1.The activity of aminopeptidase N and gallstone formation

Objective To study the activity of aminopeptidase N (APN) in the bile of patients with cholesterol gallstones (CGS) and rabbits fed with lithogenic diets (LD) to explore the lithogenic role of APN in the formation of gallstones. Methods Enzymatic activity of APN and lipid concentrations in bile of investigated CGS patients and LD fed rabbits were measured. The relationship between the activity of APN and CSI was statistically analyzed.Results APN activity in CGS patients′ gallbladder (GB) and bile duct (BD) biles 〔(66 0?24 1)*!U/ml and (38 4?9 4)*!U/ml〕 as well as in LD fed rabbits′ GB and BD biles 〔(14 5?4 08)*!U/ml and (9 13?1 67)*!U/ml〕 were significantly higher than that in the corresponding biles of their clinical and experimental control (CC and EC) groups 〔(23 0?7 7)*!U/ml〕 in GB bile of CC group and 〔(6 05?0 95)*!U/ml and (3 09?0 92)*!U/ml〕 in EC group. All P values were less than 0 05. A statistical positive correlation was found between the APN and CSI value in each of the groups.Conclusions It is more likely that APN might play certain role in the early neucleating period of the pathogenesis of gallstones based on its proneucleative potential and measurement of APN activity may serve as a predictive marker among patients with high risk of gallstone formation.

Objective To explored the role of CD40/CD40L and B7-1/CD28 interaction in the immunopathologic process of folic acid-induced nephropathy(FAN) and investigate the intervention effect of the anti-B7-1 monoclone antibody(B7-1mAb). Methods GD1 mice were administrated by i. p with FA only or co-treated with B7-1 mAb for the intervention study. Kidneys were harvested for immunohistochemical, Western blot assays and serum for renal function evaluation at day 1, 3,5, 7, 14 and 21. Results Continuous expression of CD40, B7-1 from day 1 to day 21 on the renal tubular epithelial cells was detected in this model. 4.Relationship between leptin level and insulin resistance in patients with hypertensive cerebral hemorrhage

Objective To investigate the changes of leptin level in patients with intracerebral hemorrhage and the relationship between leptin level and insulin resistance(IR). Methods 57 patients with hypertensive cerebral hemorrhage (group A ), 50 patients with hypertension (group B), 41 healthy people (group C) were selected to determine the value of fasting blood-glucose ( FBG), fasting blood-insulin( FBI), leptin levels as well as insulin sensitivity index (ISI). Results The level of FNS and leptin was significantly higher (P < 0.05 ) while the level of ISI was lower (P < 0.05 ) in group A than those in group B and C. Correlation analysis showed that leptin level in HCH patients was negatively correlated with ISI, and positively correlated with FNS. Conclusion The results indicate that higher level of leptin and IR may play an important role in the occurrence of hypertension complicated cerebral hemorrhage.
5.Clinical research of microneurosurgery for pituitary adenoma with superciliaryarch-keyhole approach

Objective To study clinical effects and complications of minimal invasive neurosurgery for pituitary adenoma through superciliaryarch-keyhole approach. Methods 127 cases pituitary adenoma in three hospitals in Shangdong province from May. 2006 to Sept. 2009 confirmed by surgery were analyzed. Patients were treated by different operative approaches, 53 cases using surgery through superciliaryarch-keyhole approach and 74 cases through conventional subfrontal approach. Clinical parameters were compared in patients undergoing surgery through different approaches. Results There were no differences in two groups, in aspects such as resection rate, improvment of endocrine hormone secretion and acuity of vision with campus visualis, electrolyte disturbances, hypothalamus damage (P >0.05). Significant differeces reside in average hospitalization days, subcutaneous fluidify, psychiatric symptom and circumscribed cerebral ischemia ( P < 0.05 ). Conclusion Advantages of minimal invasive neurosurgery through superciliary arch approach such as less trauma, better expose,shorter hospitalization days, less subcutaneous fluid accumulation and psychiatric symptoms in terms of this apporach are suitable for removing hypophyseal adenomas that grow on or around sella turcica.

Objective To summarize clinical diagnosis and microsurgical treatment method of cerebellopontine angle epidermoid cyst presenting with trigeminal neuralgia.Methods Thirty-seven cases of cerebellopontine angle epidermoid cyst presenting with trigeminal neuralgia treated by microneurosurgery procedure were analyzed retrospectively.Results Complete resection was achieved in 31 cases,and subtotal was achieved in 6 cases,with no operative mortality.Postoperatively,the symptom of trigeminal neuralgia disappeared in 28 cases,was eased obviously in 6 cases,and was eased in 3 cases.The symptom of dizziness in all 6 cases vanished,and hearing in 3 cases was improved.Three cases appeared cerebrospinal fluid leakage,and 4 cases appeared oral herpes.Symptom in 1 case aggravated after 6 months,and oral medicine was ineffective.The pain symptom disappeared after reoperation.Conclusions The suboccipital retrosigmoid approach microneurosurgery is the principal treatment method for cerebellopontine angle epidermoid cyst presenting with trigeminal neuralgia.Microsurgical technique combined with surgical skill is the guarantee for increasing total removal rate of tumors and decreasing complications.
