0.03respectivly,P>0.05),but the number of insomnia and hot flush were significantly larger in the control group than the treatment group (x2=4.24,5.93,P<0.05).No difference about the SLEDAI,ESR,CRP,Blood Complement3,Blood imune globinG was found between the two groups (t=2.16,1.04,1.18,0.26,1.61 respectively,P>0.05),and 24 hours urinary protein count decreased significantly in the treated group (1.06 ± 0.12) g/L,compared with control group (1.42 ± 0.54) g/L,(t =3.78,P < 0.05 ).Conclusion Radix Astragal,Radix Rehmanniae combined with conventional treatment of western medicine could withdraw corticosteroid smoothly,relieve symptoms and alleviate some side-effects of western medicine.
2.Preparation of Chitosan Modified TFu Solid Lipid Nanoparticles

Objective:To study the preparation methods of N3-O-toluyl-flulorouracil (TFu) solid lipid nanoparticles(SLNs) and chitosan modified TFu SLNs. Methods: TFu-SLNs and chitosan modified TFu-SLNs were prepared by film dispersion-homogenization methods. The morphology, particle diameter and zeta potential were detected. The preparation methods were optimized by single factor experiments and an orthogonal design, and the stability of the nanoparticles was also studied. Results: The mean diameter of TFu-SLNs was 160. 2nm, and the zeta potential was -33. 2 mV. The mean diameter of chitosan modified TFu- SLNs was 400. 3nm, and the zeta potential was +12. 87 mV. With the concentration increase of chitosan, the zeta potential was enhanced. The optimized TFu-SLNs had higher reproducibility and stability. Conclusion:The formula of TFu-SLNs is optimized by an orthogonal design to obtain the optimal formula of TFu -SLNs and chitosan modified TFu-SLNs.
3.Lentivirus mediated CCN1 gene on growth and migration of rat bone marrow mesenchymal stem cells

[ABSTRACT]AIM:Toinvestigatetheroleofcysteine-rich61(Cyr61/CNN1)inproliferationandmigrationof bone marrow mesenchymal stem cells ( BMSCs ) .METHODS: The lentiviral vector carrying CCN 1 ( Lenti-GFP-CCN1 ) was constructed and then transfected into the rat BMSCs .The cells were divided into non-transfection group , transfection group ( transfected with Lenti-GFP-CCN1 ) and negative control group ( Lenti-GFP ) .The fluorescence intensity of the transfected BMSCs was observed under inverted fluorescence microscope .The effects of CCN1 on the proliferation and mi-gration of BMSCs were detected by MTT assay and scratch wound healing assay .RESULTS:The proliferation of BMSCs transfected with Lenti-GFP CCN1 had no significant difference compared with negative control group and control group .The width/thickness ratio of migrated BMSCs in wound healing was significantly higher in Lenti-GFP-CCN1 group than that in negative control group and control group (P<0.05).CONCLUSION:Exogenous CCN1 promotes the migration of BMSCs.
4.Predictive value of lipoprotein-associated phospholipase A2 on risk of cerebral infarction occurrence after transient ischemic attack

Objective To investigate the predictive value of plasma lipoprotein associated phospholipase A 2 (Lp-PLA2 ) in cere-bral infarction(CI) after transient cerebral ischemia attack (TIA).Methods Plasma Lp-PLA2 level was detected in 112 TIA pa-tients ,and the incidence rate of CI was observed on 7 ,30 ,90 d after TIA.Then the grouping was performed according to the Lp-PLA2 level ,and the predictive value of Lp-PLA2 in the risk of CI occurrence after TIA was evaluated.Results Among 112 patients with TIA ,27 cases (24.1% ) developed CI within 90 d;there were 17 cases(63.0% ) of CI after TIA in the Lp-PLA2 >207 μg/L group ,which were significantly higher than that in the 175-207 μg/L group and <175 μg/L group (P<0.05);moreover the CI occurrence in TIA patients was mainly concentrated within 7 d after onset ;the patients of Lp-PLA2 ≥175μg/L were mainly distrib-uted in the moderate and high risk groups of ABCD2 score system ;in ROC curve of Lp-PLA2 for predicting CI after TIA ,with Lp-PLA2 ≥194 μg/L as the diagnostic critical point ,the sensitivity was 0.730 and specificity was 0.680.Conclusion Lp-PLA2 may be an effective risk predictive indicator of CI occurrence after TIA ,and can improve the adverse outcome of TIA patients.
5.Effect of partial foraminotomy after laminoplasty surgery on preventing C5 nerve root palsy

Objective To investigate clinical curative effect of partial foraminotomy after C4.5 ossification of posterior longitudinal ligament (OPLL) on preventing C5 nerve root palsy.Methods A total of 155 patients with OPLL were treated with expansion open-door laminoplasty (EOLP) between January 2008 and October 2012,including 75 male patients and 80 female patients,with average age of 54.5 (ranging from 38 to 75).Among them,90 cases took EOLP (open team),and 65 cases took EOLP and C4.5 partial foraminotomy (incision team).Clinical curative effect and imaging evaluation were used in 8 weeks and 12 months after operation.JOA scores,JOA scores improvement rate,C5 nerve root palsy occurrence rate,neurological function of patients with C5 nerve root palsy and the prognosis,cervical curvature index,opening angular variation,backward drift distance of anterior margin and posterior margin of the spinal cord were evaluated after 8 weeks and 1 year after the surgery.Results The operation time was 90-140 min,with the average time of 110±14 min;mean bleeding volume was 160±45 ml (ranging from 100 to 250 ml);follow-up visit time was 15.4 months on average (ranging from 12 to 18 months).JOA score and improvement rate after 8 weeks and 1 year of the operation have no statistical significance.The C5 nerve root palsy occurrence rate in open team was 8.89% (8/90),while that in incision team was 4.62% (3/65).The difference of two groups has statistical significance.C5 nerve root palsy all happened in laminar opening side.Comparison of cervical curvature index of two groups before the operation and after 8 weeks of the operation and inter-group comparison have no statistical significance.The difference between laminar opening angle after 1 year of the operation and laminar opening angle after 8 weeks has no statistical significance.The difference of backward drift distance of anterior margin of the spinal cord at different time after the operation and inter-group difference have no statistical significance.The difference in backward drift distance of posterior margin of the spinal cord between 1 year after the operation and 8 weeks after the operation has no statistical significance.Conclusion Treating OPLL with EOLP and C4.5 partial foraminotomy can reduce occurrence rate of C5 nerve root palsy,but has no significant influence on JOA score improvement rate.

Objective To explore the clinical application of treating severe soft tissue defect on distal leg and foot with sural nerve nutrient vessels retrograde island flap.Methods A total of 34 cases of patients who were treated with sural nerve nutrient vessels retrograde island flap were regared as the observation group, and the other 12 cases who were treated with local rotation skin flap grafting were regared as the control group.The postoperative survival rate of skin flap,complications,functional recovery and local sensation of the two groups were evaluated.Results The transplanted skin flap of the observation group all survived after operation.There were 3 cases of partial necrosis of the skin flap distal edge which were cured with dressing change.A few tension vesicle occured in 3 patients and they were cured with dressing change.The transplanted skin flap of the control group all survived after operation,but there were different degree of scar contracture,flap deformation and flap atrophy 6 months after surgery.The postoperative function,local sensation,scar size of the observation group were better than those of the control group with statistically significant difference (P<0.05).Conclusion To treat severe soft tissue defect on distal leg and foot with sural nerve nutrient vessels retrograde island flap is a good method which can promote severe tissue defect repair on the lower extremity and foot.
7.Effects of Jieminqufeng decoction on the histomorphological changes of nasal mucosa in rats with allergic rhinitis

OBJECTIVE To observe the curative effect of Jieminqufeng decoction to the rats with allergic rhinitis. METHODS Fourty Wistar rats were randomly divided into 4 groups. There were Jieminqufeng decoction group (A),cetirizine group (B),model control group(C) and normal control group (D). The rats with allergic rhinitis were established with ovalbumin. The behavioral changes of the rats were observed. The histological and cellular morphological changes were studied with light microscope and transmission electron microscope. RESULTS The model control (C) group had typical symptoms of allergic rhinitis. By light microscope and transmission electron microscope,a lot of eosinophilic granulocytes were found in nasal mucosa. The mucosal cells were lost or destroyed. However,the histological and cellular morphological changes of nasal mucosa in Group A,B were similar to that in Group D. CONCLUSION The Jieminqufeng decoction is an effective drug to treatment of allergic rhinitis. It can decrease the aggregation and activation of eosinophilic granulocytes,lessen inflammatory reaction and block up the allergy.
8.Research on labial and lingual alveolar bone plate thickness of the mandibular anterior teeth by CBCT

Objective To investigate labial and lingual alveolar bone plate thickness of the mandibular anterior teeth by using cone-beam computed tomography (CBCT),in order to provide a reference for immediate implantation.Methods 132 individuals with normal occlusion were examined with CBCT,and their mandibular anterior teeth were analyzed.The labial and lingual alveolar bone plate thickness at the level of the apical,1/4 of the apical,1/2 of the middle and 1/4 of the cervical were measured.The differences of the thickness were compared among the side and gender.Results There were no significant differences between the left and right sides. Except on the 1/4 of the cervical,the thickness of lingual bone plate was thicker than that of labial bone plate.The frequency of the thickness of the labial bone larger than 2 mm on the 1/4 of the cervical of the center incisor,lateral incisor,and canine were 0,0 and 1.5 1%,respectively,and on 1/4 of the middle were 0.76%,1.44% and 3.79%,respectively.The lingual bone plate thickness showed significant difference between genders.Conclusion Reference values of alveolar bone plate thickness in anterior teeth of normal occlusion were established by using CBCT,which can provide clinical instruction for immediate implant.
9.Analysis of the risk factors of central nervous system complications after allogeneic hematopoictic stem cell transplantation

Objective To analyse the etiology,clinical characteristics and risk factors of central nervous system (CNS) complications after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods The clinical features of CNS complications in patients who underwent allo-HSCT were observed,and analysis its causes and risk factors.Results 8 cases of CNS complications occured in 69 patients within 6 months after allo-HSCT and the incidence was 11.6 %,the occurrence rate of CNS complications was 21.4 % (6/28) in HLA mismatched group,higher than HLA matehed group [49 % (2/41)] (P < 0.05).Analogously,the incidence was 44.4 % (4/60) in patients with graft-versus-host disease (GVIID) (>grade 2),which was significantly higher than patients with 2 or below grade 2 GVHD [6.7 % (4/9)] (P < 0.01).But there was no significant difference in the incidence of CNS complications between ≤14 years old and >14 years old,with or without ATG,different stages of diseases,whether pretreatment with maryland respectively (P >0.05),either.Epilepsy and intracranial infection were the most common CNS complications in allo-HSCT,followed by intracranial hemorrhage.Conclusion HLA mismatched and above grade 2 GVHD are the risk factors of CNS complications in allo-HSCT.Epilepsy,intracranial infection and bleeding are common CNS complications in allo-HSCT.
10.Expression of interleukin-1 beta, interleukin-6 and cyclooxygenase 2 in cervical intervertebral disc of cervical spondylosis patients with different clinical symptoms

BACKGROUND:During degeneration of cervical spine, biochemical changes appeared in intervertebral disc cel s. During this process, a variety of inflammatory cytokines may lead to disc herniation, which stimulates the production of a variety of inflammatory factors from surrounding adjacent tissue.
OBJECTIVE:To explore the expression and significance of interleukin-1β, interleukin-6 and cyclooxygenase 2 in patients with different clinical symptoms of cervical spinal cord oppression.
METHODS:Protrusion of the intervertebral disc or disc of responsibility among patients with anterior disc resection and internal fixation were divided into three groups according to clinical symptoms:myelopathic symptom group, nerve root symptom group and cervical spine trauma group. Intervertebral disc received hematoxylin-eosin staining and immunohistochemical staining for morphological observation. Positive cel s were counted according to the result of immunohistochemical staining.
RESULTS AND CONCLUSION:(1) Hematoxylin-eosin staining results showed visible inflammatory cel infiltration and new blood vessel formation in the myelopathic symptom group and nerve root symptom group. No remarkable inflammatory cel infiltration or new blood vessel formation was seen in cervical spine trauma group. (2) Immunohistochemical staining interleukin-1β-, interleukin-6-positive cel s were seen in the myelopathic symptom group. Cytoplasm was stained tan. Cyclooxygenase 2-positive cel s showed a low number. The numbers of interleukin-1β-, interleukin-6-and cyclooxygenase 2-positive cel s were significantly more in the nerve root symptom group than in the myelopathic symptom group. The numbers of interleukin-1β-, interleukin-6-and cyclooxygenase 2-positive cel s were smal in the cervical spine trauma group. (3) Expression rate and IA value of interleukin-1β, expression rate and IA value of interleukin-6 were significantly higher in the myelopathic symptom group and nerve root symptom group than in the cervical spine trauma group (P<0.05). Expression rate and IA value of cyclooxygenase 2 were significantly higher in the nerve root symptom group than in the cervical spine trauma group. (4) These results suggested that interleukin-1β, interleukin-6 and cyclooxygenase 2 expression could be found in the cervical intervertebral disc after protrusion, and played a role in early degeneration of cervical intervertebral disc. The expressions of these inflammatory factors were significantly different in patients with different clinical symptoms of cervical spinal cord compression.
