2.Relationship between operative procedures and complication of colorectal carcinoma with acute colonic obstruction in the elderly patients

Objective To study the relationship between operative procedures and complication of colorectal carcinoma with acute colonic obstruction in the elderly patients. Methods 65 elderly patients undergoing emergency operation for acute colonic obstruction were analyzed retrospectively. One stage tumor resection and anastomosis was performed in 25 cases with right-side colonic lesion and in 26 cases with left-side colonic lesion, Hartmann operation in 8 cases, Dixon operation in 4 cases and proximal colostomy in 2 cases. Results In 65 cases, 13 cases of postopera-tive complications,the rate was 20.0% (13/65) ,which infected with toxic shock two cases,four cases of pulmonary infection,anastomotic leakage two cases, wound infection, split 2 cases, death three cases, the mortality rate was 4.6%. The rates of postoperative complications in right-side colonic lesion, left-side colonic lesion, Hartmann opera-tion and Dixon operation were 69.2% ,0,30.7% and 0 respectively(P < 0.05 ~ 0.01). Conclusion Acute obstruc-tion of colon cancer,the right-side colonic lesion were safe,feasible and few complications in colorectal carcinoma with acute colonic obstruction.
3.Comparative study on two types of digestive reconstruction after total gastrectomy in patients with gastric carcinoma

Objective To compare two types of digestive reconstruction and postoperative complications after total gastrectomy in patients with gastric carcinoma. Methods Roux-en-Y esophagojejunostomy and P-type Roux-en-Y esophagojejunostomy were performed in 50 patients with gastric carcinoma after total gastrectomy. The operating time and postoperative complications were observed. Results The gastrointestinal symptoms for Roux-en-Y esophagojeju-nostomy and P-type Roux-en-Y esophagojejunostomy serve as : diarrhea ( 11.8% and 12.1% ), dumping syndrome (3.9% and 3.3% ) and reflux esophagitis(5.9% and 6.0% ) ,the incidence of the difference was not significant ( than,P>0.05 ). The postoperative complications in P-type Roux-en-Y esophagojejunostomy( 13.1% ) slightly higher that Roux-en-Y esophagojejunostomy( 11.1% ) ,but the difference was not significant( P>0.05 ). Roux-en-Y esoph-agojejunostomy average operative time(3.1±0.3)h was significantly reduced than P-type Roux-en-Y esophagojeju-nostomy(3.7±0.2)h(t=3.35,P<0.05). Conclusion Roux-en-Y esephagojejunostomy and P-type Roux-en-Y esophagojejunostomy is a suitable type after total gastrectomy for patients with gastric carcinoma, but the former opera-tive time was shorter.
4.Analysis of Health Education Needs of Different Groups of Residents in Tianjin

Objective To investigate the degree of health knowledge and needs for health education of urban and rural inhabitants in Tianjin, and to study strategy of health education and health promotion in community. Methods Using a questionnaire survey, 900 inhabitants were investigated on their health knowledge, behavior, access to knowledge, and health education needs. Results The rate of health knowledge awareness was 65.63%, the rate of health promotion was 67.05%. 5.Comparison of the incidences of apical root cracks after canal preparation with two nickel-titanium rotary systems:an in vitro study

Objective: To compare the incidences of root cracks after canal instrumentation with HyFlex CM system and the ProTaper Universal system .Methods:Sixty mandibular incisors were mounted in resin blocks with simulated periodontal ligaments , and the apex was exposed .The control group of 20 teeth was not prepared , and the other 40 teeth were divided into 2 experimental groups ( n=20 ) .The 40 root canals of the experimental groups were instrumented using HyFlex CM and ProTaper Universal to the major apical foramen ( AF).The horizontal sections 1 mm, 2 mm, and 3 mm from the apex were observed under an optical stereomicroscope at 25 ×magnification .The presence of cracks was noted . Results:No cracks were found in the control teeth .Cracks were found in 1 of 20 ( 5%) teeth in HyFlex CM group , and 17 of 20 ( 85%) teeth in ProTaper Universal group .The difference between the two experimental groups was statistically significant (P<0.01).Conclusion: The HyFlex CM files caused fewer root cracks than the ProTaper Universal files during the root canal instrumentation .
7.Lung Injury and aquaporin-1mRNA expression during cardiopulmonary bypass

Objective To testify the lung injury induced by cardiopulmonary bypass(CPB) in canine model and observe the influence of CPB on the aquaporin-1 ( AQP1 ) mRNA expression in canine lung. Methods 8 mongrel dogs were used to perform the cardiopulmonary bypass. The hearts arrested for 90 minutes with mild hypothermia and rebeated for 6 hours. The hemodynamics, the ratio of lung dry weight and wet weight, the plasmic osmotic pressure, and the characteristics of light and fine structure were analyzed. The retro-transcription polyase chain reaction ( RT-PCR ) was used to measure the expression of AQP1 mRNA during the CPB. Results The hemodynamic data were stable in different time point during the CPB (P >0.05 ). The ratio of lung dry weight and wet weight was getting lower ( P <0.001) and the plasmic osmotic pressure was getting higher due to the prolongation of the CPB time and reperfusion time ( P <0.01). The light and electron microscopy showed the prominent aggregation of the white blood cell, severe interstitial edema and mild tear of respiratory membrane after 3 hour and 6-hour rebeat. AQP1 mRNA expression in lung was downregulated, 78.4% after 3-hour reperfusion and 55.5% after 6-hour reperfusion respectively, comparing to the level before CPB. Conclusion We recognize that the lung injury and lung edema were severe following 3-hour and 6-hour rebeat in CPB and hypothesize that the down-regulation of lung AQP1 mRNA expression may be a sign of pulmonary interstitial capillary injury induced by CPB.
8.Microsurgery for acoustic neuroma and the facial-acoustic nerve protection

Objective To explore the protection of the facial-acoustic nerve during microsurgery for acoustic neuroma via the suboccipital retrosigmoid approach. Methods The clinical data of 25 cases with acoustic neurinoma treated by the suboccipital retrosigmoid approach were retrospectively analyzed. Results Total removal was achieved in 22 patients and subtotal in 3 patients. 20 with Cochlear nerve anatomic preservation at 2 weeks after surgery and 3 patients with effective hearing and 13 patients lost effective hearing but retained measurable hearing and 9 patients completely lost hearing( the New Hannover Classification V grade). The facial nerve was preserved anatomically in 18,H-B Grade I ~ IV in 15,Grade V ~ VI in 10. There were not cerebrospinal fluid leakage, intracranial infection and death in all cases. Conclusion When operating,the relation between the tumor and the important structure surrounding could should be understood fully. Then removed the tumor piece by piece with microsurgical techniques. That the effective way to keep the facial nerve and its function being protected,and could improve the prognosis of the operation.
9.The intentional forgetting to emotional words of PTSD children after Wenchuan earthquake 18 months

Objective To compare the ability of intentional forgetting to emotional words of post traumatic stress disorder(PTSD) children and normal children by item method after Wenchuan earthquake 18 months.Methods Twenty PTSD children who met with CCMD-3 and DSM-Ⅳ criteria and twenty control group children were investigated. Sixteen positive and sixteen negative words were adopted as materials. PTSD children accomplished the words learning, arithmetical operation and memory tests ( include free recall, words completion and words recognition) in orderly. Results ( 1 )The free recall accuracy of PTSD children to positive words was significant high than negative words( (0.32 ±0.11 ), (0.28 ±0.10), P<0.05). The free recall accuracy of control group to positive words was significant high than negative words ( (0.37 ± 0.09 ), (0.34 ± 0.13 ), P< 0.05 ). ( 2 )The words completion accuracy of PTSD children to positive words was significant low than negative words( (0.38± 0.12 ), (0.49 ± 0.12 ), P < 0.05 ). The words completion accuracy of control group to positive words was significant low than negative words ( (0.39 ± 0.15 ), (0.56 ± 0.14 ), P < 0.01 ). ( 3 ) There was no difference of words recognition accuracy of PTSD children to positive and negative words ( ( 0.64 ± 0. 19 ), (0.63 ± 0. 16), P >0. 05). There was no difference of words recognition accuracy of control group to positive and negative words ((0.64 ± 0. 16), (0.65 ±0. 19), P > 0. 05). Conclusion There is a significant intentional forgetting effect on emotional words in PTSD children and normal children, and this effect were influenced not only by selective rehearsal but also retrieval inhibition.
10.The treatment of patients with ruptured intracranial aneurysms of hunt-hess Ⅲ-Ⅳ grade in the cerebral vasospasm stage

Objective To study the reasonable treatment to the patients with raptured intracranial aneurysms of hunt-hess Ⅲ-IV grade in the cerebral vasospasm stage. Methods The clinical data of 80 patients with ruptured intracranial aneurysms of hunt-hess Ⅲ-IV grade were analyzed retrospectively. All of the patients were clipped closed by microsurgery (36) or embolized intervention(44) in 3~18 days after disease. According to the time of surgical intervention after disease,they were divided into 42 patients with early(3~10 days) surgical group and 38 patients with advanced(11~18 days) surgical group. The different operative methods and different treatment periods were compared in order to evaluate the effect to the prognosis, the DIND incidence and the ruptured again aneurysms during operation incidence. Results There was no statistical significance in the effect about early or advanced surgical intervention to the prognosis, the DIND incidence and the ruptured again aneurysms during operation incidence (P > 0.05). There was no statistical significance in the effect about advanced interventional embolization or clipping closed by microsurgery to the prognosis,the DIND incidence and the ruptured again aneurysms during operation incidence(P > 0. 05). There was statistical significance in the effect about early interventional embolization or clipping closed by microsurgery to the prognosis,the DIND incidence and the ruptured again aneurysms during operation incidence(P< 0.05). The interventional embolization had better prognosis,more lower DIND incidence and more lower ruptured a-gain aneurysms during operation incidence than the clipping closed by microsurgery. Conclusion The patients with ruptured intracranial aneurysms of hunt-hess Ⅲ-IV grade in the cerebral vasospasm stage should be earlier intervened surgically,then it seems that interventional embolization is more reasonable which is worth referencing in clinic. The good operation habits during operation and the effectively strengthening management after operation had positive significance to ensure operative effect and to improve prognosis.
