3.Influence of Width and Thickness of DCI's Curved Section on Its Performance.

By establishing a series of Dynamic Cervical Implants (DCI) within C5-C6 cervical spinal segments, the biomechanical finite element analysis for DCI with different width and thickness were carried out to investigate the influence of the width and thickness of DCI's curved section on its equivalent stress and range of motion (ROM), so as to provide some theoretical basis for the optimization of DCI's design. The results show that the width of DCI's curved section has more obvious influence on the ROM of lateral bending and torsion, in comparison with the thickness of DCI's curved section. By appropriate reduction in width, the ROMs of lateral bending and torsion increase obviously, i.e. the overall movement function of patients is improved. Furthermore, the increase of equivalent stress could be counteracted by corresponding increase of thickness.

4.Observation on efficacy of endoscopic submucosal dissection and endoscopic mucosal resection in the therapy of esophageal mucosal disease

Objective To evaluate the outcome and safety of endoscopic submucosal dissection (ESD) versus endoscopic mucosal resection (EMR) in the management of esophageal mucosal lesion.Methods The clinical data of 70 patients with esophageal mucosal lesion were analyzed.Twenty-five patients were treated by ESD (ESD group) and 45 patients were treated by EMR (EMR group).The enbloc resection rate,tissue healing resection rate,operation time,complication and recurrence rate were compared between two groups.Results The enbloc resection rate,tissue healing resection rate in ESD group were significantly higher than those in EMR group [60.0%(15/25) vs.48.9%(22/45),48.0%(12/25) vs.40.0% (18/45)] (P <0.05).The local recurrence rate in ESD group was lower than that in EMR group [4.0% (1/25) vs.20.0% (9/45)] (P < 0.05).The severe complication such as delayed hemorrhage,perforation,stenosis between two groups had no significant difference (P > 0.05).The operation time in ESD group was (64.3 ±27.1) min,and significantly longer than that in EMR group [(27.6 ± 14.1) min] (P <0.05).Conclusions Compare with EMR,ESD has better enbloc resection rate,tissue healing resection rate,and lower recurrence rate.It is more safe and effective in the therapy of esophageal mucosal disease.
5.Application research of dry powder from stone of Garcinia oblongifolia in south of the Five Ridges in treatment of refractory Ⅲ and Ⅳ stage bedsore

Objective To discuss the effect of dry powder from stone of Garcinia oblongifolia in south of the Five Ridges on refractory Ⅲ and Ⅳ stage bedsore.Methods 120 cases of patients with refractory Ⅲ and Ⅳ stage pressure sore were randomly divided into the treatment group and the control group with 60 cases in each group.The treatment group applied dry powder from stone of Garcinia oblongifolia in south of the Five Ridges,while the control group used conventional infrared irradiation plus chloramphenicol powder treatment method.The treatment effect was compared between two groups.Results There were significnat differences in clinical efficacy,pain relief and cure time between the treatment group and the control group,and the treatment group acquired better effect than the control group.Conclusions The clinical efficacy,pain relief and healing time of the treatment group in the treatment of refractory Ⅲ and Ⅳ stage pressure ulcer was superior to the control group,and without skin lesions and human adverse reaction,and with the advantages of simple operation,quick,good curative effect,no pain to the patient,easy to accept and no toxic side effect.
6.Analysis of Prescribing Patterns in Outpatients based on the Latest Chinese Guideline for the Management of Hypertension

Objective To investigate the pattern of antihypertensive medication prescribing in outpatients from the Second Hospital of Tianjin Medical University, and analyze the shortcoming and deficiency compared with 2010 Chinese guidelines for the management of hypertension. Methods A total of 154 262 electronic prescribing for outpatients with hy-pertension, from January-December 2012 in a Grade 3A hospital in Tianjin, were enrolled in this retrospective survey. Data of commonly used antihypertensive medication and combination therapy in patients were analyzed. The patient data collected were divided into different groups according to age, gender, high blood pressure level and the onset of the season. Results (1)The list of the drugs commonly used for treating hypertension in outpatients were calcium antagonist (52.3%), angiotensin receptor blockers (34.0%),βblockers (25.9%), angiotensin-converting enzyme inhibitors (12.1%), fixed-dose combination (11.0%) and diuretics (1.4%).(2)The fewer combination therapy was found in outpatients than that of monotherapy (43.9%vs 56.1%). Some prescriptions were not routinely recommended by the Guideline (4.6%).(3)The combination therapy used in patients with stage 3 hypertension was higher than that of patients with stage 1or stage 2 hypertension (44.5%vs 37.7%vs 37.7%, P<0.01). The rate of combination therapy was significantly higher in cardiology department than that of other clini-cal departments (P<0.01). The combination therapy tended to be used in the elderly patients than that of non-elderly pa-tients (P<0.01). The number of prescriptions was lower in summer than that of other seasons,but the rate of combination therapy was higher in summer than that of spring, autumn and winter (P<0.01). Conclusion The prescriptions of combina-tion therapy and diuretic were inadequate in outpatients with hypertension. These findings indicate the difference between clinical prescription and the guideline for the management of hypertension.
7.Impacts of birth order and age difference on the emotional and behavioral problems among children from two-child family

Objective:
This study aims to explore the influence of birth order and age difference on the emotional and behavioral problems among children from two-child family, so as to provide advice on parenting.
Methods:
A total of 482 students in grades 2 to 5 from the 5 primary schools in Guangzhou were selected, all of whom came from two-child families (excluding twin children). Emotional and behavioral problems were investigated by using the Conner’s Parental Symptom Questionnaire (PSQ).
Results:
Among children from two-child families, higher prevalence of behavioral problems, learning problems, impulsive hyperactivity and hyperactivity index in older compared with younger children (β=-0.09,-0.16,-0.09,-0.10,P<0.05). The scores of behavioral problems and hyperactivity index were significantly higher among those of children who were 6-9 years older than young siblings, compared to those who were 0-3 years older than younger siblings(β=0.20,0.21,P<0.05). No similar pattern was found among the younger siblings(P>0.05).
Conclusion
Among school-age children from two-child families, emotional and behavioral problems of the elder ones were more serious than the younger ones’. Age difference between siblings shows impact on the emotional and behavioral development among elder children, however no significant impact is noticed among the younger ones.
8.Comparison of MRI features and pathological findings between intracranial solitary fibrous tumors and hemangiopericytomas

Objective To analyze MRI features of intracranial solitary fibrous tumors (SFT)and hemangiopericytomas (HPC)and combined with pathology to improve recognition of the diseases.Methods MRI and pathologic data of 13 intercranial SFT and 9 HPC were analyzed retrospectively.Results The intracranial SFT were round or oval,well-defined,with internal hypointensity and obviously enhanced area on T2and enhanced MRI-so called"yin-yang"pattern,rare hemorrhage,slight peritumoral edema and rare skull destruction.The HPC were lobulated or irregular,unclear margin,hyperintensity on T2,common hemorrhage,obvious peritumoral edema and common skull destruction.The differences between the two tumors were significant in relation to the tumor shape,T2signal,"yin-yang"pattern, hemorrhage and skull destruction(P<0.05).There were no significant differences in relation to the internal cystic necrosis,peritumoral edema, dural attachment and dural tail sign (P>0.05).Both of them showed the similar immunohistochemical features:the expression of Vim,CD34,CD99 and Bcl-2 were positive,and the expression of EMA was negative.Conclusion The MRI features of intracranial SFT and HPC are different in tumor shape,T2signal,"yin-yang"pattern,hemorrhage and skull destruction.MRI features are helpful to diagnosis and judge prognoses preliminary.
9.Influential factors on congenital gastrointestinal malformation: a hospital-based case-control study

Objective To investigate the influential factors on congenital gastrointestinal malformation and to provide evidences for prevention.Methods A hospital-based case-control study was conducted among infants with or without congenital gastrointestinal malformation at Children's hospital of Hunan province,from April 2011 to August 2012.Parents of 120 cases with congenital gastrointestinal malformation and another 170 controls were asked to fill in a questionnaire.Data was analyzed using SPSS 18.0 software.Results Congenital gastrointestinal malformation was associated with intakes of maternal medication (OR=3.35,95%CI:1.51-7.41) and folic acid (OR=0.28,95%CI:0.15-0.52),exposure to paints (OR=5.05,95%CI:1.32-19.29) and pesticides (OR=15.20,95%CI:1.55-148.99) prior to or during pregnancy,and also associated with medication intake of the father (OR=3.70,95%CI:1.13-12.10),smoking (OR=2.39,95%CI:1.24-4.62),drinking alcohol (OR=2.47,95%CI:1.20-5.07),exposure to the agents for indoor cleaning (OR=16.42,95% CI:1.71-157.92) and exposure to paints (OR=9.92,95% CI:2.66-36.98) before conception.Conclusion Congenital gastrointestinal malformation was affected by multiple factors.Potential risk factors for congenital gastrointestinal malformation would include intakes of medication by the pregnant mother,exposure to paints exposure and pesticide exposure before or during pregnancy,and medication intake by the father,smoking,drinking alcohol,exposure to indoor cleaning agents/paint before conception.The amount of folic acid intake by mother before or during pregnancy was likely to reduce the risk for congenital gastrointestinal malformation.
