1.Research progress in promoting stem cell function and differentiation via electrical stimulation

Nowadays,stem cells are recognized as the most promising seed cells of tissue engineering due to their self replication ability and multilineage differentiation potential.In order to apply them to tissue engineering,a variety of physical and biochemical methods were tried to promote the proliferation,function and differentiation,and good effects were achieved.Recently,as a rising method of physical stimulation,electrical stimulation received more and more attention and had been proved to have good effects on stem cell proliferation and differentiation.This paper summarized the advances in promoting effects of electrical stimulation on stem cell proliferation and differentiation,and discussed the possible mechanisms of stem cell differentiation induced by electrical stimulation.
2.Clinical value of dual-source CT in assessing adult congenital heart disease

Objective To investigate the clinical value of dual‐source CT (DSCT ) in assessing adult congenital heart disease (ACHD) .Methods The data in 29 ACHD patients with DSCT were retrospectively analyzed .Results Among 29 cases ,the major‐ity suffered from two or more cardiovascular malformations ,in which 20 cases had intracardiac malformations ,including 4 cases (13 .79% ) tetralogy of Fallot ,6 cases (20 .69% ) were atrial septal defect ,4 cases(13 .79% ) were bicuspid aortic valve (13 .79% ) and 8 cases (27 .58% )were ventricular septal defect ;19 cases had extracardiac malformations ,in which 3 cases(10 .34% ) were co‐arctation of the aorta ,4 cases (13 .79% ) were patent ductus arteriosus ,3 cases (10 .34% ) were transposition of the great arteries and 5 cases (17 .24% ) were anomalous pulmonary venous connection .Conclusion DSCT could not only clearly demonstrate the cardiovascular malformations in ACHD patients ,but also conduct the observation on the lung ,trachea and bronchia ,which can pro‐vide detailed anatomic information for surgery .
3.Bacteria Distribution and Changing Status in Infectious Species from Our Hospital

OBJECTIVE To explore bacteria distribution and changing status in infectious species from our hospital. METHODS The infectious species were collected from daily work.After bacteria culturing and genus identification by VITEK system,bacteriology data and corresponding clinical data were analyzed. RESULTS From Jan 2001 to Dec 2004,Staphylococcus aureus,S.epidermidis,Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa and fungi were isolated most frequently in order. CONCLUSIONS Dynamic supervision of causative bacteria of nosocomial infection is strongly recommended.
4.Breast reconstruction with the bipedical transverse rectus abdominis

Objective To study the operational method and the clinical application of breast reconstruction with the bipedical transverse rectus abdominis. Methods 5 cases of breast reconstruction were performed with bipedical transverse rectus abdominis post mastectomy, and 3 cases of nipple-areola reconstruction. Immediate reconstruction was 3 cases, and delayed reconstruction was 2 cases. Results 4 cases of the breast reconstruction were survival, the morphologies were natural, and touch feeling was normal. Only one case skin flap was partial necrosis, nearly 3cm?2cm. Conclutions The breast reconstruction with bipedical transverse rectus abdominis is a good choice for breast cancer patients with caesarotomy scar.
5.Clinical application of immediate latissimus dorsal myocutaneous flap in breast reconstruction after partial re-moval of breast

Objective To study the immediate conservative breast surgical reconstruction by trans-ferring the same lateral latissimus dorsal myocutaneous flap (LDM) for the treatment of stage Ⅱ and stage Ⅲ breast cancer, combined with neoadjuvant chemotherapy. Methods Breast cancer patients in stage Ⅱ and Ⅲ with pathological diagnosis by core needle biopsy, had undergone immediate conservative breast reconstruction surgery with transferring the latissimus dorsal myocutaneous (LDM). We scored the reconstructed breast twice at one month after operation and completion of radiation. The final score was decided using the mean value of the twice evaluating score. Kaplan-Meier survival was used to analyze the survival of 18 cases with traditional mastectomy. Results The tumor size ranged from 30 mm to 55 mm before neoadjuvant chemotherapy, and ranged from 25mm to 45 mm after neoadjuvant chemotherapy. The median weight of the tumor specimens in breast conservative operation was 140 g (90 g to 220 g). A-mong the 18 patients, 16 cases had more than 3 scores with satisfied cosmetics (93.33 % ). Dorsal subcu-taneous seroma in donation area was observed in 5 of 18 patients (27.78 %) and dorsal incision dehiscence was observed in 2 of 18 patients (11.11%). None of the patients developed recurrence except that 1 pa-tient (5.55 %) had distance metastasis (bone metastasis} in a mean 22-months follow-up. The Kaplan-Meier survival curve in breast conservation reconstruction patients had no different from that of traditional mastectomy patients at the same time. Conclusions Immediate conservative breast surgical reconstruction by using the same lateral latissimus dorsal myocutaneous flap is an effective method for stage Ⅱ and stage Ⅲ breast cancer patients combined with neoadjuvant chemotherapy, which can increase satisfactory breast so that cosmetic outcome can be reached.
6.The research in the effect of early systemic nursing intervention on the lime of indwelling urethral catheter after surgery of uterine cervix cancer

Objective Through early systemic nursing intervention of cervical cancer patients for promotion of the recovery of urinary bladder function, to remove the urethral catheter in advance,and reduce the occurrence of urinary bladder dysfunction. Methods 80 cervical cancer patients after total hysterectomy were divided into the treatment group and the control group with 40 cases in each group stochastically. The treatment group was given systemic nursing( psychological nursing,pelvis bottom muscles exercising,urination discontinuance exercising,abdominal muscle exercising,Valsalva maneuver exercising, Crede press exercising,open the urethral catheter timely and individually,cheiropractic with hot roller), the control group received conventional nursing and some simple training of urinary bladder function. The time of indwelling urethral catheter after the hysterectomy was compared. Results The time of indwelling urethral catheter in the treatment group and in the control group were (9.65±49)days and (15.88±03)days respectively. The incidence rate of residual urine,urine retention and infection of urinary system after surgery in the treatment group was lower than that of the control group. Conclusions Early systemic nursing intervention can shorten the time of indwelling urethral catheter after hysterectomy, reduce the occurrence of urinary bladder dysfunction .improve the surgery effect for the cervical cancer patients and ameliorate their quality of life.
7.Diagnosis and management in parotid lymphoepithelioma-like carcinoma.

OBJECTIVE:
To study the diagnosis and treatment of parotid Lymphoepithelioma-like carcinoma (LELC), and improve the rate of the diagnosis and treatment.
METHOD:
Eighteen cases with parotid LELC confirmed pathologically were collected in our department, and the clinical presentation, diagnosis, treatment and prognosis were retrospectively summarized and analyzed from 1982 to 2002.
RESULT:
All of these LELC patients were found in unilateral side. All cases received Epstein-Barr virus serological test except 2 cases because of refusing, and the testing result displayed: EBV-VCA-IGA positive rate in 93% (15/16); EBV-EA-IGA in 75% (12/ 16); EBV-DNA enzyme in 63% (10/16). Of 16 cases with facial nerve reserved, parotid superficial lobectomy were undertaken in 6 cases, resecting a majority of parotid in 5 cases, whole lobectomy in 5 cases. Facial never resection and whole lobectomy in 2 cases due to facial nerve trunk involved. Functional neck lymph dissection in superior and middle part was undertaken in 14 cases and radical neck lymph dissection in 4 cases, total neck lymph node metastasis rate was 67% (12/18). All of patients received radiotherapy to 50-70 Gy. The follow up were over 1 year, and local recurrence occurred in 2-4 years after operation for 4 cases and had to undergo reoperation. The 1, 3, 5 years survival rates of the 18 cases were 94% (17/18), 72% (13/18) and 50% (9/18), respectively. The chief Causes of death were distant metastasis and local recurrence.
CONCLUSION
There maybe be close relationship between occurrence of LELC and Epstein-Barr virus infection. Its histopathologic feature is similar to undifferentiated nasopharyngeal carcinoma and nasopharynx biopsy must be demanded before confirming diagnosis for eliminating metastasis focus. The neck lymph node metastasis rate of LELC is high and local invasion is strong. It is important to undergo enlarged local resection, neck lymph dissection and postoperative radiotherapy.

8.Effect of Tiam 1 on cell proliferation and migration in head and neck squamous cell carcinoma cells.

OBJECTIVE:
To investigate the effect of T lymphoma invasion and metastasis 1 (Tiam 1) overexpression in head and neck squamous cell carcinoma (HNSCC) cells.
METHOD:
Endogenous expression of Tiam 1 in 8 head and neck squamous cell carcinoma cell (HNSCC) lines was investigated by real-time RT-PCR. A lentivirus vector containing Tiaml was transfected into UM-SCC-47 cells, a head and neck squamous cell carcinoma cell line with little endogenous Tiaml expression. Stable clone, obtained by G418 screening, were assayed by RT-PCR and Western blot to validate the gene expression efficiency. The biological behaviors of the transduced cells were determined by cell counting, MTT and in-vitro migration assay.
RESULT:
Tiam 1 gene was highly expressed in M2 cell line and it's low level expression was found in UM-SCC-47. Cell counting and MTT assay showed that over-expression of Tiaml significantly promoted cell proliferation (P < 0.05). The cell monolayers overexpressed Tiaml that resulted in a significant increasment of cell migration in infected head and neck squamous cell carcinoma cell lines (P < 0.05).
CONCLUSION
Tiam 1 gene plays an important role in the growth and migration in head and neck squamous cell carcinoma cell lines. It may be a useful marker for metastasis of head and neck squamous cell carcinoma.

ObjectiveTo study the effects of membrane molecular weight cut off on a novel bioartificial liver(BAL) system.MethodsHealthy beagles underwent 6-hour treatment with a BAL containing membrane with 200 kDa retention rating or 1200 kDa retention rating.The functional changes and cell viability were characterized.ResultsHepatocyte performance levels such as albumin secretion,urea synthesis and viability were significantly higher in the 200 kDa retention rating group when compared with the 1200 kDa retention rating group (P<0.05).Significant levels of canine proteins were detected in the BAL medium from the 1200 kDa retention rating group.Fluorescence microscopy further verified that heavy deposition of canine IgG,IgM and complement (C3) on co-culture cells were obtained after BAL treatment in the 1200 kDa retention rating group.ConclusionsSmall membrane molecular weight cut off of BAL could reduce the transfer of xenoreactive antibodies into the BAL medium and improved the performance of the BAL.
10.Membrane molecular weight cut off impacts immunological safety of a novel bioartificial liver system

Objective To investigate the influence of membrane molecular weight cut off in our bioartificial liver(BAL)system.Methods Beagle dogs were used for a model of acute liver failure through D-galactosamine administration.The acute liver failure Beagles were divided into two groups by the membrane molecular weight cut off.Group A was treated with BAL containing 200 kDa retention rating membrane.Group B was treated with BAL containing 1200 kDa retention rating membrane.Each group underwent two six-hour BAL treatments that were performed on day 1 and day 21.BAL medium were examined and levels of IgG,IgM,and complement hemolytic unit of 50%(CH50)antibodies were measured in all Beagles and.Results BAL treatment was associated with a significant decline in levels of CH50.1200 kDa group experienced a significant increase in levels of IgG and IgM after two BAL treatments.Significant levels of canine proteins were detected in BAL medium from 1200 kDa group.Conclusions Xenogeneic immune response in the BAL system was influenced by membrane molecular weight cut off.
