Objective To understand the status of disinfection in hospitals in Zibo. Methods Sanitary investigation on the disinfection quality of indoor air ,hands of doctors and nurses,surface of objects,the sanitary quality of disinfection reagents in use and the intensities of ultraviolet ray emitted from ultraviolet lamps was carried out in 34 general hospitals with bed capacity of more than 100 and special hospitals in Zibo. Results The total qualified rates of above indexes were 86.77% for municipal hospitals ,78.77% for district (county)hospitals,61.63% for factory and mine-hospitals respectively,and between them revealed significant differences (?2=59.95,P

AIM:To study the effect of neuregulin-1 ( NRG-1β) or captopril alone and their combination on myocardial cell apoptosis and related gene expression in rats with chronic heart failure .METHODS:Young male Sprague-Dawley rats were randomly divided into sham-operated group, heart failure group, NRG-1βgroup, captopril group and combined treatment group .The volume overloaded heart failure model in the rats was established by aortocaval fistula .The cardiac function was evaluated by determining the changes of hemodynamics and plasma BNP level .The apoptotic index ( AI) of myocardial cells was assayed by TUNEL .The protein levels of p-Akt, Bax and Bcl-2 in left ventricular tissue were detected by Western blot .RESULTS: The LVEDP, AI and the expression of Bax were significantly lower in NRG-1βgroup, captopril group and combined treatment group than those in heart failure group ( P<0.05 ) , while the values of ±dp/dtmax and the protein levels of p-Akt and Bcl-2 were significantly higher in NRG-1βgroup, captopril group and com-bined treatment group than those in heart failure group (P<0.05).The effect of combined treatment group was more signifi-cant ( P<0.05) than that in NRG-1βgroup and captopril group .CONCLUSION: Neuregulin-1βor captopril alone and the combination of them effectively improve the cardiac function and inhibit myocardial cell apoptosis in chronic heart failure rats.The therapeutic effect of combination of NRG-1βand captopril is better than that of NRG-1βor captopril alone .
3.Treatment ef patients with chronic sinusitis by endoscopic and Caldwell-Luc approaches

Objective:To investigate the clinical effect of surgical procedures on patients with chronic sinusi-tis. Method: 104 patients with chronic sinusitis were treated by endoscopic and Caldwell-Luc's approaches. Thelesions were cleared away completely. Result:The clinical cure rate was 96.1% after 6~12 months (average 9months ) follow up. Conclusion:It suggests that patients with severe maxillary sinus diseases might be cured byendoscopic and Caldwell-Luc's procedure.
4.Management of Anesthesia of Hirschsprung's Disease Radical Operation By Abdominoscope

Objective To investigate the effect of the general anesthesia combined with peridural anesthesia on hirschsprung's radical operation through children abdominoscope. Methods 40 children patients with hirschsperung were elective eradicative operated by using endotrachal intubation complex vein general anesthesia combined with continuons peridural anesthesia.ECG,HR,MAP,SPO 2,P ET CO 2 were monitored, and blood gas were recorded and analyzed before pneumoperitoneum,15 minutes and 40 minutes after pneumoperitoneum,1 minute after deflation,before decannulation at the operation termination.Results Compared with before pneumoperitoneum date,HR inereased obviously 40 minutes after pneumoperitoneum,and before decannulation at the operation termination,peak pressure in airway increased obviously after pneumoperitoneum,the difference was notable,SPO 2 decreased after pneumoperitoneum,the difference was also significant.pH decreased gradually with the time prolongation,there was obviously difference.The trend of P ET CO 2 and PaCO 2 changes were analogous,P ET CO 2 had much notable difference after pneumoperitoneum,PaCO 2 had notable difference at the operation termination,and before decannulation,it was decreased to the basic value.Conlusions Hirschsprung's Disease eradical operation using general anesthesia combined with peridural anesthesia under abdominoscope guide,can immobilize ill children completely,relieve pain perfectly,muscular relaxation satisfactory,the physical interference to body is little and less complication.
5.Intracavitary hyperthermia combined with radiochemotherapy for esophageal carcinoma

Hyperthermia can enhance the sensitivity of tumors to radiotherapy and chemotherapy, In order to understand the impact of intracavitary hyperthermia on radiochemotherapy in esophageal carcinoma, the biological basis,advances,efficacy and active factors of intracavitary hyperthermia combined with radiochemotherapy in esophageal carcinoma is reviewed and evaluated.

BACKGROUND:Study on mouse models of hyperinsulinemia confirms that hyperinsulinemia can induce the occurrence of myocardial hypertrophy. OBJECTIVE:To study the effect of insulin on proliferation and hypertrophy of cardiomyocytes in neonatal mice in vitro cultured. DESIGN,TIME AND SETTING:The in vitro cytology experiment was performed at the Key Laboratory of Anatomy,Histology and Embryology of Xinxiang Medical University from January 2006 to March 2007. MATERIALS:Twenty Wistar neonatal mice aged 1-3 days were used in this study. Insulin was obtained from Sigma,USA. METHODS:Cardiomyocytes of neonatal mice were in vitro harvested,culture and purified by trypsin digestion and differential attachment technique. Cardiomyocytes were incubated in 10-8,10-7,10-6 mol/L insulin for 48 hours. Blank control group was set up. 10-7 mol/L insulin group was used for cell cycle analysis. MAIN OUTCOME MEASURES:Cell diameter,activity and counting were measured under a microscope. The expression of cyclin E in cardiomyocytes was measured using immunocytochemistry. RESULTS:Cardiomyocyte activity in each group was over 90%. Compared with the blank control group,long diameter and short diameter of cardiomyocytes in the 10-8,10-7,10-6 mol/L insulin groups were significantly increased(t=2.781 5,P 0.05) ,but the number of cardiomyocytes in the 10-6 mol/L insulin group significantly decreased(t=1.976 2,P
7.Analysis of outcome and failure reasons of late course accelerated hyperfrationation radiotherapy for esophageal carcinoma

Objective To define the prognostic factors and local failure in late course accelerated hyperfractionation radiotherapy(LCAHR) for esophageal carcinoma.Methods A retrospective study was conducted in 201 esophageal squamous cell carcinoma patients treated by LCAHR during the period between August 1994 to January 2000.The radiotherapeutic portals were set and based on CT scan and esophagograms. All patients received a mean dose of 41.4 Gy /23 F/4~5 WK. With conventional fractionation regimen during the first two thirds of the course,and followed by LCAHR with reduced fields, at dose of 27 Gy/18 d,1.5 Gy per fraction,twice daily. The total dose varied up to 67~70 Gy/40~43 F/40~49 d. Results The 1 ,3 and 5 year actuarial survivals were 72.5% ,35.6% and 31.1%. The 1 ,3 and 5 year local control rates were 82.2%, 71.3% and 71.3%,respectively. Of the 95 patients who died, 34(35.8%) did so from local failure , 32(33.7%) from distant metastasis(33.7 %), 13(3.7%) from lymphatic metastasis, 4(4.2%) frome both local and distant metastasis and 12(12.7%) from complications.Conclusions It is showed that significant improvement in local control and survival are observed after LCAHR for esophageal carcinoma in comparison to conventional fractionated regimen. The prevention and management of distant metastasis and lymphatic spread have become the major problems in the future.
8.Late course accelerated hyperfractionation radiotherapy for elderly patients with esophageal carcinoma

Objective To study the clinical results and prognostic factors of late course accelerated hyperfractionation radiotherapy (LCAHR) in the treatment of esophageal carcinoma in the elderly. Methods 105 over 60 year-old patients with esophageal carcinoma who received radical LCAHR, were retrospectively analysed. Radical tumoricidal dose of 67.9~72.0?Gy was delivered in 39~43 fractions over 42~53 days. Results The 5-year local control rate was 63.7%. The 5-year disease-free survival and overall survival rate were 22.6% and 34.4%. Acute esophagitis and bronchitis were the most common but acceptable radioreactions Grade 1~2. No significant differences were found either in the clinical response or complication, between the 60~69 year and 70~80 year groups. By multivariate analysis, T stage and KPS score were two independent prognostic factors. Of 67 death cases, 31 died of local relapse, 23 of distant metastases, 8 of both and 5 of other causes. Conclusions LCAHR toxicity ,being tolerable for the older esophageal carcinoma patients,may improve their survival and quality of life.
9.The Application of Puncture Biopsy under Ultrasound Guidance in Diagnosis of Bone Tumor

Objective To identify the application of puncture biopsy under ultrasound guidance in diagnosis of bone tumor.Methods The results of ultrasonic puncture biopsy were comparatively analysed with that of imaging diagnosis and postoperative-pathological diagnosis in 17 patients with bone tumors.Results The giant cell tumor of bone were diagnosed by puncture biopsy in 8 cases,by imaging in 12 cases and by pathology,postoperation in 9 cases,the corresponding rate was 66.7% and 88.9% respectively.Bone cyst in 2 cases,fibrous dysplasia in 2 cases,metastasis from lung carcinoma in one case,osteosarcoma in 2 cases were confirmed by both pathology and puncture biopsy.The aneurysmal bone cyst in one case was diagnosed by imaging,but no blood-like fluid was harvested in puncture,one case of metastasis in proximal femur,one case of fibrous dysplasia and one case of osteosarcoma were diagnosed by imaging,while one case,2 cases and one case diagnosed by puncture respectively.One case of bone cyst was confirmed by imaging,puncture biopsy and pathology.Conclusion The puncture biopsy under ultrasound guidance is of high diagnostic rate in bone tumors,it is a good choice for the diagnosis of bone tumor.
10.Cleaning and Sterilization of Rent Instruments in Hospitals:A Discussion

OBJECTIVE To control the quality of cleaning and sterilization of rent instruments and to secure medical safety of patients undergoing operations.METHODS We randomly chose 60 pieces of rent instruments and divided them into two groups,machinery wash group and hand wash group.Occult blood test was performed before and after washing and rinsing.RESULTS Positive rate of occult blood tests was 86.67% before either machinery or hand wash.While after machinery wash and hand wash it was 0% and 6.67%,respectively.Both wash methods had significant effect on depleting occult blood(P0.05).CONCLUSIONS Hospitals must enhance the management of rent instruments cleaning which is of poor quality at present by regulating the procedure and monitoring methods so as to ensure the quality of rent instruments cleaning and secure patients safety.
