1.Observing the effect of using humane nursing among out-patients in transfusion room

Objective To study the effect of using humane nursing among out-patients in transfusion room. Methods 4 500 adult patients with transfusion were selected as experiment group, and selected other 4 000 ones as control group. Humane nursing was performed in the experiment group, while traditional nursing was performed in the control group. The patients' satisfactory rates in these two groups were collected by questionnaires and compared with each other. Results The satisfactory rate in experiment group and control group were 99.4% and 83.3% respectively; the health education cover rate in these two groups were 100.0% and 93.7% respectively; the understanding rate were 81.8% and 72.3% respectively. Objective To provide the exact anatomic basis for guidance of inserting catheter promptly.Methods Thirty five cases of adult crania were fixed by 10% liquor formaldehyde. In turn with precise discection of surrounding tissues and exposing carotid C 5~C 1 segments, paid attention to observe and measure vessel's trend, angulations, and direction of routes. Analysis of 50 cases of negative result of normal brain DSA is then taken as the basis for further application. Results Angulations of siphon were divided into 3 kinds of angulation and 1 characteristic curve but, the difference with the counter part is insignificant. Conclusions Mastering the applied anatomy of carotid could improve catheter maneuver and the rate of success.
3.Linguistic analysis of primary progressive aphasia

BACKGROUND: Primary progressive aphasia is a degenerative disease of nervous system clinically characterized by the progressive decrease of speech ability and the relatively reserved memory. OBJECTIVE: To investigate the characteristics of speech dysfunction and the clinical features of primary progressive aphasia we by reported onel patient with primary progressive aphasia. DESIGN: A case analysis. SETTING: Department of Neurology, Beijing Tiantan Hospital affiliated to Capital University of Medical Sciences. PARTICIPANT: One male patient of 56 years old with primary progressive aphasia was selected from the Department of Neurology, Beijing Tiantan Hospital in March 2004, he had got education in senior middle school. The patient had been unable to tell the names of daily living objects at the beginning of 2001. Not only his ability of listening comprehension had gradually declined, but his characters had gradually changed except that his memory had not been affected obviously since 2003. Although he was able in self-care now, he could not normally work. METHODS: ① The spoken fluent types of the patient were evaluated with the standards for the fluency of spoken language in Aphasia battery of Chinese. Western battery aphasia was used to assess the type of aphasia of the patient. Boston diagnostic aphasia examination severity grading standard was applied to grade the severity of aphasia. ② The cognitive psychological tests of visual character-figure matching, denomination for figures and oral reading were used to judge whether the patient had verb-noun dissociation. ③The memory of the patient was assessed with clinical memory scale. MAIN OUTCOME MEASURES: ① spoken fluency, the type and grade of aphasia; ② condition of verb-noun dissociation for the patient; ③ memory of the patient. RESULTS: ① Examination of aphasia: The patient presented the spontaneous talking that named the fluent type, there were wrong meanings in his talks so that he was diagnosed as sensory aphasia. The severity of aphasia was separated into grade 3 levels. ② Cognitive psychological test: The correct rates of verbs and nouns in the test of denomination for figures were 15% and 53% respectively, and there was obvious difference (t=0.231, P < 0.05). ③ Clinical memory scale: The memory quotient was 111,and the clinical memory grade was higher than normal. CONCLUSION: The most outstanding clinical characteristics of patients with primary progressive aphasia is speech dysfunction, and there is verb specific injury.
4.Observation on efficacy of endoscopic submucosal dissection and endoscopic mucosal resection in the therapy of esophageal mucosal disease

Objective To evaluate the outcome and safety of endoscopic submucosal dissection (ESD) versus endoscopic mucosal resection (EMR) in the management of esophageal mucosal lesion.Methods The clinical data of 70 patients with esophageal mucosal lesion were analyzed.Twenty-five patients were treated by ESD (ESD group) and 45 patients were treated by EMR (EMR group).The enbloc resection rate,tissue healing resection rate,operation time,complication and recurrence rate were compared between two groups.Results The enbloc resection rate,tissue healing resection rate in ESD group were significantly higher than those in EMR group [60.0%(15/25) vs.48.9%(22/45),48.0%(12/25) vs.40.0% (18/45)] (P <0.05).The local recurrence rate in ESD group was lower than that in EMR group [4.0% (1/25) vs.20.0% (9/45)] (P < 0.05).The severe complication such as delayed hemorrhage,perforation,stenosis between two groups had no significant difference (P > 0.05).The operation time in ESD group was (64.3 ±27.1) min,and significantly longer than that in EMR group [(27.6 ± 14.1) min] (P <0.05).Conclusions Compare with EMR,ESD has better enbloc resection rate,tissue healing resection rate,and lower recurrence rate.It is more safe and effective in the therapy of esophageal mucosal disease.

7.Imaging findings of primary malignant melanomas in the nasal cavity and paranasal sinuses

Objective To analyze the CT and MR manifestations and the diagnosis of primary malignant melanoma in the nasal cavity and paranasal sinuses.Methods CT,MR features and clinical data of 1 6 cases of malignant melanoma in the nasal cavity and paranasal sinuses,which were confirmed by biopsy or surgical pathology and immunohistochemistry were analysed retrospectivelly. CT routine and enhanced scan were performed in all patients,MR routine and enhanced scan were performed in 9 cases.Results CT findings of 1 6 patients were unilateral,in which 8 cases were located in the nasal cavity,4 cases in the paranasal sinuses,and 4 cases were both in the nasal cavity and paranasal sinuses.All lesions were irregular shape with soft tissue densityand without calcification and cystic degeneration and demonstrated invasiveness to the surrounding tissue.Lesions were nonuniformly moderate to severe en-hancement.Among 9 patients with MR examination,lesions were mainly unilateral in which 4 cases were in the nasal cavity,3 cases in the paranasal sinuses,2 cases both in the nasal cavity and paranasal sinuses.Lesions were irregular in 9 cases,in which 1 case had typical high signal on T1 WI,low signal on T2 WI,while 3 cases equal signal on T1 WI,equal or slightly higher signal on T2 WI and 5 cases mixed signal.On MR enhanced scan,unevenly slight-medium enhancement were showed in the lesion and surrounding tissues. Conclusion Primary malignant melanoma of the nasal cavity and paranasal sinuses is lack of specificity on CT imaging.Typical ma-lignant melanoma,a type of rare to finding,has certain characteristics on MR,non-pigment type and mixed type are more common. CT combined with MRI can better show the site of the tumor,and the relation with surrounding tissues and adjacent bones.

Objective To investigate the morphological changes of whole brain gray matter in patients with postherpetic neuralgia(PHN),and evaluate the correlation between morphological changes of whole brain gray matter,visual analogue scale (VAS) and duration of PHN.Methods By using VBM-DARTEL method,firstly the 3D T1WI structure images of 17 patients with PHN and 17 normal control were preprocessed,then the segmented gray matter volume was compared between the two groups, and the correlation between difference of cerebral gray matter volume,VAS and duration of PHN were analyzed.Results Compared with the normal control, gray matter decrease in PHN group was found in bilateral orbital frontal regions(orbital gyrus,rectus gyri),left inferior frontal gyrus,left insular lobe, left caudate, right cingulate gyrus,left superior temporal gyrus, left cuneus, bilateral cerebellum posterior lobe and bilateral cerebellum anterior lobe.And gray matter increase in PHN group was found in bilateral superior frontal gyri, left middle frontal gyrus, vermis, pons.The gyrus volume change for two hemispheres was asymmetric (the left side greater than the right side).These different brain region changes in gray matter volume for patients with PHN were not correlated with VAS and duration of PHN.
9.Application research of dry powder from stone of Garcinia oblongifolia in south of the Five Ridges in treatment of refractory Ⅲ and Ⅳ stage bedsore

Objective To discuss the effect of dry powder from stone of Garcinia oblongifolia in south of the Five Ridges on refractory Ⅲ and Ⅳ stage bedsore.Methods 120 cases of patients with refractory Ⅲ and Ⅳ stage pressure sore were randomly divided into the treatment group and the control group with 60 cases in each group.The treatment group applied dry powder from stone of Garcinia oblongifolia in south of the Five Ridges,while the control group used conventional infrared irradiation plus chloramphenicol powder treatment method.The treatment effect was compared between two groups.Results There were significnat differences in clinical efficacy,pain relief and cure time between the treatment group and the control group,and the treatment group acquired better effect than the control group.Conclusions The clinical efficacy,pain relief and healing time of the treatment group in the treatment of refractory Ⅲ and Ⅳ stage pressure ulcer was superior to the control group,and without skin lesions and human adverse reaction,and with the advantages of simple operation,quick,good curative effect,no pain to the patient,easy to accept and no toxic side effect.
10.Effects of Different Ventilation Modes and Parameters on Intragastric Pressure and Digestive Function

Objective To study the effect of different mechanical ventilation modes and parameters on intragastric pressure and digestive function. Methods Forty patients suffered respiratory failure were selected in the study. In different modes and parameters of mechanical ventilation, a multi-channel physiological signal recording device was used to monitor the intra-trachea pressure (ITP), intra-gastric pressure (IGP) synchronously, and the emptying time of stomach, pH value and total bile acid (TBA) concentration in gastric fluid of patients. Results (1) For three types of ventilation mode(PSV,SIMV and CMV), ITP was the lowest in group PSV, the highest was in group CMV and the middle was in group SIMV. With the in-crease in ITP, IGP was also increased. CMV was the most influencing factor for the IGP (P<0.01). In different PEEP values, the greater the PEEP, the higher the ITP. (2) Under mechanical ventilation support, when ITP increased, the emptying time of stomach decreased (P<0.01), the concentration of TBA increased (P<0.05). However, there was no significant change in pH value of gastric fluid (P>0.05). Conclusion Various mechanical ventilation modes and parameter settings induced differ-ent ITP, thereby affecting the IGP and gastric emptying. The digestive function can be promoted by choosing the appropriate mechanical ventilation mode and parameters.
