1.Clinical characteristic and therapy analysis of testicular germ cell tumour:45 cases report

Objective To investigate the diagnosis and therapy of testicular germ cell tumour.Methods The clinical data of 45 cases that were confirmed by pathology testicular germ cell tumour were retrospectively analyzed.Results All 45 cases were resected successfully.Among of them,28 cases seminoma,3 cases teratoma,2 cases embryoma,2 cases choriocarcinoma,4 cases mixed testicular germ cell tumour,5 cases yolk sac tumor,1 case spermatocytic seminoma.The treatment method was selected by pathology results.Eleven cases were lost,34 cases were followed up for 3 month to 5 years,25 cases seminoma,9 cases non-seminoma,5 cases were died.Conclusion Surgical resection and adjuvant chemotherapy are important factors for testicula disease.
2.The technique of the Ilizarov's principle on tension-stress combined with limited osteotomy of triple joint for correction of the severe adult talipes equinovarus

Objective To explore the application of the Ilizarov's principle on tension-stress in correction of adult severe talipes equinovarus. Methods From January 1996 to February 2003, 12 patients (13 feet) were corrected with this method, which involved 9 males and 3 females, whose average age was 26 years old (ranged from 20-36 years). The left foot was afflicted in 4 patients, and the right foot in 7, both side in 1. Among these patients, 3 were congenital, 6 were caused by the poliomyelitis, 1 by trauma, 1 by complication from encephalitic paralysis and 1 by radiotherapy for the cavernous hemangioma of the leg. The Ilizarov distraction apparatus of the ankle and foot joint were equipped in terms of the diseases preoperatively. During the operation, the release of the postero-medial soft tissue and the limited osteotomy of triple joints of the clubfoot were performed. According to the principles of Ilizarov, the wires were inserted and the fixator installed, the distraction devices of the ankle and foot joint which had corrective function in three-dimensional directions were applied to the tibia and foot, the telescopic rods on the apparatus were rotated one week after the operation. The deformity of talipes equinovarus, internal rotation and the drooping of the forefoot were gradually corrected, and the patients could bear weight and walked on the deformity foot during the course of distractive correction. Results All the 12 patients were available at follow-up with a mean duration of 15 months (ranged from 7 months to 4 years). There was no recurrence of the deformity and the feet function was good while walking on full weight-bearing. The mean duration of traction was 71 days. 10 patients (11 feet) could gain 0? in full weight-bearing, 2 patients preserved the drooping feet deformity at 15?-20? (because of shortness in the lower extremety). None of the complications occurred postoperatively, such as infection in the incision or pin tract, necrosis of the flap, neurovascular injury and nonunion. Conclusion Utilizing Ilizarov's principle on tension-stress to correct severe adult talipes equinovarus is a safe, minimally invasive and effective method.
3.Treatment of post-traumatic elbow joint flexion contracture by elbow joint distraction arthroplasty

[Objective]To explore the method and effect of elbow joint distraction arthroplasty in the treatment of post-traumatic elbow joint flexion contracture.[Method]An Ilizarov ring distractor of elbow joint was designed.From March 2003 to July 2005,three cases of elbow joint flexion contracture aged 16~17 years (one male and two females) with a flexion deformity of 70? in one case and 45? in two cases were treated.The joint distractor was applied with the wires in the upper limb and the joint hinge at the rotational centre of the elbow joint.No skin incision was made.To correct the deformity gradually,a continuous distraction force was applied by turning the nut on the distraction bar and periodic X-ray films were taken to confirm the position of the elbow joint.[Result]The elbow joint was finally extended to 0?~10?with an average 45?correction of flexion after an average 32 days of distraction.The range of elbow joint movement increased from pre-operative 75?to post-operative 115?.[Conclusion]The elbow joint distraction arthroplasty is effective in the correcting of the elbow joint flexion contracture and improving of joint function.
4.Apparatus assembly and clinical application of Ilizarov technique for correcting ankle and foot deformities

[Objective] According to the basic principle of Ilizarov technique,the apparatus assembly,indications,operation methods and clinical effectiveness of Ilizarov technique in the correction of foot and ankle deformities were explored.[Method]In accordance with pathological changes and requirements for correctioning talipes equinus,cavus,calcaneus and forefoot varus or valgus,four standard external fixators were designed on the basis of Ilizarov apparatus assembly and tested biomechanically to correct the above four foot deformities.The innovation had expanded Ilizarov technique to correcting scar talipes equinus and severe foot valgus caused by rheumatoid arthritis and congenital fibular deficiency.[Result]The clinical applications of the four kinds of external fixators showed reasonable apparatus structure,convenient installation and adequate adjustment of distraction tension.They had been used in 105 cases and achieved satisfactory correction of deformity and functional recovery without severe complications.[Conclusion]The four new assemblies of external fixators showed simple structures,good performance,and practicality.Ilizarov technique has played an irreplaceable role in the correction of severe foot and ankle deformities.
5.Clinical trial of levocarnitine on patients with continuous blood purification

Objective To evaluate the clinical effect of levocarnitine in the treatment of continuous blood purification and its effect on matrix metalloproteinase-2(MMP-2),interleukin-18(IL-18)and heart function.Methods 118 patients with continuous blood purification were randomly divided into the control group(n=59)and treatment group group(n=59).Two groups were treated with hemodialysis treatment.Control group was given folic acid,vitamin B and alpha keto acid,treatment group intravenous given levocarnitine injection 20mg/kg+0.9%NaCl 20mL on the basis of control group.A cycle of treatment was 28 days,and treated for 3 cycles.Serum levels of MMP-2 and IL-18,heart function,clinical effect and adverse drug reaction were compared between the two groups.Results After treatment,the total effective rate in control group was 84.74%,lower than 94.92%in treatment group(P<0.05).After treatment,serum MMP-2,IL-18,left atrial diameter(LAD),left ventricular end diastolic diameter(LVDd),left ventricular posterior wall thickness(LVPWT),ventricular septal thickness(IVST),left ventricular volume index(LVMI)in two groups were lower than pre-treatment(P<0.05),and the indexes in treatment group were more lower than control group(P<0.05).The E/A and left ventricular ejection fraction(LVEF)in two groups were all increased after treatment,and those in treatment group were higher than control group,the differences were significant(P<0.05).There was no significant in the incidence rate of adverse drug reactions between two groups.Conclusion Levocarnitine in the treatment of continuous blood purification was effective,and significantly reduce the serum MMP-2,IL-18 level and improve the heart function.
6.Experimental study of carotid sinus nerve block in prevention and treatment of hemodynamics instability caused by carotid sinus stimulation

7.The modified Ilizarov device combined with minimally invasive osteotomy in the treatment of genu varum deformity

Objective To evaluate the effect of modified Ilizarov device combined with minimally invasive osteotomy in the treatment of genu varum deformity.Methods From February 2002 to May 2010,a total of 31 patients with genu varum deformity who had underwent corrections by using modified Ilizarov device combined with minimally invasive osteotomy were retrospectively analyzed,including 8 males and 23 females with an average age of 35.6 years (range,18-69 years).Fifty-six knees were involved in the 31 patients.Preoperative angle of genu varum was 17.0° ±8.8°.According to the American knee society score (KSS),preoperative score was about 34-100 point with an average of 86.2±18.5.After determining the center of rotation angulation,osteotomy was performed in the site of tibial deformity and below the fibular head with minimally invasive device respectively.Prepared modified Ilizarov device was assembled and fixed with kirschner wire.Then genu varum deformity was progressively corrected by adjusting the length of bilateral bolts.X-ray film,KSS score,subjective sensation,and occurrence of complication were used to evaluate the therapeutic effects.Results Modified Ilizarov devices were used in 56 knees for 9-20 weeks,all patients were followed up for 14-50.4 months,with an average of 30.5 months.The angle of genu varum deformity was 1.7°±0.9°,the KSS was 96.1±7.7.According to KSS,the results were excellent in 49 knees,good in 6,fair in 1,with the excellent and good rate was 98.2% (55/56).The complications included pin site infection in 11 patients and pin loosening in 3 ones; they were healed through routine dressing change and replacing pin site respectively.Conclusion Modified Iilizarov device combined with minimally invasive osteotomy for the treatment of genu varum deformity can achieve a satisfactory therapeutic effect.
8.Improvement of thoraco-umbilicus flap repair in 54 cases of soft tissue defect

Objective To investigate the technique of design and cutting method of the thoraco- um-bilicus flap.Methods (1) Made the skin flap include not only the vascularity skin of this side,but also the opposite side,to repair overlength and overbreadth wound surface.(2) Improvement of skin flap cutting steps:first to find the blood vessel under the lower abdominal wall,then to cut the skin flap,if not find to have another donor area.(3)Technology of making thin skin flap:when cutting open the skin,made the surgical blade inclined to the flap,beveling to 2/3 of the skin flap length and in hypogastrium cutting to all over the flap with the fat keep stay in the abdomen so to make the skin flap thiner.Results There was 1 case necrosis because of infection in all 54 cases be repaired.The achievement ratio was 98.1%.Follow up with 1-5 years,skin flaps appearance was good,joint extend- bend was gratification.Soft tissue infection was cured.Conclusion Modified technology of thoraco-umbilicus flap is reasonable design and easily to cut,and the achievement ratio is higher.
9.Joint distraction apparatus and Ilizarov technique for correcting deformities of the knee and ankle joint caused by muscular contracture of the leg

CONCLUSION: Based on Ilizarov’s technique and principle, the individualized designed external distraction apparatuses are fixed around the knee and the ankle-foot by transcutaneous steel pins. Slow mechanical distraction gradually corrects the flexion deformities of the knee and clubfoot.
10.Severe Tick Infestation in a Hare and Potential Risk for Transmitting Pathogens to Humans.

Severe tick infestation was found in a hare in a suburban area of Nanchang, Jiangxi Province, China. We sampled ticks and identified them based on their morphologic characteristics. Three species, Ixodes sinensis, which is commonly found in China and can experimentally transmit Borrelia burgdorferi, Rhipicephalus haemaphysaloides, and Haemaphysalis longicornis which can transmit Lyme disease were detected with an optical microscope and a stereomicroscope. Risk of spreading ticks from suburban to urban areas exists due to human transportation and travel between the infested and non-infested areas around Nanchang.
