Objective To investigate the serological patterns of hepatitis B, understand the prevalence of hepatitis B infection in neonates, infants and preschool children based on hospital data in Zhejiang province and provide a basis for prevention and control of local pediatric hepatitis B infection. Methods Five serological markers were detected on electrochemiluminescence analyzer Roche E601. The results of all serological markers in children with hepatitis B from 2006 to 2010 were exported from laboratory information system and 24 892 (15 810 boys and 9 082 girls) cases were selected for further analysis according to the exclusion criterion including duplicate analysis, children older than 7 years and children living out of Zhejiang province. SPSS13.0 software and EXCEL 2003 were employed for statistical analysis in this study. Results Average HBV vaccination coverage rates among 15 413 boys and 8 840 girls were 99. 16%(98. 87% -99. 45%) and 99. 17% (98. 29% -99. 53%), respectively. Seventeen serological patterns of hepatitis B were observed, and "anti-HBs(+)alone"(64. 997%) was the dominant patterns followed by "negative" (18. 231%), "anti-HBc(+)anti-HBs(+)"(9.911%) and "anti-HBe(+)antiHBc(+) anti-HBs(+)"(4.395%), and the positive rates for other patters were 0. 004% -0. 992%. There were significant difference of detection rates for "anti-HBc(+)alone" and "anti-HBe(+)anti-HBs(+)"patterns between male and female (χ2=7.143,8.123, all P＜0.01), and there were no significant difference of detection rates for the other 15 patterns between male and female (all P ＞0.05). Ninety-four HBsAg positive cases were detected, accounting for 0.38%. Totally 19 899 anti-HBs positive cases were detected, accounting for 79.94%. Totally 16 187 cases were "anti-HBs(+) alone", accounting for 81.3%of the total anti-HBs(+) cases. The group of 1-2 years and 2 day-2 week had the highest and lowest rate for anti-HBs(+)alone respectively. The ratios in three groups with different anti-HBs levels (10-100 mIU/ml,100-1000 mIU/ml and ＞ 1000 mIU/ml) in 16 178 anti-HBs(+) children were 23. 64%, 27.66% and 13. 70%, respectively. Totally 3 904 cases with anti-HBc(+) were detected, accounting for 15.68%. The highest positivity rate and lowest positivity rate were in the group of 2-4 week and 3-4 week respectively. In total 1 565 cases with negative results, the lowest negativity rate and highest negativity rate were in the group of 9-12 month (3. 25%) and 6-7 age (36. 21%) respectively. Conclusions The dominant serological patterns of hepatitis B in local young children is "anti-HBs (+) alone" and most of the anti-HBs concentrations ranged from 100 to 1000 mIU/ml, followed by "negative". The total positive rate of HBsAg is mild. The rate of hepatitis B serological patterns in children is associated with ages but independent of gender. For interpretation of serological patterns of HBV in children below 12 months (especially in 3 months) the status of their mothers' HBV infection should also be taken into consideration.

Objective To investigate the effects of mental health and sleep quality of the armored military performance ,pro-viding a theoretical basis for improving the performance of military officers and soldiers .Methods 276 armored military were se-lected randomly to study the performance ,including military training program evaluation(40% ) ,armored vehicles operating assess-ment(40% ) and leadership assessment(20% ) ,the test results as a percentage of the sum to exceed 80 divided into good and poor performance for the military .Symptom Checklist(SCL-90) and Scale Pittsburgh Sleep Quality Index(PSQI) were assessed ,and to evaluate the performance differences between good and poor military .Results (1)The good performance of armored military were 61 .2% ,and 38 .8% ones were poor .(2)The SCL-90 scores ,interpersonal sensitivity ,anxiety and hostility factors of good military performance were significantly lower than those with poor military performance(P<0 .05) .(3)The PSQI scores of Good military performance was 5 .83 ± 2 .94 ,and the poor was 7 .63 ± 3 .85 ;The sleep quality ,sleep latency ,sleep efficiency and daytime dysfunc-tion factors of good military performance subjective were significantly lower than those of poor military performance(P<0 .05) . Conclusion Psychological status and quality of sleep are important factors to influence performance of military armored force .

Objective To evaluate normal maturation of the fetal brain with half-Fourier rapid acquisition with relaxation enhancement(RARE) MRI.Methods The normal brains of 25 fetuses of 12-38 weeks gestational age were examined in utero with half-Fourier RARE imaging.Gyrus maturation,gray and white matter differentiation,ventricle-to-brain diameter ratio,and subarachnoid space size were evaluated with respect to gestational age.Results At 12-23 weeks,the brain had a smooth surface,and two or three layers were differentiated in the cerebral cortex.At 24-26 weeks,only a few shallow grooves were seen in the central sulcus,and three layers,including the immature cortex,intermediate zone,and germinal matrix,were differentiated in all fetuses.At 27-29 weeks,sulcus formation was observed in various regions of the brain parenchyma,and the germinal matrix became invisible.Sulcation was seen in the whole cerebral cortex from 30 weeks on.However,the cortex did not undergo infolding,and opercular formation was not seen before 33 weeks.At 23 weeks and earlier,the cerebral ventricles were large;thereafter,they gradually became smaller.The subarachnoid space overlying the cortical convexities was slightly dilated at all gestational ages,most markedly at 21-26 weeks.Conclusion Changes in brain maturation proceed through stages in an orderly and predictable fashion and can be evaluated reliably with half-Fourier RARE MRI.

Background Radiation-induced optic neuropathy (RION) is a severe complication after radiotherapy for head and neck cancer,which threatens the visual acuity and quality of life of patients.Till now,there is no recognized treatment for RION.It is of great significance to study the natural progression of the RION,and to prevent and treat RION.Objective This study was to establish an ideal radioactive optic nerve injury animal model.Methods Healthy 8-week SD rats with hygiene grade were randomly divided into normal control group and model group,with 6 rats in each group.The total 30 Gy dose of radiation with 3 portions was used to irradiate the head model group rats;ELISA was performed to analysis the changes of endothelin-1 (ET-1) and Von Willebrand factor (vWF) concentrations in blood 2,4 and 8 weeks after irradiation.Hematoxylin-eosin staining and transmission electron microscope were performed to observe the changes of optic structure.The use and care of the experimental animals complied with the ARVO statement.Results The concentrations of ET-1 in the model group were (23.18± 0.11),(27.98 ±0.22),(33.90 ±0.1 1),(65.25 ±0.38) and (43.82 ± 0.09) pg/ml before irradiation,1 day,2,4,6 weeks after irradiation,those in the normal control group were (22.65 ± 0.14),(23.18 ± 0.19),(23.68 ± 0.15),(24.23±0.12) and (23.58±0.16)pg/ml.The concentrations of vWF in the model group were (63.16±2.21),(88.32± 2.06),(123.38 ± 1.36),(191.40 ± 0.61) and (141.69 ± 0.82) pg/ml before irradiation,1 day,2,4,6 weeks after irradiation,those in the normal control group were (62.82 ± 1.56),(63.35 ±2.06),(64.12 ± 1.76),(63.52±2.02) and (63.48 ± 1.55)pg/ml.There were significant differences of ET-1 and vWF concentrations among different groups and time points (ET-1:Fgroup =32.160,P =0.012;Ftime =21.180,P =0.023.vWF:Fgroup =73.110,P=0.001;Ftime =46.180,P =0.002).The nerve fiber bundles was swelled with disordered arrangement and vacuolization 8 weeks after irradiation.Axon swell and atrophy,axons with myelin sheath layer plate separation were obtained.The rates of axon demyelination in the normal control group and model group were (1.35 ±0.79) ％ and (14.44±2.32)％,respectively.There was a statistically significant difference between the two groups (t =14.07,P＜0.01).Conclusions The total 30 Gy dose of radiation on the head of rats can make stable radioactive optic nerve injury model.This model making method is simple,cheap and practical,which is worth further study.

Objective To investigate the therapeutic effects and mechanism of biliopancreatic diversion (BPD) surgery on type 2 diabetes mellitus(T2DM) in GK rats.Methods 16 GK rats were randomly divided into 2 groups:BPD surgery group included 10 rats undergoing BPD surgery,sham-BPD group included 6 rats undergoing a sham operation.Fasting plasma glucose,insulin,glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide(GIP)were detected one week before BPD surgery and the 1st week,4th week,10th week,26th week after BPD surgery.Oral glucose tolerance test(OGTT) and insulin tolerance test(ITT) were done in the 10th week after BPD surgery.Results There was no statistical difference in fasting plasma glucose,insulin,plasma GLP-1 or GIP between the 2 groups before surgery.Plasma glucose had significant reduction in BPD group compared to that in the sham group(P ＜0.05) and insulin level had no significant difference between the 2 groups.Rats in BPD group had significant improvement in glucose tolerance and insulin sensitivity compared to those in the sham group.Serum level of GLP-1 was significantly elevated in BPD group compared to that before surgery (P =0.0337 at the 1st week after surgery; P =0.0002 at the 4th week after surgery,P ＜ 0.0001 at the 10th week after surgery,P ＜0.0001 at the 26th week after surgery) and that in sham-BPD group(P =0.0354 at the 1st week after surgery,P =0.0032 at the 4th week after surgery,P =0.0001 at the 10th week after surgery,P ＜0.0001 at the 26th week).Serum level of GIP was significantly lowered in BPD group compared to that before surgery(P =0.0189 at the 1st week after surgery; P =0.0007 at the 4th week after surgery,P =0.0003 at the 10th week after surgery,P ＜0.0001 at the 26th week after surgery) and that in sham-BPD group(P =0.0089 at the 1st week after surgery,P =0.0002 at the 4th week after surgery,P =0.0006 at the 10th week after surgery,P ＜0.0001 at the 26th week after surgery).The difference had statistical significance (P ＜0.05).Conclusion BPD surgery can significantly reduce fasting plasma glucose,improve glucose tolerance and insulin sensitivity.The change of serum levels of GLP-1 and GIP may play the major role in BPD treatment of diabetes mellitus.

Objective To detect the expression of BP1 gene in thyroid cancer and its relationship with clinicopathological features of thyroid cancer.Methods BP1 gene expression in 60 cases of thyroid cancer tissues and 20 cases of normal thyroid tissues were detected by in situ hybridization and immunohistochemistry.Results The positive expression rate of BP1 mRNA was 78.3 ％ (47/60) in the 60 cases of thyroid cancer tissues while it was 20％ (4/20) in the 20 cases of normal thyroid tissues detected by in situ hybridization.The difference had statistical significance (P ＜ 0.05).Of the 3 pathological types of thyroid cancer,the positive expression rate of papillary carcinoma was 81.6％ (40/49),follicular carcinoma 85.7％ (6/7),and medullary carcinoma 25.0％ (1/4).The expression of BP1 mRNA had statistical difference between medullary carcinoma and other pathological types like papillary carcinoma and follicular carcinoma (P ＜ 0.05).The positive expression rate of BP1 protein was 93.3％ (56/60)in the 60 cases of thyroid cancer tissues while it was 10.0％ (2/20) in the 20 cases of normal thyroid tissues detected by immunohistochemistry.The difference had statistical significance(P ＜0.05).Conclusion BP1 gene expression is up-regulated in human thyroid cancer and it is related to tumor stage and pathological type but not related to patients' age,sex or lymph node metastasis.

Objective:To observe the effects of acupuncture-moxibustion on symptoms,signs,and Th1/Th2 cytokine,and regulation on Th1/Th2 in patients with perennial allergic rhinitis(PAR) to provide experimental basis for acupuncture treating allergic rhinitis and investigate the mechanism.Methods:Thirty patients with allergic rhinitis were treated with acupuncture-moxibustion,and 30 healthy cases were the control.Clinical symptoms,signs,and nasal mucosa were observed,and contents of IL-4,IFN-γ,and granulocyte-macrophage colony stimulating factor (GM-CSF) in the supernatant of peripheral blood monocyte cultured with PHA by ELISA,before treatment,after one course of treatment,and after two courses of treatment.Results:Before the treatment,the contents of IL-4,and GM-CSF were higher (P＜0.01) and IFN-γ was lower (P＜0.01) in the PAR cases than in the control cases.In the PAR cases,the content of GM-CSF was positive correlation with that of IL-4 (P＜0.01),and negative correlation with IFN-γ (P＜0.01).After the treatment,contents of IL-4 and GM-CSF were lowed (P＜0.01),content of IFN-γ was increased (P＜0.05),and all IL-4,GM-CSF and IFN-γ had no difference compared with those in the control cases.Conclusion:Acupuncture moxibustion could improve the symptoms and signs in PAR patients,enhance the immune function.It may be achieved by regulating the expression of Th1 and Th2,correcting the imbalance of Th1/Th2.

ObjectiveTo establish pediatric serum CysC reference intervals based on the children's hospital laboratory data in Zhejiang Province and analyse the effects of CysClevels on age and gender. MethodsCysC was one of tests of a routine biochemical screening panel employed for most outpatients and inpatients in the children's hospital, and 8 127 subjects (4 264 boys and 3 863 girls) were selected from 13 567 subjects from laboratory information system according to the exclusion criterion with seriously systemic diseases, cardiovascular diseases, kidney diseases, elevated values of creatinine, urea or ALT for which 1.5-fold upper limit, serum samples with hemolysis, icterus or lipid turbidity, same patients with non-first-time CysC results and living addresses which were not in Zhejiang. The serum CysC was determined by transmission turbidimetry on Roche DPP modular automatic biochemical analyzer. SPSS 17.0 software and EXCEL 2003 were employed for statistical analysis in this study. Results The serum CysC concentration was a Gaussian distribution after log transformation. The mean Log-transformated CysC lg (CysC) concentration of boys in five age groups ( ＜ 1 month, 1 to 3 months, 4 to 11 months, 1 to 2 years and 2 to 16 years) were 0. 224,0. 170,0. 112,0. 061, -0. 011 (mg/L,lg) respectively, and the mean Log-transformated CysC lg(CysC) concentration of girls in five age groups were 0. 222, 0. 164, 0. 089, 0. 057,-0. 010 ( mmg/L, lg) respectively, and no statistically significant differences between Ig( CysC ) and gender in five age groups were found ( t values were 0. 174, 0. 362, 0. 445,- 1. 464 and - 0. 093, respectively,and corresponding P values all were greater than 0. 05 ). The mean lg ( CysC ) concentrations in five age groups were 0. 222,0. 166,0. 100,0. 059, - 0. 010 ( mmg/L, lg), and significant differences between Lg ( CysC ) and ages by Analysis of Variance were observed ( F = 309. 785 and P = 0 in between-groups totally,P = 0 in any two groups). Serum CysC levels were highest in the age of ＜ 1 month, then declined to the age of 2 years and kept stable in the age of 2 to 16 years. The serum CysC reference intervals for children were as follows:0. 95 -2. 92 mg/L in the age of ＜ 1 month, 0. 81 -2.67 mg/L in the age of 1 to 3 months, 0. 65 -2.45 mg/L in the age of 4 tol2 months, 0. 56 -2. 35 mg/L in the age of 1 year and 0. 45 -2. 13 mg/L in the age of 2 to 16 years. ConclusionsThere is no significant effect on pediatric CysC levels with gender but close correlated with age below 2 years old. It is necessary to establish appropriate age-related reference intervals of serum CysC for efficiently evaluating renal function in local children.

Most of the current experiments are obsolete and verified,the students are not interested in and the experiments are out of touch with the actual work.These are the common problems in the experiment teaching of occupational health and occupational medicine.By taking the students who majored in preventive medicine as the subjects,this study aimed to explore the effects of independent designed experiment teaching mode,which was based on dividing the groups before class,choosing the projects by students themselves,deciding the design of the program via the discussion of teacher and students and doing the experimental reports and sharing the experience.The results showed that,in the premise of the preliminary master of the theory and basic skills of occupational health and occupational medicine,carrying out independent designed experiments in the last three weeks of semester,to a certain extent,could arouse students' interest in learning,cultivate their abilities of independent thinking,practice,problem analysis and solution,and team cooperation.However,restricted by lack of teachers,inadequate equipment and high cost and other factors,this teaching mode is only suitable for small class.
