Objective To observe the effect of combination of dexmedetomidine and sufentanil on postoperative analgesia for the highly nicotine dependent patients undergoing esophagectomy. Methods Ninety highly nicotine dependent patients undergoing esophagectomy were randomly allocated into three groups (n = 30 each): low-dose sufentanil group (Group S1), high-dose sufentanil (Group S2), dexmedetomidine and sufentanil group (Group DS). All patients of three groups received postoperative PCIA with following setting: 2 mL/h;bolus: 2 mL; lock time: 5 min; 4 hours limited: 40 mL. The scores of VAS and Ramsay, effective presses/total presses of PCIA, the consumption of sufentanil during 72 h after operation, side effects and the satisfaction degree of patients were recorded. Results Compared with those of group S2, the scores of VAS (both at rest and movement) decreased significantly in group DS from 1 h to 8 h (P<0.05). Compared with those of group S1, the scores of VAS (both at rest and movement) decreased significantly in group S2 and DS from 1 h to 72 h (P<0.05). Compared with that in group S1 and S2, the consumption of sufentanil during 72 h after operation decreased significantly in group DS (P < 0.05). Compared with those in group DS, The numbers of nausea and vomiting were significant decreased in group S1 and S2(P < 0.05). Compared with those in group S1, the remedial cases were significantly decreased in group S2 and group DS (P < 0.05). Conclusion The effect of combination of dexmedetomidine and sufentanil is superior to that of sufentanil in terms of postoperative analgesia in highly nicotine dependent male patients undergoing esophagectomy.

Objective To discuss evolution regularity of Chinese Medicine Syndromes and neurological impairment of acute stroke patients with and without hypertension,and to contrast differences between the two groups.Methods Case observation table of ＂Clinical diagnosis and treatment of stroke patients dialectical law mining and verification of the program of treatment was adopted to record clinical diagnostic messages of 63 cases of non-hypertension and hypertension acute stroke patients.Results ①Contrasting syndrome dements between the two groups it showed that on the tenth day,scores of yin deficiencyin non-hypertension group score (12.83 ±9.97) increased significantly than hypertension group score (8.64±5.90) with statistical significance (P＜0.05) ; on the fourteenth day,scores ofblood deficiency in hypertension group score (12.21±7.71)increased significantly than non-hypertension group score (8.07±8.61),with statistical significance (P＜0.05).② In hypertension group,from the first day to the fourteenth day of admission,scores of wind,hot and phlegm gradually reduced,but scores of blood deficiency gradually score (9.85 ±7.23) increased,with statistical significance.In non-hypertension group,from the first day to the fourteenth day of admission,scores of wind,hot and phlegm gradually reduced,but scores of yin deficiency gradually score (12.21±7.71)increased,with statistical significance (P＜0.05).Conclusion Hypertension turns blood defiency more prominent in evolution of Chinese medicine syndromes of stroke.

Objective To explore the correlation between the change of intestinal flora and insulin resistance in pre-diabetes population,and the role of intestinal flora in the occurrence and development in the pre-diabetes population.Methods two hundreds and fifty cases of pre-diabetes in our hospital were selected and divided into the group A(pre-diabetes intervention group)and B(prediabetes non-treatment group).Fifty cases of diabetes(positive control group)served as the group C.The cohort study was adopted.The follow-up intervention lasted for two years.The intestinal flora and blood biochemical indicators were detected in different groups and at different time periods.The insulin resistance index was calculated.Results The total bacterial count before intervention in the group A and B was significantly higher than that in the group C;Enterococcus genus in the group C was significantly higher than that in the group A and B;the proteus and lactobacillus content in the group A and B was higher than that in the group C,but the difference was not significant;Bifidobacterium genus in the group C was higher than that in the group A and B.After intervention,the total bacterial amount in the group Awas significantly higher than the group B and C,but there was no statistical difference between the group B and C;enterococcus genus in the group C was significantly higher than that in the group A and B,enterococcus genus after intervention in the group A was reduced and which in the group B was increased,the difference had statistical significance;the proteus and lactobacillus content in the group A and B was higher than that in the group C,but the difference was not significant;bifidobacterium genus in the group C was higher than that in the group A and B,moreover which after intervention in the group B was increased,the difference was statistically significant.The insulin resistance before processing had no statistical difference between the groups A and B,which in the group C was higher than the group A and B,the insulin resistance after intervention in the group A was decreased,the difference was statistically significant.The total bacterial count,enterococcus and bifidobacterium were positively correlated with insulin resistance,and bacteroides,proteus and lactobacillus had no significant correlation with insulin resistance.Conclusion Intestinal flora has correlation with insulin resistance,and serves as a marker of pre-diabetes development,implementing the probiotics adjustment combined the quantization aerobic muscular movement can realize the adjustment of insulin sensitivity and improves the diabetic development in the prediabetic population.

Objective To investigate Ureaplasma urealyticum in cervical specimens of gynecologic check-up population and female sex workers(FSW)and its biovars and serotypes.Methods The gynecologic check-up patients and female sex workers were screened using Uu liquid selective medium.The positive cases were then divided into biovars and serotypes by the means of PCR.Results Ninety-eight FSW and164gyne-cologic check-up patients were investigated.In the FSW group,89cases(90.8%)were positive in culture,in-cluding biovar164cases(71.9%),biovar1+27cases(7.9%),biovar218cases(20.2%).In71biovar1positive cases,biovar1+2and co-infection of serotypes of biovar1were defined as mixed infection.Infection with single serotype of biovar1was defined as simple infection.So in71biovar1positive cases of FSW group,there were17mixed infections(23.9%)and54simple infections(76.1%).Simple infections were then subdivided into serotypes,including serotype117cases(31.5%),serotype322cases(40.7%),and serotype615cases(27.8%).In gynecologic check-up group,104cases(63.4%)were culture positive,in-cluding biovar197cases(93.3%),biovar27cases(6.7%).In97biovar1positive cases,there were11mixed infections(11.3%)and86simple infections(88.7%).In86simple infections,there were serotype122cases(25.6%),serotype337cases(43.0%),and serotype627cases(31.4%).Conclusions The posi-tive rate of Uu in FSW is higher than that of gynecologic check-up population(P

BACKGROUND:At present, the main research fields about coronary stents are the whole degradation biological materials with high biocompatibility and drug control ed release systems.
OBJECTIVE: To evaluate the safety after the two novel biodegradable stents implanted in coronary arteries in porcine models.
METHODS:The normal ful y biodegradable stents were made up of the poly-L-lactide and the antiproliferative drugs paclitaxel, and the novel biodegradable stents were added in amorphic calcium phosphate at the basis of normal biodegradable stents. (1) Five normal ful y biodegradable stents were randomly implanted into the coronary arteries of five porcines, and five novel biodegradable stents were randomly implanted into the coronary arteries of the remaining five porcines by coronary angiography. The blood biochemistry and C-reactive protein levels were measured pre-operation and at 28 days after operation. Coronary angiography was utilized to observe the lumen unobstructed at 28 days after surgery. (2) Under a microscope, seven normal ful y biodegradable stents and seven novel biodegradable stents were implanted into right external iliac arteries of 14 rabbits. Blood urea nitrogen and creatinine levels were measured before surgery and at 28 days after operation.
RESULTS AND CONCLUSION:At 28 days after operation, there were no significant changes in porcine glutamic-pyruvic transaminase, aspartate aminotransferase, triacylglycerol, total cholesterol, low density lipoprotein and C-reactive protein levels compared with that before operation, but urea nitrogen and creatinine levels were significantly higher than that before operation (P<0.05). The result of coronary angiography showed that no in-stent thrombosis or stenosis was detected in either group. There was no significant difference in urea nitrogen and creatinine levels in both groups. These results suggested that it is safe and compatible after the two novel biodegradable stents implanted in coronary arteries of porcine models, and the stents had good histocompatibility.

Objective To explore the expression of CD44v8 protein in human bladder and urothelial carcinoma ,as well as the value in diagnosis of human bladder and urothelial carcinoma .Methods RT‐PCR was used to analysis the expression of CD44v8 protein in 75 patients with bladder and urothelial carcinoma in the pathological stage and clinical stage and 20 subjects of normal bladder mucosa were collected as control .Results CD44v8 protein was negative in all normal bladder and urothelial mucosa ,the copy number was less than 1 × 102 copy/mL ;26 cases of bladder and urothelial carcinoma was positive ,and the positive rate of CD44v8 was 34 .7% ,and Ct values were less than 35 and copy number was greater than 1 × 104 copy/mL .Positive rate was correla‐ted with high pathological grades and TNM stages ,but no significant difference was observed in recurrence of tumor .Conclusion CD44v8 could be useful indicator for the assessment of pathological grades and TNM stages of bladder and urothelial carcinoma .

Objective To study the efficacy and safety of rociverine combined with tamsulosin in treatment of overactive bladder after transurethral resection of prostate (TURP).Methods 93 cases of overactive bladder patients who received TURP were collected.They were given rociverine tablets 10 mg 3 times a day and tamsulosin 0.2 mg once a day for 7 to 10 days.The efficacy and safety of treatment were assessed by changes before and after treatment in subjective indicators of overactive bladder syndrome score(OABSS),the international prostate symptom score(IPSS) and quality of life score(QOL) and objective indicators including daily frequency of micturition,urgency,urgency incontinence,nocturia,voiding volumes,combining with the adverse drug reaction incidence.Results The average treating time for the 93 cases was (5 ± 3) days.The average score of OABSS reduced 8.12(P ＜0.01,and each score≤ 4).The average score of IPSS reduced 16.69(P ＜0.01).The average score of QOL reduced 2.18 (P ＜ 0.01).There was statistical difference in objective indicators of patients between before and after treatment(P ＜ O.05).The adverse drug reaction incidence was 2.1％ (2 cases with thirsty symptom).Conclusion Rociverine tablets combined with tamsulosin in treatment of patients with overactive bladder after TURP are effective and safe.

Objective The aim of this study was to investigate the differences on origin distribution and radiofrequency ablation characteristics between premature ventricular contractions (PVC) from left and right ventricular.Methods A total of 354 frequent PVC patients were enrolled.Two hundred and eighty patients were PVCs with right ventricular origin (group RV) and 74 patients with left ventricular origin (group LV).The age,gender,ablation power,temperature and duration,time of target potential before QRS onset,fluoroscopic time,procedure time and success rate were compared between the two groups.Results Majority PVCs (79.10％) were of right ventricular origin,in which the most common site was at right ventricular outflow tract (RVOT) (55.93％);whereas,only 20.90％ were of left ventricular.There were significantly differences between group RV and group LV in age [(44.7 ± 13.6) yrs vs (49.6 ±15.9) yrs,P =0.017],gender [male:35.4％ (99) vs 55.4％ (41),P =0.002],time of target potential before QRS onset [(37.9 ± 16.2) msvs (31.3 ± 11.7) ms,P=0.008],procedure time [(85.1 ± 36.9) minvs (100.8 ± 45.5) min,P=0.017],fluoroscopic time [(12.3 ± 9.2) min vs (15.9± 10.8) min,P =0.028] and success rate (96.4％ vs 89.2％,P =0.012).Conclusions Radiofrequency ablation of PVCs was a safe and effective method.Subjects with RV PVCs are younger and more females than subjects with LV PVCs.The radiofrequency ablation for RV PVCs was easier,and the success rate was higher than that for LV PVCs.

Objective To investigate the expression of CD44 varant 2(CD44v2) in bladder and urothelial carcinoma ,and to study its significance in the diagnosis of human bladder and urothelial carcinoma .Methods Real‐time fluorescent quantitative PCR was used to analyze the expression of CD44v2 protein in 70 bladder and urothelial carcinoma tissue samples from patients in different pathological and clinical stages .Meanwhile ,20 tissue samples of normal bladder mucosa were collected as controls .Results CD44v2 expression was negative in normal bladder and urothelial mucosa ,the gene copies were less than 1 × 102 copies/mL ,while the posi‐tive expression rate of CD44v2 was 42 .9% (30/70) ,and Ct values were less than 35 and copy number was greater than 1 × 104 cop‐ies/mL .Positive expression rate was correlated with high pathological grades and TNM stages .Conclusion CD44v2 could be an useful indicator for the early assessment of bladder and urothelial carcinoma .
