1.Uncertainty of Measurement for Cefixime in Human Plasma by HPLC-MS/MS

Objective To develop a method on evaluating the uncertainty during the determination of cefixime in human plasma by high performance liquid chromatography-tandem mass spectrometry ( HPLC-MS/ MS). Methods We analyzed various factors that can cause the uncertainty in the whole process of determination, which includ repeatability, weighting,preparation of standard solutions, sample preparation, equipment error and calibration curve fitting. The expanded uncertainty was evaluated by calculating uncertainty of each component and combined. Results The expanded uncertainty for low ( 80 ng ? mL-1 ), medium ( 560 ng ? mL-1 ) and high ( 9 600 ng ? mL-1 ) level of cefixime was 14. 93, 23. 65, 137.95 ng?mL-1 ,respectively (P= 95%). Conclusion The uncertainty of determining cefixime in human plasma by HPLC-MS/ MS is mainly caused by the calibration curve fitting,repeatability and HPLC-MS/ MS error.
2.Determination of Gallic Acid and Kaempferol-3-O-Rutinoside Contents inNymphaea Candida Presl. by HPLC

Objective To establish an HPLC method for the content determination of gallic acid and kaempferol-3-O-rutinoside inNymphaea candida Presl..MethodsThe separation was carried out on a Wondasil C18 column (250 mm×4.6 mm, 5μm). The mobile phase was methanol-0.1% acetic acid solution, with gradient elution;the flow rate was 1.0 mL/min;the detection wavelength was set at 270 nm;the column temperature was 30℃.Results The linear ranges of gallic acid and kaempferol-3-O-rutinoside were 0.065-0.585μg (r=0.999 1), 0.133-1.197μg (r=0.999 5), respectively. The average recoveries of gallic acid and kaempferol-3-O-rutinoside were 102.71%, 102.08%, with RSD of 1.97%, 0.46%, respectively.Conclusion This method was rapid, accurate, and reliable. 3.Clinical observation of formula for clearing heat,eliminating phlegm and relaxing bowel on treating encephalopathy complicated with pulmonary infection

Objective The efficacy and safety of rectal instillation with formula for clearing heat, eliminating phlegm and relaxing bowel on treating encephalopathy complicated with pulmonary infection were evaluated by randomized controlled clinical trials. Methods Eighty eligible cases of encephalopathy complicated with pulmonary infection were randomly divided into treatment group and control group. Control group was given conventional treatment and treatment group was given rectal instillation treatment with formula for clearing heat , eliminating phlegm and relaxing bowel. The changes of index were observed before and after treatment in two groups including temperature,antifebrile time,white blood cell count,neutrophils ratio,C-reactive protein,procalcitonin, pneumonia severity index,and the efficacy score of TCM syndrome. Results Rectal instillation with formula for clearing heat and eliminating phlegm to relax bowel can be better improved encephalopathy complicated with pulmonary infection with syndrome of phlegm-heat obstructing in which the patients have yellow sputum, constipation,yellow urinary and other symptoms. It can reduce TCM syndrome score and level of serum calcitonin, relieve inflammation,enhanc antibacterial effect,and improve the clinical curative effect. Conclusions The curative effects of rectal instillation with formula for clearing heat and eliminating phlegm to relax bowel on treating encephalopathy complicated with pulmonary is definite which has no adverse reaction and safety use. It is worthy of further popularizing in clinic.
4. A novel cavernosum-free surgical technique for concealed penis of children

Objective:
To explore the feasibility and applicability of a novel cavernosum-free surgical technique for concealed penis of children.
Methods:
From June 2017 to August 2017, 38 boys with concealed penis were treated using our new cavernosum-free surgical technique. The median age was 8.5 years(4-13 years). The technique consisted of 3 steps: Step 1, making a horizon incision and expose the glans; Step 2, suturing skin of penis to the corresponding side of inguinal ligament medial crus at 2 and 10 o′clock without fixing of cavernosum.Step 3, pruning the foreskin and suturing together. No catheter was placed after surgery.
Results:
All the patients were able to urinate 2 to 6 hours after operation. The numeric rating scales score of postoperative pain was 1-3. Wound dressing was removed 5 to 7 days post-operatively. All of the parents had good cosmetic appearance, penis stretched naturally without deflection. The length of penis increased more than 3.0 cm on average after operation.Edema was observed in 3 cases, but disappeared in 5-7 days.No serious complications such as bleeding or winding occurs.
Conclusions
The novel cavernosum-free surgical technique is easy to perform. It provides a safe and effective technique for concealed penis of children.
5.Statistical analysis of SCI papers in a particular hospital during 2013 and 2017

Objective This article conducted the statistical analysis of SCI papers published during 2013-2017 at the first affiliated hospital of Sun Yat-sen university ,to understand the SCI paper outcomes as to provide reference for future policy making and research administration development .Methods We analyzed the SCI papers published by the First Affiliated Hos-pital of Sun Yat-sen University from 2013 to 2017 ."Article" and"Review" type SCI papers published by the hospital as the first completion unit were statistically analyzed from the aspects of quantity ,impact factors ,journals and discipline distribu-tion .Results A total of 2268 SCI papers were published in 5 years ,distributed in 1162 journals .The quantity and the average impact factor both increased year by year ,but mainly concentrated in traditional dominant disciplines .Conclusions The SCI papers were increasing annually since 2013 .However ,we still need to improve the quantity and quality of high-level articles ,to promote the development of disciplines ,to strengthen the construction of public experimental platforms ,to continue building talent teams ,to improve the reward and evaluation system ,and to further enhance the overall scientific research level of the hospital .
6.Discussion on the main rankings and its impact on the discipline construction of the affiliated hospital in universities

Objective:Through the analysis of the evaluation index system of the major rankings of universities and hospitals, this paper aims to provide a reference for the discipline construction of affiliated hospitals in universities.Methods:This paper comprehensively analyzes and compares the evaluation objectives and indicators of the five major rankings of universities and the three major rankings of hospitals in China and abroad.Results:Each main rankings have its own characteristics that both positive and with possible limitations.Conclusions:Hospital management departments can refer certain indicators in order to identify possible gaps of the hospital discipline development. Also, tailored corresponding measurements for discipline development can be developed in combination with real-needs.
7.Mechanism of the therapeutic effect of Tetrandrine on striatum injury caused by microwave radiation

Objective:To study the therapeutic effect Tetrandrine (TET) on striatal injury caused by microwave radiation and underlying mechanism.Methods:C57BL/6N mice were randomly divided into blank control group (C), radiation control group (R), TET group (TET) and TET combined with radiation group (TET+ R). The mice of radiation group were exposed to 2.856 GHz 8 mW/cm2 microwave on whole-body for 15 min. TET (60 mg/kg) was injected intraperitoneally once a day for 3 consecutive days. The TET structure was verified by ultraviolet spectrophotometry. The open field experiment was used to detect the change of anxiety in mice. Histopathological and ultrastructural changes of the striatum were observed by light microscopy and transmission electron microscopy (TMT). Quantitative real-time PCR (qPCR) was used to detect gene expression changes of voltage-gated calcium channel (VGCC) subtype in the striatum.Results:The open field experiments showed that the time and distance of mice to explore the central region after microwave radiation were significantly lower than that before radiation ( t=4.60, 5.18, P<0.01), and the TET administration significantly improved these changes ( F=1.43, 4.37, P < 0.05). 7 d after microwave radiation, some neuronal nuclei in the striatum of mice contracted and could be stained deeply, which was more obvious in the globus pallidus area. The partial neuronal apoptosis, swelling and cavitation of glial cell mitochondria, blurring of synaptic gaps, and widening of perivascular gaps in the striatum were observed by TMT. The above lesions were significantly rescued after TET administration. But both microwave radiation and TET administration had no significant effect on the gene expressions of striatal VGCC ( P > 0.05). Conclusions:TET has a therapeutic effect on anxiety-like behavior and structural damage of striatum caused by microwave radiation, which is independent of the expression of striatal VGCC genes.
8.Discussion on the construction of research integrity in medical science under new circumstances

Objective This paper aimed to discuss the construction strategy to promote research integrity in medical science from the perspective of hospital research administrators,and to provide theoretical basis for fulfilling the national strategic requirement of implementing "Scientific research integrity construction ".Methods Analyze and summarize the connotation,manifestations,causes and countermeasures of research integrity and research misconduct.Results The misconduct in medical scientific research occurred in various aspects,including scientific research projects,research papers,scientific achievements and other different stages.Behind reasons include weak sense of research integrity,lack of moral integrity education and training,improper academic evaluation system,lack of sound supervision,reward and punishment mechanism,etc.Conclusions A lot of strategies could adopted to promote the construction of research integrity and development of medical scientific research,for instance,strengthening scientific research integrity education,establishing a multi-dimensional evaluation system,improving the supervision,reward and punishment mechanism of scientific research integrity,as well as promote a better scientific research atmosphere.
9.The mediating role of illness perception and illness distress on glycemic control and depression among people with type 2 diabetes

Objective:To investigate the status of depression and physiological, psychological and social factors among people with type 2 diabetes(T2DM), as well as the mediating effects of illness perception and diabetes distress on glycemic control and depression.Methods:A total of 511 patients with type 2 diabetes were recruited and investigated using general demographic questionnaire, self-rating depression scale(SDS), brief illness perception questionnaire(BIPQ) and diabetes distress scale(DDS). Body mass index (BMI) and hemoglobin a1c (HbA1c) were detected in laboratory.Results:The mean score of SDS was (57.48±9.94). The distribution of depression condition were 138(27.0%)without depression, 179(35%)with mild depression, 174(34.1%)with moderate depression and 20(3.9%)with severe depression.SDS score was significantly positively correlated with poor glycemic control ( r=0.157, P<0.01), illness perception( r=0.359, P<0.01) and four dimensions of diabetes distress( r=0.177-0.354, P<0.01). Partially mediating effect of illness perception( B=0.216, 95% CI=0.112-0.372) was found in glycemic control and depression, the proportion of effect was 25.9%.The chain mediating effect ( B=0.086, 95% CI=0.042-0.149) of illness perception and diabetes distress was also found between glycemic control and depression, whose indirect effect size was 10.3%. Conclusion:Glycemic control is significantly related with depression.Illness perception and diabetes distress are partly mediating the effect between glycemic control and depression.
