Objective To investigate the effect of Bushen-Tongmai decoction on plasma renin activity (PRA), plasma levels of brain natriuretic peptide (BNP) and angiotensinⅡ(AngⅡ) in patients with chronic heart failure. Methods A total of 100 patients with chronic heart failure from January 2011 to December 2013 were randomly divided into a control group and a treatment group, with 50 patients in each group. The patients in the control and treatment groups received conventional treatment and Bushen-Tongmai decoction plus conventional treatment for 8 weeks, respectively. Laboratory measurements including PRA, BNP and AngⅡwere performed before and after treatment.Cardiac function was measured before and after treatment with echocardiography. Results The PRA , plasma levels of BNP and AngⅡ after treatment in the treatment group were significantly decreased than before treatment (treatment group:t=2.990, 4.030, 4.401, all P<0.01);after treatment, there was significant difference in PRA, BNP and AngⅡbetween the treatment group and the control group(t=2.622, 2.863, 3.809, all P<0.05). Left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD) and left ventricular ejection fraction (LVEF) by echocardiography in both groups were significantly improved (treatment group:t=9.167, 11.030, 13.273, all P<0.01;control group:t=5.189, 7.626, 6.906, all P<0.01). Improvements of LVESD, LVEDD and LVEF in the treatment group were grater than those in the control group (t=4.037, 2.701, 7.458, all P<0.01). Conclusion Bushen-Tongmai decoction can effectively decrease PRA , plasma levels of BNP, AngⅡ, and improve cardiac function in patients with chronic heart failure.

Objective　To explore left atrial(LA) structur e and function changes in 86 cases of essential hypertension disease (EH) and ef fects of age,gender,hypertension duration,hypertension stage and left ventricula r function on them.Methods　 Using M-mode echocardiogram to detect LA diameter, Usi ng ultrasonic acoustic quantification (AQ) to detect LA area,LA systolic and dia stolic function,LV systolic and diastolic function. Results　①Compared with normal group, LA diameter and LA area we re larger in EH group, LA systolic and diastolic function were impaired. ② Gend er had no effect on LA structure and functione in EH patients; LA diameter and L A area dilated with age, LA systolic function was impaired with age. ③ Hyperten sion duration had no effect on LA structure and function in EH patients. ④LA d iameter and area dilated with hypertension stage. With the development of hypert ension, LA diastolic function was first impaired,then LA systolic function was a lso impaired.⑤LA structure and function had signifficant relationship with LV d iastolic function, LA might dilate and LA systolic function might be impaired wi th LV diastolic function damage.Conclusions　LA diameter and LA area are dilated and LA function i s impaired in EH patients, which changes become more signifficant with age. LA s tructure change and systolic function impairment have a relationship with LV dia stolic dysfunction.

Objective To investigate and compare the clinical values of routine electrocardiogram and dynamic electrocardiogram in arrhythmia in coronary heart disease diagnosis.Methods 73 patients with coronary heart disease were collected and monitored by routine electrocardiogram and dynamic electrocardiogram,and two groups were e stablished including control group using routine electrocardiogram and observation group using dynamic electrocardiogram.The positive diagnosis rate and the specific diagnosis diresults of arrhythmia in coronary heart disease of both groups were compared.Results The positive diagnosis rate of arrhythmia in coronary heart disease of observation group(75.3％ ) was significantly higher than that of control group(54.8％ ) ( x2 =6.78,P ＜0.05).And the positive diagnosis rate of the observation group was also significantly higher than that of the control group in ventricular premature beats two,triple law,pairs of premature ventricular contractions,atrial premature beats two,triple law,atrial premature beats in pairs,atrioventricular block and paroxysmal ventricular tachycardia( all P ＜ 0.05 ).Conclusion Compared with routine electrocardiogram,the dynamic electrocardiogram in anrnythmia in coronary heart disease diagnosis could obtain higher positive diagnosis rate,and it is worthy of clinical application.

Luminol dependent chemilumi-nescence (CL) method was used to observe effect of nifedipine on polymorphonuclear leukocytes (PMN) oxygen metabolism in coronary heart disease (CHD). Studied subjects including 24 cases of coronary heart disease non using nifedipine, 26 cases of coronary heart disease using nifedipine group (10.mg tid on 3~7 days), 30 cases of normal drug-free subjects.The results showed that: 1. PMN-CL in using nifedipine CHD group was sigrlifficantly lower, its PMN- CL backgroud, peak value andphargocyte index was signifficantly lower than that in non using nifedipine CHD group. 2. PMN-CL peak value and phargocyte index in using nifedipine CHD group had no signifficantly differences compared with normal group. 3. Parameters of PMN- CL in non using nifedipine CHD group were signifficantly higher than that in normal group.

Objective To explore the effect of lymph node metastases in gastric cancer on vagniae vasorum of gastric vessels,in order to provide evidence for the surgical treatment of gastric cancer.Methods The study included 90 specimens of left and right gastric arteries from 47 patients who underwent radical gastrectomy for carcinoma from January 2004 to 2006.All specimens were dealt with frozen section method,then they were stained with HE and immunohistochemical method respectively.Results Metastatic cancer cells were not found inside vagina vasorum in stage Ⅰ specimens.Cytokeratiu positive of tumor cens in or on vagina vasorum was showed on 25 slices from 10 tumors.Among them,2 slices from 1 tumor belonged to stage Ⅱ.8 slices from 3 tumors belonged to stage Ⅲ and 15 slices from 6 tumors belonged to stage Ⅳ.There was significant differences among cytokeratin positive rate in different stages of gastric cancer(P＜0.01).Conclusion Vaginae vasorum dissection is recommended when clearing lymph node around arteries in radical gastrectomy for carcinomas in stage Ⅱ above.

Objective To evaluate the role of polymorphonuclear (PMN) in patients with acute myocardial infarction (AMI).The preventive effects of Vitamin C-a potent free radical scavenger,on parameters of PMN oxygen free radicals (OFR) production in patients with AMI were also studied.Methods 60 patients with AMI were randomized to receive either conventional treatment only (group Ⅰ,n=30) or conventional treatment supplemented with vitamin C infusion,3.0 g/d,for 7 days (group Ⅱ,n=30).Parameters of PMN OFR production were assayed by a method named as polymorphonuclear chemiluminescence (PMN-CL) on the 1st,3rd,7th and 10th day after been hospitalized.62 healthy controls of similar age,sex were also studied.Results Parameters of PMN-CL increased significantly in patients with AMI compared with that in the healthy controls.Parameters of PMN-CL decreased significantly in group Ⅱ on the 3rd,7th and 10th day compared with that in group Ⅰ(P<0.05,0.01,0.001,respectively).ST segment score was significantly higher in group Ⅱ than that in group Ⅰ on the 3rd,7th and 10th day.Conclusion These results indicate that the PMN act as a potential contributor to extension of tissue injury induced by OFR.Supplementation with vitamin C may suppress PMN OFR production and could be beneficial in preventing myocardial necrosis.
