2.Relationship between operative procedures and complication of colorectal carcinoma with acute colonic obstruction in the elderly patients

Objective To study the relationship between operative procedures and complication of colorectal carcinoma with acute colonic obstruction in the elderly patients. Methods 65 elderly patients undergoing emergency operation for acute colonic obstruction were analyzed retrospectively. One stage tumor resection and anastomosis was performed in 25 cases with right-side colonic lesion and in 26 cases with left-side colonic lesion, Hartmann operation in 8 cases, Dixon operation in 4 cases and proximal colostomy in 2 cases. Results In 65 cases, 13 cases of postopera-tive complications,the rate was 20.0% (13/65) ,which infected with toxic shock two cases,four cases of pulmonary infection,anastomotic leakage two cases, wound infection, split 2 cases, death three cases, the mortality rate was 4.6%. The rates of postoperative complications in right-side colonic lesion, left-side colonic lesion, Hartmann opera-tion and Dixon operation were 69.2% ,0,30.7% and 0 respectively(P < 0.05 ~ 0.01). Conclusion Acute obstruc-tion of colon cancer,the right-side colonic lesion were safe,feasible and few complications in colorectal carcinoma with acute colonic obstruction.
3.Comparative study on two types of digestive reconstruction after total gastrectomy in patients with gastric carcinoma

Objective To compare two types of digestive reconstruction and postoperative complications after total gastrectomy in patients with gastric carcinoma. Methods Roux-en-Y esophagojejunostomy and P-type Roux-en-Y esophagojejunostomy were performed in 50 patients with gastric carcinoma after total gastrectomy. The operating time and postoperative complications were observed. Results The gastrointestinal symptoms for Roux-en-Y esophagojeju-nostomy and P-type Roux-en-Y esophagojejunostomy serve as : diarrhea ( 11.8% and 12.1% ), dumping syndrome (3.9% and 3.3% ) and reflux esophagitis(5.9% and 6.0% ) ,the incidence of the difference was not significant ( than,P>0.05 ). The postoperative complications in P-type Roux-en-Y esophagojejunostomy( 13.1% ) slightly higher that Roux-en-Y esophagojejunostomy( 11.1% ) ,but the difference was not significant( P>0.05 ). Roux-en-Y esoph-agojejunostomy average operative time(3.1±0.3)h was significantly reduced than P-type Roux-en-Y esophagojeju-nostomy(3.7±0.2)h(t=3.35,P<0.05). Conclusion Roux-en-Y esephagojejunostomy and P-type Roux-en-Y esophagojejunostomy is a suitable type after total gastrectomy for patients with gastric carcinoma, but the former opera-tive time was shorter.
4.Comparison of the incidences of apical root cracks after canal preparation with two nickel-titanium rotary systems:an in vitro study

Objective: To compare the incidences of root cracks after canal instrumentation with HyFlex CM system and the ProTaper Universal system .Methods:Sixty mandibular incisors were mounted in resin blocks with simulated periodontal ligaments , and the apex was exposed .The control group of 20 teeth was not prepared , and the other 40 teeth were divided into 2 experimental groups ( n=20 ) .The 40 root canals of the experimental groups were instrumented using HyFlex CM and ProTaper Universal to the major apical foramen ( AF).The horizontal sections 1 mm, 2 mm, and 3 mm from the apex were observed under an optical stereomicroscope at 25 ×magnification .The presence of cracks was noted . Results:No cracks were found in the control teeth .Cracks were found in 1 of 20 ( 5%) teeth in HyFlex CM group , and 17 of 20 ( 85%) teeth in ProTaper Universal group .The difference between the two experimental groups was statistically significant (P<0.01).Conclusion: The HyFlex CM files caused fewer root cracks than the ProTaper Universal files during the root canal instrumentation .

Objective To investigate the degree of health knowledge and needs for health education of urban and rural inhabitants in Tianjin, and to study strategy of health education and health promotion in community. Methods Using a questionnaire survey, 900 inhabitants were investigated on their health knowledge, behavior, access to knowledge, and health education needs. Results The rate of health knowledge awareness was 65.63%, the rate of health promotion was 67.05%. Logistic regression results showed that the education background level (OR=1.323, 95%CI=1.113~1.573) and income per capita monthly(1.607, 1.317~1.962)were the influential factors for the rate of health knowledge awareness (P

Objective To investigate the impact of the COX-2 specific inhibitor parecoxib on postoperative serum cytokines and early cognitive function in the elderly patients undergoing knee replacement.Methods Seventy-two elderly patients undergoing knee replacement were divided into a control group (group Ⅰ)and parecoxib group (group Ⅱ).MMSE score were compared at preoperative 1 d and postoperative 1 d,3 d,7 d.Before induction of anesthesia (T1 ),postoperative 6 h (T2 ),1 d (T3 ),3 d (T4 ),7 d (T5 ),the serum IL-1β,IL-6 concentrations were measured,meanwhile the serum S-100βwere measured in T2 .Results At postop-erative 1 d,3 d,the MMSE score in group Ⅰ was significantly lower compared with the preoperative (P <0.05).IL-6 in T2 ,IL-1βin T3 increased in both group.In group Ⅱ,three days later,the exaltation of plasma inflammatory factors returned to the preopera-tive level,but this process last untill T5 in group Ⅰ.The plasma concentration of IL-6 in T2 ,T3 ,T4 and IL-1βin T3 ,T4 in group Ⅰwere higher than group II (P <0.05).At T2 ,the serum S-100βin group Ⅰ was (1 625 ± 364)pg/mL higher than group Ⅱ(1 263 ±174)pg/mL(P <0.05).Conclusion The protective effection of COX-2-specific inhibitor parecoxib upon postoperative cognitive decline in elderly patients undergoing orthopedic surgery may be related to inhibition of peripheral cytokine release.
8.The effect of real time continuous blood glucose monitoring system versus intermittent blood glucose monitoring in critically ill patients under the intensive insulin therapy: a Meta-analysis

Objective To evaluate the value of real time continuous blood glucose monitoring system (RT-CGMS) versus intermittent blood glucose monitoring (IGM) in the critically ill patients under intensive insulin therapy (IIT).Methods A systematic searching randomized and controlled trials (RCT) in databases was performed for meta-analysis by Review Manager 5.2 software.Outcomes were hypoglycemia episode,alteration of mean blood glucose level,the percentage of time at a blood glucose level within optimal target range,and the early mortalities.Results Six studies,totally 531 patients,were included in this meta-analysis.The pooled SMD of mean blood glucose level was =-0.21 (95% SMD:-0.43-0.01,P=0.07).The pooled SMD of percentage of time at a blood glucose level within optimal target range was 0.20 (95% SMD:-0.09-0.49,P =0.18).The pooled OR of hypoglycemia episode frequency was 0.20 (95% CI:0.09-0.43,P < 0.01).The pooled OR of early mortalities was 0.35 (95% CI:0.14-0.89,P =O.03).Conclusions In critically ill patients under the intensive insulin therapy,RT-CGMS had obvious beneficial effect on reducing hypoglycemic events.RT-CGMS had no obvious beneficial effect on keeping blood glucose level within optimal target range.
9.CT Findings of Sacroiliac Joints in Ankylosing Spondylitis(An Analysis of 80 Cases)

Objective To sum up CT findings of sacroiliac joints in ankylosing spondylitis with an attempt at further improving early and even accurate diagnosis of the disease.Methods CT features of sacroiliac joints in 80 cases with anklyosing spondylitis proved clinically were analysed,and their application in making the diagnosis was evaluated.Results 73 of the 80 cases had abnormal CT findings of the joints,with different features according to different stages of the disease.In early stage,most cases were with symmetric involvement of the joints showing shaggy appearance of the articular surface with interruption or disappearance of the joint cortex,subcortical microcysts and mild sclerosis of iliac surface of joint .In advanced stage,cases were with irregularity of sacroiliac articular surfaces,showing serrated or brush-like appearance,subcortical systic destruction,sclerosis or hypertrophic changes around the joints,and pseudo-widening,irregular narrowing or even vacuumization of the joint space.In the late or stable stage,ossification of ligaments,bony ankylosis of the joints with obliteration of joint space and osteoporosis or osteosclerosis were seen.Conclusion CT study of sacroiliac joints could be helpful in making an early and even accurate diagnosis of ankylosing spondylitis as well as in clinical staging of the disease.
10.Single lung transplantation and ventricular septal defect repair for Eisenmenger syndrome: report of one case

Objective To evaluate the operative technique,perioperative management and ontcome of single lung transplantation and ventricular septal defect repair for Eisenmenger syndrome.Methods A 16 years old male patient with end-stage ventricular septal defect who received open heart exploration 12 years ago underwent right single lung transplantation and ventricular septal defect repair on Oct.22,2004.The operation was carried out under cardiopulmonary bypass(CPB) with beating heart.The donor lung was perfused with modified LPD solution.Tacrolimus(FK506),mycophenolate mofetil (MMF) and corticosteroid regime was used for immunosuppressive therapy.CPB time was 244 min,and donor lung ischemic time was 6 h.Results During the operation,the pulmonary artery pressure was reduced from 110/60 mm Hg to 53/39 mm Hg after transplantation.The patient weaned from the ventilator at the 12th postoperative day.An acute rejection episode occurred on the 14th postoperative day.ICU stay was 30 days.99m-Tc-MAA pulmonary perfusion scan demonstrated predominant right lung blood flow.UCG assessed improved heart functions,intact ventricular septum and mean pulmonary artery pressure of 45 mm Hg.The exercise tolerance was prominently improved.The patient was discharged on the 62nd day after transplantation.Conclusions Single lung transplantation and congenital heart defect repair is effective for end-stage left-to-right shunt congenital heart diseases with satisfying short-term results.Closed follow-up and surveillance are needed for long-term outcomes.
