1.Clinical analysis of low dose thalidomide combined with conventional chemotherapy in the treatment of multiple myeloma

Objective To investigate the efficacy of low dose thalidomide combined with chemotherapy in treatment of multiple myeloma(MM)and the clinical curative effect of the treatment method.Methods From June 2010 to June 2012,80 patients were diagnosed with multiple myeloma as the research object in Traditional Chinese Medicine Hospital of Haining,randomly divided into control group and observation group,with 40 cases in each group.The control group adopted conventional chemotherapy,the observation group were given low dose thalidomide combined with conventional chemotherapy.The clinical effect and adverse drug reactions were compared. Results The total effective rate of observation group (87.5%)was significantly higher than that of control group(65.0%)(P<0.05).The adverse reactions were 87.5%(35/40)in observation group and 100.0%(40/40)in control group,there was no significant difference between the two groups.The mean progression-free survival(PFS)and overall survival(OS)in observation group(5.5 ±1.2 months;1 1.5 ±2.4 months)were higher than those in control group(3.7 ±0.8 months;8.5 ±1.3 months)(P<0.05 ). Conclusion Low dose thalidomide combined with conventional chemotherapy in the treatment of multiple myeloma can improve clinical effect and decrease recurrence rate,with high safety and less adverse reactions.
2.Study on the changes of NSE, S100B protein and NPY levels in children with viral encephalitis

Objective To study the changes of neuron-specific-enolase (NSE), S100B protein and neuropeptide Y (NPY) levels in serum and cerebrospinal fluid of children with viral encephalitis and their clinical significance. Methods The NSE, S100B protein and NPY levels in the serum and cerebrospinal fluid of 50 children with viral encephalitiswere were measured, and another 20 children without central nervous system infection were selected as controls. The levels of serum and CSF NSE S100B protein and NPY in critical patient[serum: (21.93 ±5.39)μg/L,(0.71 ±0. 31)μg/L, (32. 5 ± 62. 8) μg/L;GSF: (13.05 ±4.41)μg/L, (1.23 ± 0. 66) μg/L, (407.3 ±68. 1 ) μg/L] were higher than ordinary patients [serum: ( 15.93 ± 4. 02 ) μg/L, ( 0. 42 ± 0. 14 ) μg/L,(234.7 ±51.2)μ.g/L;GSF:(8.05 ± 1.77) μg/L,(0. 63 ±0.26)μg/L, (320.2 ±59.5) μg/L] ( P <0. 01 ). Conclusion NSE, S100B protein and NPY can be used to evaluate encephalitis condition, brain damage degree and prognosis of viral encephalitis.
3.The study on the changes of endothelin-1 and troponin-1 levels in kawasaki disease

Objective To discuss the clinical significance of the changes of endothelin-1 and troponin-1 levels in kawasaki disease after acute stage. Methods 60 patients of kawasaki disease in which 12 cases of patient were complicated with coronary artery disease and 48 cases of patient who were not complicated with coronary artery disease were enrolled this study. Blood plasma endothelin-1 and serum cardiac troponin I were determined in kawasaki disease acute phase and recovery phase and 30 healthy children. Results Acute phase ET-1 and cTnI [(90.19±3.43)ng/L, (1.35±0.14)μg/L] and recovery phase ET-1 and cTnI [(89.09±2.44)ng/L, (1.12±0.11)μg/L ] in coronary artery lesions of (CAL) group had no significant difference(P>0.05). The concentration values of ET-1, cTnI in no coronary artery lesions (NCAL) group of acute phase (64.49±4.78)ng/L,(0.62±0.02)μg/L were different with convalescent phase (50.47±4.49)ng/L,(0.07±0.05)μg/L. There were significant difference between the the two phases (P<0.01). Plasma endothelin-1 and serum troponin I concentrations were positively correlated in acute phase of Kawasaki disease in children (r=0.93,0.96,P<0.01). Conclusions Plasma endothelin-1 and serum cardiac troponin I test for the diagnosis of Kawasaki disease with coronary artery disease may be early indicators of risk monitoring, they are valuable indexes in diagnosis and treatment of Kawasaki disease.
4.Changes of Plasma Von Willebrand Factor Content in Chronic Obstructive Pulmonary Disease and Late Lung Cancer Patients with Qi Deficiency

Objective To observe the changes of plasma Von Willebrand factor(vWF) content in chronic obstructive pulmonary disease(COPD) and late lung cancer(LLC) patients with Qi deficiency.Methods The changes of vWF content in 26 COPD patients with Qi deficiency and in 15 LLC patients with Qi deficiency were observed before and after treatment,and were compared with those in 30 healthy volunteers.Results Plasma vWF content before treatment was increased in COPD and LLC patients with Qi deficiency(P0.05).Conclusion Abnormal platelet activation exists in COPD and LLC patients with Qi deficiency,the function of vascular endothelial cells decreases,and the blood is in hypercoagulable state,which accords with the traditional Chinese medical theory of Qi failing in promoting blood circulation.

High expression of Fightless I (FLII) is associated to multiple tumors. Based on our previous study that FLII might be involved in the nuclear export, we assessed the possible interaction of FLII with the nuclear envelop associating proteins Importin β and Nup88. We first constructed GST-FLII, GST-LRR recombinant plasmids and transformed them into the Rosetta strain to produce GST-FLII, GST-LRR fusion protein. After purification of these proteins, GST-pull down, as well as co-immunoprecipitation, were used to test the interaction of FLII with Importin β and Nup88. FLII interacted with Importin β and Nup88, and FLII LRR domain is responsible for these interactions. Thus, FLII may play a role in nuclear export through interaction with Importin β and Nup88.

6.Case analysis of hospitals′s infringement on patients′ right to informed consent from the perspective of the Civil Code of the People′s Republic of China

Objective:To analyze the problems existing in the cases of hospitals infringing upon patients′ informed consent right after the promulgation of the Civil Code of the People′ s Republic of China, and to provide reference for improving the legal system of informed consent. Methods:Search for judgment documents published from January 1, 2021 to June 30, 2023 on the China Judgment Document Network using the keyword " liability dispute for infringing on patients′ informed consent rights", and conduct descriptive and textual analysis on the cases in the judgment documents.Results:68 judgment documents were included in total.There were 11 judgment documents on infringement of patients′ right to informed consent, and all related cases have undergone appraisal. The content of infringement mainly included infringement of patients′ right to know their condition (12/20), infringement of patients′ right to access their medical records (6/20), infringement of patients′ right to bodily integrity without consent (2/20). There were 15 judgment documents which judge that the right of informed consent of patients wasn′t infringed, and only one case in the document has been appraised. The main focus of controversy was the issue of the priority of the exercise of the right to informed consent (2/15), the fulfillment of the obligation to inform the right to informed consent (5/15), the issue of surgery without notification (1/15), and the issue of determining the authenticity of medical records and providing medical materials (7/15).Conclusions:The legal protection of patients′ right to informed consent still faces judicial difficulties such as unclear order of the exercise of rights subject, insufficient specificity of the scope of the exercise of the right, conflicts between the right to informed consent and medical intervention, and lack of unified identification methods for infringement liability. It is suggested to standardize the order of the exercise subject of the right to informed consent, clarify the content of the obligation of the medical party to inform, construct a system for medical intervention, and unify the identification methods for disputes related to the right to informed consent.
7.Association of thyroid-stimulating hormone receptor (TSHR) gene rs2268458 polymorphism with Graves disease and Hashimoto's thyroiditis

Objective To investigate the association of single nucleotide polymorphisms in the rs2268458 locus of thyroid-stimulating hormone receptor (TSHR) gene with Graves disease (GD) and Hashimoto's thyroiditis (HT).Methods Part of the cases diagnosed through the epidemiological investigation project about thyroid diseases of Cangzhou City in 2016 was selected as the case group.The case group was subdivided into GD group and HT group according to the diagnosis.At the same time,healthy people with similar gender and age to the case group were selected as the control group.All subjects were from the Han nationality and were not related to each other.The genotypes and alleles of the TSHR gene rs2268458 (C/T) of all subjects were detected by restriction endonuclease (RFLP).And based on genotyping analysis of patient risk (odds ratio,OR) and 95% confidence interval (95%CI).Results There were 87 cases in GD group [aged (43.17 ± 12.56) years old],including 64 females and 23 males.There were 31 cases in HT group [aged (44.41 ± 16.51) years old],including 26 females and 5 males.In the control group,there were 147 cases [age (40.26 ± 9.31) years old],including 80 females and 67 males.HardyWeinberg equilibrium test was performed on each group.The results showed that P > 0.05,suggesting that the study samples were representative of the population.The results of genetic analysis showed that in females,the C allele frequency of GD patients was significantly higher than that of the control group [x2 =4.632,36.7% (47/128) vs 25.0% (40/160),P < 0.05,OR =1.741,95%CI =1.048-2.891].The frequency distribution of each genotype (CC,CT,TT) at TSHR rs2268458 (T/C) was statistically different (x2 =6.104,P < 0.05),and the frequency of TC ± CC combined genotype was significantly higher in GD patients than in controls (x2 =6.092,P < 0.05,OR =2.333,95% CI =1.184-4.598),however,there was no statistical difference between the HT group and the control group in genotype (CC,CT,TT) frequency distribution and alleles (P > 0.05).In males,there was no statistically significant difference in genotype (CC,CT,TT) frequency distribution and allele between groups (P > 0.05).Conclusion Among women in Cangzhou,the single nucleotide polymorphism of rs2268458 in TSHR gene is associated with the susceptibility to GD,but not to HT,and C genotype increases the risk of GD by dominant inheritance.
8.Effect of psychological intervention on postpartum depression

Objective:To investigate the effect of psychological intervention on postpartum depression.Methods:A total of 250 pregnant women who were hospitalized for delivery in the obstetrics ward of Jiaxing Maternity and Child Health Care Hospital from January 2021 to January 2022 were included in this randomized controlled study. The patients were randomly assigned to an observation group ( n = 125) and a control group ( n = 125) using a random number table. The observation group received psychological intervention, while the control group underwent routine ward rounds. Prior to and after intervention, the levels of depression [assessed by Edinburgh Postnatal Depression Scale (EPDS) score and Self-rating Depression scale (SDS) score] and anxiety [measured by Self-rating Anxiety Scale (SAS) score] were compared between the two groups. Results:Before intervention, the EPDS, SDS, and SAS scores in the observation group were (16.95 ± 4.20) points, (65.83 ± 7.52) points, and (61.12 ± 7.59) points, respectively. In the control group, these scores were (16.62 ± 4.03) points, (64.49 ± 7.98) points, and (60.88 ± 7.85) points, respectively. At 7 days post-discharge, the EPDS, SDS, and SAS scores in the observation group were (11.54 ± 2.39) points, (51.12 ± 4.62) points, and (49.71 ± 4.22) points, respectively. In the control group, these scores were (15.93 ± 3.87) points, (63.21 ± 6.95) points, and (59.65 ± 6.43) points, respectively. At 7 days post-discharge, the EPDS, SDS, and SAS scores in the observation group were significantly decreased compared with those before intervention ( t = 18.35, 27.09, 21.60, all P < 0.001). At 7 days post-discharge, the EPDS, SDS, and SAS scores in the observation group were significantly lower than those in the control group ( t = -10.79, -16.19, -14.44, all P < 0.001). There were no significant differences in EPDS, SDS, and SAS scores in the control group between before intervention and 7 days post-discharge ( t = 1.95, 1.91, 1.92, P = 0.051, 0.056, 0.055). Conclusion:Psychological intervention can effectively alleviate postpartum depression and anxiety in postpartum women and deserves clinical promotion.
9.STAT3, a Poor Survival Predicator, Is Associated with Lymph Node Metastasis from Breast Cancer.

PURPOSE: The aim of this study is to explore signal transducer and activator of transcription 3 (STAT3) expression in breast cancer and to analyze the detailed mechanism that STAT3 contributes to the progression of breast cancer. METHODS: We retrospectively analyzed the clinicopathologic characteristics and overall survival (OS) of 140 breast cancer patients after curative surgery, and detected STAT3 expression, phosphorylated STAT3 (pSTAT3) expression, Ki-67 expression, vascular endothelial growth factor (VEGF)-C and -D expression in breast cancer tissues, and adjacent nontumor tissues. Survival analysis and relationship analysis were adopted for demonstrated the important mechanism of STAT3 contribution to progression of breast cancer. RESULTS: STAT3 expression, pSTAT3 expression, Ki-67 expression, VEGF-C expression, and VEGF-D expression in breast cancer tissues were significantly higher than those in adjacent nontumor tissues, respectively. With survival analysis, only number of lymph node metastasis (N stage) was identified as the independent predictors of the OS of breast cancer patients. Besides, we demonstrated there was the most prominent correlation between STAT3 expression and lymph node metastasis in breast cancer tissues by using the multinominal regression method. CONCLUSION: STAT3, a poor survival biomarker potential association with lymph node metastasis, was suitable for predication the OS of breast cancer patients after curative resection.

10.Efficacy of endoscopic ligation resection and endoscopic submucosal excavation for small gastrointestinal stromal tumors originating from muscularis propria

Clinical data of 43 patients who underwent endoscopic resection for gastrointestinal stromal tumors (GIST) of length ≤1.2 cm at the Digestive Endoscopy Center of the 909th Hospital from January 2016 to December 2018 were retrospectively analyzed. The patients were divided into the endoscopic ligation resection (ELR) group ( n=27) and the endoscopic submucosal excavation (ESE) group ( n=16). The general, perioperative and follow-up data of the two groups were compared. The results showed that there was no significant difference in the general data between the two groups. The operation time was 20.0 (18.0,25.0) min in the ELR group and 27.5 (23.0,37.5) min in the ESE group, showing significant difference ( U=92.5, P=0.001). The en bloc resection rates were 100.0% (27/27) in the ELR group and 81.3% (13/16) in the ESE group, showing significant difference ( P=0.045). The postoperative hospital stays were 3 (2,4) days in the ELR group and 5 (4,6) days in the ESE group, showing significant difference ( U=125.5, P=0.020). There was no significant difference in the intraoperative bleeding rate, intraoperative hemorrhage volume, intraoperative perforation rate, number of hemostatic clips or postoperative complications including hemorrhage, fever and peritonitis between the two groups ( P>0.05). During the follow-up, there was no recurrence or metastasis of GIST in both groups. ELR and ESE can be safe and effective for small GIST ≤1.2 cm in diameter. Compared with the ESE group, the operation time and postoperative hospital stay are shorter with higher en bloc resection rate in the ELR group.
